Length of hospital stay among oral and maxillofacial patients: a retrospective study

  • Objectives: Many conditions of the oral and maxillofacial region require hospitalization and in-patient care. The average length of stay (LOS) of these patients varies and is usually affected by multiple confounding variables. However, even with an increasing number of hospital admissions, published evidence on the factors that affect the LOS of oral and maxillofacial patients is lacking. Therefore, this study assessed the LOS of in-patients at the oral and maxillofacial surgery department of a government-funded, multi-specialty hospital in Malaysia, based on their reasons for admission and other factors. Materials and Methods: Our samples were collected retrospectively over a 5-year period and included patients with maxillofacial infections, post-trauma stabilization, facial bone fracture surgery, benign and malignant lesion surgery, dentoalveolar surgery, and other maxillofacial surgeries as reasons for admission. Factors potentially affecting LOS were also recorded, and their significance was determined using multiple logistic regression analyses. A P-value of less than 0.05 was considered to be statistically significant. Results: A total of 1,380 patients were included in this study. Most (84.5%) of our in-patients were of Malay ethnicity, and males outnumbered females in our sample by 502 subjects. The median LOS of our in-patients was 3 days. Sex, ethnicity, age, reason for admission, and American Society of Anesthesiology (ASA) classification were factors that significantly affected LOS. Conclusion: The median LOS reported in this study was 3 days. LOS was significantly affected by sex, ethnicity, age, reason of admission and ASA classification.

Clinical Observations on Myocardial Protection with Cold Blood Potassium Cardioplegia According to the Duration of Ischemic Arrest (냉혈K+ 심정지약을 사용한 개심술 예에서 심정지시간에 따른 심근보호효과에 관하여)

  • The efficacy of cold blood potassium cardioplegia during periods of ischemic arrest was assessed in 88 patients undergoing open heart surgical procedures at Chonnam National University Medical School from December, 1987 to January, 1989. The purpose of this study was to determine if the aortic cross clamping time[ACCT] over 120 minutes correlated with operative mortality, incidence of postoperative ventricular tachyarrhythmias, needs of postoperative inotropic support and serum enzyme levels. The patients were divided according to aortic cross clamping time[less than 120 minutes and 120 minutes or greater]. The results were as follows: 1. The operative mortality was 3.2% in ACCT<120min group and 7.7% in ACCT>120 min group. 2. The incidence of postoperative ventricular tachyarrhythmia was 1.6% in ACCT <120min group and 11.5% in ACCT>120min group[p<0.05]. 3. The incidence of postoperative inotropic support in congenital heart disease was 13.0Fo in ACCT<120min group and 45.0%o in ACCT>120min group[p<0.05]. The incidence in acquired heart disease was 26.0% in ACCT<120min group and 40.0% in ACCT> 120min group. 4. After cardiopulmonary bypass, serum GOT, LDH, CPK and CPK - MB were elevated prominently. Children showed higher value of the enzymes examined than adults did before and after cardiopulmonary bypass. In congenital heart diseases, postoperative serum GOT, LDH, CPK and CPK - MB levels of ACCT>120min group were significantly higher than those of ACCT<120min group. Postoperative serum GOT, LDH and CPK - MB levels of ACCT>=120min group were significantly higher than those of ACCT<120min group also in acquired heart diseases. The results suggest that the myocardial protective effect with cold blood potassium cardioplegic solutions was not sufficient when the aortic cross clamping time was over 120 minutes.

  • Mantle xenoliths in alkali basalt from Boun, Gansung area, and Baegryung island in S. Korea are spinel lherzolites composed of olivine, orthopyroxene, clinopyroxene, and spinel. The xenoliths generally show triple junctions among grams, kink-banding in olivine and pyroxenes, and protogranular and eqigranular textures having m orlentatron of specific direction. Anhedral brown spinels are disseminated in the intergranular spaces of minerals. Mineral compositions are very homogeneous without compositional zonation from rim to core in grains regardless different locahties. Olivine shows Fo. component of 89.0-90.2 and low CaO of 0.03-0.12wt%, orthopyroxene is enstatite with En component of 89.0 - 90.0 and $Al_2O_3$ of 4-5wt%, and clinopyroxene is diopside having En. component of 47.2-49.1 and $Al_2O_3$ of 7.42-7.64wt% from Boun and 4.70-4.91wt% from Baegryung showing local variation. Spinel shows the distinctive negative trend with increasing of A1 and decreasing of Cr, and Mg value and Cr number are 75.1-81.9 and 8.5-12.6, respectively. To estlmate T and P for these mantle xenoliths pyroxene-geothermometers (Wood and Banno, 1973; Wells, 1977; Mercier, 1980; Sachtleben and Seck, 1981; Bertrand and Mercier, 1985; Brey and Kohler, 1990) and Al-solubility geobarometer (Mercier, 1980; Lane and Ganguly, 1980) are used. Temperatures of Mercier (1980) and Sachtleben and Seck (1981) are compatible and equilibrium temperatures of xenoliths, average value of these two, aiie from $970^{\circ}C$ to $1020^{\circ}C$, and equihbrium pressures derived from Mercier (1980) are in the range of 12-19 Kb (42-63 Km). These temperatures and pressures seem to be reasonble wlth the consideration of Al-isopleths in MAS system (Lane and Ganguly, 1980) and Fe effect on Al-solubility in orthopyroxene (Lee and Ganguly, 1988). Equllibrium of temperatures and pressures of xenoliths in P-T space belong to ocenanic geothem among the Mercier's mantle geotherms (1980) and are completely different from continental geotherms of S. Africa (Lesotho) and S. India having different geologcal ages. anera1 compositions of spmel-lherzohtes in S. Korea and eastern China are primitwe and paleogeothems of both are very s~mllar, but degrees of depletion of upper mantle could be locally different from each other since eastern China has various depleted xenoliths due to different degrees of partial melting.

A Study on Pullout-Resistance Increase in Soil Nailing due to Pressurized Grouting (가압 그라우팅 쏘일네일링의 인발저항력 증가 원인에 관한 연구)

  • Pressurized grouting is a common technique in geotechnical engineering applications to increase the stiffness and strength of the ground mass and to fill boreholes or void space in a tunnel lining and so on. Recently, the pressurized grouting has been applied to a soil-nailing system which is widely used to improve slope stability. Because interaction between pressurized grouting paste and adjacent ground mass is complicated and difficult to analyze, the soil-nailing design has been empirically performed in most geotechnical applications. The purpose of this study is to analyze the ground behavior induced by pressurized grouting paste with the aid of laboratory model tests. The laboratory tests are carried out for four kinds of granitic residual soils. When injecting pressure is applied to grout, the pressure measured in the adjacent ground initially increases for a while, which behaves in the way of the membrane model. With the lapse of time, the pressure in the adjacent ground decreases down to a value of residual stress because a portion of water in the grouting paste seeps into the adjacent ground. The seepage can be indicated by the fact that the ratio of water/cement in the grouting paste has decreased from a initial value of 50% to around 30% during the test. The reduction of the W/C ratio should cause to harden the grouting paste and increase the stiffness of it, which restricts the rebound of out-moved ground into the original position, and thus increase the in-situ stress by approximately 20% of the injecting pressures. The measured radial deformation of the ground under pressure is in good agreement with the expansion of a cylindrical cavity estimated by the cavity expansion theory. In-situ test revealed that the pullout resistance of a soil nailing with pressurized grouting is about 36% larger than that with regular grouting, caused by grout radius increase, residual stress effect, and/or roughness increase.

Physicochemical Properties of Jatjook as Influenced by Various Levels of Pinenut (잣의 첨가량에 따른 잣죽의 특성)

  • The purpose of this study was to investigate the sensory and physicochemical properties of jatjook(rice gruel cooked with pinenut) containing various levels of pinenut. For this purpose, the various methods and recipe of Jatjook appeared in the literatures were evaluated to select the most preferable Jatjook. Chemical composition of Jatjook containing various levels of pinenut were 69.75∼83.04% of moisture, 2.25∼4.85% of crude protein, 1.77∼ 13.09% of crude fat, and 0.13∼0.51% of ash. C18-2,(linoleic acid) was the highest in the fatty acid contents of jatjook. Vitamin I contents of jatjook were 0.50∼3.50mg/100g. By the color difference meter, the values of L(lightness) and b(yellowness) were increased by increasing the amount of pinenuts, whereas the value of a(redness) showed negative value by increasing the amount of pinenut. Viscosity of Jatjook was decreased by increasing the amount of pinenut. As a result of the sensory evaluation fo. Jatjook made with various levels of pinenut, Jatjook having mixture ratio of 1 rice, 1 pinenut, and 5 water was the most preferable.

Ecological studies on plant communities of Quelpart Island -The communities of the Crinum maritimum and Torreya nucifera- (제주도 식물군락의 생태학적 연구 -비자림 및 문주란 자생지를 중심으로-)

  • This investigation was one of the series of ecological studies on the plant communities of Quelpart Island which was held in 1969. It was aimed at studying the ground vegetation of Torreya nucifera and the soil properties of the ground in which it grows. Observaton of the flora in a place where wild growth of Crinum maritimum, soil properties and distribution of micro-organisms in a place where wild growth occurs, were also made. 1) A few kinds which have important values among the ground vegetation fo Torreya nucifera forest are Liniope graminifolia, Convallaria keiskei, Sasaquelpaertensis, Carex kingiana, Pteridium aquilinum, Pyrola japonica, Hedera tobleri, etc. 2) While the studies of 25 Quadrats were being held, 29 species in A plot, 30 species in B plot, and 26 species in C plot have been found. 3) The kind which has the highest important value among the species located in naked plot(outside of Torreya nucifera forest) is Zoysia japonica and 18 other kinds were found. 4) The soil fertility through all depths are found higher in the forest soil than in that of open places. 5) P2O5, K, and total exchangeable bases etc. have shown the difference of contents according to the depth and generally had lower value as much as it goes down. 6) The content of N was a lot higher than any other forest soil and P2O5, K and total exchangeable bases seemed to be a little lower. 7) The overall flora of Todo consists of 71 species (9 species of wood plant and 62 species of herbs) added 28 kinds of non-reported species. 8) Themineral contents are the highest at a place of wind growth of Crinum maritimum which has much organic matter. The subsoil of this place is more fertile than surface soil. 9) The soil of Todo which is sandy soil, consists of shellfish shell and quartz, and has low fertility and alkali. 10) In the total occurrences of micro-organisms, bacteria was the greatest in number followed by actinomycete and fungi, in that order. 11) A survey of the soil depth and the ecological distributional pattern of micro-organismsm revealed that the number of micro-organisms is the greatest on the surface, gradually decreasing in proportion to the depth. 12) It was found that a comparatively greater number of micro-organisms occurred when the soil showed an adequate increase in water content.

Cross Resistance of Cypermethrin-and Methomyl-Resistance and Linkage Group Analysis on Cypermethrin Resistance in House Fly(Musca domestica L.) (Cypermethrin과 Methomyl 저항성 집파리의 교처저항성과 Cypermethrin 저항성에 대한 연관군 분석)

  • The house fly (Musca domestica L.) strains were derived from the Yumenoshima III strain by selecting with cypermethrin and methomyl for 19 and 16 generations, respectively. The resulting strains, cypermethrin resistance strain (Cyp-R19) and methomyl resistance strain (Met-R16), showed high level of resistance by 12906 and 51 times, respectively, comparing with the susceptible SRS strain. The Cyp-R19 strain was resistant to synthetic pyrethroids such as deltamethrin, esfenvalerate, fenpropathrin, $\beta$-cyfluthrin, showing > 11000, 1231, 103, 292 times higher $LD_{50}$ values than the SRS strain, respectively. It was also resistant to 3 organophosphates and 2 carbamates such as fenitrothion, profenofos, pyridaphenthion, benfuracarb, methomyl, showing resistance ratios fo 51, 17, 49, 39 and 62 comparing to SRS strain. The Met-R16 strain was resistant to synthetic carbamate benfuracarb, showing 6 times higher $LD_{50}$ value than SRS strain. It was also resistant to 4 organophosphates such as acephate, fenitrothion, profenofos and pyridaphenthion, showing > 40, 103, 19, 60 times higher $LD_{50}$ value. It was also resistant to 5 pyrethroids and a pyrrole such as cypermethrin, deltamethrin, esfenvalerate, fenpropathrin, $\beta$-cyfluthrin and chlorfenapyr, showing 3030, 249, 4063, 34, 330 and 86 times higher $LD_{50}$ values than the SRS strain. Cyp-R14 strain which was selected for 14 generations by cypermethrin and developed 11014 times higher resistance to the SRS strain was used in the dominance and linkage group analysis. Cypermethrin resistance inheritance was incompletely dominant in house fly as judged by the reciprocal cross between the resistant and susceptible strains. The linkage group analysis for the major factors responsible for this resistance was carried out by the$ F_1$male-backcross method, using susceptible multi-chromosomal marker aabys strain. The major factors for cypermethrin resistance were located on the 1st, the 3rd and the 4th chromosomes, and the effect of the 3rd chromosome was most prominent.

Effect of Maternal Dietary $\omega$3 and $\omega$6 Polyunsaturated Fatty Acids on the Fatty Acid Composition of the Second Generation Rat Brain (어미 쥐의 $\omega$3계 및 $\omega$6계 지방산 식이가 제2세대 쥐의 뇌조직 지방산 성분에 미치는 영향)

  • The change in fatty acid composition in brain tissue of the second generation rats(Sprague-Dawley strain) was studied using four different fat diets(Corn oil=CO, Soybean oil=SO, Perilla oil=PO, Fish oil=FO, 10% by Wt). The experimental diets were started from pregnancy in four different groups, each consisting of 9 rats. The seound generation rats were fed the same diet as their mothers. Animals were anesthetized with ether at 0, 3, 9 & 16 weeks of age. Whole brains were dissected out, brain tissues were, then, homogenized and lipids were extracted from brain tissues. The fatty acid compositions were measured after methylation by gas-liquid chromatography at 0, 3, 9 and 16 weeks of age of offspring. The changes in the relative concentrations of polyunsaturated fatty acids(PUFA) or more specifically docosahexaenoic acid(22 : 6, $\omega$3, DHA), the major $\omega$3 fatty acid component in rat brain at different age were similar to changes in the amount of DNA in brain tissue showing the maximum value during the lactation. The changes in saturated fatty acid(SFA) content showed a contrasting patten to those of PUFA, while monounsaturated fatty acid(MUFA) increased steadily throughout the experimental period. At birth, the relative concentrations of $\omega$3 series fatty acids the relative concentrations of PUFA, MUFA and SFA converged to very similar values respectively regardless of the dietary fatty acid compositions. In brain tissue, it is of value to note that while changes in relative concentrations of linoleic acid (18 : 2, $\omega$6, LA) and arachidonic acid(20 : 4, $\omega$6, AA) showed a precursor-product-like relationship, $\alpha$-linolenic acid(18 : 3, $\omega$3, $\alpha$-LnA) and DHA showed a different pattern. Even when the $\omega$3 fatty acid content in very low in maternal diet(CO), the second generation rat brain tissues appeared to secure DHA content, suggesting an essential role of this fatty acid in the brain. The fact that a large amount of $\alpha$-LnA in the maternal diet did not have a significant effect on the second generation rat brain $\alpha$-LnA content, indicated that DHA seemed essential component for the brain development in our experimental condition. In all groups, the relative content of $\alpha$-LnA in the brain tissues remained relatively constant throughout the experimental period at the very low level. The study of the specific concentrations and essential role(s) of DHA in each parts of brain tissue is needed in more details.

Temporal and spatial variations of SST and Ocean Fronts in the Korean Seas by Empirical Orthogonal Function (경험직교함수 분석에 의한 한반도 주변해역의 해수면온도 및 수온 전선의 시.공간 변화)

  • In the Korean seas, Sea Surface Temperature (SST) and Thermal Fronts (TF) were analyzed temporally and spatially during 8 years from 1993 to 2000 using NOAA/AVHRR MCSST. As the result of EOF method applying SST, the variance of the 1st mode was 97.6%. It is suitable to explain SST conditions in the whole Korean seas. Time coefficients were shown annual variations and spatial distributions were shown the closer to the continent the higher SST variations like as annual amplitudes. The 2nd mode presented higher time coefficients of 1993, 94, and 95 than those of other years. Although the influence is a little, that can explain ElNINO effect to the Korean seas. TF were detected by Sobel Edge Detection Method using gradient of SST. Consequently, TF were divided into 4 fronts; the Subpola. Front (SPF) dividing into the north and south part of the East sea, the Kuroshio Front (KF) in the East China Sea (ESC), the South Sea Coastal Front (SSCF) in the South sea, and the Tidal Front in the West sea. TF located in steep slope of submarine topography. The distributions of 1st mode in SST were bounded in the same place, and these results should be considered to influence of seasonal variations. To discover temporal and spatial variations of TF,SST gradient values were analyzed by EOF. The time coefficients fo the 1st mode (variance : 64.55%) showed distinctive annual variations and SPF, KF, and SSCF was significantly appeared in March. the spatial distributions of the 2nd mode showed contrast distribution, as SPF and SSCF had strong '-' value, where KF had strong '+' value. The time of '+' and '-' value was May and October, respectively. Time coefficients of the 3rd mode had 2 peaks per year and showed definite seasonal variations. SPF represented striking '+' value which time was March and October That was result reflected time of the 1st and 2nd mode. We can suggest specific temporal and spatial variations of TF using EOF.
