자기결합방식의 이식형 인공중이의 제안 (Proposal of Magnetic Coupling Type Implantable Middle Ear Hearing Aid)

  • 본 논문에서는 회로가 간단하고 체내에 삽입되는 부분이 영구자석만.:-로 이루어져있는 새로운 방식의 이식형 인공중이를 제안하였다. 트랜스듀서의 갭 조정이 필요없어 수술이 간편해지며, 코일을 외부로 보이지 않게 옷 내부에 둘 수 있어서 미용적인 측면에서도 우수하다. 제안된 이식형 인공중이의 성능을 측정하고 난청자의 청각 역치에 맞는 시스템을 설계하기 위하여 트랜스듀서를 통해 내이로 전달되는 힘을 정량적으로 해 석하였으며, 진동특성 실험을 통해서 이 해석의 타당성을 증명하였다. 또한 사람의 측두골을 이용하여 직접 침골(incus)의 진동변위를 측정하였다. 이 실험으로 제안된 시스템이 실제로 사람의 몸에 설치되었을 때 이소골(ossicles)을 충분히 진동시킬 수 있음을 증명하였다.

외이염 발생견에서 고막평가를 위한 초음파 및 이경 검사법 비교 (Comparison of Ultrasonography and Video-otoscopy for the Evaluation of the Tympanic Membrane in Otitis Dogs)

  • The aim of this study was to compare ultrasonography with video-otoscopy for the evalution of the external ear canal and the tympanum. Fifty dogs with clinical signs ot the bilateral ear diseases were examined using otoscopy under general anesthesia. Ultrasonographic examination was performed with an 11 MHz linear-arrary probe and a 6.5MHz convex probe, with the external ear canal filled with saline. Saline functioned as a contrast media and enhanced image quality of ultrasonography. The images of 100 ears before and after the ear cleansing were obtained at otoscopy and ultrasonography. On the video-otoscopic images, the tympanic membrane was identified in 59 ears before the ear cleansing and 91 ears after cleansing. On the ultrasonography, the tympanic membrane was detected in 98 ears before the ear cleansing and in all ears after cleansing. With these results, it was concluded that ultrasonography with infusing saline into the ear canal, could be a useful diagnostic procedure to identify the tympanic membrane in otitis dogs.

3D CT Image Processing for 3D Printed Auricular Reconstruction of Unilateral Microtia Patient

  • Purpose Microtia is congenital anomaly of external ear and the reconstruction method for the external ear of microtia patient was based on autogenous costal cartilage framework. The application of 3D printing technique in medical science has made more possibility of human tissue restoration, and we tried to apply this technique in auricular reconstruction field. Materials and Methods As for unilateral microtia patient, the contralateral side ear is normal and reconstructive surgeon tried to mimic it for reconstruction of affected ear. So, we obtained facial CT scan of microtia patient and made mirror image of normal side ear. Moreover, to make the 3D scaffold based on the mirror image of normal ear and to apply this scaffold for the auricular reconstruction surgery, we included auriculocephalic sulcus and anterior fixation part. Results We could successfully obtain mirror image of normal ear, auriculocephalic sulcus and anterior fixation part for 3D scaffold printing. Conclusions Using this CT image processing and 3D printing technique, we will be able to make the scaffold for auricular reconstruction of unilateral microtia patient, and perform auricular reconstruction in near future.

Single Dose Topical Application of Clotrimazole for the Treatment of Otomycosis: Is This Enough?

  • Background and Objectives: To determine the efficacy of filling the external auditory meatus with 1% clotrimazole at a single visit for the treatment of otomycosis. Subjects and Methods: This prospective study included 40 patients who were referred to our clinic with complaints of ear itching, pain, and fullness, and were diagnosed with unilateral otomycosis. After cleaning the mycotic hyphae from the external auditory meatus, the ear canal was filled with 1% clotrimazole, using an intravenous catheter and syringe. The patients received follow-up examinations on post-treatment days 7, 15, and 45. Results: The follow-up otomicroscopic examinations revealed that 95% of the ear canals were entirely clean and that all symptoms had resolved. The post-treatment scores of pain, aural fullness and itching were significantly lower than the pre-treatment scores (p<0.01). Conclusions: Filling the external auditory meatus with 1% clotrimazole at a single visit is an easy, efficient, and cost-effective treatment for otomycosis. Additionally, high patient compliance makes this treatment superior to long-term topical therapy.

  • Congenital aural atresia is an ear malformation evident at birth, involving various degrees of failed external ear canal development. A true external ear canal is desirable, as devices that replace the canal are inconvenient and expensive. Therefore, an optimal surgical technique is required. Here, we review useful preoperative and operative techniques. Surgical correction is often not the preferred treatment; the hearing outcome is no better than the outcomes afforded by bone-conduction devices, and surgery may be associated with recurrence or complications such as meatal stenosis. Preoperative evaluation and appropriate management are important. Several means of preventing meatal stenosis are discussed in this review.

완전 이식형 인공중이용 마이크로폰의 위치 결정을 위한 물리적 귀 모델의 구현 (Implementation a Physical Ear Model for Determinating Location of the Microphone of Fully Implantable Middle Ear Hearing Device)

  • 일반적으로, 완전 이식형 인공중이의 이식형 마이크로폰은 측두골 부근에 이식되어진다. 이 경우, 마이크로폰의 진동막이 외부 충격이나 생체 노이즈의 영향 받을 수 있으며 이런 문제를 해결하기 위하여 이식용 마이크로폰을 외이도 내에 이식하는 방법을 고려할 수 있다. 그러나 이소골에 이식된 진동 트랜스듀서에 의해서 고막으로부터 외이도 방향으로 음향 반향이 발생할 가능성이 있다. 본 논문에서는 사람의 귀와 유사한 특성을 가지는 귀 모델을 제작하여 프로브 마이크로폰의 위치를 변경해가며 외이도의 음향 반향을 측정하였고 실험결과를 통하여 외이도 내에서의 마이크로폰의 이식 위치를 추정해 보았다. 분석 결과 외이도에 마이크로폰을 이식할 경우 고막에서 멀어질수록 반향 신호가 감소함을 확인할 수는 있었으나, 반향 신호를 완전히 제거할 수 없기 때문에 외이도에 마이크로폰을 이식하기에는 부적합한 것으로 예상된다.

완전 이식형 인공중이의 하이브리드 음향센서 특성 평가를 위한 귀 물리모델 (A Physical Ear Model for Evaluating Hybrid-acoustic Sensor Characteristics of Fully Implantable Middle-ear Hearing Aid)

  • In this paper, biomimetic based physical ear model proposed for measuring the characteristics of a hybrid-acoustic sensor for fully implantable middle-ear hearing aid. The proposed physical ear model consists of the external ear, middle-ear, and cochlea. The physical ear model was implemented based on the anatomical structure and CT images of the human ear. To confirm the characteristics of the ear model, the vibrational characteristics of the stapes was measured after applying sound pressure to the tympanic membrane. The measured results were compared with the vibrational characteristics of the human temporal bone specified by ASTM F2504-05. Through the comparison results, the feasibility of the proposed ear model was confirmed. Then, after attaching the hybrid-acoustic sensor to the ear model, the output characteristics of the ECM and acceleration sensor were measured according to the sound pressure. The measured results were compared with previous studies using human temporal bone, and the usefulness of the proposed physical ear model was verified through the analysis results.

『향약집성방(鄕藥集成方)·인후문(咽喉門)』의 외치법(外治法)에 관한 연구(硏究) (A Literature Review of External Treatment for Throat Disorders found in Hyang-Yak-Jib-Sung-Bang - Throat)

  • This literature review focuses on external treatments for the throat, as found in the classic Korean text, Hyang-Yak-Jib-Sung-Bang Throat. Sections of the text related to external treatment of throat disorders were reviewed. Examples of frequently used external methods include gargling, application of powder, and brushing of the throat. These methods are still used in modern Korean Medicine treatments for Ear-Nose-Throat (ENT) disorders. This discussion is provided with intention to invigorate the ancient practices of throat treatments, by highlighting benefits and results of the therapeutic protocols described in Hyang-Yak-Jib-Sung-Bang Throat. Discovering new effective herbal medicines and developing more precise external treatments will advance ENT treatment in Korean medicine.
