영상장치 센서 데이터 QC에 관한 연구 (A study on imaging device sensor data QC)

  • Currently, Korea is an aging society and is expected to become a super-aged society in about four years. X-ray devices are widely used for early diagnosis in hospitals, and many X-ray technologies are being developed. The development of X-ray device technology is important, but it is also important to increase the reliability of the device through accurate data management. Sensor nodes such as temperature, voltage, and current of the diagnosis device may malfunction or transmit inaccurate data due to various causes such as failure or power outage. Therefore, in this study, the temperature, tube voltage, and tube current data related to each sensor and detection circuit of the diagnostic X-ray imaging device were measured and analyzed. Based on QC data, device failure prediction and diagnosis algorithms were designed and performed. The fault diagnosis algorithm can configure a simulator capable of setting user parameter values, displaying sensor output graphs, and displaying signs of sensor abnormalities, and can check the detection results when each sensor is operating normally and when the sensor is abnormal. It is judged that efficient device management and diagnosis is possible because it monitors abnormal data values (temperature, voltage, current) in real time and automatically diagnoses failures by feeding back the abnormal values detected at each stage. Although this algorithm cannot predict all failures related to temperature, voltage, and current of diagnostic X-ray imaging devices, it can detect temperature rise, bouncing values, device physical limits, input/output values, and radiation-related anomalies. exposure. If a value exceeding the maximum variation value of each data occurs, it is judged that it will be possible to check and respond in preparation for device failure. If a device's sensor fails, unexpected accidents may occur, increasing costs and risks, and regular maintenance cannot cope with all errors or failures. Therefore, since real-time maintenance through continuous data monitoring is possible, reliability improvement, maintenance cost reduction, and efficient management of equipment are expected to be possible.

신생아 및 영아기 윤상인두 협조불능의 임상적 고찰 (Clinical Features of Cricopharyngeal Incoordination in Newborns and Infants)

  • 목 적: 윤상인두 협조불능은 원인 불명의 드문 질환으로 윤상인두의 이완과 연관된 인두의 수축 지연이 특징이다. 출생 초기에 연하곤란을 일으키는 타 질환과의 감별과 임상 경과에 대한 이해가 조기 진단과 적절한 치료를 위해 필요하나 이에 대한 연구가 많지 않다. 방 법: 2000년 1월부터 2006년 12월까지 부산대학교 병원 소아과에서 임상 소견과 비디오 식도 촬영 또는 식도 조영술에서 윤상인두 협조불능으로 진단받은 17예를 대상으로 병록지와 검사 소견을 후향적으로 분석하였다. 결 과: 남녀 비는 1:1.1 (남자 8예, 여자 9예)이었다. 평균 나이는 12.5${\pm}$17.5일로 7일 이내 10예(58.8%), 7~30일 5예(29.4%), 30일 이상 2예(11.7%)였다. 체중은 3백분위수 미만 8예(47.1%), 10백분위수 이하 11예(64.7%)였다. 미숙아 6예(35.3%), 저출생 체중아 7예(41.2%), 부당경량아 2예(11.8%)였다. 동반 질환은 심장기형 3예, 염색체 이상 2예, 후두연하증 1예였다. 주요증상은 재발성 흡인성 폐렴 10예(58.8%), 수유곤란 9예(52.9%), 수유 시 호흡곤란 4예(23.5%), 구토와 목맴 4예(23.5%), 기침 3예(17.6%)였다. 흡인 정도는 경도 흡인 12예(70.5%), 중증도 흡인 3예(17.6%), 중증 흡인 2예(11.8%)였다. 15예(88.2%)에서 비위관 식이를 하였다. 추적관찰이 가능했던 10예 중 9예에서 경구 식이가 가능하였다. 그 시기는 1개월까지 3예(30.0%), 6개월까지 3예(30.0%), 12개월까지 2예(20.0%), 24개월까지 1예(10.0%)였다. 하지만 흡인의 정도가 심할수록 비위관식이의 기간이 의미 있게 길었다(상관 계수 0.73, p<0.05). 결 론: 출생 직후 또는 영아 초기에 연하곤란과 반복되는 흡인성 폐렴이 있으면 비디오 식도 조영술을 이용하여 윤상인두 협조불능의 감별이 필요하다. 윤상인두 협조불능은 예후가 양호한 질환이지만 조기 진단과 관급식이 합병증의 예방과 질환의 관리에 중요하며, 흡인의 정도가 심할수록 비위관 영양 기간이 길어지므로 완전 호전될 때까지 철저한 추적관리가 중요하다.

Effect of Feeding Chemically Treated Mustard (Brassica campestris) Straw on Rumen Fibre Degrading Enzymes in Sheep

  • The aim of the experiment was to study the changes in the activities of various rumen fibre degrading enzymes due to the feeding of chemically treated mustard (Brassica campestris) straw in sheep. Mustard straw (MS) (<5 cm particle size) was treated either with urea (4% (w/w), or with 2% sodium hydroxide (NaOH), or with alkaline hydrogen peroxide (2% NaOH and 1.5% hydrogen peroxide ($H_2O_2$)) and/or supplemented with 2% (w/w) urea. Seven maintenance type rations were prepared using MS (70 parts) with molasses (5 parts) and concentrate (25 parts). They were untreated MS (CMS), urea treated MS (UMS), urea supplemented MS (MSUS), alkali treated MS (AMS), alkali treated and urea supplemented MS (AMS-US), alkali $H_2O_2$ treated MS (AHMS) and alkali $H_2O_2$ treated and urea supplemented MS (AHMS-US). They were then compressed into a complete feed block with the help of block making machine. Forty two male hoggets of Malpura breed sheep were equally distributed into each treatment group and (were) offered feed and water ad libitum. At the end of 21 days of feeding trial, rumen liquor was collected through stomach tube from three animals in each group at 0 h, 4 h, 8 h, 12 h of post feeding. Results showed that the level of enzyme varied from 8.52 to 11.12, 40.85 to 50.37, 3.22 to 3.78, 2.09 to 2.77 and 31.44 to 44.24 units/100 ml SRL respectively for carboxymethyl cellulase (CMCase), $\alpha$-amylase, microcrystalline cellulase (MCCase), filter paper (FP) degrading enzyme and $\alpha$-glucosidase. Processing of MS affected the enzyme activities, in a way, that NaOH and AHP treatment significantly reduced CMCase and FP degrading enzyme. The effect of urea treatment showed an increase in the activity of MCCase and $\alpha$-glucosidase. But the supplementation of urea increased the activity of CMCase, FP degrading enzyme and $\alpha$-glucosidase. The CMCase, $\alpha$-amylase, $\alpha$-glucosidase activities were highest at 4hr whereas MCCase and FP degrading enzyme had maximum activities at 12 h post feeding Results suggested that MS might need longer time in the rumen for its effective degradation.

How to Manage the Pediatric Nutritional Support Team: Updates

  • Pediatric patients in hospital are at risk of malnutrition at admission and even during their hospitalization. Although the concept of nutritional support team (NST) was introduced to hospitals for optimal nutritional care since 1960s and the benefits of pediatric NST have been proven by many studies and reports in terms of patient clinical outcome and cost saving, the pediatric NST is not widespread yet. The pediatric NST composed of pediatricians, dieticians, pharmacist, and nutrition support nurses as core members dedicated to nutritional care in children should be independent of central NST or other disciplines, but closely cooperate with other teams in hospitals. There is no doubt that a multidisciplinary NST is an effective way to provide appropriate nutritional support to an individual patient. Therefore, the implementation of the pediatric NST in hospitals should be recommended to provide optimum nutritional support including enteral tube feeding and parenteral nutrition and to assess pediatric patients at risk of malnutrition.

  • In the present study, front lower arm and engine cradle which are automotive chassis parts were developed using the hydroforming technology. For systematic establishment of parts development process, material properties of tube were reflected at the start of design and problems of initial design drawing were solved by forming analysis. Design and manufacturing technology of hydroforming die were established and the relationship between internal pressure and feeding stroke was studied during try-out. Durability and buckling strength of hydrofromed parts were estimated.

만성 농흉에 대한 Modified Eloesser's Operation 과 그 의의 (Modified Eloesser's Operation and Its Result In Chronic Empuema with Poor General Condition or Chachecic State)

  • During the past 3 years, 15 patients who could not be anesthetized generally because of poor general condition or cachecic state, of 111 patients with empyema, have been treated with modified Eloesser`s operation under the local anesthesia with 2% procaine. There were 13 males and 2 females ranging from 21 years to 61 years of age. The etiology was tuberculosis [6 cases], pyogenic pneumonia [5 cases], lung abscess [1 case], post-trauma [2 cases] and malignancy[1 case]. " The over-all mortality rate was 6.7%[1 case] and cause of its death was poor oral feeding because of post-traumatic psychosis. Modified Eloesser`s operation was performed after closed tube thoracostomy and irrigation with 1% zephanon solution for over 2 weeks. And then the other operation was not performed and all patients except 1 case appeared good progression (Complete healing; 4 cases, Progressive healing 10 cases, death: 1 case).

경부 식도협착 재건술에 있어서 유리공장 이식편의 이용 (Reconstruction of the Cervical Esophagus Using the Free Jejunal Graft)

  • The cervical esophageal stricture has various surgical modalities and difficulties in reconstruction. We had experienced a case of successful reconstruction of the cervical esophageal restenosis using the free jejunal graft, on 30 year old man had had esophageal stricture after ingestion of lye. He had undergone colon interposition[esophagocologastrostomy] with left colon feeding gastrostomy. But restenosis was occurred just above of the cervical esophagocolostomy site several times of balloon dilatation were failed. So, we decided to use of the free jejunal graft. The free jejunal graft was isolated about 15cm length with it`s vascular arcades. The graft was irrigated with the mixed solution as isotonic saline, heparin and papaverine chloride. The artery of graft was anastomosed to the branch of the external carotid artery in end to side with continuous sutures of the 8.0 Prolene. The vein of the graft was anastomosed to the branch of the anterior facial vein in end to end with continuous sutures of the 8.0 prolene. Postoperative course was uneventful and the patient was discharged after removal of the tracheostomy cannula and gastrostomy tube.

Fistulas between the Esophagus and Adjacent Vital Organs in Esophageal Cancer

  • Esophageal fistulas may occur in an advanced stage or as a potentially life-threatening complication of treatment. They can be divided into esophageal-respiratory and esophageal-aorta fistulas. The diagnosis is confirmed with fluoroscopy using dilute barium oral contrast, followed by thin-section computed tomography, which defines the precise location and extent of the fistula. Flexible esophagoscopy and bronchoscopy are required for confirmation and anatomic assessment of the suspected fistula and provide additional information for treatment planning. Contamination is traditionally controlled by surgical exclusion, along with a jejunal feeding tube. Currently, fully covered self-expanding metal stents are the primary treatment option.

사각 형상 플랜지 형성을 위한 액압성형시 공정 조건에 따른 성형특성 (Effect of Process Parameters on Hydroforming Characteristics of a Rectangular Shape Flange)

  • Hydroforming has attracted the attention of manufacturing industries for vehicles and transportation systems. A wide range of products such as subframes, camshafts, radiator frames, axles and crankshafts are made by the hydroforming process. Hydroformed parts often need to be structurally joined to other components during assembly. Therefore it is useful if the hydroformed automotive parts can be attached with a localized flange. In this study, a hydroforming process to produce a rectangular shape flange is proposed. FE analysis to form the flanged rectangular shape was performed by Dynaform 5.5. The hydroforming characteristics at various die aspect ratios and feeding conditions were analyzed and optimal process conditions which can avoid defects are suggested. For validation purposes, hydroforming experiments to form the flange were conducted with the optimized conditions. The results show that the flanged parts can be successfully formed with a hydroforming process without additional processing steps.

Rectovaginal Fistula and Atresia Ani in a Kitten: A Case Report

  • A 2-month-old intact female domestic short hair presented with physical abnormalities, including discharge of watery feces from the vagina, depression, and dehydration. The rectovaginal fistula and atresia ani type 3 were diagnosed on the vaginogram. During the anoplasty, the fistula could not be identified because the patient was too small. Instead of colotomy, feces were flushed out using a feeding tube. To prevent complications, a 1-cc syringe tip was sutured at the surgical site after the anoplasty. At the 3-month telephone follow-up after surgery, the cat was reported to have no further clinical signs.
