Full mouth rehabilitation of a patient with difficulties in guiding centric relation: A case report (중심위로의 유도가 어려운 환자의 수복 증례)

  • The Gothic arch tracing method using a Gothic arch tracer which is one of the centric relation recording methods can reproduce mandibular movement more accurately by describing the path of mandibular curvilinear motion. This case reports that we have satisfactory results by recording reproducible centric relation using a gothic arch tracing method in a patient who has difficulty to induce centric relation by operator due to parafunctional movement.

HORIZONTAL PLANE JAW MOVEMENTS IN EDENTULOUS PATIENTS BY USE OF EXTRAORAL TRACING DEVICE (구외 묘기장치를 이용한 무치악 환자의 수평면상 하악운동에 관한 연구)

  • Ten subjects who were going to wear conventional complete dentures were selected for this study. Theree subjects were women and seven were men. The average age was 63.1 years(range : 44 to 76 years). With the Height tracer (extraoral tracing device) in place the subject was instructed to go through the entire range of mandibular movements. The extreme lateral pathway of the incisor point, the so-called Gothic arch, was thereby inscribed by the stylus on the tracing plate. The mandibular movements in this study were peformed voluntarily by the subject(self guided technique) and guided by the dentist(chin-point technique and bimanual technique). The Gothic arch tracings were analysed and the Gothic arch angles and eccentric movement distances were measured. The results were as follows : 1. The apex position of the Gothic arch tracings of mandibular movements in edentulous patients varied both anterioposteriorly and mediolaterally. 2. The Gothic arch tracing had some lateral deviation during protrusion. 3. The average Gothic arch tracing angle was $136.7{\pm}12.0^{\circ}$ by subjects self guided technique, $135.7{\pm}5.9^{\circ}$ by chin-point technique, $136.6{\pm}6.5^{\circ}$ by bimanual technique. But there were no statistical differences in the reliability among the three techniques. 4. The average mandibular eccentric movements were irregular and the mandibular eccentric movement distances varied with a wide range.

Full mouth rehabilitation with vertical dimension elevation in the patient with severely worn dentition: case report (과도한 치아 마모 환자에서 수직고경 증가를 동반한 전악 수복 증례)

  • Excessive teeth abrasion causes pathologic changes of oral environment and masticatory system due to decrease in occlusal vertical dimension. When establishing new occlusal plane and recovering vertical dimension, accurate diagnosis and analysis are essential. In this case, after appropriate centric relation in elevated vertical dimension was taken using Gothic arch tracer, full mouth rehabilitation was performed. A 70-year-old male patient had the chief complaint that he could not eat due to teeth wear. He showed generalized teeth wear and decreased vertical dimension. Elevation of vertical dimension was planned by model analysis. According to increased vertical dimension, centric relation was recorded using Gothic arch tracer and temporary prostheses were applied. Appropriate occlusion was established by temporary prostheses for 4 months. Final prostheses were fabricated using vertical dimension adapted by temporary prostheses. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

  • It was clinically important to substitute the physiologic centric relation to the therapeutic position of the patients who needed the oral rehabilitation or occlusal treatment. There were several methods for recording the centric relation. One of the known methods was to use the gothic arch tracer. However the existing intraoral device was difficult to adjust the three dimensional angulation of the recording plate and recording stylus depending on the hinge movement arch of the individual. The purpose of this study was to develop new intraoral tracer which had adjustable stylus within hinge movement arch for the record of centric relation and to evaluate the clinical application of this device. The results were as follow; 1. A stylus of new developed intraoral tracer was so adjustable that the recording of mandibular positions could be reproducible within the hinge movement arc. 2. A record plate of new developed intraoral tracer was so adjustable to parallel with the occlusal plane that lateral recording of mandibular position was able to obtain stably. This study showed that new developed intraoral tracer allowed the determination of the treatment position which can be used in the full mouth rehabilitation and occlusal treatments.

  • In order to create denture occlusion that is functional, comfortable and balanced, it is necessary to both determine a vertical dimension of occlusion that is in harmony with the patient's musculature and to record a relatively repeatable jaw relation position. This require clinical skill to establish an accurate, verifiable and reproducible vertical dimension of occlusion (VDO) and centric relation (CR). Correct vertical relation depends upon a consideration of several factors, including muscle tone, inter-dental arch space and parallelism of the ridges. Centric relation is considered to be a repeatable position from which all opening and lateral mandibular movements begin and recording this position is a critical step in the fabrication of dentures. Any errors made while taking maxillo-mandibular jaw relation records will result in denture that are uncomfortable. The purpose of this paper is to review a very simple and efficient technique for accomplishing these two important steps in denture fabrication.

  • A patient with para-functional habits can cause widespread teeth wear, along with temporomandibular joint disorders and myofascial pain syndrome in the masticatory muscles. Prolonged teeth wear is associated with a decrease in vertical occlusal dimension, leading to issues such as changes in facial morphology, decreased masticatory efficiency, and temporomandibular joint disorders. To achieve a three-dimensional full-mouth rehabilitation in patients with decreased vertical occlusal dimension, accurate diagnosis, analysis, and proactive treatment planning are essential. Prosthetic treatment accompanied by the restoration of physiological vertical occlusal dimension and the re-establishment of a normal occlusal plane is necessary. This case report presents a full-mouth rehabilitation case involving a patient with overall teeth wear, showing decreased vertical occlusal dimension which results in discomfort in the temporomandibular joint and aesthetic dissatisfaction. The report highlights the successful outcome achieved through the use of occlusal stabilization splint for temporomandibular joint stability and Gothic arch tracing devices for the re-establishment of intermaxillary relationships. Also, through adjustments and adaptation assessment using provisional prostheses, favorable outcomes were achieved both functionally and aesthetically.

Comparison of the replicability of centric relation registration techniques using Arcus Digma (Arcus Digma를 이용한 하악 중심위 유도방법에 따른 하악위의 재현성에 관한 비교 연구)

  • The purpose of this study was to compare the replicability of centric relation made by means of self guided method, bimanual manipulation, chin-point guidance, leaf gauge method, gothic arch tracing. The EPA test of Arcus $Digma^{(R)}$ was employed for examining the reproducibility of centric relation. 10 adults with normal occlusion were participated in this study. In this limitation of study, the following results were obtained; Condyle position was different depending on different methods of guiding centric relation. Left and right temporomandibular joints showed a little different degree of repeatability. Bimanual manipulation using anterior jig was the most replicable. Chin point guidance provided the second most repeatable method, and Gothic arch tracing showed the third, the leaf gauge was the forth, the self-guided method was the worst.

Relationship of TMJ sound and mandibular positions recorded by a newly developed intra oral tracer (새로 개발된 구내묘기장치에 의해 채득된 하악위와 측두하악관절잡음과의 관계)

  • It is clinically important to determine the physiologic mandibular position as the therapeutic position of the patients who needs the oral rehabilitation and occlusal treatment. Several methods have been employed for the recording the mandibular position. The gothic arch tracer is one of methods to record the mandibular position. The purpose of this study is to record the border position, chewing position, and myocentric position using the newly developed intra oral tracer in 10 subjects with TMJ clicking sound and 10 subjects without TMJ clinking sound.. This study showed that newly developed intra oral tracer allowed clinician the determination of the treatment position on the same horizontal plate which can be used in the full mouth rehabilitation and occlusal treatments. There was no statistically significant difference between clicking group and nonclicking group in the distance of border position-chewing position and the distance of border position-myocentric centric position.
