Optimal protocol for teleconsultation with a cellular phone for dentoalveolar trauma: an in-vitro study

  • Purpose : Dental trauma is frequently unpredictable. The initial assessment and urgent treatment are essential for dentists to save the patient's teeth. Mobile-phone-assisted teleconsultation and telediagnosis for dental trauma could be an aid when a dentist is not available. In the present in-vitro study, we evaluated the success rate and time to transfer images under various conditions. Materials and Methods : We analyzed the image quality of cameras built into mobile phones based on their resolution, autofocus, white-balance, and anti-movement functions. Results : The image quality of most built-in cameras was acceptable to perform the initial assessment, with the autofocus function being essential to obtain high-quality images. The transmission failure rate increased markedly when the image size exceeded 500 kB and the additional text messaging did not improve the success rate or the transmission time. Conclusion : Our optimal protocol could be useful for emergency programs running on the mobile phones.

Study on the Patterns of Helicopter Emergency Medical Services in Ullung Island (울릉도 지역의 헬리콥터를 이용한 응급환자 후송 실태)

  • Objective: The aim of this study was to evaluate the patterns of helicopter emergency medical services (HEMS) in Ullung Island. Methods : The authors reviewed the records from emergency room diaries and the lists of helicopter transfers in the Ullung Public Health Medical Center over the 5-year period from Jan 1, 1997 to Dec 31, 2001. Results : One hundred thirteen cases were transferred by helicopters in 88 flights. According to year, the number of flights was 13(14.8%) and the number of cases was 15(13.3%) in 1997; 17(19.3%) and 21(18.6%) in 1998; 18(20.5%) and 20(17.7%) in 1999; 17(19.3%) and 20(17.7%) in 2000; and 23(26.1%) and 37(32.7%) in 2001. According to the kind of helicopter, the number of flights was 46(52.3%) and the number of cases was 60(53.1%) by Maritime police; and 19(21.6%) and 28(25.1%) by 119 rescue. According to time zone, there were no night flights. According to sex and age, there were 75 male cases(66.4%) and 28 cases(28.3%) of patients aged sixty years and over. The number of flights was 11(12.5%) and the number of cases was 15(13.3%) in November; 10 flights(11.4%) and 14 cases(12.4%) in March; and 7 cases(8.0%) in each of September, October and April. The most common season of helicopter transfer cases was autumn. According to transfer area, there were 48 cases (42.5%) in Pohang city, Gyeonsangbukdo; 35(31.0%) in Gangnung city, Gangwondo; and 17(15.0%) in Daegu metropolitan city. According to condition, there were 27 cases(23.9%) of cerebro-vascular accident, 13(11.5%) of fracture and 11(9.7%) of head injury. According to admission department, there were 42 cases(37.2%) in Neurosurgery, 21(18.6%) in Internal Medicine and 13(11.5%) in Orthopedic Surgery. According tothe Korea Standard Classification of Disease(3-KSCD), circulatory systemic disease(IX) and injury, intoxication and others (XIX) were the two most frequent categories with 34 cases(30.1%) each, followed by digestive system disease (XI) with 23 cases(20.4%). Conclusions : HEMS in Ullung Island leave much to be desired. Helicopters cannot make a night flight and are not equipped with medical facilities. HEMS in islands such as Ullung Island are essential. We hope that night flights, equipment-monitoring systems for emergency patients in the helicopters, and a law related to HEMS in the island will all be established.

Comparison of Job Satisfaction and Nursing Performance between Nurses on Fixed Nights and Nurses on Three shifts, and Nurses Understanding of Fixed Night Shift System (밤번고정근무 간호사와 3교대근무 간호사의 직무만족도, 간호업무수행정도 비교 및 밤번고정근무제도에 대한 인식)

  • Purpose: The purpose of this study was to compare job satisfaction and nursing performance of nurses on fixed night shifts (FNS) and nurses working three shifts (W3S), to identify the environment of night work and recognition of the viability of the FNS system. Methods: The research was conducted with 106 nurses on FNS and 257 on W3S. The nurses had worked for 1 to 11 years at A hospital, Seoul. Results: Job satisfaction was significantly higher (t=-3.51, p< .001) for nurses on FNS. However, no significant difference was found for nursing performance (t=-1.019, p=.309) between the two groups. Both groups of nurses were mostly satisfied. Subjective fatigue scores (0 to 10 points) during the night for nurses on FNS (6.02) were lower than for W3S nurses (7.28) A high percentage (88.7%) of nurses on FNS indicated a willingness to further participate in FNS. Conclusion: Results indicate that the demonstration FNS system showed its effectiveness. Therefore, efforts are needed to enhance and revitalize FNS and to upgrade the system to control patient transfer time from emergency departments to wards, and to provide nurses needed resources for emergency or intensive care.

Interhospital Transport System for Critically Ill Patients: Mobile Extracorporeal Membrane Oxygenation without a Ventilator

  • Background: Extracorporeal membrane oxygenation (ECMO) has been successfully used as a method for the interhospital transportation of critically ill patients. In South Korea, a well-established ECMO interhospital transport system is lacking due to limited resources. We developed a simplified ECMO transport system without mechanical ventilation for use by public emergency medical services. Methods: Eighteen patients utilized our ECMO transport system from December 2011 to September 2015. We retrospectively analyzed the indications for ECMO, the patient status during transport, and the patient outcomes. Results: All transport was conducted on the ground by ambulance. The distances covered ranged from 26 to 408 km (mean, $65.9{\pm}88.1km$) and the average transport time was $56.1{\pm}57.3minutes$ (range, 30 to 280 minutes). All patients were transported without adverse events. After transport, 4 patients (22.2%) underwent lung transplantation because of interstitial lung disease. Eight patients who had severe acute respiratory distress syndrome showed recovery of heart and lung function after ECMO therapy. A total of 13 patients (70.6%) were successfully taken off ECMO, and 11 patients (61.1%) survived. Conclusion: Our ECMO transport system without mechanical ventilation can be considered a safe and useful method for interhospital transport and could be a good alternative option for ECMO transport in Korean hospitals with limited resources.

  • With the development of recent IT fusion technology, IT fusion technology of medical equipment has been dramatically development. In an attempt to complement these problems, in this paper, in the process of endotracheal intubation, intubated by inserting an endoscopic camera to Into Activation tube, the streaming is a real-time monitoring and wireless video transmission method using, there is a purpose of enabling the monitoring in smart devices increase the success rate of endotracheal intubation of first aid purchases.

  • In the field of clinical medicine and research, the analysis of such as protein and DNA at the molecular level and even at the cell level are necessary for disease diagnosis and treatment. In many cases, a real time image of samples is needed for the accurate analysis and manipulation of samples since experimental samples are degenerated with time. In this research, a three-dimensional fluorescence microscope device was developed for taking images of protein and DNA inside a single cell and the server-client based image analysis system was made for an integrated management of the real-time images taken from the microscope device. The system consists of a fluorescent measurement device, the associated software and a client program on smartphone. The developed system allows doctors or experimental managers to receive and look at the real-time experimental images taken from the samples of patients anywhere in the emergency, to analyze results and to instantly diagnose the disease and to transfer the results to the patients. As a result, the system is able to be utilized in the implementation of ubiquitous health as well.

Accurate transfer of bimaxillary orthognathic surgical plans using computer-aided intraoperative navigation

  • Objective: To examine the accuracy of computer-aided intraoperative navigation (Ci-Navi) in bimaxillary orthognathic surgery by comparing preoperative planning and postoperative outcome. Methods: The study comprised 45 patients with congenital dentomaxillofacial deformities who were scheduled to undergo bimaxillary orthognathic surgery. Virtual bimaxillary orthognathic surgery was simulated using Mimics software. Intraoperatively, a Le Fort I osteotomy of the maxilla was performed using osteotomy guide plates. After the Le Fort I osteotomy and bilateral sagittal split ramus osteotomy of the mandible, the mobilized maxilla and the distal mandibular segment were fixed using an occlusal splint, forming the maxillomandibular complex (MMC). Real-time Ci-Navi was used to lead the MMC in the designated direction. Osteoplasty of the inferior border of the mandible was performed using Ci-Navi when facial symmetry and skeletal harmony were of concern. Linear and angular distinctions between preoperative planning and postoperative outcomes were calculated. Results: The mean linear difference was 0.79 mm (maxilla: 0.62 mm, mandible: 0.88 mm) and the overall mean angular difference was 1.20°. The observed difference in the upper incisor point to the Frankfort horizontal plane, midfacial sagittal plane, and coronal plane was < 1 mm in 40 cases. Conclusions: This study demonstrates the role of Ci-Navi in the accurate positioning of bone segments during bimaxillary orthognathic surgery. Ci-Navi was found to be a reliable method for the accurate transfer of the surgical plan during an operation.

  • Worldwide casualties caused by earthquakes, floods, fire or other disaster has been increasing. So many researchers are being actively done technical studies to ensure golden-time. In this paper if a disaster occurs, use the IoT technologies in order to secure golden-time and transmits the message after to find the user of the accident area first. When the previous job is finished, gradually finds a user of the surrounding area and transmits the message. For national emergency information, OPEN API of Korea Meteorological Administration was used. To collect detailed information on a relevant area in real time, this study established the system that connects and integrates Crowd Sensing technology with BLE (Bluetooth Low Energy) Beacon technology. Up to now, the CBS based on base station has been applied. However, this study designed and mapped DB in the integration of Beacon based user positioning and national administrative address system in order to estimate local users. In this experiment, the accuracy and speed of information dif6fusion algorithm were measured with a rise in the number of users. The experiments were conducted in a manner that increases the number of users by one thousand and was measured the accuracy and speed of the message spread transfer algorithm. Finally, became operational in less than one second in 20,000 users, it was confirmed that the notification message is sent.

Clinical Characteristics of Asthmatic Patients Who Visited Emergency Room (응급실로 내원한 천식환자들의 임상적 특성)

  • Background : Despite remarkable progress of understanding the pathophysiology and therapy of bronchial asthma, asthma morbidity and mortality are on the rise. Also hospitalization and attending rates of emergency department for asthma have been increasing gradually. We analyzed clinical characteristics and prognosis of patients who visited emergency room due to asthma attack in order to define clinical characteristics of these group of patients. Method : We reviewed 105 adult asthmatic patients who attended emergency department of Korea University Hospital between August 1995 and July 1996, retrospectively. Results : 103 patients(56 female, 47 males, mean age : 48.6 years) attended-68 self referral, 18 practitioner referral and 17 OPD transfer- and 86 patients(83.5%) were admitted. Attending emergncy department was clearly more frequent in December(13.6%) and May(12.6%). Time lag between onset of asthmatic attack and arrival at the hospital was $14.2{\pm}15.5$ hour and initial peak expiratory flow rate was $166.7{\pm}68.3L/min$.(43.3% predicted) The commonest cause for visiting emergency room was aggravation of asthma due to upper respiratory tract infection in mild asthmatics. About half of them had history of previous ER visits. Their prognosis was not bad, but after discharge, about half of patients escaped from OPD follow-up. Conclusion : As a group they merit detailed attention and follow up arrangement. Clinician need to monitor and review the treatment plans, the medications, the patient's management technique, and the level of asthma control. For this group, plans for longer term treatment, including asthma education program and adjustment of overall treatment plan should be made.

  • U-Healthcare is one of the major applications in ubiquitous sensor network. U-Healthcare has potential to become a critical service for the people who immediately require emergency ambulatory attention. This paper describes about the real time pulse monitoring and reporting system, consisting of two components: thus, the one is a reliable bio-sensor that continuously monitors the pulse information of the subject, and the other is the automatic transfer system that transmits pulse information to both his/her family and hospital care system through the Base Station. In the hospital, this bio-information can be used to treat the patient accordingly. I designed the pulse information monitored by a bio-sensor module that transfers the pulse information to both the Base Station and the central monitoring system through transmitting protocols such as Zigbee and TCP/IP, as well as designed the architecture of information packets for the corresponding protocols. Furthermore, the central monitoring system automatically parses the pulse information of the subject into the web database server, which can continuously provides the real time information and status of the subject via an internet browser to the clients who are family members of the subject and the authenticated medical care personnel as well.
