뇌교경색에 따른 Wallenberg 증후군 치험 1례에 관한 고찰 (Clinical Observation on 1 Case of The Wallenberg's syndrome Caused by The Pons Infarction)

  • Objective: The main symptoms of the Wallenberg's syndrome are ataxia of gait, clumsiness of ipsilateral limbs, nausea and vomiting, vertigo, visual disturbance such as difficulty in focusing blurred vision diplopia, numbness, dysphagia, hoarseness, hiccup, nystagmus, Homer's syndrome. The purpose of this paper is to report the patient with the Wallenberg's syndrome who was improved by oriental medical treatment. Methods: In terms of exuberance of Yang(vital function) of the liver and stagnation of the liver Ki(energy), we let the patient take Chunghunhwadam-Tang, Hwangryeonhaedok-Tang and Dangkwihwalhyeol-Tang, at the same time, treated with acupuncture, Sa-Am and Dong-Si acupuncture therapy twice a day. Results: We could know that symptoms of the Wallenberg's Syndrome were improved after observing left sensory paralysis was almost recovered, hiccup was completely healed and vertigo was reduced to 1/10. Conclusions: We confirmed that oriental drugs and acupuncture by differentiation of syndromes make the patient with the Wallenberg's Syndrome reduce and improve symptoms he has.

  • xWe studied about the causes of hypertension in the occidental and oriental medical records and the results are as follows. 1. The causes of hypertension in oriental medicine can be divided into wind, fire, phlegm and deficiency of Qi in our body, and broadly into three categories as emotions in excess, improper diet and damages of endogenous etiological factors. 2. Hypertension is the disease of undermining of fundamental aspects while prevailing incidental aspects and liver, heart and kidneys are the organs mainly related to hypertension. 3. The main pathological factors of hypertension are as follows; the disorder of Yin and Yang makes the situation of the deficiency of the vital essence of kidneys and the exuberance of the vital essence of liver and they are the fundamentals to be taken hypertension and finally make the state of the deficiency of both Yin and Yang. 4. In the point of occidental medical view, the essential hypertension is a disease of unknown etiology and we think it occurs not on the only one factor but on the various factors and secondary hypertension occurs from the diseases of the substances and the vessels of kidneys.

주의력결핍 과잉운동장애에 대한 한의학적 접근 (Oriental Medical Approach to Attention-deficit/hyperactivity disorder(ADHD))

  • Attention-deficit/hyperactivity disorder(ADHD) is one of the most common childhood-onset psychlatric disorders. It is distinguished by symptoms of inattention, hyperactivity, and impulsivity. ADHD may be accompanied by learning disabilities, depression, anxiety, conduct disorder, and oppositional defiant disorder. The etiology of ADHD is unknown, and the disorder may have several different causes. Individual with ADHD present in childhood and may continue to show symptoms as they enter adolescence and adult life. Public interest in ADHD has increased along with debate in the media concerning the diagnostic process and treatment strategies. The purpose of this study is oriental medical approach to ADHD. This study was progressed for oriental diagnosis and treatment for ADHD. In oriental medicine, the reason of ADHD was deficiency of the kidney, hyperactivity of the liver(腎虛肝亢), deficiency of the heart and the spleen(心脾不足), heart disturbed by phlegm and heat(痰熱擾心). The method of medical treatment was nourishing the kidney and checking exuberance of yang(滋腎潛陽), relieving mental stress and promoting wisdom(寧神益智), nourishing the heart and strengthening the spleen(養心健脾), tranquilzation(安神定志). removing heat-phlegm(淸熱化痰), inducing resuscitation and tranquilzation(開窮安神). The prescription was commonly used as Liuwei Dihuang Wan jiajian(六味地黃丸加減), Guipi Tang he Ganmai Dazao Tang jiajian(歸脾湯合甘麥大棗湯加減), Huanglian Wendan Tang jiawei(溫黃連溫膽湯加味). It should help primary care providers in their assessment of a common child health problem.

소양(瘙痒)의 병인.병기(病因.病機)에 대한 연구 (Study about the Cause and Mechanism of the Itching)

  • After studying the cause and mechanism of the itching with the perspective of chronic medical books including Neijing, and the annotators, the conclusions are as follows. 1. The itching, which has correlation with the heart, is the deficiency symptom(虛症). It is generated when the lung-metal(肺金) is infected by the fire pathogen(火邪) and developed to depression(鬱), because of the heart fire flaming upward of circuit year(歲火太過). The treatment is to make metal depression scatter(金鬱泄之). 2. The itching occurs between the dermis(膚) and the epidermis(皮). 3. The itching generates when the excess of yang exuberance(熱盛極) precede the transmission of the wind-heats(風熱), the wind-cold(風寒), and the wind-dampness(風濕). It causes the blood deficiency(血虛) and the blood stasis(血瘀) because of the skin dryness. 4. The itching usually generates during summer, causes the body fever(身熱) and the dermis pain(膚痛). When it becomes worse, it causes generalized itching and vesicles at the whole body(浸淫). 5. The itching is related to the spirit(魄), which works in unconsciousness and the movement of defense qi(衛氣) in night.

현운(眩暈)의 원인(原因)과 기전(機轉)에 대(對)한 문헌적(文獻的) 고찰(考察) (The bibliographical study on the cause and originative of vertigo)

  • This study has been carried out to investigate the cause and originative mechanism of vertigo by referring to 46 literatures. The results were as follows; 1. The 1st factors causing vertigo are exuberance of fire in the Liver (肝火偏亢). and ascending of Yang of Wind (風陽升動) resulting from thought excess (思慮太過) and melancholy (憂鬱). 2. The 2nd factors causing vertigo is a malnurtrient of the brain resulting from dispersion of the Liver function (肝血虛) and failure in ascending of the Clear Yang (淸陽不升) due to hemorrhage and so on. 3. The 3rd factors causing vertigo are failure in ascending to the Brain and deficiency of blood of the Liver (肝血虛) resulting from the injury of the essence of the Kidney (肝精虧損). 4. The 4th factors causing vertigo is a ascending of exogenous pathogenic factors (外邪) to the Brain on deficiency state. 5. The 1st factors causing vertigo are Stagnatum of clear Yang (淸陽不振) and pershing of Yang (亡陽) resulting from loss of water and damage of active thin body fluid (津氣虧損). 6. The obesity is beonged to excessiveness Symptom-Complex (實證) and the thin to deficiency Symptom-complex (虛證). 7. The vertigo is connective with the Live (肝), Spleen (脾) and the Kidney (腎), but among those, most intimative viscera is the Liver (肝).

중풍환자의 흉중번열증(胸中煩熱症)에 대한 양격산화탕(凉膈散火湯)투여 2례(例) (Two Cases of Chest Fever with Dysphoria in Stroke Patients Treated with Yangkyuksanhoa-tang)

  • Fire is known as one of the most common causes of stroke. Chest fever with dysphoria caused by heat in the stomach is one of the fire symptoms, meaning feeling oppressed in the chest. We observed two stroke patients who had chest fever with dysphoria. These patients' clinical symptoms were headache, thirst, heat in the upper part of the body and constipation. They were diagnosed as Chest fever with dysphoria. It is assumed that Exuberance of Yang causes heat. We treated them with Yangkyuksanhoa-tang(凉膈散火湯) based on Dong Ui Su Se Bo Won for about 2 weeks. During this period, we observed that their symptoms of heat in the upper part of the body and constipation had been improved. We suggest that Yangkyuksanhoa-tang makes the balance between water and fire, and improves chest fever with dysphoria of stroke patients.

양허증(陽虛證)의 임상적 질환 범위에 대한 고찰 (Study on Clinical Diseases of Yang Deficiency Pattern)

  • Yang deficiency pattern is a representative syndrome differentiation. This article is a study on to which categories of modern diseases yang deficiency is assigned by reference to modern clinical papers and the meaning of yang deficiency interpreted with a perspective of Korean Medicine and a modern perspective. Yang deficiency, yang qi deficiency, lack of yang qi and yang qi debilitation are the words found in "Nei Ching" and yang qi can be interpreted as something to warm, drive and arouse. Zhangzhongjing considered recovery or loss of Yang as the key to life in "Shanghanlun". Danxi proposed "Yang being liable to hyperactivity, Yin being insufficient" and emphasized pathological ministerial fire of Yang exuberance rather than physiological ministerial fire of Yang deficiency. Zhangjingyue proposed "Yang not being in excess, Yin being often deficient" and understood growth and decline of yin qi are all led by yang qi and put emphasis on true yin in addition to yang qi. Diseases of yang deficiency pattern are related with decline of metabolic level, hypofunction of internal secretion, disorder of immune function, disorder of automatic nerve system, sympathetic nerve inhibition, metabolic disorder of microelements, increase of cGMP, change of microcirculation, low speed of blood stream, kidney malfunction. Diseases related with kidney are sterility, polycystic ovary syndrome, spinal stenosis, edema, renal failure, IgA nephropathy, erectile dysfunction, nephritis, prostatitis, benign prostatic hyperplasia, decrease of adrenal cortical hormone by nephrotic syndrome, myelodysplastic syndrome. Disease related with heart are heart failure, arrhythmia, cardiomyopathy, atherosclerosis heart disease, hypertension, hyperlipidemia, pulmonary heart disease. Diseases related with spleen are irritable bowel syndrome, ulcerative colitis. Diseases related with liver are hypothyroidism, liver cirrhosis ascites, hepatitis B, chronic hepatitis, hepatic diabetes. Diseases related with lung are allergic rhinitis, cough variant asthma, bronchial asthma, pulmonary emphysema. And diabetes mellitus, metabolic syndrome, aplastic anemia, headache, encephalatrophy, Alzheimer's disease are also related with yang deficiency.

한약 및 침치료 후 피부습진의 SCORAD 점수를 호전시킨 성인 아토피성 피부염 환자 증례보고 (A Case Report of an Adult Atopic Dermatitis Patient Whose Eczema and SCORAD Index Were Improved by Herbal Medicine and Acupuncture Therapy)

  • This case reports the effect of herbal medicine and acupuncture therapy on the scoring atopic dermatitis (SCORAD) index for an adult atopic dermatitis patient. An adult male in his 30s with complaints of severe eczema on the whole body was diagnosed with heat toxin blazing exuberance pattern (熱毒內蘊) and treated with herbal medicine, including an orally-administrated decoction, externally-applied decoction, extracted powder capsule, and acupuncture for a total of 30 days. We observed skin symptoms, SCORAD index, follow-up photographs, and any adverse events. After treatment, SCORAD index decreased from 74.5 to 38.7 (51.9% from baseline). Skin symptoms such as redness, itching, and exudation were improved without any adverse events reported. This case suggests that herbal medicine and acupuncture therapy could be effective in improving eczema in adult atopic dermatitis patients.

신(腎)과 갑상선(甲狀腺)의 상관성(相關性)에 관(關)한 고찰(考察) (A Study on the relationship between Shin(腎) with Thyroid)

  • So far we oriental medical doctors have referred to Shin(腎) as endocrine system, especially suprarenal gland, sexual gland and autonomic nervous system, thyroid etc. as well as kidney. But the sight on thyroid is weak and the relationship with Shin(腎) hasn't been suggested clearly. The purpose of this study is to find out the relationship between Shin(腎) with thyroid. The following are the results. 1. Ki Gi(氣機) of Shin(腎) is similar to the fuction of thyroid in that they are the base of human metabolism, influencing all the metabolism of human tissue. 2. Shin Yang(腎陽) and thyroid hormone are almost identical in that they are the base of Yang Gi(陽氣), the former as source of heat energy, the latter as energy hormone. 3. Shin(腎) and thyroid hormone are almost the same in that they activate growth of human body. 4. Ki Gi(氣機) of Shin(腎) such as Ju Gol(主骨), Saeng Su(生髓), Tong uh Noi(通於腦) is similar to the effects of thyroid hormone on bones, central nervous system and hair 5. The symptoms of deficiency of Shin Eum(腎陰虛) are almost identical with those of hyperthyroidism, so the process of Yang Hwa Gi(陽化氣) caused by exuberance of Yang due to deficiency of Shin Eum(腎陰虛陽亢) is similar to excessive metabolism caused by hyperthyroidism. 6. The process of Eum Seong Hyung(陰成形) caused by preponderance of Eum due to deficiency of Shin Yang(腎陽虛陰盛) is similar to the lowering of metabolism caused by hypothyroidism. 7. Deficiency of Shin Eum(腎陰虛) is similar to hyperthyroidism, deficiency of Shin Yang(腎陽虛) to hypothyroidism. But there are major difference in edema and the fuction of intestine. To conclude, Ki Gi(氣機) of Shin is similar to the function of throid in many respects. I hope that there will be further studies on the relationship beteen thyroid malfuction with deficiency of Shin Yang(腎陽虛) or of Shin Eum(腎陰虛) in the future.

형방지황탕가미로 치료한 만성 난치성 설사 치험 3례에 대한 임상적 고찰 (Three Case Reports of Treatment of Chronic Intractable Diarrhea Using Hyungbangjihwang-tang-gami)

  • Objective: This study reports the effect of Hyungbangjihwang-tang-gami as a treatment for Soyang-in patients with chronic diarrhea. Methods: We treated three male patients with chronic diarrhea who did not improve after taking herbal medicine, as determined by zang-fu (organ) pattern diagnosis. After changing our approach to a Sasang constitution, we prescribed Hyungbangjihwang-tang-gami and assessed the effect by the frequency or form of defecation. Results: The patients' complaints worsened when they were administered a herbal medicine unsuited to their Soyang constitutions, which caused an exuberance of Yang. Changing the herbal medicine to Hyungbangjihwang-tang-gami improved their symptoms. Conclusions: This study suggested that Soyang-in patients may have side effects when administered unsuitable herbs, even if they were diagnosed using zang-fu (organ) pattern diagnosis. When we treat patients, we have to consider their Sasang constitution, as well as the zang-fu (organ) pattern diagnosis.
