Usefullness of panoramic radiograph for the improvement of periodic oral examination (구강검진의 효과 증진을 위한 파노라마방사선사진의 필요성에 관한 연구)

  • Purpose : This study was designed to evaluate the efficacy and utility of panoramic radiograph for the improvement of the periodic oral examinations. Materials and Methods : Clinical examinations and panoramic examinations were done for the 242 subjects of oral examinations. The results of panoramic radiograph interpretation were compared with the clinical findings. Two questionnaires were created. One was carried out before the panoramic examination and the other done afterwards, to find out the subjects' cognition and satisfaction for the clinical and panoramic examinations. Results : 1. Panoramic findings showed a higher detection rate of 31.9% for periodontal diseases, and 23.1% for dental caries than clinical findings. 2. The additional abnormalities detected through panoramic examinations were impacted tooth in 81 subjects (33.6%), maxillary sinus abnormalities in 28 subjects (11.6%), condylar abnormalities in 5 subjects (2.1%), congenital and acquired dental anormalies in 59 subjects (24.5%), and other miscellaneous abnormalities in 34 subjects (14.1%). 3. 164 subjects (67.8%) were satisfied with the current periodic oral examination, and 75 subjects (31.1%) hoped for better accuracy. 4. In the first and second questionnaire, 154 subjects (67.0%) and 163 subjects (70.6%) responded respectively that panoramic examination was necessary, and 193 subjects (83.2%) responded that it actually helped. Conclusion : The panoramic examination was revealed to improve the effectiveness of the periodic oral examination and to increase the satisfaction of the subjects of examination.

  • Methodology to evaluate the effective doses to adults undergoing various diagnostic x-ray examinations were established by Monte Carlo simulation of the x-ray examinations. Anthropomorphic mathematical phantoms, the MIRD5 male phantom and the ORNL female phantom, were used as the target body and x-ray spectra were produced by the x-ray spectrum generation code SPEC78. The computational procedure was validated by comparing the resulting doses to the results of NRPB studies for the same diagnostic procedures. The effective doses as well as the organ doses due to chest, abdomen, head and spine examinations were calculated for x-rays incident from AP, PA, LLAT and RLAT directions. For instance, the effective doses from the most common procedures, chest PA and abdomen AP, were 0.029 mSv and 0.44 mSv, respectively. The fact that the effective dose from PA chest x-ray is far lower than the traditional value of 0.3 mSv(or 30 mrem), which results partly from the advances of technology in diagnostic radiology and partly from the differences in the dose concept employed, emphasizes necessities of intensive assessment of the patient doses in wide ranges of medical exposures. The methodology and tools established in this study can easily be applied to dose assessments for other radiology procedures; dose from CT examinations, dose to the fetus due to examinations of pregnant women, dose from pediatric radiology.

The Experience of Inservice Inspection for Yonggwang Nuclear Power Plant Unit 6 (영광 원자력발전소 6호기 가동중검사 수형 경험)

  • As the increase of the operation year of nuclear power plants, the probabilities of the degradation of the major facilities and materials in the nuclear power plants are increased. The integrity of those facilities shall be monitored and verified by the non-destructive examination methods with the regulation codes, so called inservice inspection(ISI). The ISI of Yonggwang unit 6 was performed in four different parts, 1) non-destructive examinations for the components, piping weldments and structures, 2) automated ultrasonic examinations for pressure vessels, 3) visual examinations for the interior structures of the reactor, 4) eddy current examinations for the steam generator tubes. As the results, there was no severe indication and all detected indications were evaluated as non-relavent. Especially for the examinations of the piping weldments, PD(Performance Demonstration) was applied as a W examination method defined in the 1995 edition of ASME Code Sec. XI. The implementation of the PD for the piping weld results in an improvement of the reliability of the UT examinations.

Establishment of Local Diagnostic Reference Levels of Pediatric Abdominopelvic and Chest CT Examinations Based on the Body Weight and Size in Korea

  • Objective: The purposes of this study were to analyze the radiation doses for pediatric abdominopelvic and chest CT examinations from university hospitals in Korea and to establish the local diagnostic reference levels (DRLs) based on the body weight and size. Materials and Methods: At seven university hospitals in Korea, 2494 CT examinations of patients aged 15 years or younger (1625 abdominopelvic and 869 chest CT examinations) between January and December 2017 were analyzed in this study. CT scans were transferred to commercial automated dose management software for the analysis after being de-identified. DRLs were calculated after grouping the patients according to the body weight and effective diameter. DRLs were set at the 75th percentile of the distribution of each institution's typical values. Results: For body weights of 5, 15, 30, 50, and 80 kg, DRLs (volume CT dose index [CTDIvol]) were 1.4, 2.2, 2.7, 4.0, and 4.7 mGy, respectively, for abdominopelvic CT and 1.2, 1.5, 2.3, 3.7, and 5.8 mGy, respectively, for chest CT. For effective diameters of < 13 cm, 14-16 cm, 17-20 cm, 21-24 cm, and > 24 cm, DRLs (size-specific dose estimates [SSDE]) were 4.1, 5.0, 5.7, 7.1, and 7.2 mGy, respectively, for abdominopelvic CT and 2.8, 4.6, 4.3, 5.3, and 7.5 mGy, respectively, for chest CT. SSDE was greater than CTDIvol in all age groups. Overall, the local DRL was lower than DRLs in previously conducted dose surveys and other countries. Conclusion: Our study set local DRLs in pediatric abdominopelvic and chest CT examinations for the body weight and size. Further research involving more facilities and CT examinations is required to develop national DRLs and update the current DRLs.

Changing Gadolinium-Based Contrast Agents to Prevent Recurrent Acute Adverse Drug Reactions: 6-Year Cohort Study Using Propensity Score Matching

  • Objective: To determine the preventive effect of changing gadolinium-based contrast agents (GBCAs) to reduce the recurrence of GBCA-associated acute adverse drug reactions (ADRs). Materials and Methods: This retrospective, observational, single-center study-conducted between January 2016 and December 2021-included 238743 consecutive GBCA-enhanced MRI examinations. We focused on a subgroup of patients who experienced acute GBCA-associated ADRs during any of these examinations and subsequently underwent follow-up GBCA-enhanced MRI examinations up until July 2023. The follow-up examinations involved either the same (non-change group) or different (change group) GBCAs compared to the ones that initially caused the acute ADR. Baseline participant characteristics, generic profile of the GBCAs, administration of premedication, history of prior ADR to iodinated contrast media, and symptoms of GBCA-associated acute ADRs were retrospectively analyzed. Multivariable logistic regression with generalized estimating equations and propensity score matching were used. Results: A total of 1042 instances of acute ADRs (0.44%; 95% confidence interval [CI]: 0.41%-0.46%) were reported. Three-hundred and seventy-three patients underwent GBCA-enhanced MRI examinations after experiencing GBCA-associated acute ADRs within the study period; 31.9% (119/373) reexperienced acute ADRs at any of the follow-up examinations. The ADR recurrence was significantly lower in the GBCA change group than in the non-change group according to multivariable logistic regression (adjusted odds ratio [OR]: 0.35; 95% CI: 0.13-0.90; P = 0.03) and analysis with propensity score matching (14.3% [6/42] vs. 36.9% [31/84], respectively; OR: 0.32, 95% CI: 0.11-0.94; P = 0.04). A history of an ADR to iodinated contrast media (OR: 1.14, 95% CI: 0.68-1.90; P = 0.62) and premedication (adjusted OR: 2.09, 95% CI: 0.93-4.68; P = 0.07) were not significantly associated with GBCA-associated acute ADR recurrence. A separate analysis for recurrent allergic-like hypersensitivity reactions demonstrated similar results (adjusted OR: 0.20, 95% CI: 0.06-0.65; P < 0.01). Conclusion: Changing GBCAs may reduce the risk of GBCA-associated acute ADR recurrence.

Acute Oral Toxicity of KDRD-002 in Rats (랫드에 대한 KDRD-002의 급성경구독성시험)

  • Acute oral toxicity studies of KDRD-002 (Coriolus versicolor polysaccharide :DDB= 19.2:1) were carried out in Sprague-Dawley rats of both sexes. In this study, we daily examined number of deaths, clinical signs, body weights and pathological examinations for 7 days after single oral administration of KDRD-002 with different dose levels. KDRD-002 did not show any toxic effect in rats and oral LD$_{50}$ value was over 3.25 g/kg in rats.s.

Changes in Blood Pressures, Blood Profiles and Physical Conditions among Adults in the Daejeon Area (대전지역 일부 직장인의 혈압, 혈액성분 및 체위의 변화)

  • The body mass index, blood pressures and blood profiles of adults, who worked at three colleges in the Daejeon area, were evaluated for use in developing well-balanced menus at their workplace cafeterias and/or in creating nutrition education programs tailored to their health statuses. The data analyzed were from physical examinations performed in 2002 and in 2004 by the National Health Insurance Corporation. Overall, levels of systolic and diastolic blood pressure and serum cholesterol were lower in the 2004 samples than in those of 2002, but most samples in 2004 showed higher levels of SGOT. In 2002, statistical differences occured in the levels of blood pressure and serum cholesterol among age groups, but not in 2004. And in 2004, the older the age group, the higher the BMI. Among subjects in their twenties, three variables (systolic and diastolic blood pressure, and serum cholesterol) showed statistical differences; this increased to four variables (systolic and diastolic blood pressure, blood glucose and serum cholesterol) among subjects in their thirties and to six variables (Hb, SGOT, systolic and diastolic blood pressure, blood glucose and serum cholesterol) among those in their forties, followed by a decline to four variables (systolic and diastolic blood pressure, Hb and serum cholesterol) among subjects in their fifties. In general, the results of physical examinations showed improvements in blood pressure and blood profiles in 2004 in comparison with results in 2002. The level of SGOT was the exception. Despite these improvements, of those employees who had physical examinations in 2004, $5.2\%$ had high blood glucose and $18.1\%$ hypertension, $27.9\%$ hypercholesterolemia, $8.3\%$ registered abnormal liver functions, and $31.3\%$ were classified as overweight and obese. Moreover, because some vulnerable blood profiles were found in the older age groups, various programs, including nutrition education, and/or development of healthful diets as well as excercise activities, may be needed within their workplaces.

A study on manganese health hazards among experienced welders (용접경력자의 망간에 의한 건강 장해에 관한 연구)

  • This study was conducted to evaluate the health hazards and to develop early diagnostic methods of the manganism in experienced welders and to know the meaning of signal intensities on the brain Magnetic Resonance images. It was carried out from December 1996 to february 1997 with 277 male welders, the duration of welding was at least 5 years or more. The study was consisted of a questionnaire, physical examination and measurements of blood & urine manganese concentrations. Brain Magnetic Resonance imaging was done on 19 study subjects by random sampling. As the duration of welding increases, the positive rates of clinical symptoms, neurological examinations and blood manganese concentrations were also increased. However, physical examinations and urine manganese concentrations were not statistically significant with the duration of welding. Authors couldn't observe any Parkinsonism-like diseases. There were statistically significant correlations between duration of welding and blood manganese concentration(r=0.16, p<0.01). There were not statistically significant correlations between duration of welding and urine manganese concentrations (r=0.06). There were statistically significant correlations between blood & urine manganese concentration(r=0.34, p<0.01). By viewing brain Magnetic Resonance images, 13 welders(68.4 %) among 19 welders were found to have signal intensities. The positive rates of clinical symptoms, physical examinations, neurological examinations and blood & urine manganese concentrations were not statistically different between those with signal intensities and those without signal intensities. We would like to suggest that some non-specific clinical symptoms and neurological signs are correlated with the duration of welding but any Parkinsonism-like diseases had not been observed with these welders. Next we suggest that the high signal intensities on TlWI of brain Magnetic Resonance images are not the sign of manganese intoxication but the sign of manganese deposition.

A Study on Activities of Diagnostic X-ray Examination(II) (X선진단(X線診斷) 부문(部門)에 있어서 업무량(業務量)에 관(關)한 조사연구(調査硏究)(II))

  • This study was carried out with statistical materials during the last two years of period from Jan. 1975 to Dec. 1976 which presented at radiologic department of 5 hospitals in Seoul City. The primary purpose of this study was to obtained more detailed informations related to the activities of radiologic technologists in diagnostic X-Ray examinations part and to provide some basic materials for managements in activities of then and manpower managements of their organization and practice. From the results of this study, the following conclusions were obtained [1] During two year from the January of 1975 to the December of 1976, total number of case in X-ray examinations were 464,830 case and 22,029 case in 1975 and 24,461 in 1976. And ratio of icreased in X-Ray examinations by year was 11.09 per cent. [2] Regarding the examined portion of X-Ray examination, a great propotion was chest examination as 56.88 per cent. [3] An average, the required time per case in X-Ray exam. was 9.28 minutes and make used of 1.94 sheets of X-Ray film per case in radiography. [4] An average, ratio of increased in X-Ray film by year was 12.71 per cent and ratio of failed film in it was 2.23 per cent. [5] The frequency rate of film size showed the highest distribution of $8"{\times}10"$(28.17%) and the highest distribution of X-Ray film by month was July(8.93%). [6] An average, the amount of activities per a diagnostic X-Ray equipment was 34.92 case and make used of 67.81 sheets of X-Ray film in a day. [7] The mean number of case in X-Ray examinations by radiologic technologists was 29.29 cases and make used of 56.87 sheets of X-Ray film in a day. Also, the average number of case was reading by radiologists was 32.42 case and 62.97 sheets of X-Ray film in a day.
