미만성 이상각화증을 동반한 침습성 표피양 낭의 세침흡인 세포학적 소견 - 1예 보고 - (Fine Needle Aspiration Cytology of Unusual Epidermoid Cyst with Diffuse Parakeratosis and Aggressive Growth - A Case Report -)

  • An extremely unusual case of epidermoid cyst showing diffuse parakeratosis and aggressive clinical behavior is presented. A destructive bone lesion with surrounding ill-defined soft tissue lesion was found by computed tomography in a 63 year-old man complaining of painful swelling of the right buttock. He had a history of surgical excision twice for epidermoid cysts of soft tissue of the right hip during recent one year On aspiration cytology, the aspirate was highly cellular and mostly composed of desquamated nucleated squamous cells. Operation finding revealed that the iliac bone was Irregularly destroyed and filled with gray-white cheesy material and necrotic bone bedris. Adjacent gluteus muscle showed scattered gray-white lesions. The curettage specimen showed bone necrosis and desquamated squamous cells filling the marrow spaces. The lesion within muscle revealed epidermoid cyst with diffuse parakeratosis.

악성 흉선종의 세침흡인 세포학적 소견 - 침윤성 흉선종 2례와 흉선암종 2례 - (Fine Needle Aspiration Cytology of Malignant Thymoma - Two cases of Invasive Thymoma and Thymic Carcinoma -)

  • We report 4 cases of malignant thymoma which were composed of 2 cases of invasive thymoma and 2 cases of thymic carcinoma. The cytologic findings of invasive thymoma were similar to those of benign thymoma. The distinctive cytologic features of thymic carcinoma were necrotic background, irregular clusters and individually scattered arrangement of anaplastic epithelial cells, and some scattered mature small lymphocytes. These findings may be found in the Hodgkin's lymphoma, seminoma, and metastatic squamous ceil carcinoma, undifferentiated carcinoma, and large ceil carcinoma of the lung. But, the feature of irregular clustering of anaplastic epithelial cell haying scanty cytoplasm was different from Hodgkin's lymphoma and seminoma. Clinical and radiologic findings as well as cytologic finding were helpful in differential diagnosis of thymic carcinoma from metastatic carcinoma.

유방의 세침흡인 세포검사 -악성 병변의 일반적인 세포 소견- (General Cytological Characters of Malignant Breast Lesions)

  • Fine needle aspiration (FNA) cytology of breast disease is recognized to be highly accurate and cost effective, especially when this is used in combination with clinical examination and imaging as part of a triple approach. A probabilistic/categorical approach is used for the classification of breast FNA specimens. Criteria are defined from the perspective of the likelihood of making a definitive diagnosis of cancer on excision. This approach is an accurate way of classifying breast FNA specimens, and this can be reliably applied regardless of the level of experience of the pathologist for interpreting the case. When a definitive diagnosis of malignancy is made, the next step is to determining the specific histologic types of the malignancy according to their cytological features. In order to make an accurate diagnosis of carcinoma and for correct typing a tumor, an adequate, correctly sampled aspirate without any other artifacts is required.

  • The role of respiratory cytology is to detect and classify pulmonary disease, with an emphasis of neoplastic disease, so that proper therapy can be instituted. As in many branches of cytology, the recognition of malignancy in the cells obtained from the respiratory tract is more straightforward than identifying the type of tumor cell. It is important to accurately determine the true cytopathological cell type in cases of primary lung cancer and to know the accuracy of the diagnosis achieved by the cytological procedures. The well differentiated tumors have characteristic cytoplasmic and nuclear abnormalities that enable physicians to firmly categorize these lesions, as in squamous cell or adenocarcinoma, but some moderately and most poorly differentiated tumors show few distinctive features. This article reviews the malignant and reactive pulmonary cytologic findings and we also report on some of their pitfalls and the cytologic criteria.

비 산부인과 세포 검사 진단에 있어서세포 군집절편(Cell-Block)법의 유용성 (The Efficiency of Cell Block in the Diagnosis of Non-gynecologic Cytology)

  • Because of the lack of resources on the importance of cell block, the diagnosis of cytologic specimen has been overlooked. Out of 1,243 cases of non-gynecologic cytology specimen, about 87.1% has been proven to be diagnostically useful. About 0.9% of those were rendered diagnostic by cell block alone. If cell block was not made it could have resulted in misdiagnosis. The effect on making cell block might not be directly linked to the patient but to the pathologist, it can be a helpful secondary tool in lowering the chance of giving false negative results hence. Giving the patient the opportunity of an adequate treatment. This study has proven that cell block has diagnostic value in non-gynecologic cytology. We are doing our best to increase the production rate of cell block and to improve the quality of cytology smears and cell block, so that we can increase the accuracy of diagnosis.
