단순 모델을 사용한 추상복합 건물의 효율적인 전이 시스템에 관한 연구 (Evaluation of an Effective Load Transfer System Applied to a Simple Model of a Wall Frame Structural System)

  • 상부 벽식, 하부 골조로 이루어진 주상복합 건물은 주어진 대지 내에 여러 기능을 수용하기 위한 건물의 구조로서 대도시에서는 광범위하게 사용되고 있다. 그러나 이러한 전이보 시스템은 구조시스템의 수직적 불연속성으로 인하여 많은 문제점을 가지고 있다. 본 연구에서는 현재 일반적으로 주상복합건물에 사용중인 춤이 깊은 전이보를 가진 구조시스템을 조사하고 아치 시스템으로 대체하기 위하여 기존의 전이보를 사용하였을 경우 발생하는 아치거동의 형태를 규정하고 이를 아치의 형태로 나타내어 전이보 시스템을 대체하였다. 전이보 시스템을 대체한 보다 효율적인 아치 시스템에 대한 여러 가지 고찰을 통해 아치 시스템의 적용성 여부를 판단하고, 이 시스템과 기존의 전이보 시스템을 중력방향 하중과 횡 방향 하중에 대하여 비교하였다. 전이보 시스템과 아치 시스템을 중력방향 하중과 횡 방향 하중에 대해 비교한 결과, 전이보 시스템이 가지고 있는 구조적인 문제점을 많이 감소시킬 수 있었다. 전이보 시스템을 대체할 아치 시스템은 아치 작용으로 인해 발생하는 전이보의 전단 거동에 대한 불확실성, 전이보 자체의 큰 강성으로 인하여 기둥 부분에 발생하는 추가 모멘트 벽 하단부에 발생하는 전단 응력 집중 현상 및 벽 하부 중앙과 전이보 중앙부의 비효율성 등과 같은 문제점을 감소시킬 수 있는 효율적인 시스템으로 판단된다.

  • This clinical case highlights the failure of long length implants, and the prosthodontic procedures necessary to rehabilitate the maxillary dentition of a patient with microstomia. The integrated digital technology of intra-oral scanning, computer-aided design, and three-dimensional printing can provide an alternative method to make conventional impressions for patients with microstomia who cannot insert the appropriate tray in their mouths.

청년 전악X선사진에 있어서 매복치 및 과잉치에 관한 연구 (THE STUDY OF IMPACTED AND SUPERNUMERARY TEETH IN YOUNG MAN BY THE FULL MOUTH ROENTGENOGRAMS)

  • The author has observed 906 full mouth roentgenograms in Korean young man who had the impacted and supernumerary teeth. Impacted and supernumerary teeth were analysed upon several items, such as incidences and positions in the dental arch. The result were as follows; 1. The incidences of impacted teeth were 59.05% and the ratio of upper jaw to lower one was 1:3. 2. The frequency of the supernumerary teeth were 5.52% and the ratio of maxilla to mandible was 49:1. 3. No significant differences between right and left side in both jaws, but incidence rate of the supernumerary teeth was 74% in the maxillary central incisor area. 4. The positions of impacted and supernumerary teeth were vertical and inverted shapes in maxilla, whereas horizontal and mesio-angular forms in mandible.

Full Arch Restoration through Orthognathic Surgery after Implantation on the Patients with Mandibular Prognathism and Loss of Posterior Teeth: A Case Report

  • In case of loss of many teeth due to dental caries or periodontal disease, improvement of masticatory function and aesthetics can be obtained through implant treatment. However, if the patient does not have a normal intercondylar relationship, it is difficult to achieve an ideal occlusal relationship with only prosthetic treatment. In particular, oral reconstruction with orthodontic treatment or orthognathic surgery is necessary for patients with mandibular prognathism. However, if the posterior occlusion collapses due to severe caries or periodontal disease, orthognathic surgery may be difficult. The occlusal vertical stop is very important for the stability of the mandibular position during occlusal reconstruction through orthognathic surgery. The patient in this case had posterior occlusion collapsed due to the caries of a large number of posterior teeth, and showed mandibular prognathism and long face. We planned a full arch restoration with orthognathic surgery and extracted the hopeless teeth. To secure the vertical stop required for orthognathic surgery, the implant was placed before surgery. After the orthognathic surgery and the final prosthesis application, the results were satisfactory for the improvement of the aesthetics and the restoration of the masticatory function.

치과용 모형재 색상에 따른 디지털 모형의 체적 안정성 연구 (The study on the dimensional stability of digitized dental stone replicas according to difference color of gypsum materials)

  • Purpose: The aim of study was to compare the dimensional stability of digitized dental stone replica using different color of gypsum materials using a white light scanner with three-dimensional software. Methods: A master model(500B-1, Nissin dental product, Japan) with the prepared lower full arch tooth was used. Several type IV stones(white, yellow, green) were used for 30 stone casts(10 casts each) duplicated a master model of mandible. The master model and the replicas were digitized with the non-contacting white light scanner to create 3-dimensional digital models. The linear distance between the reference points were measured and analyzed on the Delcam Copycad$^{(R)}$(Delcam plc, UK) 3D graphic software. One-way analysis of variance(ANOVA) combined with a Tukey multiple-range test were used to analysis the data(${\alpha}$=0.05). Results: There were considerable differences in mean values between gypsum materials within each color(white, yellow, green), and this difference was statistically significant, p=0.001. Conclusion: Digitization of dental materials on optical scanner was affected by color. Three different color of gypsum materials showed clinically acceptable accuracies of full arch digital model produced by them. Besides, these results will have to be confirmed in further clinical studies.

동영상 촬영방식의 구강스캐너로 채득된 전악치열 디지털모형의 정확도 분석 : CS-3600 시스템을 중심으로 (Accuracy of full arch digital model obtained from rendering-based intraoral scanner(IOS) : An example of CS-3600 system)

  • Purpose: The purpose of this study was to evaluate and compare the accuracy of definitive casts that are fabricated from digital intraoral impression and conventional impression technique. Methods: A master model(ANNA-4, Frasaco GmbH, Tettnang, Germany) with the prepared upper full arch tooth was used. Conventional impression and then stone model(n=10) were produced from this master model, and on the other hands, digital impressions were made with the CS-3600 intraoral scanner(n=10). Six linear measurements were recorded between landmarks, directly on each of the stone models on two occasions by a single examiner. Measurements were made with a digital caliper to the nearest 0.01mm from manual models and with the software(Delcam PowerSHAPE) from the virtual models. The t-student test for paired samples and intraclass correlation coefficient(ICC) were used for statistical analysis. Results: The measurement of two methods showed good reliability. The ICC of the two models were 0.88~0.91(stone model) and 0.94~0.99(digital model). The mean differences to master model for stone model and digital model were 0.10~0.14mm, and 0.14~0.20mm, respectively. Conclusion: The definitive casts obtained with digital intraoral technique model had significantly larger dimensions as compared to those of the stone model. However, the differences to the master model detected appear to provide enough accuracy and reliability for clinical application.

Biomechanical behavior of CAD/CAM cobalt-chromium and zirconia full-arch fixed prostheses

  • PURPOSE. To verify the influence of computer-aided design/computer-aided manufacturing (CAD/CAM) implant-supported prostheses manufactured with cobalt-chromium (Co-Cr) and zirconia (Zr), and whether ceramic application, spark erosion, and simulation of masticatory cycles modify biomechanical parameters (marginal fit, screw-loosening torque, and strain) on the implant-supported system. MATERIALS AND METHODS. Ten full-arch fixed frameworks were manufactured by a CAD/CAM milling system with Co-Cr and Zr (n=5/group). The marginal fit between the abutment and frameworks was measured as stated by single-screw test. Screw-loosening torque evaluated screw stability, and strain analysis was explored on the implant-supported system. All analyses were performed at 3 distinct times: after framework manufacturing; after ceramic application in both materials' frameworks; and after the spark erosion in Co-Cr frameworks. Afterward, stability analysis was re-evaluated after 106 mechanical cycles (2 Hz/150-N) for both materials. Statistical analyses were performed by Kruskal-Wallis and Dunn tests (α=.05). RESULTS. No difference between the two materials was found for marginal fit, screwloosening torque, and strain after framework manufacturing (P>.05). Ceramic application did not affect the variables (P>.05). Spark erosion optimized marginal fit and strain medians for Co-Cr frameworks (P<.05). Screw-loosening torque was significantly reduced by masticatory simulation (P<.05) regardless of the framework materials. CONCLUSION. Co-Cr and Zr frameworks presented similar biomechanical behavior. Ceramic application had no effect on the biomechanical behavior of either material. Spark erosion was an effective technique to improve Co-Cr biomechanical behavior on the implant-supported system. Screw-loosening torque was reduced for both materials after masticatory simulation.

Full-arch accuracy of five intraoral scanners: In vivo analysis of trueness and precision

  • Objective: To evaluate the trueness and precision of full-arch scans acquired using five intraoral scanners and investigate the factors associated with the dimensional accuracy of the intraoral scan data. Methods: Nine adult participants (mean age, 34.3 ± 8.3 years) were recruited. Four zirconium spheres (Ø 6 mm) were bonded to the canines and the molars. Following acquisition of reference scans using an industrial-grade scanner, five intraoral scanners, namely i500, CS3600, Trios 3, iTero, and CEREC Omnicam, were used to scan the arches. Linear distances between the four reference spheres were automatically calculated, and linear mixed model analysis was performed to compare the trueness and precision of the intraoral scan data among the different scanners. Results: The absolute mean trueness and precision values for all intraoral scanners were 76.6 ± 79.3 and 56.6 ± 52.4 ㎛, respectively. The type of scanner and the measured linear distances had significant effects on the accuracy of the intraoral scan data. With regard to trueness, errors in the intermolar dimension and the distance from the canine to the contralateral molar were greater with Omnicam than with the other scanners. With regard to precision, the error in the linear distance from the canine to the molar in the same quadrant was greater with Omnicam and CS3600 than with the other scanners. Conclusions: The dimensional accuracy of intraoral scan data may differ significantly according to the type of scanner, with the amount of error in terms of trueness being clinically significant.

가상 골격-치열 하이브리드 이미지 생성을 위한 구강 스캐너의 활용 (Intraoral Scan for Virtual Skull-Dentition Hybrid Images of Young Patients)

  • 콘빔 전산화 단층촬영(CBCT)은 치열을 왜곡시켜 추가 치열 이미지가 필요하다. 치열 이미지로 주로 사용되는 석고 모형을 대신해 구강 스캐너로 CBCT의 치열 이미지를 보완할 때의 임상적 활용 가능성을 평가하였다. 만 12 - 18세의 20명에게 상악에 대한 석고 모형, 구강 스캔 이미지, CBCT이미지를 획득하였다. 비교를 위해 두 종류의 구강 스캐너를 이용하였는데, 그 중 하나의 구강 스캐너로는 전악을 3분할하여 각각을 스캔한 후 이들을 병합해 전악 이미지를 획득하는 방법을 추가로 시행하였다. 구강 스캐너를 이용해 얻은 가상 골격-치열 하이브리드 이미지를 석고 모형을 통해 얻은 이미지와 중첩하여 각 기준점에서의 좌표 값의 차이와 거리를 측정하였다. 결과적으로 구강 스캐너로 분할하여 스캔하는 방법을 시행했을 때 평균 거리 2 ㎛를 보여 가장 적게 나타났다. 구강 스캐너를 적절히 이용하면 가상 골격-치열 하이브리드 이미지를 위한 치열 이미지로 훌륭히 활용될 수 있을 것이다.

Comparison of 3D accuracy of three different digital intraoral scanners in full-arch implant impressions

  • PURPOSE. This in vitro study aimed to evaluate the performance of digital intraoral scanners in a completely edentulous patient with angled and parallel implants. MATERIALS AND METHODS. A total of 6 implants were placed at angulations of 0°, 5°, 0°, 0°, 15°, and 0° in regions #36, #34, #32, #42, #44, and #46, respectively, in a completely edentulous mandibular polyurethane model. Then, the study model created by connecting a scan body on the implants was scanned using a model scanner, and a 3D reference model was obtained. Three different intraoral scanners were used for digital impressions (PS group, TR group, and CS group, n = 10 in each group). The distances and angles between the scan bodies in these measurement groups were measured. RESULTS. While the Primescan (PS) impression group had the highest accuracy with 38 ㎛, the values of 104 ㎛ and 171 ㎛ were obtained with Trios 4 IOSs (TR) and Carestream 3600 (CS), respectively (P = .001). The CS scanner constituted the impression group with the highest deviation in terms of accuracy. In terms of dimensional differences in the angle parameter, a statistically significant difference was revealed among the mean deviation angle values according to the scanners (P < .001). While the lowest angular deviation was obtained with the PS impression group with 0.185°, the values of 0.499° and 1.250° were obtained with TR and CS, respectively. No statistically significant difference was detected among the impression groups in terms of precision values (P > .05). CONCLUSION. A statistically significant difference was found among the three digital impression groups upon comparing the impression accuracy. Implant angulation affected the impression accuracy of the digital impression groups. The most accurate impressions in terms of both distance and angle deviation were obtained with the PS impression group.
