  • Effects of soil disinfection, fungicide application, and narrow ridge cultivation on ginger rhizome rot development were examined in two naturally-infested fields at Seosan, Choongnam province. Soil disinfection treatments were assigned to main plots, and fungicide and ridge treatments to sub-plots in a split plot design with three replications. The rhizome rot started in late July, and progressed rapidly until late September with the peak incidence in mid-august to early September. Soil disinfection by dazomet application showed the most prominent inhibition effects in both fields, where the disease was reduced by the treatment from 17.5% to 4.8% in one field, and from 51.0% to 2.2% in the other field. Three to five applications of fungicide metalaxyl-copper during the growing season inhibited the disease by 89.7% in one field, but less effectively in the other field. Narrow ridge cultivation reduced the disease effectively by 78.1% and 63.9%, compared to the unridged control plots in each field, respectively. Germination rate of seed-rhizomes and growth of ginger plants were similar between treatments, except when the plots received improper aeration after applying dazomet, and then the germination rate was significantly reduced. The greatest yields were obtained in the disinfected plots, regardless of rhizome rot incidence, except one control plot with very little disease. Ginger yield was negatively correlated with disease severity. However, the yield of ridge plots averaged 58∼59% compared to those of the unridged plots, due mainly to the half planting rate of the ridge plots. In spatial progress, the disease in the disinfected plots started from a single focus of the inoculum, and spread into the adjacent areas only, whereas in the untreated plots, the disease started from many foci that were distributed over the plot, and rapidly progressed to make an epidemic during the season. The soil density of P. myriotylum in the disinfected plots was not changed or, if not, increased slightly during the season. However, in the untreated plots it increased rapidly to reach the density 3 to 5 times greater by the end of the season.

Assessment of the Availability of Health Insurance Data for Epidemiologic Study of Childhood Aseptic Meningitis (소아 무균성 뇌막염의 역학적 연구를 위한 건강보험자료원의 유용성 평가)

  • Objectives : Aseptic meningitis is a major cause of Korean childhood morbidity late spring and early summer. However, the nationwide incidences of the disease have not been reported. This study was conducted to evaluate the availability of National Health Insurance data (NHID) for the study of an epidemiological trend in the surveillance of aseptic meningitis in children. Methods : All the claims, under A87, A87.8, and A87.9 by ICD-10, among children below 15 years of age, to the National Health Insurance Corporation, between January and December 1998, were extracted. A survey of the medical record of 3,874 cases from 136 general hospitals was peformed. The availability of the NHID was evaluated by the three following methods: 1) The diagnostic accuracy (the positive Predictive value : proportion of the confirmed aseptic meningitis among the subjects registered as above disease-codes in NHID) was evaluated through a chart review, and according to age, gender, month and region of disease-occurrence. 2) The distribution of confirmed cases was compared with the distribution of total subjects from the NHID, for subjects in General hospitals, or the subjects surveyed. 3) The proportion of confirmed CSF test was confirmed, and the relating factor, which was the difference in CSF-test rate, analyzed. Results : Among 3,874 cases, CSF examinations were peformed on 1,845 (47.6%), and the CSF-test rates were different according to the medical utility (admission vs. OPD visit) and the severity of the symptoms and signs. The diagnostic accuracy for aseptic meningitis, and during the epidemic (May-Aug) and sporadic (Sept-Apr) periods, were 85.0 (1,568/1,845), 86.0 (1,239/l,440) and 81.2% (329/405), respectively. The distributions by age, sex, month or period (epidemic/sporadic) and region, in the confirmed cases, were similar to those in the NHID, in both the subjects at General hospitals and in those surveyed, to within ${\pm}7%$. Conclusions : In this paper, the NHID for the subjects registered with an aseptic meningitis disease-code might be available for an epidemiological study on the incidence-estimation of childhood aseptic meningitis, as the NHID could include both the probable and definite cases. On the basis of this result, further studies of time-series and secular trend analyses, using the NHID, will be peformed.

Tuberculosis Surveillance and Monitoring under the National Public-Private Mix Tuberculosis Control Project in South Korea 2016-2017

  • Background: The national Public-Private Mix (PPM) tuberculosis (TB) control project provides for the comprehensive management of TB patients at private hospitals in South Korea. Surveillance and monitoring of TB under the PPM project are essential toward achieving TB elimination goals. Methods: TB is a nationally notifiable disease in South Korea and is monitored using the surveillance system. The Korea Centers for Disease Control and Prevention quarterly generates monitoring indicators for TB management, used to evaluate activities of the PPM hospitals by the central steering committee of the national PPM TB control project. Based on the notification date, TB patients at PPM hospitals were enrolled in each quarter, forming a cohort, and followed up for at least 12 months to identify treatment outcomes. This report analyzed the dataset of cohorts the first quarter of 2016 through the fourth quarter of 2017. Results: The coverage of sputum, smear, and culture tests among the pulmonary TB cases were 92.8% and 91.5%, respectively. The percentage of positive sputum smear and culture test results were 30.7% and 61.5%, respectively. The coverage of drug susceptibility tests among the culture-confirmed cases was 92.8%. The treatment success rate among the smear-positive drug-susceptible cases was 83.2%. The coverage of latent TB infection treatment among the childhood TB contacts was significantly higher than that among the adult contacts (85.6% vs. 56.0%, p=0.001). Conclusion: This is the first official report to analyze monitoring indicators, describing the current status of the national PPM TB control project. To sustain its effect, strengthening the monitoring and evaluation systems is essential.

  • Lead poisoning is one of the earliest identified and most known occupational disease. Its acute effects have been recognized from antiquity when this condition principally afflicted manual workers and slaves, actually scarcely considered by the medicine of that time. The Industrial Revolution caused an epidemic of metal intoxication, urging scientists and physician of that period to study and identify specific symptoms and organ alterations related to chronic lead poisoning. During the 20th century, the acknowledgment of occupational and environmental toxicity of lead fostered public awareness and legislation to protect health. More recently, the identification of sub-clinical effects have greatly modified the concept of lead poisoning and the approaches of medicine towards this condition. Nowadays, lead poisoning is rarely seen in developed countries, but it still represents a major environmental problem in certain areas. Consequently, it may appear as a paradigm of "occupational and environmental disease," and the history of this condition seems to parallel the historical development of modern "Occupational and Environmental Health" as a more complete medical discipline.

Structural Factors of the Middle East Respiratory Syndrome Coronavirus Outbreak as a Public Health Crisis in Korea and Future Response Strategies

  • The recent Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak has originated from a failure in the national quarantine system in the Republic of Korea as most basic role of protecting the safety and lives of its citizens. Furthermore, a number of the Korean healthcare system's weaknesses seem to have been completely exposed. The MERS-CoV outbreak can be considered a typical public health crisis in that the public was not only greatly terrorized by the actual fear of the disease, but also experienced a great impact to their daily lives, all in a short period of time. Preparedness for and an appropriate response to a public health crisis require comprehensive systematic public healthcare measures to address risks comprehensively with an all-hazards approach. Consequently, discussion regarding establishment of post-MERS-CoV improvement measures must focus on the total reform of the national quarantine system and strengthening of the public health infrastructure. In addition, the Korea Centers for Disease Control and Prevention must implement specific strategies of action including taking on the role of "control tower" in a public health emergency, training of Field Epidemic Intelligence Service officers, establishment of collaborative governance between central and local governments for infection prevention and control, strengthening the roles and capabilities of community-based public hospitals, and development of nationwide crisis communication methods.

Epizootiologic Surveys on Canine Parvovirus Infections of Dogs Kyungbuk West Area (경북서부지역에서의 개 파보바이러스 감염상황 조사)

  • The disease syndrome characterized by the acute vomiting and diarrhea with high mortality had been greatly epidemic in Kyungbuk West Area since March 1990 and it was followed serologically for the classification of the agent. The agent present in feces of dogs associated with this syndrome had characteristic feature in agglutinating pig red blood cells that was specifically inhibited by anti-CPV reference dog serum. This also showed the serological identity with the reference CPV antigen in Hemagglutinating inhibition test. The result obtained were summarized as follows : 1. During 5 years(March. 1990∼September. 1994), 1,470 dogs were investigated on the actual condition of CPV infections. The Infection rate of CPV from dogs was 62.5% and mortality rate was 59.8%. 2. Among 24 fecal samples collected from the dogs with enteric disease, all showed the hemagglutinating activity to porcine erythrocyte ranging from 40 to 5,120 of HA titers. 3. Among 12 sera samples collected from the dogs with enteric disease, all showed the serological identity with the reference CPV antigen from 5 to 5,120 of HI titers. 4. Bacteriologic examination of fecal specimens resulted in the isolation of pathogeric bacteria such as Staphylococcus sp, Streptococcus sp, Escherichia coli and Bacillus. Cultures for salmonella sp and Clostridium remaind negative. 5. The prevalence and identification of internal parasites were determined by fecal examination using the floatation methods. From 20 fecal samples 12(60.0%) were isolated and their species were Toxacara canis, Toxascaris leonina, and coccidium.

The Larval Trombiculid Mites of Korea (한국산 Trombiculid mites에 관한 연구)

  • In Korea ,little attention was paid to chiggers until 1950 so only to reports on four species of chiggers were pulished before Korean War. Since 1950 (beginning of Korean War), a marked progress in the study of chiggers has been made in connection with investigations of Epidemic hemorrhagic fever occurring among the United Nations troops which was suspected as a chigger-borne infectious disease and the first report of tsutsugamushi disease in Korea called natives attention to chiggers as the vector mites of this newly known disease in Korea. This paper is mostly based on specimens of author's collections from 3269 Rodentia Insectivora, 9 Chiroptera, 24 Aves , 35 Amphibia and 3 Reptilia during the time from December 1955 to December 1956 but four species were introduced here from works of others . There are reported here five species of chiggers previously known only out of Korea and a new species also. This new species was collected by author but Lipovsky informed his colleague had collected the same one in Korea and they would publish it as new one in near future. This is the reason of describing the new species without specific name . Of course, this paper is incomplete in view of the geographic distribution, seosonal change and host relation etc, but will serve as a brief summary of the chiggers fauna of Korea up to the present. Herbal Medicine for Coronavirus Disease 2019 (COVID-19) in Children: a Literature Review

  • Objectives Few studies to date reviewed herbal medicines prescribed to pediatric patients with coronavirus disease 2019 (COVID-19). We aimed to summarize and analyze these herbal medicines. Methods Nine databases were searched for studies published up until November 24, 2020. Nine case reports and one case series involving 17 pediatric patients were included in the study. The clinical symptoms, interventions, outcomes, pattern identification, composition, and frequency of herbal medicine administration were analyzed. Results The symptoms of pediatric patients with COVID-19 treated with herbal medicines were mild or moderate. Herbal medicines have been used to alleviate symptoms, such as fever and cough. No adverse events were reported. All of the patients who received herbal medicines were discharged from the hospital after symptomatic relief. Pattern identification revealed that the lungs were the most affected organs. Epidemic toxins, wind, dampness, and heat were the primary etiological factors. More pediatric patients with COVID-19 were prescribed a combination of herbal and Western medicines than herbal medicines alone. The frequently used herbs include Glycyrrhizae Radix et Rhizoma, Lonicerae Flos, Scutellariae Radix, Armeniacae Semen, Citri Reticulatae Pericarpium, and Astragali Radix. Conclusions Our review can serve as a reference for the treatment of COVID-19 in children.

Characteristics of Vibrio isolated from cultured file fish, Stephanolepis cirrhifer in Korea (우리나라 양식 쥐치, Stephanolepis cirrhifer에서 분리된 비브리오의 특성)

  • An epidemic was occurred in Stephanolepis cirrhifer during acclimation in laboratory tanks (water temperature was about $17^{\circ}C$). Diseased fish showed an unique external sign, large cloudy skin surface. We investigated the cause of the disease, and isolated one pure cultured bacterium, that was identified as Vibrio anguillarum by biochemical analysis and sequence analysis of the l6S rRNA and recA genes. During the outbreak, about 30% of S. cirrhifer showed the large cloudy skin surface, and approximately 10% of the stocked fish died. Fish were effectively cured by treatment with oxolinic acid.

Vulnerability Assessment for Public Health to Climate change Using Spatio-temporal Information Based on GIS (GIS기반 시공간정보를 이용한 건강부문의 기후변화 취약성 평가)

  • To prevent the damage to human health by climate change, vulnerability assessment should be conducted for establishment of adaptation strategies. In this study, vulnerability assessment was conducted to provide information about vulnerable area for making adaptation policy. vulnerability assessment for human health was divided into three categories; extreme heat, ozone, and epidemic disease. To assess vulnerability, suitable indicators were selected by three criteria; sensitivity, adaptive capacity, and exposure, spatial data of indicators were prepared and processed using GIS technique. As a result, high vulnerability to extreme heat was shown in the low land regions of southern part. And vulnerability to harmful ozone was high in the surrounding area of Dae-gu basin and metropolitan area with a number of automobiles. Vulnerability of malaria and tsutsugamushi disease have a region-specific property. They were high in the vicinity of the Dimilitarized zone and south-western plain, respectively. In general, vulnerability of human health was increased in the future time. Vulnerable area was extended from south to central regions and from plain to low mountainous regions. For assessing vulnerability with high accuracy, it is necessary to prepare more related indicators and consider weight of indicators and use climate prediction data based on the newly released scenario when assessing vulnerability.
