Current Trend in Use of Occupational Therapy Assessment Tool by Pediatric Occupational Therapist (국내 아동 작업치료사의 평가도구 사용 동향)

  • Objective : The purpose of this study is to investigate trend in use of occupational therapy assessment tools used by pediatric occupational therapist. Methods : Survey questionnaire developed for this study were used to for data collection. The questionnaires are divided into 10 area: occupational performance, activities of daily living, education, play, sensory-perceptual, motor and praxis, cognition, social interaction skills, development, and physical examination. Total 105 responses were analyzed using descriptive statistics analysis and frequency analysis. Results : For the general process of evaluation in pediatric occupational therapy, major response on the initial evaluation time was 30~60minutes (41.9%), and major response on the re-evaluation period was 3~6months (41.0%). The major assessment tool for each area Canadian Occupation Performance Measure (55.2%) for occupational performance assessments, Wee Functional Independence Measure (57.1%) for activities of daily living assessments, School Function Assessment (2.5%) for education assessments, Knox Preschool Play Scale (28.6%) for play assessments, Developmental Test of Visual Perception (94.3%) for sensory-perceptual assessments, Bruininks-Oseretsky Test of Motor Proficiency (42.9%) for motor and praxis assessments, Evaluation of Social Interaction (6.1%) for social interaction skills assessments, Denver Developmental Screening Test (92.4%) for development assessments, Clinical Observation (89.5%) for physical examination assessments. Conclusion : The study identified the most assessment tools used for specific area by pediatric occupational therapists. The results can be used as a basic data to educate about pediatric occupational therapy evaluation, as well as to develop new assessment tools in pediatric setting in future.

Music Therapists' Perceptions on Professionalism (직업전문성에 대한 음악치료사의 인식)

  • This study investigated how music therapists perceive the professionalism of the profession. A total of 98 certified music therapists completed the online survey for a 46.7% response rate, and 78 questionnaires were included in the analysis after excluding incomplete responses. The results of this study demonstrated that music therapists show a high level of standards for music therapy professionalism in general. Meanwhile, the respondents gave their lowest rating on the perception of their occupation as a social service addressing the needs of society. The majority of respondents rated the level of their professionalism as high and they tended to identify individual competency as the most important factor. Also, there were significant differences in perceived professionalism depending on the length of clinical practice and level of education. These results indicate that the quality of training may contribute to a high level of perception on professionalism. Also, lower levels of perception on social aspects of the professionalism support the need for continuing education to broaden the understanding of professionalism. Given the importance of professionalism in enhancing the quality of care for clients and expanding the profession, further studies were suggested to increase understanding of the music therapy professionalism and propose strategies for maintaining the professionalism of music therapists.

Analysis of Psychological Counseling Requirement of Small Business Owners according to Psychological Exhaustion, Work-Family Conflicts, and Business-Related Anxiety (소상공인 창업자의 심리적 탈진, 일-가정 갈등, 사업운영 불안에 따른 상담요구 분석)

  • This study was carried out to analyze the effect of psychological burnout, work-family conflict, business-related anxiety on counseling requirement for small business owners. To achieve the research purpose, a survey was executed on small 100 business owners doing business in Chung-nam Province including whole and retail sellers, food and lodging establishments, service providers and manufacturers for five weeks from Jan. 1, 2016 through Jan. 312, 2016. All the questionnaires distributed were used for final analysis using SPSS V22. Study findings are as follows: first, most small business owners were found to have had counseling experience; second, no difference was shown in psychological burnout in terms of general characteristics of survey respondents; third, it is found that small business owners with shorter operation career want more counseling opportunity; fourth, it was analyzed that for any counseling service for small business owners, publicity efforts on the availability of counseling service must be precedented; fifth, it was also analyzed that to support small business owners counseling service should be provided in various areas including inter-psychological elements like emotion, conflict with spouse and children. Such findings may lead to a possible conclusion that counseling service can contribute to the success and survival of small business owners both at home and work place.

  • Vast volumes of studies of the medical and public health aspects of fertility and family planning have been published by the various institutes of health related research and university scholars in Korea. None of them, however, have dealed with the population problems associated with biologically and mentally handicapped people. It must be emphasized that the purpose of Korea's population policy should be to improve the quality of the population rather than to decrease its rate of increase. In this spirit, the first report of this study is to identify problems related with mentally and physically less fitted population, and to attempt to offer the possible solutions to the health planners and policy-makers. Several nation-wide surveys of the handicapped people in Korea have been compared. Each survey shows a wide range of difference in the prevalence of the handicaps(see Table 13). In this study, the data on the handicaps are collected by two independent system ; one by the nation-wide survey and the other by the reporting system existing at the Seoul National University Demonstration Health Project. The Chandrasekar-Deming technique was used to estimate the total number of handicaps. The estimates are summarized in the tables 8, 9 and 10. Estimate of total number of handicapped people in Korea is 601, 400 with the prevalence rate of 16.1 per thousand persons. Even if taking a number of conditions which may result in a biased estimate of the total number of the handicaps into consideration, the proportion of handicapped people in Korea has increased in the past two decade as the result of the rapid decline in fertility and childhood mortality, which consequently prolonged life expectancy of persons with congenital or acquired impediment. An increase in the proportion of handicapped people will eventually bring about serious problems of social welfare, medical care, and population qualities from various aspects including eugenics. To tackle the problem, there must be an increased emphasis on the prevention of handicaps from the government and private sector. Based on the amount and quality of data, and from the practical point of view, this study prepared a set of recommendations for the government to strengthen its programs of the preventive activities during the prenatal period and early childhood, early finding from routine examinations, and proper social and medical rehabilitation.

Follow-up study on Activities of Daily Living of the Elderly in Rural Community (농촌지역노인의 일상생활 동작능력에 관한 추적연구)

  • This study was conducted to find out the factors which have an effect on the rural aged's natural history by disability in Activities of Daily Living(ADL) and ADL decline with 475 old people which were chosen except the dead 90 and unconfirmable 56 old people by performing follow-up survey for 4 years since 1993 on the basis of presurveyed data about 621 home old people over 60 years residing in 13 villages at Kangdong-myon, Kyongju-city, Kyongsangbuk-do Province, Korea. Such activities of daily living as bathing, dressing, going to toilet, transfer, feeding and continence were examined. 1. In the follow-up survey for 4 years, the ADL distribution of the dead was remarkably lower than the subject group. 2. It was also shown in the 4-year follow-up survey that 82.1% of high ADL group maintained high ADL while 77.8% of low ADL group died within 4 years. 3. The occurrence percentage of disabilities of each group by ADL item for 4 years appeared high in such an order as bathing, continence, dressing, going to toilet, transfer and feeding. 6.7%(8.1% for female and 4.6% for male) of the old people who were the high ADL group at the time of the first survey had disabilities that occurred after 4 years. 4. In the change of ADL according to general characteristics used to analyse the factors which have an effect on ADL decline, there was a significant difference in age and job, that is, the jobless old people had the higher degree of ADL decline. In the change of ADL according to behavioral pattern, for the male old people there was a significant difference in support of living expenses and subjective health condition. In this case, the degree of ADL decline was higher if they depended on their sons & daughters or spouses and felt that they were not healthy.

Factors Influencing Korean International Adoptee's Search for Their Birthparents (국외입양인의 뿌리찾기에 영향을 미치는 요인)

  • This study examines the factors influencing Korean international adoptee's search for their birthparents. Considering that the search for birthparents is general needs for adoptees, Korean government should support their searching activities and, first of all, understand their characteristics. The research model was constructed based on the results of previous studies, and the data set of conducted by ministry of health and welfare was reanalyzed for this study. The subjects of the survey were Korean-born adoptees (who are more than 16 years old) in North America, Europe, and Australia. The research questionnaire was translated to English and French, and the survey was conducted on line. A total of 290 questionnaires were included in the analysis. Since survey was conducted on line, the missing rate of the data was relatively high. So, multiply imputed five data sets were used for analysis. Among the variables included in research model, the age group of adoptees, experience of identity crisis in their life, the first time when they became actively interested in Korean roots, the age at the time of adoption, and the attitudes of adoptive parents toward their search were significantly related to their search for birthparents. Adoptees in the age group of 30~34 had more actively participated in search compared to their reference group (which is the age group of more than 35 years old). The earlier they became actively interested in Korean roots, they tended to be more active in searching activities. Also, the experience of identity crisis in life and the age at the time of adoption were positively related to their search. Although most of adoptive parents have supported their search, the adoptees who reported that they didn't know their adoptive parents' attitude toward search, or their parents deceased had more actively participated in search for their birthparents. Some implications for adoption policy and practice were discussed based on the results of the study.

Relationship of Oral Health Status and Oral Health Care to the Quality of Life in Patients of Dental Hospitals and Clinics (치과내원 환자들의 구강건강상태와 구강건강관리 행태에 따른 삶의 질)

  • The purpose of this study was to examine the relationship of oral health status and oral health care to life. The subjects in this study were the patients who visited dental hospitals and clinics in several regions for seven months from March 2013. They got dental checkups, and a survey was conducted. The patients who received general treatment scored highest in Oral Health Impact Profile-14. Among the subfactors, the patients who received general treatment scored higher in functional limitation (8.27), physical pain (7.24), physical restrictions (7.90), social restrictions (7.79), physical limitations (8.14) and social difficulties (8.24). The patients who received orthodontic treatment scored higher in handicap (8.78). All the differences were statistically significant. As for gender gaps, the men scored higher in every factor. By age group, the older patients led a worse quality of life. By occupation, the homemakers mostly led a worse quality of life, and the patients who had no systemic diseases lived a better quality of life. All the differences were significant. Concerning self-rated health status and self-rated oral health state, the patients who found themselves to be in better health and in better oral health led a better quality of life. The differences were significant. Regarding oral health care, the patients who didn't get regular dental checkups and who could visit a dental clinic whenever they had a pain lived a better quality of life. The differences were significant. In terms of oral health status, a higher quality of life was found among the patients who had sound teeth, whose teeth received no sealant treatment, whose teeth received no filling therapy, who had no missing tooth, whose teeth received no prosthetic treatment and who had no dental implants teeth.

Analysis of Cancer Nursing Research in Digestive System in Korea (소화기계 암환자의 국내 간호연구 분석)

  • Purpose: This study was to analyze the research trend centering on the theses to cancer nursing research in digestive system released in Korea. Method: The researcher collected the academic degrees and theses published on the book of the academic soceity from January 1993 to August, 2004, and examined 38 domestic papers of cancer nursing research in digestive system. Results: 1) As for the subject, the results were : patients with stomach cancer 25(66%), colon cancer 4(11%), rectal cancer 3(8%), and others(taxi drivers 2, family of cancer patents. 2) As for the research designs the result were : quantitative studies were 33(87%), and qualitative studies were 9(23%). 3) As for key concepts of survey, the results were : life patterns of patients with rectal cancer, oral intake of stomach cancer patients, fatigue of stomach cancer patients undergoing chemotherapy, nursing needs when discharging from hospital after operation with gastrectomy, and so on. 4) As for the comparative studies, the results were : risk factors between colorectal patients and general population, early symptom and risk factors between stomach cancer patients and general population, and risk factors between stomach cancer and patients with gastritis. 5) As for main concepts of correlational studies, the results were : quality of life, health belief, fatigue, health promotion behavior, social support, straitanxiety. 6) The treatment of experimental research, the results were : information services, arc reflex massage, acupressure, educational program for discharge, 7)As for the qualitative studies, in terms of subjects, stomach cancer patients were 2, spouse of patients with stomach cancer was 1, rectal cancer patients were 2. In the theme of the qualitative studies, the results were: experience of family of patients with stomach cancer, experience of long term survival of patients with rectal cancer, experience of disease process of rectal cancer patiens. 8) As for the used instrument in studies, the results were : Strait-anxiety Scale by Spielberger, Nausea and Vomiting Scale by Rhodes, Social support by Tae and Lee, Health belief by Champion, Becker, and Moon. QOL by NCCN, Roh, Pdilla, Kwon, Revised Fatigue Scale by Piper, Health Locus of Control by Wallston and Wallston, Uncertainty Scale by Mishel. Conclusions: More research needs to be encouraged in various subject of cancer patients in digestive system. More nonexperimental and experimental researches should be conducted for the establishment of the basis of practical and theoretical framework and the providing good quality of care for cancer patients.

Brain Death and Heart Transplantation in Korea: A Questionnaire Survey (한국에서의 심장이식수술 - 설문조사를 중심으로 -)

  • Recent social and medical developments have significantly changed both the concept of death and the criteria for its pronouncement. Instead of considering. the heart as central to the determination for death, it is now prevalent to use death of the brain as adequate for death regardless of cardiac function in most western countries. But the brain death theory is not yet legally accepted despite growing public interest in our country due mainly to customary and moral reasons. Subsequently heart transplantation, which necessitates the concept of brain death, still remains a possible surgical entity in the future. As a part of endeavor the evaluate social atmosphere for the legal acceptance of the concept of brain death and the availability of potential heart donors, a four-page questionnaire on brain death and cardiac transplantation was given to the sophomore students of a medical college in Seoul[n=116, group I] and their family members[n=83, group II ]. The groups were chosen under the assumptions that they lacked sophisticated medical knowledge but had general medical interest so reliable data could be obtained. The majority of respondents in both group I and II thought that they knew the concept of brain death[group I 99.1%, group II 93.3%] and the definition of heart transplantation[group I 94%, group II 67.6%] at least to some extent, but only a small proportion of them was proven to have correct knowledge: brain death[74.4%, group II 39. 8%], heart transplantation[group I 31.9%, group II 30.1%]. Most respondents answered in the affirmative for the legal approval of brain death[group I 87.8%, group Il 97.9%]. The possibility of medical usage of brain dead organs was the biggest reason for brain death[group I 52.9%, group Il 47.9%]. Ninety-one percent of group I and 89.1 percent of group II responded that they were willing to give permission for donating the heart of brain-dead family members. Fifty-nine percent of group I and 51.9 percent of group II wanted their own heart donated. These results suggest, despite some inherent sampling limitations, that favorable responses to brain death and heart transplantation can be obtained among the general public if they are properly informed.

A Study on Job Stress of Container Termainal Workers (항만하역 근로자들의 직무 스트레스에 관한 연구)

  • Objectives: The objective of this study is to evaluate the job characteristics of container terminal workers by applying the Job Strain model, and to assess the relationship among the general characteristics, job characteristics and psychosocial distress. Methods: A self-administrated questionnaire survey was performed to the container terminal workers in Pusan. Among the 200 male workers who answered the questionnaires, white-collar workers and blue-collar workers were 100, respectively. Karaseks Job Content Questionnaire was utilized to evaluate the job characteristics and Psychosocial well-being index (PWI) was applied to measure the extent of their psychosocial stress. Results: In white-collar workers, the skill discretion, created skill, decision-making authority, decision-making latitude, psychological job demand, and supervisor support of the job characteristics were significantly high, while in blue-collar workers physical exertion was significantly high. The result of Psychosocial well-being index (PWI) reveals that blue-collar workers were more stressed than white-collar workers, especially, the indices of depression (factor 2), sleeping disturbance and anxiety (factor 3), General well-being and vitality (factor 4) were significantly increased; whereas, in white-collar workers, only the index of social performance and self-confidence (factor 1) was significantly increased. And PWI scores were significantly increased in the lower social support and psychological job demand. By the multiple logistic regression analysis for PWI, blue-collar workers had increased odds ratio of 2.66(95% CI;1.11-6.41) compared with white-collar workers. The unmarried workers increased odds ratio of 3.54(95% CI;1.18-10.62) compared with married workers. And workers who have not own house increased odds ratio of 2.35(95% CI;1.15-4.79) compared with workers who have own house. Particularly, odds ratio of work-shift in blue-collar workers was 11.10(2.14-57.64). Conclusion: Skill discretion, created skill, decision-making authority, decision-making latitude, psychological job demand, and supervisor support were increased in white-collar workers. Decreased skill discretion and increased physical exertion were found in blue-collar workers, which is supported the Job Strain model. Job stress of blue-collar workers was comparatively higher than that of white-collar workers, especially, skill discretion, decision-making authority, decision-making latitude, job insecurity, physical exertion were noticeable factors. Especially, sleeping, smoking, and work shifting turned out to be a main cause that increases stress. Therefore, in order to decrease the job stress, a health promotion program to change the health behaviors should be activated and an organized job stress management program should be introduced. Especially, working condition for blue-collar such as physical exertion and work-shift should be improved.
