Studies on application of Radix Puerariae main blended Prescription from Donguibogam (동의보감(東醫寶鑑) 중(中) 갈근(葛根)이 주약(主藥)으로 배오(配伍)된 처방(處方)의 방제학적(方劑學的) 고찰(考察))

  • Through investigation on prescriptions from Dongeuibogam with Radix Puerariae as an ingredient, the following conclusions were reached. The 59 prescriptions with Radix Puerariae as an ingredient are mentioned from chapter of Pediatrics to chapter of Intestinal Convulsions. And 85% of those appears from chapter of Pediatrics to chapter of coughs. Prescriptions with Radix Puerariae as the main ingredient are used for skin diseases, cold, epidemics, abscess, diabetes, stroke, feverish diseases, intestinal diseases, weakness and cough. The dosage of Radix Puerariae varies from 0.875g to 11.25g. 3.75g was the most frequently used dosage which accounts 44%. And those with small dosage of 0.875 to 1.125g were for use on infants or pregnant women. The grounding pathology used for prescriptions with Radix Puerariae as an ingredient appeared to be mostly the pathology related to the Six External Qi. Among this, wind with cold(風寒), wind, cold with heat and humidity(風寒署濕), catching cold(傷寒), successively catching cold(兩感傷寒), cold of the Yangmyung and heat clogging(陽明傷寒火鬱), abnormal weather(時令不正) are the majority. Through the prescriptions with Radix Puerariae as an ingredient, it could be concluded Radix Puerariae that has various functions such as sweating and removing rash(發表透疹), exhalation and curing muscular ache(發散解肌), producing body fluid and ceasing thirst(生津止渴), opening abscess and healing the scar(托瘡生肌), removal of fever(解熱), resolving hangover(醒酒). Seungmagalguntang(升麻葛根湯) is used as the base prescription the most, and various prescriptions and herbs are combined according to the case.

  • Objectives : This study was aimed to investigate the methods for acupuncture and moxibustional treatment to insomnia Methods : The insomnia is classified by seven cause at main subject. According to this, arrange the acupuncture and moxibustional treatment for classical books, and explain eight meridian related to insomnia. Results : 1. Insomnia means deficiency of sleeping and it is the word generally used when a short period of sleeping, difficulty for deep sleeping and difficulty for recovery of original vital energy comes out. In oriental medicine, they understand that uneasiness condition occurred by abnormal operation of the internal organs as heart, liver, gallbladder, spleen, stomach, kidney, etc. caused by external affection or internal injury becomes Insomnia. 2. Cause of insomnia can be classified majorly as fire-transformation of liver and gallbladder , inner shaking of phlegm-heat , insufficiency of both the heart and the spleen, disharmony between heat and kidney, the dysfunction of the stomach, timidity of heart and gallbladder, the deficiency of Qi of the lung. They make insomnia with bad influence upon body and spirit. 3. The meridian system such as Heart Meridian of Hand Soeum, Pericardium Meridian of Hand Gworeum, Stomach Meridian of Foot Yangmyeong, Spleen Meridian of Foot Taeeum, Bladder Meridian of Foot Taeyang, Kidney Meridian of Foot Soeum, Gallbladder Meridian of Foot Soyang, Liver Meridian of Foot Gworeum and Lung Meridian of Hand Taeeum are used to treat insomnia. Sinmun acupoint and Naegwan acupoint are often used in particular because the they are good for calmming the spirit, the heart and purging the heart of (pathogenic) fire. 4. Especialy, Back-Su points of Bladder Meridian of Foot Taeyang was used by each causes. The Back-su Points was mainly used for heal the insomnia because the Back-Su points has good competent to control the ability of internal organs by direct effect to it. It is because the vitality flows through around back-Su. Conclusions : It comes to a conclusion as follows with research for relevancy of the main cause of insomnia and meridian system includes meridian point.

Multi-Residue Analysis of Fipronil and Its Metabolites in Eggs by SinChERS-Based UHPLC-MS/MS

  • A method for simultaneous detection of fipronil (F) and its metabolites fipronil desulfinyl (FD), fipronil sulfide (FS), fipronil sulfone (FSO) in chicken eggs was applied and validated. It includes single-step, cheap, effective, rugged, safe-based method (SinChERS) for sample preparation and ultra high performance liquid chromatography coupled with mass spectrometry (UHPLC-MS/MS) for chemical analysis. Results suggested that formic acid enhanced the recovery of 4 target residues and 1% supplementation to acetonitrile gained higher recoveries than that of 5%. SinChERS integrated extraction and clean-up steps into one, with shorter time (1.5 h) to operate and higher recoveries (97%-100%) than HLB, Envi-Carb-NH2 and quik-easy-cheap-effective-rugged-safe method (QuEChERS), and it consumed the smallest volume of extracting solvent (10 mL) as QuEChERS. Quantitative analyses using external standard method suggested the linear ranges of 4 target compounds were 1-20 ㎍/L with R2 >0.9947. The limit of detection (S/N>3) and quantification (S/N>10) were 0.3 ㎍/kg and 1 ㎍/kg. Recoveries ranged from 89.0% to 104.4%, and the relative standard deviations (n=6) at 1, 10, and 20 ㎍/kg were lower than 6.03%. Thirty batches of domestic eggs (500 g each) were detected by the established SinChERS-based UHPLC-MS/MS and no target residues were detected in all samples. The method developed in this study is a rapid, sensitive, accurate and economic way for multi-residue analysis of fipronil and its metabolites in eggs.

Safety and antifatigue effect of Korean Red Ginseng capsule: A randomized, double-blind and placebo-controlled clinical trial

  • Background: In physical activity or labor, the human body is in a state of high intensity stress, and all parts or physiological functions of the body respond positively to maintain or balance the need for movement. The human body has many physiological changes in the process of movement, and fatigue is the external manifestation of various complex changes inside the human body. Fatigue is also a physiological mechanism of self-protection after the body reaches a certain level of activity, which can prevent the occurrence of life-threatening excessive functional failure. The generation of fatigue is a very complex process, and its mechanism has not been concluded yet. Therefore, it is an important work to search and screen the effective components of natural plants that have anti-fatigue effect and to explore their mechanism. Methods: This was a 8-week, randomized, double-blind, placebo-controlled clinical trial. A total of 110 subjects who passed physical examination were included according to the scheme design, and randomly divided into a test group which was given KRG and a placebo control group. The calculation is carried out according to the standard of sub-high-intensity exercise test. Results: There was no adverse effect on safety index of subjects after taking red ginseng capsule. After KRG treatment, subjective strength grade is significant lower than placebo treatment. Blood lactic acid content is significantly get lower after trial in KRG group, and significant lower than placebo group. Creatine phosphokinase(CK) content is significantly get lower after trial in KRG group, and significant lower than placebo group. Conclusion: According to the criterion in the test scheme, the result shows that KRG is helpful on relieving physical fatigue.

  • While both functional verification and performance evaluation of a system are necessary for the development of effective and reliable communication systems, they have been usually performed individually through functional modeling with formal language tools and performance modeling with professional network performance evaluation tools, respectively. However, separate and duplicated modeling of a system may cause increase of the cost and inconsistency between the models. In order to overcome this problem, this paper proposes an integrated design technique that estimates the performance of a communication protocol designed in SDL with SDL-OPNET co-simulation. The proposed technique presents how to design a co-simulation system with the environment functions of Tau and the external system module of OPNET. InRes protocol was used as an example to show the applicability and usefulness of the proposed technique.

  • As well-known, the extended Kalman filter (EKF) is a powerful tool for parameter identification with limited measurements. However, traditional EKF is not applicable when the external excitation is unknown. By using least-squares estimation (LSE) for force identification, an EKF with unknown input (EKF-UI) approach was recently proposed by the authors. In this approach, to ensure the influence matrix be of full column rank, the sensors have to be deployed at all the degrees-of-freedom (DOFs) corresponding to the unknown excitation, saying collocated measurements are required. However, it is not easy to guarantee that the sensors can be installed at all these locations. To circumvent this limitation, based on the idea of first-order-holder discretization (FOHD), an improved EKF with unknown input (IEKF-UI) approach is proposed in this study for the simultaneous identification of structural parameters and unknown excitation. By using projection matrix, an improved observation equation is obtained. Few displacement measurements are fused into the observation equation to avoid the so-called low-frequency drift. To avoid the ill-conditioning problem for force identification without collocated measurements, the idea of FOHD is employed. The recursive solution of the structural states and unknown loads is then analytically derived. The effectiveness of the proposed approach is validated via several numerical examples. Results show that the proposed approach is capable of satisfactorily identifying the parameters of linear and nonlinear structures and the unknown excitation applied to them.

A Study on the Therapeutic Usage of Natural Pigments in the Joseon Period (조선시대에 사용된 천연염료의 한의학적 활용 가능성 고찰)

  • Objectives : This study examined the possibility of therapeutic application of natural pigments in the Joseon Period other than aesthetic purposes. Methods : The medicinal properties of natural pigments as written in the texts Sangayorok, Gyuhapchongseo, Imwongyeongjeji were searched and analyzed in the texts Bencaogangmu, Encyclopedia of Eastern Medicine, and the Encyclopedia of Chinese Medicinals focusing on their Qi, flavor, effects and application. Of the conditions they treated, we examined whether they treated skin conditions using external application. The Korean Medical effects of these pigments were examined based on the content of the skin chapter of the Donguibogam. Results : We were able to extract 47 kinds of natural pigments, most of which had cold nature and bitter tasting properties. In addition, most of the pigments had the effect of treating skin conditions, illnesses due to micro-organisms, and of detoxifying. Conclusions : After consideration of medicinal properties and effects of the herbs that were used as pigments in the Joseon period, it could be concluded that as these herbs not only function as pigments but also hold properties that prevent skin conditions, their therapeutic functions could have been considered when using them as pigments.

Research Trends of the Traditional Korean Medicine Treatment for Atopic Dermatitis -Based on the Journal of Pediatrics of Korean Medicine- (아토피 피부염의 한의학적 치료에 대한 연구 동향 -대한한방소아과학회지를 중심으로-)

  • Objectives The goal of this review was to investigate clinical, experimental and literature studies of Korean medicine on atopic dermatitis (AD) of Korean children in Korean medicine literature, seeking for the better research methods for more effective treatments. Methods Electronic investigations were practiced on AKOP (The Association of Korean Oriental Pediatrics, http://www.akop.or.kr) to collect theses which were published by J Pediatr Korean Med. The key word 'Atopy' was used for searching to ensure that every related thesis was collected. The publication date was limited from 1996 to 2016. The selected literatures were assessed mainly according to distributions of study type, publication year, scale, treatment and its efficacy. Results 55 papers were selected from 59 studies. In these collected 55 related theses, there were 29 experimental studies (52.73%), 18 clinical studies (32.73%), 8 review studies (14.55%). In the 29 experimental studies, there were 20 studies (68.97%) using NC/Nga mouse as subjects. The mite antigen was used to trigger AD by 8 studies (27.59%). The studies have been utilizing DNCB and DNFB instead of mite antigen since 2011. All the experimental studies showed that the Korean medicine was effective in AD. In total 18 clinical studies, 3 studies (16.67%) were case reports, only 1 thesis (5.56%) was conducted by case control study and the rest were case series studies. 7 studies (38.89%) of 18 studies diagnosed AD according to Hanifin and Rajka's diagnostic criteria (1980), only 1 study (5.56%) according to the Korean standard and the rest 10 studies (55.56%) didn't mention diagnostic criteria. 12 clinical studies (66.67%) showed efficacy in treating AD. There were 10 studies (55.56%) only employing internal treatments and 1 thesis (5.56%) didn't mention the exact prescription. Naesowhajungtang-kamibang, Saenghyeoryunbue-um were used 3 times (16.67%) as the internal medicine respectively. 5 theses (27.78%) were combined with acupuncture therapy. Hegu (LI4), Sanyinjiao (SP6) were used in 4 theses, and Taichong (LR3), Quchi (LI11), Zusanli (ST36) in 3 theses. The number of acupoints chosen from The Spleen meridian of Foot-Taiyin was the largest. There were 8 review theses. 2 were about the foreign oriental medicine, 3 were about the external medicine or external treatment methods, and 3 other studies were about severity scoring systems, the methodological study in the latest clinical study, overview for pattern and results of herbal medicine-derived AD clinical researches. Conclusions The experimental studies and clinical studies showed the effectiveness of Korean medicine treatments. However, this study still needs improving by conducting more comparative studies and using better research methods, in order to find more effective treatments to improve clinical efficacy.

A literal study on the Gu-Chang (구창의 문헌연구)

  • Gu-Chang is a disorder characterized by recurring ulcers confined to the oral mucosa. Despite much clinical and research attention, the causes remain poorly understood. In this paper, we will compare Gu-Chang with Recurrent Aphthous Stomatitis(RAS) in order to know what is the similiarity between Gu-Chang and RAS. So we will arrange various oriental and western medical literatures which are important. As a result of arrangement of the causes, symptoms and therapys of Gu-Chang, we can conclude through the studies as follows. 1. The etiologies of Gu-chang are following. In the Sthenia syndrome, there are evil heat of external factor, heat of heart and spleen, insomnia, heat of upper warmer, stress and diet, heat of lung and heart, excessive heat of upper warmer, inappropriate food intake, heat conveyance of organ, heat of stomach merdian, moistured heat of spleen and stomach and stasis of liver energy. In the Asthenia syndrome, there are deficiency of stomach energy, deficiency of upper warmer leading to heat, deficiency of middle warmer leading to cold, deficiency of lower warmer leading to heat, deficiency of middle energy, deficiency of blood, decreased fire and deficiency of soil, yin fire of lower warmer, deficiency of heart yin, deficiency of spleen yin and deficiency of qi and blood. 2. In western medicine the causes of RAS is presumed as local, microbial, systemic, nutritional, genetic, immunologic factors. 3. Once Gu-chang is compared with RAS, in the deficiency of yin leading to hyperactivity of fire, deficiency of yin leading to floating of fire and stasis of liver energy, recurring of Gu-chang is similar to RAS. Although recurring of Gu-chang due to tripple warmer of excessive fire has no recurrance, since there are the degree of Pain, site of lesion, dysphagia etc, it is similar to major RAS. It is may be believed that Sthenia Gu-chang is similar to major RAS, shape of recurring, site of lesion, degrree of Pain and white color of Asthenia Gu-chang are similar to minor RAS, but there is no similarity concerning herpes RAS in the literatures that describe the symptoms. 4. Generally, the treatment of Gu-chang is divided into Asthenia and Sthenia Syndrome. The method of cure to Sthenia syndrome is heat cleaning and purge fire, Asthenia syndrome is nourish yin to lower and adverse rising energy and strength the middle warmer and benefit vital energy. 5. Following is the medication for Sthenia syndrome. Heat of heart and spleen is Do Jok San, Yang Gyek San, Juk Yup Suk Go Tang, evil heat of external factor is Yang Gyek San Ga Gam, Stasis of liver energy is Chong Wi Fae Dok Yum, moistured heat of spleen and stomach is Chong Gi Sam Syep Tang. The medication for Asthenia Syndrome is following. Deficiency of upper warmer leading to heat is Bo Jung Ik Gi Tang, deficiency of middle warmer leading to cold is Bu Ja Lee Jung Tang, deficiency of lower warmer leading to heat is Yuk Mi Ji Hwang Tang, deficiency of yin leading to hyperactivity of fire is Ji Baek Ji Hwang Hwan, deficiency of yin leading to floating of fire is Lee Jung Tang Ga Bu Ja Medicine for external use were Yang Suk San, Boo Wyen San, Rok Po San, Yoo Hwa San ate. 6. In western medicine, there is no specific treatment for RAS, and management strategies depend on dinical presentation and symptoms and includes antibiotics, oral rinses, glucocorticoids, immunomodulatory drugs, vitamines, analgesics, laser and antiviral agents.

Measurement of Nursing Service Quality using SERVQUAL Model (SERVQUAL 모델을 이용한 간호 서비스 질 측정)

  • This study is a descriptive analytic research measuring nursing service quality, using SERVQUAL model, to make fundamental data and strategies for nursing service improvement. Data were collected by self-reported questionnaire from 202 patients and 142 nurses, from June 7 to 14, 1999. The reliability of instrument were adequate(Cronbach ${\alpha}=.94$). SAS program was utilized for statistical analysis of collected data. The results were as follows; 1. There was a gab between patient's expectation and perception on nursing service(Gap B). Gap D was indicated an affecting factor to decide nursing service quality. Gap C was indicated an indirect affecting factor of nursing service quality. Because it was not statistically significant in total item analysis, but in individual item analysis, 7 items were appeared statistically significant. Gap A was not a gap occurrence factor of nursing service quality. 2. Focuses of nursing service quality improvement strategies were; (1) to direct qualitative improvement of nursing service in order to correspondence patient's nursing service expectation. (2) to make nurse's service activity modified because nurse's practice were not reached patient's expectation level. (3) to need internal, external factor analysis affecting nurse's service activity. 3. Nursing service quality was decided by rather environmental inappropriateness provided nursing service than itself. Therefore, to make nursing service quality improvement, it is required to improve nursing service environment. For this, followings are required; (1) to strengthen nurse's education on lower part of nursing service satisfaction and QI activities. (2) to balance demand and supply of nursing personnel. 3) to fix computerized system for reducing other duties weight except nursing care through analysis of nursing activity. (4) to construct rational cooperating system among related departments. 4. The important parts for nursing service quality improvement were indicated as follows: (1) Gap B: 'prompt reaction', 'examination symptom before patient's complaint', 'hearted nursing service reducing patient's dissatisfaction', 'explanation goals of nursing activities', 'having special Knowledge enough', 'maintenance position comfortably', 'management of patient's physical hygiene'. (2) Gap C: 'maintenance physical safety', 'explanation about hospital rules and facilities'. (3) Gap D: 'tender, safe injection and wound care'. Because above items are mostly improved through nurse's attitude change and quality improvement, it is required to establish nursing standardization and to strengthen nurse's clinical education. As the based on above results, followings are suggested; 1. SERVQUAL model is very useful to make strategies for nursing service quality improvement because it indicates multiple factors affecting hap occurrence. 2. At individual items analysis of Gap C, statistically significant 7 items appeared higher nurse's perception level than patient's perception level on nursing service were trouble perception level on nursing service quality improvement. So. it need further research to analysis about these difference occurring factors. 3. At analysis of Gap D, it is indicated that in nursing service performance process, multiple factors lowing nursing service quality were intruded. So it needs further research to analysis what these factors are and how each factors affect on nursing performance process. 4. nursing service quality measurement is changeable according to sample select time or sampled subject's characteristics. So to develope strategy for nursing service quality improvement is based on the results of periodical analysis.
