Purpose: To identify age, gender, medication, seasons and place of fall, and areas of the fractures from the fall among the hospitalized elderly patients in order to provide the basic data for future fall prevention program for the elderly. Methods: This study was conducted for 106 elderly patients admitted into a university hospital by fractures from the fall during the period from January 1, 1999 to December 31, 1999. Data on the age, gender, medication, season and place of the fall, areas of the fracture were collected based on their medical records. Result: The age range of the subjects were from 60 to 96 years old. The subjects were aged between 60-69 years old 49(46.2%), between 70-79 years old 31(29.2%), between 80-89 years old 24(22.6%), and over 90 years old 2(1.9%). Male patients comprised was 34(28.3%), while female patients comprised 76(71.7%). The fall occurred in Winter most frequently 34(32%). The place of the fall included room 81(76.4%), streets 13(12.3%), bathroom 6(5.7%), stair 4(3.8%), and mountain 2(1.9%). Twenty-two subjects (20.8%) had medication regularly, while 84 subjects (79.2%) had no medication. The areas of the fracture from the fall included upper extremities 20(18.9%) and lower extremities 86(81.1%). Radius fracture (7.5%) was the area where the fracture occurred most frequently in upper extremities and femur fracture (52.8%) was the area where the fracture occurred most frequently in lower extremities. A significant difference was found in the fracture area by age, season and place of the fall (p<.05). No significant difference was found in the fracture area by gender and medication. In all age groups, seasons and places of the fall, occurrence of fracture in lower extremity was significantly higher than that in upper extremity.
Purpose: The purposes of this study were to examine 1) functional status at 2 months after hip fracture surgery 2) health care utilization after a fall episode and 3) fear of falling experienced during first 2 months after a fall episode. Method: With a convenient sample of 99 elderly from six university or general hospitals with hip fracture from a fall, data were collected at 2-3 days before discharge and at 2 months after hip fracture surgery. Result: 1) At 2 months after hip fracture from a fall, significant proportion (25.3%) of elderly was not able to walk indoors. 2) Average length of hospital stay was 27.6 days with a range of 8 to 86 days. About 51% subjects received physical therapy during hospital stay, and only 6.1% subjects received physical therapy following discharge from the hospital. 3) Significant proportion (72.7%) had fear of falling after the fall episode. About 51% reported that they restricted their activities because they had fear of falling. Conclusion: Fall is a dreaded event which result in loss of independence and restriction of activity. Development and application of fall prevention program is critical especially for those with risk factors of fall.

In recent years, the importance of the characterization of fall for a fracture prevention system keeps increasing since fracture from a fall can lead to serious health problems. Fall is one of the major sources which increase morbidity in elderly people. In terms of the cost and the influence to the quality of life, the most serious injury with hip fractures is caused by falls. The traditional methods in characterizing fall patterns have been mainly by the epidemiological surveys. With surveys, the exact data of fall patterns can not been acquired. In this paper, we measured and analyzed with the parameters related to fall pattern such as velocities and accelerations during the motion of falls using 3D motion capture program. We acquired the parameters of the fall pattern of intentional and unexpected fall. The result showed that the variation of velocity and acceleration during fall was very important in characterizing fall pattern, which of vital importance for the development of a fracture prevention system and for the safety of the elderly.

Purpose: This study aims to establish a theoretical framework for the fall prevention behavior improvement program by verifying the associations between fracture risk, fall risk perception, and fall prevention behaviors in women with osteoporosis. Methods: A survey was conducted in 122 women who were diagnosed as osteoporosis by having T-score of the femur bone density below -2.5 standard deviation from the bone density examination performed in 2 orthopedic clinics located in B metropolitan city between July 2014 and September 2014. The risk of fracture, level of fall risk perception, and fall prevention behaviors were measured. Results: Fall risk perception had a complete mediating effect on the relationship between the fracture risk and fall prevention behaviors in women with osteoporosis. Conclusion: The perception of fall risk is important to enhance fall prevention behaviors in women with osteoporosis, and the development of various education programs to improve awareness of fall risk is needed.

Social activities of the elderly have been increasing as our society progresses toward an aging society. As their activities increase, so does the occurrence of falls that could lead to fractures. Falls are serious health hazards to the elderly. Therefore, development of a device that can detect fall accidents and prevent fracture is essential. In this study, we developed a portable fall detection system for the fracture prevention system of the elderly. The device is intended to detect a fall and activate a second device such as an air bag deployment system that can prevent fracture. The fall detection device contains a 3-axis acceleration sensor and two 2-axis tilt sensors. We measured acceleration and tilt angle of body during fall and activities of daily(ADL) living using the fall detection device that is attached on the subjects'. Moving mattress which is actuated by a pneumatic system was used in fall experiments and it could provide forced falls. Sensor data during fall and ADL were sent to computer and filtered with low-pass filter. The developed fall detection device was successful in detecting a fall about 0.1 second before a severe impact to occur and detecting the direction of the fall to provide enough time and information for the fracture preventive device to be activated. The fall detection device was also able to differentiate fall from ADL such as walking, sitting down, standing up, lying down, and running.

Purpose: The study aimed to examine the effects of Tai Chi fall prevention program on risk factors for fall, fear of falling, and quality of life among elderly women living in the community. Methods: A quasi-experimental pretest-posttest design with non-equivalent control group was used. Sixty participants were recruited from 4 senior citizen centers in a city. The 1-hour Tai Chi fall prevention program was provided three times a week for 6 months to the experimental group. Risk factors for fall including fracture risk and bone mineral density, fear of falling, and quality of life were measured at the baseline and at 6 months. Results: Participants were 75 years old in average. At the completion of 6 month program, the experimental group showed lower fracture risks, less fear of falling, and higher scores in several domains of quality of life than the control group, after controlling for the pretest scores, fall experience, and regular exercise habit. Conclusion: The Tai Chi fall prevention program was safely applied to elderly women with improvement in fracture risk, fear of falling, and several domains of quality of life. Future study is necessary to confirm the longer effect of the Tai Chi program for the prevention of fall episodes.
Finite element analysis is one of the most used tools for studying femoral neck fracture. Nerveless, consensus concerning either the choice of material characteristics, damage law and /or geometric models (linear on nonlinear) remains unreached. In this work, we propose a numerical quasi-brittle damage model to describe the behavior of the proximal femur associated with two methods to evaluate the Young modulus. Eight proximal femur finite elements models were constructed from CT scan data (4 donors: 3 women; 1 man). The numerical computations showed a good agreement between the numerical curves (load - displacement) and the experimental ones. A very encouraging result is obtained when a comparison is made between the computed fracture loads and the experimental ones ($R^2=0.825$, Relative error =6.49%). All specific numerical computation provided very fair qualitative matches with the fracture patterns for the sideway fall simulation. Finally, the comparative study based on 32 simulations adopting linear and nonlinear meshing led to the conclusion that the quantitatively results are improved when a nonlinear mesh is used.
Tillaux fractures, a rare type corresponding to Salter-Harris type 3, are observed in approximately 3% of pediatric ankle fractures. They are known to occur primarily via a supination-external rotation mechanism. Maisonneuve fractures, on the other hand, are proximal fibula fractures involving injury on syndesmosis, deltoid ligament, or medial malleolus fracture, occurring via a pronation-external rotation mechanism. These two types of fractures occur through different mechanisms of injury. In this study, a nine-year-old female pediatric patient presented to the outpatient department with ankle pain following a fall from a trampoline and was diagnosed with concurrent Tillaux-like fracture and Maisonneuve fractures. Surgical treatment was performed, resulting in successful recovery.

Falls are serious health hazards to the elderly and we need more thorough understanding of falls including the progress of falls and the impact area in various fall directions. Many of the traditional methods of falls research dealt with voluntary falls by younger subject since older subject can easily get fracture from voluntary falls. So, it has been difficult to get exact data about falls of the elderly. Here, we tried to capture the characteristics of the movements of major joints using three dimensional motion capture system during falls experiments using a moving mattress that can safely induce unexpected falls. Healthy younger subjects participated in the actual falls experiment and the moving mattress was actuated by a pneumatic system. The kinematic parameters such as velocities of major segments were imported to a computer simulation environment and falls to hard surfaces were simulated in a computational environment using a realistic human model of aged persons. The simulation was able to give approximations to contact forces which can occur during actual falls. And we designed impact absorption system to reduce the impact during falls. We can adapt this system to fracture prevention system that we are going to study.
