Characteristics of teeth referred to a dental university hospital for endodontic reason (근관치료적 이유로 치과대학병원으로 의뢰된 치아들의 특성)

  • Purpose: The aim of this study is to investigate the characteristics of patients and teeth referred to a university dental hospital for endodontic problem. Materials and Methods: From January, 2017 to December, 2018, patients who were referred to a university dental hospital for endodontic problem were collected from clinical records. A total of 1171 patient records were analyzed. The status of the referred teeth was divided into three groups according to whether they were treated endodontically based on radiographs and clinical records at the time of referred visit. Results: 69.9% of the referred teeth were maxillary and mandibular first and second molars. The average time from referral to actual visit is 9.03 days and 65.6% of the case referred with referring letter. The most primary reasons of referral were persistent clinical symptom (pain, swelling, and sinus tract) (37.9%), diagnosis difficulty (16.7%), blockage of canal space (13.8%) and difficult tooth anatomy (11.4%). In the case of referral before endodontic treatment, the most primary reason of referral was failure to make a proper diagnosis. If the teeth were referred in the middle of endodontic treatment, the most primary reason of referral was persistent clinical symptom and blockage of canal space. In the case of referral after root canal filling, the most primary reason of referral was persistent clinical symptom. Conclusion: In the case of molars, the rate of persistent clinical symptom and blockage of canal space were the most primary reason of referral, and the rate of apical surgery and management of trauma was high in the case of anterior teeth.

  • This study was performed to evaluate whether growth Prediction method can be used to diagnose and make treatment plan in skeletal Class III malocclusion patients or not. The sample was consisted of 25 patients(13 males, 12 females) who had been diagnosed with skeletal Class III malocclusion at first visit and after that had returned to take ortognathic surgery. Growth prediction performed with Ricketts' growth prediction method from first cephaogram. was compared with actual growth of the second cephalogram. The findings of this study were as follows ; 1. There was significant difference between actual growth and growth prediction in Porion Location, Ramus Position, Facial Depth, Facial Axis, Mandibular Plane angle, Maxillary Convexity. So, for these items Ricketts' growth prediction method is not proper to predict growth. 2. Although the growth amount of mandibular body was similar to normal growth amount, mandible was positioned anteriorly because of Porion Location and Ramus Position. 3. In skeletal Class III malocclusion patients, the tendency of mandibular prognathism might be aggreviated because of anterior placement of ramus and anterosuperior rotation of Pogonion.

Microbiology and Antibiotic Selection for Diabetic Foot Infections (감염된 당뇨병성 족부 병변의 균주 조사 및 항생제 선택)

  • Purpose: To help the empirical antibiotics selection in diabetic foot infection patients, we investigated prevalence of microorganisms and their antibiotics sensitivity results. Materials and Methods: Patients who came to our clinics to treat diabetic foot infections with deep ulceration and were followed up more than 6 months until complete recovery were adopted. From March 2006 to June 2009, there were 140 patients who corresponded with such a inclusion criteria. Wound cultures were done by deep tissue or bone debris at first visit to our clinics. Microorganisms which was documented by wound culture and most susceptible antibiotics by minimum inhibitory concentrations were surveyed retrospectively. Results: Microorganisms were confirmed in 113 cases (80.7%). In the other 27 cases (19.3%), there were no cultured microorganisms. In bacterial growth group, there were 72 cases (63.7%) of gram-positive bacteria and 41 cases (36.3%) of gram-negative bacteria. All of them were aerobic microorganisms and there were no anaerobic microorganisms. Methicillin-sensitive staphylococcus aureus was the most common pathogen and accounted for 35 cases (31.0%). As other common pathogens, there were Enterobacter cloacae (11 cases, 9.7%), pseudomonas aeruginosa (10 cases, 8.8%), Methicillin-resistant staphylococcus aureus (10 cases, 8.8%) and enterococcus faecalis (6 cases, 5.3%), and so on. Common susceptible antibiotics in gram positive microorganism were vancomycin (60 cases, 83.3%), teicoplanin (60 cases, 83.3%), nitrofurantoin (60 cases, 83.3%) and ciprofloxacin (53 cases, 73.6%). In gram negative ones, common susceptible antibiotics were imipenem (35 cases, 85.3%), piperacillin/tazobactam (33 cases, 80.5%) and gentamicin (31 cases, 75.6%). Conclusion: Methicillin-sensitive Staphylococcus aureus in gram positive and enterobacter cloacae in gram negative was the most common pathogen in each group. Ciprofloxacin and gentamicin might be adaptable as a first-line empirical antibiotics in infected diabetic foot patients.

  • The purpose of this study is to compare the differences between first visits and the recurred time of TMD patients about the number of visits, the treatment plans, the symptoms and the results after the conservative treatments. This could lead us to assess the prognoses of TMDs treated conservatively. We investigated 54 patients who have visited the Department of Oral Medicine in PNUH from 1991 to 2001, diagnosed as TMDs and treated conservatively with medications, physical therapies and splints. The treatments were terminated since the subjects have shown much improvements and resumed when TMDs recurred from 1992 to 2002. We researched the diagnoses, symptoms, the number of visits, the treatment plans and the results comparing the time before and after the treatments were carried out and following are the results. 1. Both the primary and the recurred groups improved after having been treated conservatively. 2. Both the primary and the recurred groups showed no differences in pains and MCOs when they first visited though the noises and LOMs turned out to be more serious in primary group. 3. Both the primary and the recurred groups had no differences in pains, noises, LOMs, MCOs when the treatments were over. 4. The results of treatments were not affected by treatment plans, sex, diagnoses in both primary and recurred groups. 5. Most of the patents tended to visit less than 10 times in recurred patients.

Analysis of Atopic Dermatitis Patients according to the Sasang Constitution (아토피 피부염 환자의 체질분석)

  • Objectives : The aim of this study was to investigate the relationship between atopic dermatitis and the Sasang constitution. Methods : I examined 64 patients with moderate to severe atopic dermatitis for a long time who visited Department of Dermatology in Kangnam Korean Hospital, Kyunghee University and classified them into 4 types according to Sasang constitution under consulting a Sasang specialist. 1. The Sasang specialist classified the patients using the Questionnaire for the Sasang Constitution Classification Ⅱ(QSCCⅡ), body components examination, facial morphology measurement and Meridian function examination. 2. I analyzed the distribution of sex and age, first onset age, history period, digestion condition, existences of past history and family history, basal material volume, immunoglobulin E and eosinophil. Results and Conclusions : 1. As the distribution of sex and age, 28 patients(pts) were male and 36 pts were female. 20's female group was the most common. 2. As the distribution of Sasang constitution types, Soeumin were 28(43.7$\%$) pts, Taeumin were 24(37.5$\%$) pts, Soyangin were 11(17.2$\%$) pts, and Taeyangin was 1(1.6$\%$) pI. 3. As the mean age at first visit, Soeumin's was 20yrs, Taeumin's and Soyangin's were 27yrs, and Taeyangin's was 15yrs. 4. As the mean onset age, Soeumin's was 7yrs the lowest, Taeumin's was 15.7yrs, Soyangin's was 10.54yrs and Taeyangin's was 15yrs. 5. As the history period. Soeumin's was 13.8yrs. Taeumin's was 11.8yrs. Soyangin's was 16.5yrs. Tae yangin's was 13yrs. 6. As the existence of past history of atopic diseases, Soeumin were 12(43$\%$) pts, Taeumin were 7(29$\%$) pts and Soyangin were 5(45$\%$) pts. 7. As immunoglobulin E, 21(75$\%$) pts of Soeumin were normal, 7(25$\%$) pts were abnormal.. 22(92$\%$) pts of Taeumin were normal, 2(8$\%$) pts were abnormal. 8(73$\%$) pts of Soyangin were normal, 3(27$\%$) pts were abnormal. As the defecation condition, 19(68$\%$) pts of Soeumin were normal, 9(32$\%$) pts were abnormal. 14(58$\%$) pts of Taeumin were normal, 10(42$\%$) pts were abnormal, 8(73$\%$) pts of Soyangin were normal, 3(27$\%$) pts were abnormal. 8. The mean value of immunoglobulin E of Soeumin was 1236, Taeeumin was 442, Soyangin was 2010, the mean value of eosinophil of Soeumin was 3.5, Taeeumin was 2.47, Soyangin was 4.28.

Two Cases of Korean Medicine Treatment for Patients with Parkinson's Disease Evaluated Using a Three-Dimensional Gait Analysis System (3차원 보행분석기로 평가한 보행장애 및 자세불안정을 주소로 하는 파킨슨병 환자 한의 치험 2례)

  • Objective: This study examined the effectiveness of Korean medicine treatments in two patients with Parkinson's disease complaining of discomfort stemming from postural instability and gait disturbance (PIGD). Methods: Two patients were treated for 3 months. They visited the clinic once a week for the first month and thereafter once every 2 weeks. The Unified Parkinson's Disease Rating Scale (UPDRS) and a three-dimensional gait analysis were performed at the first visit and at 1, 2, and 3 months thereafter. Results: In Case 1, gait speed, stride length, and swing phase increased. Double support decreased until 2 months after treatment but increased slightly after 3 months. Among the kinematic parameters, tilt and rotation increased. The total UPDRS Part III score decreased from 51 points to 29 points after 3 months of treatment. In Case 2, gait speed, stride length, and swing phase increased, but double support decreased. Among the kinematic parameters, tilt, rotation, and obliquity decreased. The total UPDRS Part III score decreased from 11 points to 7 points after 3 months of treatment. Conclusions: This study suggests that Korean medicine can be an effective treatment for patients with Parkinson's disease who experience discomfort due to PIGD.

A Research on the Total Emergency Room Time for Child Patients under 14 years suffering from External Injuries using Medical Information (의료정보를 활용한 14세 이하 외상환자 응급실 총 경유시간에 미치는 요인)

  • This research focused on a total of 378 patients with external injuries under the age of 14 who visited the emergency center at a university hospital in Korea, during the months of January, April, August and November between January and December of 2007. In addition, the survey was conducted only on even number days and ranged across the whole year to reflect the impact of seasonal characteristics on the collected data. The research focused on determining the characteristics and inflicting cause of these patients with external injuries, analyzed the total time spent in the emergency room and obtained the following results. 1. When classifying the patients into different genders, the proportion of males (67.5%) was higher than that of females (32.5%). According to the different age groups, the highest ratio, at 61.1 %, was patients under the age of six. 2. Looking at the total number of minutes spent in the emergency room, the longest amount of time occurred during April with 162.7 minutes, followed by 121.9 minutes in January and 92.4 minutes in November. August had the shortest period of time spent in the emergency room, a significant statistical difference from the other periods of the year (p<0.001). 3. Regarding the amount of time required for each examination, patients required to provide a urine test spent an average of 204.7 minutes while those who did not spent 113.5 minutes on average. This is a 5% statistical difference among the two groups (p>0.05). 4. Looking at the five most commonly diagnosed problems in the emergency room, the total number of people with these top five commonly diagnosed illnesses comprised 55.6%, or 210 patients out of 378. 5. Utilizing the Decision Tree Model to estimate the total number of minutes required per visit, the first classifications were made using a chemical examination factor. People subject to chemical classification spent an average of 177.7 minutes, which was longer than the overall average of 115.2 minutes, and those exempt from chemical examination spent an average of 103.8 minutes, which was shorter than the average Conclusion; Effort to curtail the total time spent in emergency rooms is vital in guaranteeing efficient management of hospitals and providing medical services. The delay experienced by many comprehensive professional medical centers must be resolved through the establishment of effective delivery of medical services, increased supply of patient rooms and other policy oriented implementations. However, for now, this problem must be resolved by increasing the level of patient satisfaction and guaranteeing effective operation of patient rooms, which will significantly contribute to the general management and success of hospitals and institutions.

The strong association of left-side heart anomalies with Kabuki syndrome

  • Purpose: Kabuki syndrome is a multiple congenital malformation syndrome, with characteristic facial features, mental retardation, and skeletal and congenital heart anomalies. However, the cardiac anomalies are not well described in the Korean population. We analyzed the cardiac anomalies and clinical features of Kabuki syndrome in a single tertiary center. Methods: A retrospective analysis was conducted for a total of 13 patients with Kabuki syndrome. Results: The median age at diagnosis of was 5.9 years (range, 9 days to 11 years and 8 months). All patients showed the characteristic facial dysmorphisms and congenital anomalies in multiple organs, and the diagnosis was delayed by 5.9 years (range, 9 days to 11 years and 5 months) after the first visit. Noncardiac anomalies were found in 84% of patients, and congenital heart diseases were found in 9 patients (69%). All 9 patients exhibited left-side heart anomalies, including hypoplastic left heart syndrome in 3, coarctation of the aorta in 4, aortic valve stenosis in 1, and mitral valve stenosis in 1. None had right-side heart disease or isolated septal defects. Genetic testing in 10 patients revealed 9 novel MLL2 mutations. All 11 patients who were available for follow-up exhibited developmental delays during the median 4 years (range, 9 days to 11 years 11 months) of follow-up. The leading cause of death was hypoplastic left heart syndrome. Conclusion: Pediatric cardiologist should recognize Kabuki syndrome and the high prevalence of left heart anomalies with Kabuki syndrome. Genetic testing can be helpful for early diagnosis and counseling.

Analysis on Outpatients with Postpartum Disease at the Korean Gynecology Clinic of Traditional Korean Medicine Hospital (한방병원에 내원한 산후병 환자의 특성 및 증상에 대한 분석)

  • Objectives: The purpose of this study was to analyze the current characteristics of outpatients with postpartum disease and provide a treatment instruction in the clinical field. Methods: To analyze characteristics of outpatients with postpartum disease, We searched medical records from January 1, 2015 to December 31, 2017 using Z34 (Supervision of normal pregnancy), O94 (Sequelae of complication of pregnancy), U327 (産後風) and 388 postpartum patients were analyzed. Results: 1. The general characteristics of the subjects are as follows. The average age was $33.73{\pm}3.62$ years old, 356 (91.75%) patients are full term pregnancy, 201 (51.81%) patients are first delivery, 79 (20.36%) patients had past history of abortion and 63 (16.24%) patients had past history of gynecologic disease. 2. The most subjects visited in April (10.31%). The mean duration from delivery date to visiting date was $131.5{\pm}214.6$ days. The mean number of visits after the initial visit was $1.91{\pm}1.7$. 3. The characteristics related with pregnancy and delivery are as follows. The number of subjects taken assisted reproductive technology was 19 (4.9%) and suffering from gestational disease was 28 (7.22%). The mean body weight difference between full term pregnancy and visiting date was $-8.24{\pm}3.57kg$. The number of subjects discharging lochia on visiting date was 167 (43.04%) and breastfeeding was 262 (67.53%). 4. The most chief complaint was arthralgia in 217 (55.93%) followed by general weakness, cold sensation, edema, sweating disorder, dizziness, lower abdominal pain, digestion disorder, heat sensation, defecation disorder and urination disorder. 5. The most accompanied symptom was arthralgia in 322 (82.99%), followed by sleep disorder, cold sensation, general weakness, sweating disorder, defecation disorder, edema, dizziness, digestion disorder, urination disorder, lower abdominal pain and heat sensation. 6. A total of 356 subjects were taken herbal medicine. The most prescribed herbal medicine to subjects was Gungguijohyeol-eum-gamibang (29.59%). Conclusions: These results could be helpful to diagnose and treat postpartum patients in Korean gynecologic clinical fields.

A Survey Research on the Actual Condition for Intra-oral Treatment in Patients with a Visit to Dental Hospital in K Region (K지역 치과 병원에 내원한 일반 성인의 구강 내 치료 상태 실태조사)

  • This study carried out the survey of the actual condition targeting 303 patients with execution of treatment after medical examination among patients who visited from January 2, 2008 to December 31, 2008 at dental hospital where is located in K region, and analyzed by using a program of SPSS13.0. The ratio of treatment in posterior was high with 76.9% in central incisor, 62.3% in the 1st bicuspids, and 45.3% in the first permanent molars as for health in the right teeth of maxillary, and with 77.9% in central incisor, 64.4% in the 1st bicuspids, and 47.6% in the first permanent molars as for the left teeth. In the lower jaw, the ratio of treatment in posterior was high with 92.1% in central incisor, 73.9% in the 1st bicuspids, and 42.6% in the first permanent molars as for health in the right teeth, and with 92.1% in central incisor, 75.9% in the 1st bicuspids, and 43.2% in the first permanent molars as for the left teeth. The upper left cuspid was treated in older patients by age(${\chi}^2=32.33$, p=0.000), in the more patients with high blood pressure(${\chi}^2=12.60$, p=0.000), and in case of systemic disease. The significant difference was shown(${\chi}^2=6.37$, p=0.012). The older patients led to being treated cuspid among right teeth in the lower jaw. The significant difference was shown according in the more patients with high blood pressure(${\chi}^2=8.26$, p=0.004). The left cuspid in the lower jaw was treated in the older patients by age the statistically significant difference was shown(${\chi}^2=12.10$, p=0.007).
