침술 후 발생한 표재 비골 신경 및 심 비골 신경의 손상(1예 보고) (Compression Neuropathy of Superficial Peroneal Nerve and Deep Peroneal Nerve Following Acupuncture Treatment (A Case Report))

  • Acupuncture has been widely used to treat a variety of disease and symptoms. But various complications have been reported. Among them, peripheral nerve injuries have been reported less frequently than other complications. The purpose of this report is to describe what we believe to be the first case of delayed superficial and deep peroneal nerve compressive neuropathy caused by fibrotic mass formed by neglected broken acupuncture needle.

  • Melorheostosis is a rare disease, belonging to the sclerotic bone dysplasia group. Initially described by Leri and Joanny in 1922, its etiology remains unknown. Onset is usually insidious, with deformity of the extremity, pain, limb stiffness, and limitation of motion in the joints. The typical radiographic appearance consists of irregular hyperostotic changes of the cortex, resembling melted wax dripping down one side of a candle. Treatment is usually symptomatic and conservative; however, conservative treatment is unsatisfactory due to functional issues when involving the distal extremity. We report on two cases of melorheostosis with synovial chondromatosis of the foot treated by mass excision.

Foot Rot of Bok Choy and Kale Caused by Rhizoctonia solani AG-2-1 in Korea

  • Foot rot symptoms were occasionally observed on young bok choy (Brassica rapa subsp. chinensis) and kale (B. oleracea var. viridis) plants grown in vinyl greenhouses located in Icheon and Yangpyeong, Gyeonggi Province, Korea. These observations were made during disease surveys in April 2020. The incidence of diseased plants in the vinyl greenhouses investigated was 0.5-1.0% in bok choy and 0.5-5.0% in kale. Five isolates of Rhizoctonia sp. were obtained from diseased roots of bok choy and three isolates of Rhizoctonia sp. were taken from diseased stems of kale. All the Rhizoctonia sp. isolates were identified as Rhizoctonia solani AG-2-1 based on the morphological characteristics and anastomosis test. Three isolates each of R. solani AG-2-1 from bok choy and kale were tested for pathogenicity in their host plants by artificial inoculation. The tested isolates induced foot rot symptoms on the inoculated bok choy and kale plants. The symptoms on the bok choy and kale, induced by the artificial inoculation, were similar to those observed on plants from the vinyl greenhouses that were investigated. This is the first report of R. solani AG-2-1 causing foot rot in bok choy and kale in Korea.

  • When a patient represents pain in foot, physician can easily overlook compression neuropathy of peripheral nerve as it is uncommon. Among nerve entrapment syndrome encountered in the foot, selective compression in lateral branch of deep peroneal nerve (DPN) is rare. We report a case of a patient with pain and dysesthesia in dorsolateral foot which turned out as lateral branch of deep peroneal nerve entrapment syndrome caused by talonavicular joint effusion. We would like to share diagnostic work up flow and conservative treatment courses. This case manifests the importance of the deep peroneal nerve and its branches in clinical setting of pain and ankle instability.

대한 정형외과 전공의 평가시험에서 족부-족관절 분야 문항 분석 (An Analysis of the Foot and Ankle-related Questions on the Korean Orthopedic In-Training Examination)

  • Purpose: The purpose of this study was to evaluate the questions pertaining to the foot and ankle on the Korean Orthopedic In-Training Examination (KOITE) between 2012 and 2016. Materials and Methods: Questions regarding the foot and ankle on KOITE during the five-year period (2012~2016) were analyzed. Number of foot and ankle questions, topics, taxonomic classification, imaging modalities, and cited references related to each question were analyzed. Results: The average number of foot and ankle questions was 35, accounting for 7.0% (35/500) of all KOITE questions. All questions were categorized into three topic areas: disease (51.4%, 18/35), trauma (31.4%, 11/35), and basics (17.1%, 6/35). Taxonomy 3 (decision-making type questions) was the most common (42.9%, 15/35). References that are commonly used for each question were Campbell's Operative Orthopedics (100%, 35/35) and the textbook of The Korean Orthopaedic Association (74.3%, 26/35). Conclusion: This analysis can be valuable to not only orthopaedic surgery residents to improve their knowledge of orthopedics, but also to instructors for optimizing their education programs.

가축매몰지 조성비용 평가 (Cost Analysis for the Carcass Burial Construction)

  • A lot of burial sites for the disposal of the contagious livestock slaughtered by foot and mouth disease were constructed in a short time for preventing the rapid spread of the virus. More than 4,700 carcass burial sites were constructed in 2011. Approximately 7 million poultry and 3.5 million livestock including head of cattle and swine were buried in farm land. However, the available cost data for a burial site was not provided. This study was to performed in order to understand the economic issues related to carcass burial disposal. The data from a local government was analyzed and the assumption data web based was constructed to evaluate the cost for constructing a carcass burial. The results showed that the local government paid KRW 5,386 for the burial disposal of a mortality (swine). It was estimated that the cost could be reduced through an appropriate measures.

중족골통에서의 소족지 중족 절골술 (Lesser Metatarsal Osteotomies for Metatarsalgia)

  • Metatarsalgia means the pain under the lesser metatarsal heads. The many causes of metatarsalgia can be categorized into three groups: local disease in the region, altered forefoot biomechanics, and systemic disease affecting the region. Surgical options need to be considered if nonsurgical treatment fails. The metatarsal osteotomies are designed primarily to reduce the weightbearing forces on the metatarsal head by elevating or shortening the metatarsal. Many lesser metatarsal osteotomies have been described, and their success depends on many factors. Regardless of the method employed, it is important to maintain or restore the metatarsal cascade to maintain an even pressure under the lesser metatarsal heads and prevent transfer lesions. The surgeon must understand the effects of the metatarsal osteotomy on the forefoot patho-biomechanics and decide, using a combination of clinical examinations and imaging, whether the desired effect of the osteotomy is to shorten or elevate the metatarsal head or both.

가축매몰지 환경관리에 있어 차단방역의 필요 및 절차 (Needs of Biosecurity and Protocols for the Environmental Management of Carcasses Burial)

  • Avian influenza (AI) and foot and mouth disease (FMD) are two main contagious pathogenic viruses causing massive mortality burial, as burial is a primary measure to quaranteen the causative viruse(s). Biosecurity is a set of preventive measures designed to reduce the risk of propagation of infectious diseases. Main objectives of this paper were to discuss the needs of biosecurity and develop protocol outlines for environmental management of burial sites. Pathological characteristics of contagious viruses should be considered during environmental management practices. Current practice prescribes to minimize the potential for on-farm pollution and the spread of infectious diseases, policy makers should understand robust knowledge regarding biosecurity to make informed decisions on future legislation.

인공 고관절시술자환자의 중심동요 특성에 관한 연구 (Investigation of postural sway characteristics of patients after total hip replacement)

  • This research investigates the possibility of using the postural sway of patients as a reference for measruing the progress of the disease and healing of patients who received total hip replacement; the progress of the disease by X-ray pictures, the pain felt by the patients, and the difficulties encountered in moving the joints. The measurements on the force platform were taken during a 25 second period standing on both feet and during a 5 second period standing on one foot with both eyes open. The result of the research showed that the trace and area of the overall length of postural sway was a good indicator which represented the healing progress of the patients who received total hip replacement, and the weakening of the muscles and the recovery process for 3 or 4 month after receiving the operation, which did not appear on X-ray pictures, were exhibited on the postural sway. Finally, standing on a single foot represented the healing progress much better than standing on both feet.

당뇨병성 신부전증 환자의 선단 궤양에 의한 하지절단(1예 보고) (Lower Leg Amputation by Calciphylaxis in Diabetic Nephropathy Patient (A Case Report))

  • Calciphylaxis is a rare disease that appear in patients with secondary hyper-parathyroidism or chronic renal failure or that show defect in calcium phosphate metabolism which is characterized by fibrin deposit or calcification of medial wall of vessels causing gradual ischemic skin necrosis. Calciphylaxis is a disease with poor prognosis as skin necrosis can progress rapidly. If left untreated, calciphylaxis will progress to sepsis with high mortality. The treatment is controversial but kidney transplantation or parathyroidectomy is suggested to recover calcium-phosphate metabolism. The authors have experienced calciphylaxis in a patient with chronic renal failure caused by DM nephropathy with characteristic skin lesion and rapid skin necrosis. We describe this case with documentary reviews.
