Effects of diffusional barriers on the extent of presystemic and systemic intestinal elimination of drugs

  • In the present study, a pharmacokinetic model to address the effects of the diffusional barrier between splanchnic bed and enterocytes on the extent of presystemic and systemic intestinal elimination of drugs was developed. The model is composed of five compartments, ie., gut lumen, enterocyte, splanchnic bed, liver and central compartments. The equations for various pharmacokinetic parameters important for estimating the quantitative differences between presystemic and systemic intestinal and hepatic elimination of drugs were derived. A simulation study demonstrated that the diffusions[ barrier present between splanchnic blood and enterocytes can have significant effects on oral bioavailability and systemic clearance of drugs. In conclusion, the model can be useful for a better understanding of the effects of diffusional barrier on the extent of administration-route dependent intestinal and hepatic elimination of drugs, especially those with high hydrophilicity and/or charge(s) under physiological conditions.

제주도의 돼지유행성설사 발생 상황과 병리학적 고찰 (Prevalence and pathologic study of porcine epidemic diarrhea in Jeju)

  • From April 2014 to September 2015, 153 piglets from 52 farms in Jeju were diagnosed with porcine epidemic diarrhea (PED). The major PED cases were focused on suckling piglets (144 piglets, 94.1%), particularly in 1-7-day-old piglets. Histopathologically, severe villous atrophy was observed in the small intestine, especially in the jejunum and ileum. The mean villous height to crypt depth ratios of the jejunum and ileum were 1.4:1 and 1.5:1, respectively. The major histopathologic findings of the small intestine were cytoplasmic vacuolation, cuboidalization, squamation, and exfoliation of the mucosal enterocytes in the villi. The cytoplasmic vacuolations in the enterocytes were the most prevalent lesions in the small intestine and were more severe in the ileum than in the jejunum. According to immunohistochemistry methods, the PED virus (PEDV) antigens were presented in the cytoplasms of the enterocytes, and were distributed more prevalently in the ileum than in the jejunum. PEDV antigens were also detected in the colon of 26 piglets (19.5%). Sequence comparison and phylogenetic analysis indicated that 12 PEDV had more than a 98.9% homology with each other. These PEDV strains were highly homologous with the genogroup 2 North American group.

  • In vitro intestinal uptake of non-soluble polystyrene microspheres (NPMS) was visualized with and without oleic acid using a fluorescence microscopy. Fluorescent polystyrene latex microspheres with 1${\mu}$m larger size were used as models for nonspecifically absorbed nonbiodegradable particulates. The NPMS could not penetrate the enterocytes but a few NPMS could be penetrated via Peyer's patches. When the oleic acid was mixed with NPMS, the transporting efficiency of NPMS through enterocytes as well as Peyer's patches was significantly enhanced. The modification of the intestinal membrane permeability and surface feature of the NPMS in the presence of oleic acid might be a clue to the transport of NSPM although the detailed mechanism is still under investigation.

  • The field of stem cell research has been rapidly expanding. Although the clinical usefulness of research remains to be ascertained through human trials, the use of stem cells as a therapeutic option for currently disabling diseases holds fascinating potential. Many pediatric gastrointestinal tract diseases have defect in enterocytes, enteric nervous system cells, smooth muscles, and interstitial cells of Cajal. Various kinds of therapeutic trials using stem cells could be applied to these diseases. This review article focuses on the recent achievements in stem cell applications for pediatric gastrointestinal tract diseases.

  • Absorption of fat-soluble vi tamin, retinol occurs mainly in the proximal part of small intestine. But its intestinal transport mechanism isn't yet clear. The aim of the present study was to investigate on the mechanism of absorption of retinol by determining a concentration-dependent kinetic of retinol absorption in rats. The study was carried out by applying in vivo technique in which vitamin solution was infused to intestinal lumen and at the same time thoracic duct and choledochus duct were canulated to collect samples. The investigations showed that retinol is absorbed in the small intestine by a saturable, carrier-mediated transport system, i.e. wi thout signi ficant differences between the proximal and distal halves of the small intestine. The transport of retinol taken up by the enterocytes occured via different mechanisms: while the main vitamin A transport via the thoracic duct was saturated by limiting transport factors such as retinol-CRBP-II-complex formation and retinol esterification with increasing substrate concentrations, the transport of retinol metabolite product via the portal vein was proportional to the substrate concentration.ration.

국내에서 분리된 돼지 유행성 설사증 바이러스 접종자돈의 병리학적 소견 (Pathological findings of experimentally infected piglets with a porcine epidemic diarrhea virus isolated in Korea)

  • Twelve colostrum-deprived piglets were inoculated oral1y with porcine epidemic diarrhea virus isolated from Korea. The piglets were euthanatized from at 12 hour postinoculation(PI) at 6-hour intervals. At 24 to 36 hours PI, all infected piglets showed severe yellowish to watery diarrhea, dehydration, depression, and anorexia. At necropsy, the intestinal wall looked thin, mesenteric vessels congested, and mesenteric lymph nodes edematous. The histological findings showed marked villous atrophy and fusion, severe degeneration of enterocytes and monomuclear cell infiltration in the lamina propria. On electron microscopy, villous epithelial cells of infected jejunum contained viral particles at 18 hour PI. The viral particles were pleomorphic spheres with a mean diameter of 95∼180nm including 18nm projections.

Principles of Physiology of Lipid Digestion

  • The processing of dietary lipids can be distinguished in several sequential steps, including their emulsification, hydrolysis and micellization, before they are absorbed by the enterocytes. Emulsification of lipids starts in the stomach and is mediated by physical forces and favoured by the partial lipolysis of the dietary lipids due to the activity of gastric lipase. The process of lipid digestion continues in the duodenum where pancreatic triacylglycerol lipase (PTL) releases 50 to 70% of dietary fatty acids. Bile salts at low concentrations stimulate PTL activity, but higher concentrations inhibit PTL activity. Pancreatic triacylglycerol lipase activity is regulated by colipase, that interacts with bile salts and PTL and can release bile salt mediated PTL inhibition. Without colipase, PTL is unable to hydrolyse fatty acids from dietary triacylglycerols, resulting in fat malabsorption with severe consequences on bioavailability of dietary lipids and fat-soluble vitamins. Furthermore, carboxyl ester lipase, a pancreatic enzyme that is bile salt-stimulated and displays wide substrate reactivities, is involved in lipid digestion. The products of lipolysis are removed from the water-oil interface by incorporation into mixed micelles that are formed spontaneously by the interaction of bile salts. Monoacylglycerols and phospholipids enhance the ability of bile salts to form mixed micelles. Formation of mixed micelles is necessary to move the non-polar lipids across the unstirred water layer adjacent to the mucosal cells, thereby facilitating absorption.

In Situ Hybridization에 의한 돼지 유행성설사증 (Porcine Epidemic Diarrhea)의 진단 (Rapid and Easy Detection of Porcine Epidemic Diarrhea Virus (PEDV) by in situ Hybridization)

  • Molecular diagnostic techniques have been used to identify porcine epidemic diarrhea virus (PEDV), a causative agent of acute enteritis in swine, but they were difficult to be petformed and time-consuming. To detect PEDV in a rapid and easy way, we developed biotinylated cDNA probe for N gene encoding the nucleoproteins of PEDV. Formalin-fixed and paraffin-embedded tissues from 24 naturally infected pigs were used for the experiment. The ISH produced a positive reaction in all cases. When intestinal tissues were hybridized with PEDV probe, strong signals were seen in the villus enterocytes of the jejunum and ileum. Hybridization signals were also found in the duodenum from one pig and in colon from dnother. In conclusion, ISH with a biotinylated cDNA probe was provided to be a useful diagnostic method for detecting PEDV effectively in routinely processed tissue sections.

Water Extract of Kudzu Root (Pueraria radix) Decreases Apolipoprotein B100 and B48 Production in Vitro

  • We have previously demonstrated that kudzu root extracts have a hypocholesterolemic effect on rats fed diets high in fat and cholesterol. To further elucidate the mechanism involved, in this study we investigated the effect of water extracts of kudzu root, Pueraria radix, on the production of apolipoprotein B$_{100}$ (APo B$_{100}$) in HepG$_2$ liver cells and secretion of apolipoprotein B$_{48}$ (Apo B$_{48}$) in Caco$_2$ cells. Human cell lines, HepG$_2$ liver cells and Caco$_2$ intestinal epithelial cells, were grown with various concentrations (0%, 0.5%, 1.0%, 1.5%, 2.0%) of water extracts of kudzu root in the media. The kudzu root extract decreased Apo B$_{100}$ production and secretion. Treatment of HeP G$_2$ cells with the kudzu root extract also significantly decreased the intracellular total and free cholesterol concentration, and also decreased esterified cholesterol but was only significant at the highest dose of 2%. Apo B$_{48}$ production, but not secretion, from enterocytes was lowered by the kudzu root extracts. This research provided evidence that the hypocholesterolemic properties of kudzu root may be a consequence of decreased production and secretion of Apo B$_{100}$ in the liver and Apo B$_{48}$ in the intestine.

제주지역 양돈장에서 발생한 자돈 설사형 바이러스의 면역조직화학적 확인 (Immunohistochemical study of causative agents for the porcine viral diarrhea disease on newborn piglets in Cheju)

  • In an outbreak of acute porcine diarrhea in newborn piglets, an etiological study was carried out using piglets submitted in Cheju Province Institute for Livestock Promotion(Cheju Veterinary Service for the disease diagnosis). Sixteen piglets(2-7 days old) were collected from 4 farms during outbreaks of diarrhea disease(from January to April 2000). Specimens were taken after necropsy and examined by immunohistochemistry using of monoclonal antibodies for porcine epidemic diarrhea(PED) virus, transmissible gastroenteritis(TGE) virus, and porcine rotavirus. Immunohistochemistry showed that PED virus antigens, but both TGE virus and rota virus antigens not, were localized in the some epithelial cells of the intestines of 14 animals among 16 piglets examined. PEB virus antigens were mainly detected in the cytoplasm of enterocytes. Infected cells, which were most abundant in the villous epithelial cells of the jejunum and ileum, were uncommon in the crypt, epithelial cells, the lamina propria and Peyer's patches of piglets examined. The results suggest that PED virus is one of the most prevailing agents in an outbreak of fatal diarrhea in newborn piglets on Cheju island and PED virus was need to further study to prevent this disease.
