Gastric salvage after venous congestion during major pancreatic resections: A series of three cases

  • Pancreatic resections, depending on the location of the tumor, usually require division of the vasculature of either the distal or proximal part of the stomach. In certain situations, such as total pancreatectomy and/or with splenic vein occlusion, viability of the stomach may be threatened due to inadequate venous drainage. We discuss three cases of complex pancreatic surgeries performed for carcinoma of the pancreas at a tertiary care center in India, wherein the stomach was salvaged by reimplanting the veins in two patients and preserving the only draining collateral in one case after the gastric venous drainage was compromised. The perioperative and postoperative course in these patients and the complications were analyzed. None of these 3 patients developed any complication related to gastric venous congestion, and additional gastrectomy was avoided in all these patients. Re-establishment of the Gastric venous outflow after extensive pancreatic resections helps to avoid additional gastric resection secondary to venous congestive changes.

  • We examined the stomaches of slaughtered pigs to evaluate the gastric lesions. Gastric ulcer accompanying parakeratosis and hyperkeratosis were most frequently observed in the mucosa layer of pars oesophagea. Diffuse congestion and focal necrosis was also frequently seen on the fundus. Pierce wounds by toothpick were seen from 3 stomaches.

Effect of Dioscorea Rhizoma on Gastritis by Acute Gastric Ulcer in Rats (산약이 흰쥐의 급성 위궤양에 의한 위장 장애에 미치는 효과)

  • In order to verify the effects of Dioscorea Rhizoma on gastrointestinal damages, we investigated the protective role of the crude extracts on induced gastric ulceration in rats. Rats administered of Dioscorea Rhizoma extracts showed gradually decreased congestion and hemorrhage, but control group did not show any symptom in gastric tissue. Moreover, Dioscorea Rhizoma extract had a role in lowering gastrin and histamine levels in gastric ulcer rats, thereby inhibiting the gastric tissue damages. Excessive production of malondialdehyde shown in gastric ulcer rats was declined in all rats administered with Dioscorea Rhizoma extract as well as the levels of SOD and GPX surged by acute gastric ulcer. Also, the increased activity of CAT showed an effect in activation of antioxidant enzyme to normal state. All data suggest that Dioscorea Rhizoma extract was verified to be highly effective resource in improving the gastrointestinal function of rats by preventing from gastric tissue damage in acute gastric ulcer and restoring the activities of plasma substances and antioxidant enzymes.

  • Portal hypertensive gastropathy (PHG) is part of a complex syndrome which occurs as a complication of chronic liver disease and portal hypertension. The gastric mucosa in these patients shows typical congestion of ‘mosaic-like’ pattern and vulnerable to various noxious agents such as NSAIDs and ethanol. We previously reported that DA-9601, a quality-controlled extract from Artemisia asiatica, exhibits cytoprotection against various gastritis models. In the present study we investigated the effect of DA-9601 on ethanol-induced gastric damage in PHG rats. Experimental PHG was produced by CBD ligation in SD rats. DA-9601 was orally administered at a dose of 30 mg/kg or 100 mg/kg daily for 2 weeks.

Protective Effect of Rhizoma Dioscoreae on the Gastric Mucosal Lesions Induced by Alcohol in Rats (산약이 알콜로 유도된 흰쥐의 위 점막 손상 예방에 미치는 영향)

  • The aim of this study was carried out to investigate the protective effect of Rhizoma Dioscoreae on the gastric mucosal lesions induced by alcohol in rats. Experimental groups were treated by oral infusion with Rhizoma Dioscoreae extract at the dose of 0.03465 g/ml(OA-RD1 group), 0.0693 g/ml(OA-RD2 group), and 0.1386 g/ml(OA-RD3 group), while D.W group was administrated with the distilled water and control group did not pretreated. Experimental groups pretreated for 14 days, and given orally 1 ml of 75% alcohol two times(30min interval). The animals were killed 1hr 30min after alcohol treatment, and measured rats body weight, absolute stomach weight, relative stomach weight, SOD activity, glutathione peroxidase(GPx) activity, observed gastric mucosal lesions. The body weight was unremarkable changed. In once as dose intake group's absolute stomach weight was increased and In once as dose intake group, twice as dose intake group's relative stomach weight was increased. SOD activity, glutathione peroxidase activity in twice as dose intake group is remarkably increased. Light microscopy Observations of congestion, hemorrhage, and erosion in gastric mucosal lesions were shown severely in control group than OA-RD1, OA-RD2, OA-RD3. These results suggest that the proposed gastroprotective effect may involve activation of antioxidant effect. And Twice as dose is especially effective.

Protective Effects of Crude Mucin and Saponin from Dioscorea Rhizoma on Gastric Ulcer Induced by Alcohol in Rats (산약의 조추출 mucin과 saponin이 급성 위궤양이 유도된 흰쥐에 미치는 보호 효과)

  • The aim of this study was to investigate the effect of crude mucin and saponin from Dioscorea Rhizoma on acute gastric ulcers in rats. The gastric ulcer group (GU group) and mucin-applied group (DR-M group) exhibited serious bleeding of the mucous membrane of the stomach due to the ulcers, as well as blood congestion for three days. The saponin-applied group (DR-S group) exhibited less mucous membrane bleeding, and reddened and inflamed membranes recovered dramatically within 24 hours. After developing an acute pgastric ulcer, the tissues of the stomach, intestine, and liver in the control group and the DR-M group exhibited edema in the submucous membrane, as well as serious bleeding. However, the DR-S group recovered quickly from mucous membrane bleeding due to gastric ulcer. The DR-M group did not show any notable changes in serum formation or activity of antioxidant enzymes compared to the GU rats. Increased AST and ALT activities were detected from the first day with saponin application in the gastric ulcer rats. As the AST and ALT activities decreased, the gastric ulcers recovered with the increased activities of the antioxidant enzymes. Accordingly, this study suggest that mucin in Dioscorea Rhizomahas no effect on the recovery of damaged stomachs due to gastric ulcers, but saponin is mainly responsible for decreasing tissue damage by activating antioxidant enzymes.

Effects of Fermented Rice Wine Using Mycelium of Phellinus linteus on the Gastric Mucosa of Rat (상황버섯 균사체를 이용한 발효주가 흰쥐의 위점막에 미치는 영향)

  • It was examined the effect of fermented rice wine using mycelium of Phellinus linteus (FWPL) on the gastric mucosa of rat. The gastric mucosal lesions were not seen macroscopically in normal, but ethanol-administrated rats produced congestion and edema with a few local lesions. The administration of FWPL showed a similar pattern as like normal except trace histopathological changes. The results of Western blot analyses showed that the higher expression of inducible nitric oxygenase (iNOS), cyclooxygenase (COX)-1, COX-2, tumor necrosis factor-$\alpha$ and c-fos, especially COX-2, in the ethanol-administrated rat compared with normal rat. But FWPL-administrated rat showed a trace increase of these expression compared to normal rat. About immunohistochemical observations, weaker iNOS reactions were detected in mucous cells of epithelim of ethanol administrated rat compared with normal and FWPL-administrated rat. These results suggested that FWPL-administrated rat showed a trace changes on the mucus barrier-related protein expression compared with ethanol-administrated rat and thus FWPL may be use to develop a functional alcoholic beverage.

The Effect of Chronic Alcohol Administration to Alteration of Liver, Kidney and Stomach in Mouse (장기 알콜투여가 생쥐의 간 및 신장, 위장조직 손상에 미치는 영향)

  • Alcohol is a major risk factor for several diseases and excessive, long-term alcohol consumption are caues physical alteration-fatty liver, hepatitis, cirrhosis, breaking down, Wernicke-karsakoff's syndrome, weight loss, and poor immunity-in virtually all organ and tissue. This study was observed that liver, kidney, and stomach were altered in mouse by the effect of chronic alcohol administration. The mouse were sacrificed to obtain the tissue after mouse were orally injected with 25 % ethanol $18m{\ell}/kg/day$ for 120days. The tissue were stained by hematoxylin and eosin and then observed by light microscope. The results of this study were as follows : 1. The congestion was appeared in liver after 120days alcohol admistration. 2. The destruction of glomerulus were increased and the parietal cell of Bowman's capsule were swelled such as cuboidal cell after 120days alcohol administration. The congestion was appeared in alcohol administrated group. 3. The mucosa and gastric pit were destructed and the ulceration was appeared in stomach after 120days administration. The parietal cells and chief cells were damaged. Above results were shown that the tissue were damaged by chronic alcohol administration.

GET Imaging Evaluation of Patients with Esophageal Cancer (식도암 환자의 GET 영상 평가)

  • Purpose: Measure gastric emptying time (GET: Gastric Emptying Time) is a non-invasive and quantitative evaluation methods, mainly by endoscopic or radiological examination confirmed no mechanical obstruction in patients with symptoms of congestion is checked. Such tests are not common gastric emptying time measured esophageal cancer patients (who underwent esophagectomy) patients after surgery for gastric emptying time was measured test. And the period of time for more than one year after the gastric emptying time measurement was performed. By comparing the two kinds of tests in the chest cavity after surgery as the evaluation of gastrointestinal function tests evaluate the usefulness of GET, and will evaluate the characteristics of the image. Materials and Methods: 93 patients who underwent esophagectomy with gastric emptying time measurement of subject tests immediately after surgery and after 1 year or longer were twice. Preparation of the patient before the test is more than 12 hours of overnight fasting is important, in addition to the medicine or to stop smoking, and diabetes insulin injections should be early in the morning is ideal to test. Generally labeled with $^{99m}Tc-DTPA$ resin which is used to make steamed egg, seaweed and fermented milk with a high viscosity after eating, three hours in the standing position was measured. Evaluation of gastric emptying curves on the way intragastric radioactivity level by 50% the time (half-time [T1/2]) was calculated, based on the half-life was divided into three steps: over 180 minutes was defined as delayed gastric emptying, within 180minutes was defined as intermediate gastric emptying and when all the radioisotopes were dumped into the jejunum as soon as swallowed, was defined as rapid gastric emptying. Results: Gastric emptying time of a typical images stomach of antrum and fundus additional images appear stronger over time move on to the small intestine. but esophageal cancer who underwent esophagectomy side of the thoracic cavity showed a strong image. Immediately after surgery, the half-time (T1/2) of rapid gastric emptying appeared to 12.9%, intermediate gastric emptying appeared to 52.7%, delay gastric emptying appeared to 34.4%. After more than a year the results of the half-life after surgery, 67% of rapid gastric emptying to intermediate gastric emptying was changed, 69% of delay gastric emptying to intermediate gastric emptying changed. Intermediate gastric emptying worse in patients rapid gastric emptying and the delay gastric emptying is 24% in the case. Conclusion: Esophagectomy for esophageal cancer who underwent half-time measurement test (T1/2) rapid gastric emptying and delay gastric emptying are the result of the comparison over time, changes were observed intermediate gastric emptying. Mainly seeing of gastric emptying time measurement in the esophagus instead of thoracic cavity to check the evaluation of gastrointestinal function can be useful even means. And segmentation criteria and narrow time interval of checking if more accurate information and analysis of the clinical diagnosis and evaluation seems to be done.

Ulcer Healing Effects of Vitamin E on Chronic Gastric Ulcer Induced by Alcohol in Young Adult Rats (알코올로 유도한 만성위궤양 흰쥐 모델에서 비타민 E 보충이 위궤양 치유에 미치는 영향)

  • This study was carried out to examine the effects of vitamin E on chronic gastric ulcer induced by alcohol treatment in rats. Chronic gastric ulcer model was established by oral administration of 70% ethanol at one time and supply of 15% ethanol for additional 7 days. Male Sprague-Dawley rats, approximately 200 g, were fasted for 24 hours and orally gavaged with 1 mL of 70% ethanol for the induction of acute ulcer. A supply of 15% ethanol dissolved in distilled water for 7 days were followed to maintain chronic gastric ulcer. Acute ulcer group was sacrificed at 3 hours after oral administration of 1 mL of 70% ethanol. Chronic groups were divided into three groups according to vitamin E levels; low-vitamin E (LVE, 0 mg/mL oil/day), normalvitamin E (NVE, 1 mg/mL oil/day) and high-vitamin E (HVE, 10 mg/mL oil/day). These groups were fed vitamin E free diets which were made of vitamin E free vitamin mix followed AIN-93M pattern for 7 days. Histological findings of congestion, hemorrhage and necrosis in gastric tissue were shown severely in acute ulcer group and LVE group of chronic ulcer groups. The concentration of gastrin in serum was significantly higher in LVE group. The content of histamine in stomach was lower in acute ulcer group but there was no significant difference among the chronic groups regardless of vitamin E levels. Content of malondialdehyde (MDA) in gastric tissue was higher in HVE group and activities of antioxidant enzyme, glutathione peroxidase (GPx) and catalase, were lower in HVE group. Myeloperoxidase (MPO) activities as a marker of neutrophils infiltration was significantly higher in LVE group. These results suggested that vitamin E supplementation has positive effects on healing of alcohol-induced chronic gastric ulcer through alleviation of gastric tissue injuries and reduction of the MPO activity in gastric tissue and gastrin in serum.
