  • The purpose of study was to evaluate the effects of the $CO_2$ laser irradiation on demineralization inhibition and rehardening of human primary tooth enamel by laser fluoresecence measurement. Enamel specimens were made from the human primary teeth. The center spots of the specimens about 2 mm diameter were irradiated by $CO_2$ laser at the conditions of focused continuous or defocused pulsed, 3 or 6 W, for 4 seconds, before or after the demineralization by Coca-Cola for 24 hours at $37^{\circ}C$. The Diagnodent was used to measure the degree of demineralization and rehardening. There was no significant difference between focused continuous and defocused pulsed irradiation. 6W irradiation inhibited the demineralization but 3W did not. 6W irradiation rehardened the demineralized enamel but 3W did partially. The color of enamel was changed to brown to black after 6W irradiation but 3W caused no color change. $CO_2$ laser irradiation showed the effects on demineralization inhibition and rehardening of human priamary tooth enamel, and the laser fluoresecence measurement technique seemed to be a valid evaluation method.

  • To maintain the strength obtained with metal ceramic restorations, excellent gradient of translucency, wear resistance in the same range as enamel and good marginal integrity, all ceramics have been developed with many advantages in recent years. Authentic (Ceramay, Germany) is a type of low fusing, leucite-reinforced pressable ceramic that allows both the laying technique and the staining technique or the combination of the two. Two-phase glass ceramic will allow achieving unmatched esthetics even with the simplest laying or staining technique. The ingots and laying porcelain come in Vita shades. Case selection has to be the most crucial thing to achieve optimal esthetics, and cooperation with technician has to be emphasized to satisfy patient's highest esthetic demands.

  • In this study, attempt has been made to evaluate the effect of $CO_2$ laser irradiation on enamel surface structure and the bond strength of sealant material. Conventional acid etching was used as a control technic for comparison. The results obtained from this experiment were as follows; 1. The highest mean shear bond strength value was observed in samples of Group I (acid-etching) with the statistical significance(p<.05) between all the other groups. 2. The shear bond strength in Group IV was the lowest among laser etching groups. but there were no significant difference between them(p>.05). 3. Scanning electron microscopic observation showed that the rough and irregular surface was created by $CO_2$ laser treatment with the formation of numerous pores, micro-cracks, and small bubble-like inclusion. Increasing the energy density induced localized surface melt with a thin smooth glaze-like appearance. 4. In acid-etched control specimen cohesive failure predominated, whereas adhesive failure was the main mode in laser-treated group. Based upon the above-mentioned results, it can be assumed that the $CO_2$ laser is not an adequate substitute for the acid-etch technique in enamel preconditioning. More studies are required to explore the effective condition of laser irradiation which could attain the better bond strength of restorative materials.

  • Objectives: The purpose of this study was to determine the effect of resin infiltration technique on color and surface hardness of white spot lesion (WSL) with various degrees of demineralization. Materials and Methods: Ten human upper premolars were cut and divided into quarters with a $3{\times}4mm$ window on the enamel surface. Each specimens were separated into four groups (n = 10) and immersed in demineralization solution to create WSL: control, no treatment (baseline); 12 h, 12 hr demineralization; 24 h, 24 hr demineralization; 48 h, 48 hr demineralization. Resin infiltration was performed to the specimens using Icon (DMG). $CIEL^*a^*b^*$ color parameters of the enamel-dentin complex were determined using a spectroradiometer at baseline, after caries formation and after resin infiltration. Surface hardness was measured by Vickers Micro Hardness Tester (Shimadzu, HMV-2). The differences in color and hardness among the groups were analyzed with ANOVA followed by Tukey test. Results: Resin infiltration induced color changes and increased the hardness of demineralized enamel. After resin infiltration, there was no difference in color change (${\Delta}E^*$) or microhardness among the groups (p < 0.05). Conclusion: There was no difference in the effect of resin infiltration on color and hardness among groups with different extents of demineralization.

  • Sixty premolars extracted for orthodontic treatment were divided into four groups, and the residual resin was removed with four different rotary finishing instruments at a fixed speed of $18,500{\pm}300 rpm$ on the low speed handpiece. The instruments were G1; No.169L carbide fissure bur, G2: No.2 round bur, G3; No.4 round bur, G4: No.8 round bur. Then, the enamel received a S-second polishing with a rubber cup and a pumice. To find the extent of loss on the enamel at this point, prophylaxis was done with the rubber cup and pumice prior to bonding of the bracket(P1) and removal of residual resin by means of appropriate procedure applicable to each respective group(P2) followed. The final polishing was done with the rubber cup and pumice(P3), and the enamel surface roughness was measured each by the surface measuring instrument. The whole process was observed under a scanning electron microscope to gain the following results: At P2, the enamel surface roughness in G1 showed most smoothly with $2.60{\pm}0.55{\mu}m;\;in\;G2,\;3.24{\pm}0.80{\mu}m;\;in\;G3,\;3.44{\pm}0.94{\mu}m;\;in\;G4,\;3.89{\pm}0.54{\mu}m$, the roughest. G2 and G3 showed no statistical significance(P>0.05). At P3, the enamel surface roughness in G1 showed most smoothly with $2.29{\pm}0.47{\mu}m;\;in\;G2,\;2.44{\pm}0.56{\mu}m;\;in\;G3,\;2.44{\pm}0.56{\mu}m;\;in\;G4,\;2.92{\pm}0.43{\mu}m$, the roughest. G1 vs G2, G3, and G2 vs G3 had no statistical significances(p>0.05). In all groups, P2 and P3 showed rougher in surface roughness than P1, and P2 rougher than P3(p<0.01). In a case of 5-second prophylaxis with the rubber cup and the pumice on a virgin, normal enamel, fine scratches were found under the scanning electron microscope. In all four groups, unremovable gouges remained even after polishing with the ubber and pumice; residual resin was not observed with naked eye when finished with the rubber and pumice, but the resin debris was observed under the scanning electron microscope.

  • Clinical application of composite resin recently draw great concerns in dentistry. Especially due to advantages such as esthetics, adhesiveness, simple clinical procedures, various shapes and kinds of composite resins are widely being applied to prosthodontics, conservative dentistry, and orthodontics. But, clinical problems attributable to the polymerization shrinkage of composite resin have been proposed, and we have to regard clinical problems such as secondary caries, loss of restoration, fracture of the surrounding tooth structure, marginal discoloration, and tooth sensitivity, and many portions are remained to be overcome. Therefore, this study attempts to analyze stress distribution between resin and tooth structure which is generated during polymerization shrinkage of composite resin using three dimensional finite element method. Three dimensional finite element models with conventional box-shape cavity and erosion/abrasion type V-shape lesion cavity in upper central incisor were developed. These cavities were filled with four different types of placement techniques. (bulk filling, horizontal increment filling, oblique occlusal increment filling, oblique gingival increment filling) The stresses generated by polymerization shrinkage of composite resin were calculated. The results analyzed with three dimensional finite element method were as follows : 1. The increment filling technique showed the highest maximum normal stress in both conventional box-shape and V-shape cavities and showed a tendency to decrease after complete polymerization. 2. The bulk filling technique resulted in increased stresses during the curing process in both conventional box-shape and V-shape cavities and the highest maximum normal stress occurred after complete polymerization. 3. The bulk filling resulted in the lowest maximum normal stress in both box-shape and V-shape cavities 4. Regardless of placement method, in conventional box-shape cavity, the maximum normal stress increased in dentin floor, enamel, dentin sequence and in V-shape cavity, the maximum normal stress increased in enamel, dentin sequence.

  • Statement of problems: Self-etch adhesives exhibit some clinical benefits such as ease of manipulation and reduced technique-sensitivity. Nevertheless, some concern remains regarding the bonding effectiveness of self-etch adhesives to enamel, in particular when so-called 'mild' self-etch adhesives are employed. This study compared the microtensile bond strengths to ground enamel of the two-step self-etch adhesive Clearfil SE Bond (Kuraray) to the three-step etch-and- rinse adhesive Scotchbond Multi-Purpose (3M ESPE) and the one-step self-etch adhesive iBond (Heraeus Kulzer). Purpose: The purpose of this study was to determine the effect of a preceding phosphoric acid conditioning step on the bonding effectiveness of a two-step self-etch adhesive to ground enamel. Material and methods: The two-step self-etch adhesive Clearfil SE Bond non-etch group, Clearfil SE Bond etch group with prior 35% phosphoric acid etching, and the one-step self-etch adhesive iBond group were used as experimental groups. The three-step etch-and-rinse adhesive Scotchbond Multi-Purpose was used as a control group. The facial surfaces of bovine incisors were divided in four equal parts cruciformly, and randomly distributed into each group. The facial surface of each incisor was ground with 800-grit silicon carbide paper. Each adhesive group was applied according to the manufacturer's instructions to ground enamel, after which the surface was built up using Light-Core (Bisco). After storage in distilled water at $37^{\circ}C$ for 1 week, the restored teeth were sectioned into enamel beams approximately 0.8*0.8mm in cross section using a low speed precision diamond saw (TOPMET Metsaw-LS). After storage in distilled water at $37^{\circ}C$ for 1 month, 3 months, microtensile bond strength evaluations were performed using microspecimens. The microtensile bond strength (MPa) was derived by dividing the imposed force (N) at time of fracture by the bond area ($mm^2$). The mode of failure at the interface was determined with a microscope (Microscope-B nocular, Nikon). The data of microtensile bond strength were statistically analyzed using a one-way ANOVA, followed by Least Significant Difference Post Hoc Test at a significance level of 5%. Results: The mean microtensile bond strength after 1 month of storage showed no statistically significant difference between all adhesive groups (P>0.05). After 3 months of storage, adhesion to ground enamel of iBond was not significantly different from Clearfil SE Bond etch (P>>0.05), while Clearfil SE Bond non-etch and Scotchbond Multi-Purpose demonstrated significantly lower bond strengths (P<0.05), with no significant differences between the two adhesives. Conclusion: In this study the microtensile bond strength to ground enamel of two-step self-etch adhesive Clearfil SE Bond was not significantly different from three-step etch-and-rinse adhesive Scotchbond Multi-Purpose, and prior etching with 35% phosphoric acid significantly increased the bonding effectiveness of Clearfil SE Bond to enamel at 3 months.
