Vascularisation of Urethral Repairs with the Gracilis Muscle Flap

  • Background The ability to achieve a long-term, stricture-free urethral repair is one of the ongoing challenges of reconstructive urologic surgery. A successful initial repair is critical, as repeat procedures are difficult, owing to distortion, scarring, and short urethral stumps. Methods We describe a technique in which the gracilis muscle flap is laid on or wrapped around the urethral repair site to provide a well-vascularised soft tissue reinforcement for urethral repair. This technique promotes vascular induction, whereby a new blood supply is introduced to the repair site to improve the outcome of urethral repair or anastomotic urethroplasty. The surface contact between the muscle flap and the repair site is enhanced by the use of fibrin glue to improve adherence and promote inosculation and healing. We employed this technique in 4 patients with different urethral defects. Results After a follow-up period of 32 to 108 months, all of the urethral repairs were successful without complications. Conclusions Our results suggest that the use of a gracilis muscle flap to vascularise urethral repairs can improve the outcome of challenging urethral repairs.

Primary repair of symptomatic neonates with tetralogy of Fallot with or without pulmonary atresia

  • Recently, surgical outcomes of repair of tetralogy of Fallot (TOF) have improved. For patients with TOF older than 3 months, primary repair has been advocated regardless of symptoms. However, a surgical approach to symptomatic TOF in neonates or very young infants remains elusive. Traditionally, there have been two surgical options for these patients: primary repair versus an initial aortopulmonary shunt followed by repair. Early primary repair provides several advantages, including avoidance of shunt-related complications, early relief of hypoxia, promotion of normal lung development, avoidance of ventricular hypertrophy and fibrosis, and psychological comfort to the family. Because of advances in cardiopulmonary bypass techniques and accumulated experience in neonatal cardiac surgery, primary repair in neonates with TOF has been performed with excellent early outcomes (early mortality<5%), which may be superior to the outcomes of aortopulmonary shunting. A remaining question regarding surgical options is whether shunts can preserve the pulmonary valve annulus for TOF neonates with pulmonary stenosis. Symptomatic neonates and older infants have different anatomies of right ventricular outflow tract (RVOT) obstructions, which in neonates are nearly always caused by a hypoplastic pulmonary valve annulus instead of infundibular obstruction. Therefore, a shunt is less likely to preserve the pulmonary valve annulus than is primary repair. Primary repair of TOF can be performed safely in most symptomatic neonates. Patients who have had primary repair should be closely followed up to evaluate the RVOT pathology and right ventricular function.

Cone Repair in Adult Patients with Ebstein Anomaly

  • Ebstein anomaly is a rare congenital heart malformation typically involving the tricuspid valve and the right ventricle that has a wide range of anatomical and pathophysiological presentations. Various surgical repair techniques for Ebstein anomaly have been reported because of its near-infinite anatomical variability. Cone repair for Ebstein anomaly can achieve nearly anatomical reconstruction of the tricuspid valve with promising outcomes. In this article, the surgical techniques for cone repair in adult patients with Ebstein anomaly are described in detail, and clinical experiences and technically challenging cases are presented.

Preventive Replacement Policy with the Number of General Repairs (일반수리회수에 의한 장비 교환 정책)

  • This paper presents a model for determining the optimal number of general repairs and supplementary input cost limit rate in addition to minimal repair cost rate to implement preventive maintenance. The basic concept parallels the periodic replacement model with minimal repair at failure introduced by Barlow and Hunter(1960) and Park(1979), only difference being the replacement signalled by the number of previous general repairs performed on the system. A general repair brings the state of the system to a certain better state than before repaired. Numerical examples are provided.

Repair of Achilles tendon tear with distal wire pulling (강선 견인을 이용한 아킬레스 건 파열의 수술적 치료)

  • Purpose: This study was designed to evaluate whether the method "Distal pulling with wire" after primary repair of Achilles tendon, can result in early restoration of ankle motion. Materials and Methods: In 14 cases of tendocalcaneus repair, the proximal stump was pulled down distally by wire and fixed the ends of wire to the transcalcaneal K - wire. We evaluated the range of motion of ankle joint at 4th, 6th, 12th week in 10 patients whom we followed up for more than 1 year. We evaluated the results of Achilles tendon repair by Hooker's criteria. Results: Range of motion of ankle joint revealed as follows. Degree of mean dorsiflexion improved - $5.1^{\circ},\;15.0^{\circ},\;22.4^{\circ}$ at 4th, 6th and 12th week respectively, while plantarflexion improved $21.5^{\circ},\;32.7^{\circ}$ and $42.3^{\circ}$ respectively. At one year follow up, seven of them had no problems in active daily life and sports activities. According to Hooker's criteria, the result rated excellent in nine, satisfactory in one. Conclusion: Early gain of ankle joint motion was possible by "Distal pulling with wire" after primary repair of Achilles tendon.

  • The purpose of this study is to examine the circumstances of repair works after enacting "Low for Conservation of Joseon Treasure, Historic Site, Scenic Beauty & Natural Monument (朝鮮寶物古跡名勝天然記念物保存令)". For this purpose, the official documents produced by the Government-general Museum of Joseon were analyzed. They included the contents of repair works of Hwaumsa Kakhwangjeon(華嚴寺 覺皇 殿). As a result, this study reveals that the repair work of Hwaumsa Kakhwangjeon had become a momentum of introducing specific guidelines such as "Guidelines for Architectural Treasure's Repair 寶物建造物修理施行準則", "Regulations for administration of Hwaumsa Kakhwangjeon's repair work 華嚴寺覺皇殿修理工事取扱手續)". The guidelines and regulations systematized the planning phase of the repair works and provided to organize the field office formed by experts. In spite of these positive changes, the repair works were controlled by the central government to the certain extent, and the Japanese repair techniques were mingled with traditional techniques recklessly.

  • The compatibility of concrete repairs is proposed by D. Cusson et al. and N. K. Emberson et al. But this is the general compatibility of concrete repairs. This study is actualized the general compatibility of concrete repairs on the flexural specimen under bending load. This study is obtained following results. 1) As a results of analysis for repair effects on failure shape, debonding between concrete and repairs, yielding load and ultimate/yielding ratio, the repair effects is ascertained that the repair R3 is much excellent than the repair R7, but on the other hand R7 is very high than R3 on the viewpoint of compressive strength, where repair R7, R3 is a product. 2) Therefore the compatibility of concrete repair proposed by D. Cusson et al. and N. K. Emberson et al. must be reanalyzed for structures types of column, beam, wall, slab et al.

  • Reinforced concrete structure is deteriorated, as time goes on. So many repair materials are developed for the repair. But repair materials have not been adequately applied so far. Because the datum which evaluated the repair materials are not sufficient. The object of this study is estimation f repair materials that is in general use and establish method of application. To acquire the result, we have made experiments on chemical attack, carbonation and chloride permeability test. The carbonation and chloride permeability are very different. Some repair materials are poorer than portland cement mortar.

On Optimal Replacement Policy for a Generalized Model (일반화된 모델에 대한 최적 교체정책에 관한 연구)

  • In this paper, the properties on the optimal replacement policies for the general failure model are developed. In the general failure model, two types of system failures may occur : one is Type I failure (minor failure) which can be removed by a minimal repair and the other, Type II failure (catastrophic failure) which can be removed only by complete repair. It is assumed that, when the unit fails, Type I failure occurs with probability 1-p and Type II failure occurs with probability p, $0\leqp\leq1$. Under the model, the system is minimally repaired for each Type I failure, and it is repaired completely at the time of the Type II failure or at its age T, whichever occurs first. We further assume that the repair times are non-negligible. It is assumed that the minimal repair times in a renewal cycle consist of a strictly increasing geometric process. Under this model, we study the properties on the optimal replacement policy minimizing the long-run average cost per unit time.

  • Today, apartment houses account for a very high proportion of the types of residence. This study analyzed the difference between the bidding construction cost using the long-term repair allowance and using the general repair cost, and analyzed the difference of factors affecting the amount of use of these two items. For this purpose, related data on apartment housing complexes located in seven representative districts of Busan from 2017 to 2021 were collected and analyzed from the apartment housing management information system. As a result of statistical analysis, the type of bidding variable had the greatest negative(-) effect on the bidding construction cost, and the next factor that affected was different between the amount of long-term repair allowance used and general repair cost used. In addition, there was a difference in statistical significance in the type of bidding by construction year for the number of bidding works between the case of using the long-term repair allowance and the case of using the general repair cost. The results of this analysis will help to find ways to reduce bidding construction costs. However, it is expected to find additional meaning by including more detailed variables such as technical ability.
