Effectiveness of Additional Acupuncture Therapy for Lung Cancer Patients - A single group before-and-after, preliminary study - (폐암 환자에 대한 부가적인 침치료 효과 - 단일군, 전후, 예비임상 연구 -)

  • This study is a single group before-and-after study aimed to evaluate the effectiveness of additional acupuncture therapy for lung cancer patients being treated with chemotherapy. Acupuncture therapy was conducted twice a day at least for a week. The effectiveness outcome measures were the Korean Functional Assessment of Cancer Therapy (FACT-L) and the Korean Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) at the baseline, discharge and four weeks after discharge, and visual analogue scale(VAS) checked daily from the baseline to discharge. Ten patients were enrolled, one patient had excluded due to acute exacerbation of lung cancer. The data of nine patients showed some improvement in all of FACT-L, FACIT-F and VAS, however, there were no statistical significance. Additional acupuncture therapy for lung cancer patients treated with chemotherapy might be beneficial to improve the quality of life(QoL) and to improve fatigue. Prospective trials with complex traditional Korean medicine interventions are needed to further investigation.

Aerodynamic and Structural Design of A High Efficiency Small Scale Composite Vertical Axis Wind Turbine Blade (복합재가 적용된 고효율 소형 수직축 풍력터빈 블레이드의 공력 설계 및 구조 설계에 관한 연구)

  • Recently, the wind energy has been widely used as a renewable energy resource due to lack and environmental issues of the mostly used fossil fuel. This work is to develop a 500W class blade design of vertical axis wind turbine system which will be applicable to relatively low speed region like Korea and for the domestic use. For this wind turbine a high efficiency and low noise turbine blade was designed with the proposing aerodynamic design procedure, and a light composite structure blade. Structural analyses were performed using the Finite Element Method and fatigue life of the designed blade is estimated. Finally, in order to check its performance, the manufactured blade was tested by using truck and the results of test was good with respect to its analysis result.

The Analysis of Research in Arthritis (관절염에 관한 국내 간호학 연구 논문 분석)

  • The purpose of this article was to identify trend in Arthritis research and to provide ba an approach of Arthritis research. The results of analysis of 69 Arthritis articles public year of between 1970-1999 found that most articles(27.6%, n=19) were published since few(10.1%, n=7) were before 1994. Vast majority of articles(79.8%, n=55) were printed in The Journal of Rheumatology Health, and 79.8%(n=55) of those were quantitative research, 8.7%(n=6) were qualitative research, and 14%(n=1) was methodological research. 29.0%(n=20) of quantitative research were carried in experimental design and over half(50.8%, n=35) in nonexperimental design. Having concerned the types of treatment(or intervention) used in experimental study, the most common treatment was 'exercise' (45.0%, n=9), and other less common were 'self-help education(15.0%, n=3), home care(10.0%, n=2) supportive care(10.0%, n=2), flexibility exercise combined with local heat therapy(5.0%, n=1), relaxation training(5.0%, n=1). Of the types of exercise, aquatic exercise was 30.0%(n=6). The effect of exercise on outcome variable was found to be most strong among the other types of treatment. 18 articles of nonexperimental research were correlational research. From the analysis of correlational research, 29 concepts were identified, and the relationship between depression and other variable were strongly reported. In descriptive studies, most commonly physiological variables were examined in 5 articles(29.5%). Only one study(5.9%) explored patient's perception of the causal factors and those causality. In qualitative researches, two study were carried out by the means of grounded theory. Majority of those subjects were RA patients, and the largest number of sample were 23 and the smallest was 1 In two studies, patient's perceived causality of RA was identified as a research problems, and an experience of ADL, fatigue Phenomenon, an experience of having a total knee replacement with osteoarthritis were each found in one study. Methodological research was carried out to test validity and reliability of the RA Fatigue Scale.

The Effects of Work-Related Stress on Workers who are Engaged in the Medium Enterprises (직무 스트레스가 중소기업에서 근무하는 작업자에게 미치는 영향)

  • This study aims at providing some basic material to be made use of for establishing plans through which one can reduce job-related stress, through conducting a survey on the sources of stress and then distinguishing its symptoms by the sources. As the methods of the study, we conducted a questionnaire survey of some selected medium enterprises' employees in the National Capital region and analysed the results. The questionnaire was designed by using the dynamic stress model of Cooper and Eaker and consisted of 85 questions to examine the sources of stress, the characteristics of personalities, and symptoms by each type of stress. The total valid respondents were 392 persons indicating 65.3% of the response rate. For analysing the results, a rating scale was used, and the reliability and validity were tested by using Cronbach's alpha coefficient. As the result of the study, those workers in medium enterprises were found to be put under much stress by responsibility, physical environment factors and career development factors and so on. As physical symptoms of job-related stress, eye fatigue, stiffness in their shoulders, headache, worry or anxiety, and waist ache presented most in the respondents. In addition, as behavioral symptoms, chronic fatigue was most indicated. In organizational symptoms, dissatisfaction about the company and lack of self-motivation for the job performance were found to show most. Moreover, the results indicated that the closer personalities to those appearing in persons mostly of type A they had, the more they were put under intensive stress by the vagueness of their roles and responsibility among all the job characteristics.

Evaluations of Structural Performance of Recycled Aggregate Concrete According to Replacement Ratios (치환율에 따른 순환골재 콘크리트의 구조성능 분석)

  • This study is a fundamental research in order to establish the design code of recycled aggregate concrete structure. The structural properties of recycled aggregate concrete such as flexure, shear, fatigue, compression, and bond development are experimentally investigated and confirmed. In this study, laboratory-scale reinforced concrete beam, column, and pull-out test specimens using recycled coarse aggregate are manufactured. Then, the structural performances of recycled aggregate concrete according to replacement ratios of recycled coarse aggregate are evaluated. Also, finite element analysis using commercial code DIANA is carried out to predict the test results and the analysis results are compared with test results in this study. Structural test results showed that the structural performances of recycled aggregate concrete specimens with 60% replacement ratio are reduced by approximately 15-20%. These results indicated that the replacement ratio of recycled coarse aggregate within 30% is a suitable to use for structural members. The results of finite element analysis showed that the specimens with 30% replacement ratio possessed similar or more excellent structural performance than normal concrete specimens. However, recycled aggregate concrete with 60% replacement ratio of recycled coarse aggregate must be carefully considered for structural applications due to significant decrease of the failure loads.

Lifetime Estimation of an Axle Drive Shaft by Calibrated Accelerated Life Test Method (CALT 방법을 이용한 액슬구동축의 수명 예측)

  • In this paper, a method to predict the fatigue life of an axle drive shaft by the calibrated accelerated life test (CALT) method is proposed. The CALT method is very effective for predicting lifetimes, significantly reducing test time, and quantifying reliability. The fatigue test is performed by considering two high stress and one low stress levels, and the lifetime at the normal stress level is predicted by extrapolation. In addition, in this study, the major reliability parameters such as the lifetime, accelerated power index, shape parameter, and scale parameter are determined by conducting various experiments. The lifetime prediction of the axle drive shaft is verified by comparing the experimental results with load spectrum data. The results confirm that the CALT method is effective for lifetime prediction and requires a short test time.

Occurrence of Cognitive and Neurological Symptoms in Norwegian Dentists

  • Objectives: Previous investigations have presented some evidence of late cognitive effects in dental personnel exposed to metallic mercury. We wanted to examine if Norwegian dentists have an increased prevalence of symptoms consistent with neurological and/or cognitive malfunction. Methods: The study group consisted of 406 dentists from central Norway and 217 controls from the general population, all under the age of 70. They had responded to a standardised postal questionnaire (Euroquest) inquiring about seven symptoms in regard to neurology, psychosomatics, memory, concentration, mood, sleep disturbances, and fatigue. A score was calculated for each symptom based on 4 to 15 single questions scored on a scale from 1 (seldom or never) to 4 (very often). Results: The dentists and controls had a participation rate of 57.2 % and 42.9 % respectively. The dentists reported no more cognitive symptoms than the controls, with low average symptom scores from 1.16 for neurological symptoms in males to 1.73 for fatigue in females. Corresponding figures for the controls were 1.22 and 1.77. There were a total of 1.2 % of the dentists and 1.8 % of the controls who reported having three or more of the seven symptoms "often" or more frequently. Conclusion: Norwegian dentists do not report more cognitive and neurological symptoms than controls from the general population.

A Case Report of a Mitral Stenosis Patient Treated with Korean Medicine (승모판 협착증 환자에 대한 한방치험 1례)

  • Introduction: This study aimed to report on the effectiveness of using Korean medicine to improve symptoms of mitral stenosis. While some patients that do not require surgery due to mild mitral stenosis are treated with medication, the medication does not always alleviate the symptoms. Thus, we administered Korean medicine treatments to those patients because there are not many case reports on using this approach to treat symptoms of mitral stenosis. Case Presentation: A 48-year-old male patient with mitral stenosis complained of symptoms, such as palpitation, dyspnea, chest pain, fatigue, and swelling in both legs. To reduce the patient's symptoms, we administered Korean medical treatments, including herbal medicine (Samulanshin-tang-gamibang) and acupuncture. To evaluate the results, we used the numeric rating scale (NRS) and the New York Heart Association (NYHA) functional classification. After 17 days of treatment, the NRS score decreased for palpitation (8→2), dyspnea (8→1), chest pain (5→0), fatigue (96→2), and swelling in both legs (6→2). The NYHA class also changed from Class II to Class I. No adverse reaction was noted. Conclusion: According to the results, Korean medicine treatments can be considered to be effective for mitral stenosis patients. To confirm this finding, a prospective study is needed in the future.

A Case Report of Primary Peritoneal Carcinoma Patient Treated with Paclitaxel/Carboplain Regimen Combined with Traditional Korean Medicine on the Improvement of Quality of Life (Paclitaxel/Carboplain과 한의 치료를 병행한 원발성 복막암종 환자의 삶의 질 개선에 대한 증례보고)

  • Objective: The purpose of this study is to report the therapeutic possibility of a primary peritoneal carcinoma (PPC) patient with Traditional Korean Medicine (TKM). Methods: A 37 years old female patient diagnosed with PPC suffered from operation site pain, neck pain, nausea, headache and fatigue after undergoing operation and chemotherapy (Paclitaxel/Carboplain). The patient was treated with acupuncture, pharmacoacupuncture, moxibution and herbal medicine. Laboratory study and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety and adverse events of treatments. The performance status was measured by Eastern Cooperative Oncology Group (ECOG) scale. Results: After treatments, cancer did not show any progression. Related symptoms such as postoperative pain, nausea, fatigue and quality of life have improved. Conclusion: This case study implies that TKM may have therapeutic possibility in treating PPC patient.

A Case Report on Opioid-Induced Constipation in a Patient with Cancer Treated by Jowiseunggi-tang (조위승기탕으로 호전된 암 환자의 마약성 진통제로 인한 변비 치험 1례)

  • Objective: Jowiseunggi-tang (JWSGT) is a traditional herbal medicine commonly used for purgative activity in constipation. This study evaluates JWSGT for the treatment of opioid-induced constipation (OIC), the most common and debilitating gastrointestinal effect of opioid use. Methods: A 64-year-old man with floor of mouth cancer was hospitalized for OIC, and JWSGT was administered orally twice a day for 10 days, along with acupuncture, moxibustion, and cupping. The primary outcome measures were defecation type according to the Bristol Stool Form Scale (BSFS) and the frequency of bowel movements. The Brief Fatigue Inventory (BFI) and the Functional Assessment of Cancer Therapy-General (FACT-G) were used as secondary measures. Results: After three days of JWSGT administration, spontaneous bowel movements were observed two to three times per week with improved BSFS from type 2 to 4. A reduction in BFI score (8.7 to 2.0) and an increase in FACT-G score (44.3 to 59.0) suggested an improvement in fatigue level and quality of life. Conclusion: This is the first report to assess the efficacy of JWSGT for the management of OIC in patients with cancer, and JWSGT may be an effective option to improve symptoms and quality of life in this group.
