Impact of Changes in Medical Aid Status on Health Care Utilization

  • Background: South Korea operates a Medical Aid (MA) program targeting selected low-income individuals to ensure medical service delivery to the disadvantaged while enhancing self-sufficiency of work-capable beneficiaries. However, as reasons behind welfare exits are diverse and do not always infer poverty relief or the provision of appropriate levels of health care services, this study aimed to investigate the association between changes in MA status and health care utilization. Methods: This study used the 2006 to 2015 National Health Insurance claims data. The impact of changes in annual MA status on health care utilization (yearly number of outpatient visits, inpatient visits, length of stay, and emergency department [ED] visits) was investigated using the generalized estimating equation model. Results: In 117,943 adult subjects aged 20 to 64, compared to the 'MA to MA' group, the 'MA to MA exit' group showed general decreases in utilization (outpatient visits: β=-3.93, p<0.0001; hospital admissions: relative risk [RR], 0.87; 95% confidence interval [CI], 0.83-0.91; length of stay: β=-3.64, p<0.0001; ED visits: RR, 0.83; 95% CI, 0.77-0.90). Similar patterns were found in the 'MA exit to MA exit' group (outpatient visits: β=-5.72, p<0.0001; admissions: RR, 0.91; 95% CI, 0.87-0.94; length of stay: β=-5.87; p<0.0001; ED visits: RR, 0.81; 95% CI, 0.75-0.88). Likewise, in 74,747 older adult subjects aged 65 or above, the 'MA to MA exit' group showed reduced levels of utilization (outpatient visits: β=-1.51; p=0.0020), as well as the 'MA exit to MA exit' group (admissions: RR, 0.92; 95% CI, 0.89-0.95; length of stay: β, -5.45; p<0.0001; ED visits: RR, 0.90; 95% CI, 0.83-0.97). Conclusion: MA exit was associated with general decreases in health care utilization. Utilization patterns of individuals with experiences of receiving MA benefits should be monitored to promote the ideal use of health care services while preventing potential financial barriers present in accessing medical care.

  • 방송 통신의 융합과 미디어 산업으로의 진화로 인해, 시청자들은 다양한 플랫폼과 개인 미디어를 통한 콘텐츠의 수신에 적응하고 있다. 이런 환경의 변화로 공영방송은 공적 책임을 재정의하고, 지속 가능한 사회를 위한 역할을 정립하는 공영미디어로의 변화를 모색하고 있다. 특히, 공익적 콘텐츠가 상업서비스에 의해서도 제공되는 환경에서 차별화된 공공서비스의 형태로 그 가치를 입증하는 것은 사회구성원들에게 재원, 서비스에 대한 정당성 입증을 위해 필요하다. 본 논문에서는 2010년대 디지털 방송의 시대가 되고, 2020년 미디어 산업이 재편되면서 공영방송사에 요구되는 공적 책임과 상응하는 공공서비스의 변화를 해외 공영방송 사례를 통해 분석하고, 향후 공영미디어로서의 가치를 입증하기 위해 중심이 되어야 하는 공적 책임과 공공서비스의 방향성을 도출하였다. 특히, 미디어 환경에서 방송사, 시청자의 기술적인 준비가 필수적이기 때문에 공공서비스의 방향성은 기술 방향성을 위주로 분석하였다. 도출한 방향성은 한국을 포함한 모든 공영방송사에 적용 가능한 공통적인 방향성이다. 본 논문에서 제언한 공적 책임과 기술 발전의 방향성이 공영미디어의 역할을 명확히 하고, 생활에 이바지하는 차별화되는 서비스가 되는데 일조하기를 기대한다.

맞벌이 부인의 가족지원서비스 필요도 결정요인 : 전문직과 생산직 모델 비교 (Determinants of Dual-earner Wives' Needs for Family-supportive Services: A Comparison of Professional and Blue-collar Models)

  • 맞벌이 부인의 가족지원 서비스 필요도를 결정짓는 요인을 조사하기 위해, 맞벌이 부인의 스트레스와 역할과중을 증대시키는 직장과 가정의 상황 구조적 요인들을 나타내는 여섯 가지 스트레스원, 역할과중, 스트레스, 네 종류의 맞벌이가족 지원서비스 (보육비지원 서비스, 직장내 보육서비스, 휴직제도, 유동근무제도)에 대한 필요도 간의 상호 관계를 설명할 수 있는 포괄적인 이론모델을 개발하였다. 맞벌이가족 지원서비스 필요도를 결정짓는 요인에 있어서의 계층간 공통점과 차이점을 비교하기 위해, 전문직 맞벌이 부인과 생산직 맞벌이 부인의 모델을 별도로 개발하여 비교하였다. 본 연구에서 개발된 모델은 진주.사천지역에 거주하는 234명의 전문직 맞벌이 부인과 208명의 생산직 맞벌이 부인으로부터 수집된 설문조사 자료를 통하여 검증되었다. 공변량 구조분석을 이용하여, 변인들 간의 구조관계를 설명할 수 있는 최적의 이론구조모델이 선정되었다 (전문직 모델: df=141, GFI=0.928, CFI=0.965; 생산직 모델: df=141, GFI=0.902, CFI=0.912). 자료분석 결과, 현재 이용 중인 보육서비스에 대한 불만족도가 높을 수록, 근무시간이 길 수록, 맞벌이 부인들의 역할과중은 증가하며, 역할과중은 스트레스를 증가시키는 것으로 나타났다. 스트레스가 증가할 수록, 맞벌이 부인들은 휴직제도를 필요로 하는 것으로 나타났다. 한편, 자녀가 어릴 수록 보육비 지원서비스를 필요로 하고 있으며, 자녀의 나이가 증가될 수록 유동 근무제도를 필요로 하는 것으로 나타났다. 보육비지원 서비스에 대한 필요도가 높을 수록 직장내 보육서비스에 대한 필요도가 높으며, 직장내 보육서비스의 필요도가 높을 수록 휴직제도에 대한 필요도가 높게 나타났다. 또한 휴직제도에 대한 필요도가 높을 수록 유동근무제도에 대한 필요도도 높게 나타났다. 이와 같은 전문직과 생산직 맞벌이 부인 모두에게 일반화될 수 있는 9가지 공통적 인과관계를 제외하고는 전문직과 생산직 부인의 서비스 필요도 결정요인에 있어 상당한 차이점이 발견되었다. 두 집단의 공통적 요인과 차별적 요인에 대한 논의를 바탕으로, 각 직장의 환경과 개인의 필요에 따라 맞벌이 부인이 가장 필요로 하는 서비스를 차별적으로 제공할 수 있는 방안이 제시되었다.

iVisher: Real-Time Detection of Caller ID Spoofing

  • Voice phishing (vishing) uses social engineering, based on people's trust in telephone services, to trick people into divulging financial data or transferring money to a scammer. In a vishing attack, a scammer often modifies the telephone number that appears on the victim's phone to mislead the victim into believing that the phone call is coming from a trusted source, since people typically judge a caller's legitimacy by the displayed phone number. We propose a system named iVisher for detecting a concealed incoming number (that is, caller ID) in Session Initiation Protocol-based Voice-over-Internet Protocol initiated phone calls. Our results demonstrate that iVisher is capable of detecting a concealed caller ID without significantly impacting upon the overall call setup time.

Municipal Solid Waste Management: Challenges and Opportunities in Nepal

  • Nepal is one of the least urbanized countries in the world where more than 80% of the total population live in rural areas. In recent years, the rate of urbanization became rampant which ultimately accelerated immense pressure on municipal services, especially on managing the ever increasing amount of wastes. Due to lack of technology, infrastructure and financial capacity management of increasing amount of solid waste has become a major challenge in municipalities of Nepal. The indiscriminate dumping of solid wastes already affected the urban environment by creating a serious occupational health and environmental hazard to the vicinity of the dumping sites. However, there is great possibility of recovering methane from the landfill sites since the typical Nepalese municipal solid waste contains more than 65% of organic wastes. Despite having enormous potential of generating electricity from hydropower, Nepal is facing acute shortage of energy. Therefore, comprehensive scientific research and development is necessary for making solid waste to environmentally friendly by converting waste to the energy.

The Role of Electronic Trade Specialized Service Provider under Global Trade Environment

  • This thesis examines the problems faced in the electronic trade specialized service provider for which improvements are necessary, and suggests various ways of overcoming those problems. First, from the legal aspect, it is necessary to clarify the scope of duties as well as to expand the threshold for entrance and business areas through the amendment of related laws. Second, from the operating aspect, the e-trade specialized service provider should be able to provide convenient work spaces through user-centered interface by connecting, integrating, automating and simplifying various services scattered around through the establishment of an operating system. Third, from the institutional aspect, trade firms should be able to perform e-trade duties without difficulties by specializing and increasing the size of the e-trade specialized service provider system and strengthening job competence. For this, it is required to improve legal registration requirements to enhance the reliability of the e-trade specialized service provider system and secure its financial capacity.

영양사 직무기술서 (2000년) 의 영양사 직무영역별 특성 분석 3 : 학교 (Analysis of the Dietitian's Job description in the school)

  • The purpose of this study was to verify the dietitian’s job description(2000) for dietitians working at schools. The survey was carried out for the frequency, criticality and difficulty of each job description with 4 point scales. The number of subject was 125 dietitians, who have been working over 3 years at their position. The results are following ; 1. Duties with high frequency and criticality were menu managements, purchasing, storage and inventory controls, food productions, meal services, managing equipments and facility, waste managements, sanitation managements and self promotions. 2. Duties with middle frequency and high criticality were human resources managements and financial managements. 3. Duties with low frequency and high criticality were nutrition assessment (life cycles, certain diseases, specific condition), medical nutritional theraphy, nutrition education and public health nutrition. 4. For difficulty, duties related food service had lower points than that of nutrition service.

  • Changed the business environment, private sector focuses on the performance management, so to manage the performance he uses BSC that overcomes the limitation of financial indicators. Also, it is getting emphasized to evaluate the performance about services that the public sector provides and obliged for the public sector to be responsible for managing and improving its performance as the private sector has been doing. But lacks of knowledge and experience on performance management and evaluation are blocking the public sector from introducing and implementing new performance management systems. To solve those problems, this study has derived general guidelines and KPIs that public sector should keep in mind when BSC is introduced new management system. This study focused on the public hospital which is the performance management and evaluation system of Government administration.
