광고를 통해 본 일제강점기 치과 장비 및 기구 광고에 관한 연구 (The study on advertisement of dental devices & instrument during Japanese colonized period)

  • This article is purposed of reviewing the development history of Japanese dental devices and instrument, and their related advertisement activities during the Japanese colonized period in Korea in early 20th century. Japanese dental devices and instrument were redesigned to accommodate their ergonomic shape above the simple imitation, and it implies the excessive desires brought them frustrations. The tragic earthquake on Sep. l, 1923, medical insurance law enforcement on Jan. 1, 1927, celebration of "Cavity prevention Day" started on Jun. 4, 1928, and the attack of Manchuria and China by Japan after 1931, all of these historical incidents become the preliminary requirement for the development of dental devices. On Nov. 1, 1937, Japanese government started to control dental materials, driving the campaigns for excluding foreign products and encourging the use of local products. In 1939, Nakajima dental manufacturers used this political and social atmosphere on their advertisement as saying "Our Nakajima's products have no compromise with the short raw materials, but only commitment to our quality". Since after 1940, the price and supply have been strongly under control, and the control group was appeared to manage all of supply and distribution of raw materials, regular price system, and specifications. At last, the Japanese national power were devastated in its production and distribution capacities, and get to the frustrated period. The main advertised dental devices and instruments in Korea during the Japanese colonized period were 1) dental chair, unit and cabinet, 2) dental x-ray, 3) compressors, 4) dental needles, 5) small instrument and carryon medical(emergency) kit, 6) oral hygiene and pyorrhea alveolaris, infrared rays, sunlight lamp, ultrashort wave treatment devices, 7)crown former, electric furnace, casting machine, articulator, electric lathe, and laboratory equipments, etc.

치과위생사 통합교육과정 모형 접근에 관한 연구 (A Study on Approach toward Integrative Dental-Hygienist Curriculum Model)

  • The purpose of this study was to classify the courses of the dental-hygiene curricula into several categories by field, to incorporate the subjects in the same category into an integrated course, and to suggest how to ensure the successful phase-in application of integrative education according to Ronald M. Harden's 11-stage integrative ladder model. The findings of the study were as follows: 1. When the existing curricula were analyzed, it's found that many credits were provided to the courses in the area of basic dentistry that offered both theory and practice. In particular, the subjects tested by the national examination were offered by every college. In the field of public oral health, the largest number of credits was allocated to theory of oral prophylaxis and practice courses. In clinical area, clinical practice, in the area of dental office management, dental insurance course was given the largest credit. There were 31 to 61 major subjects in the colleges, which indicated that the major subjects were segmented in detail. 2. It seemed necessary to incorporate the subjects in the field of basic dentistry into oral biology, and theory of oral prophylaxis/practice, preventive dentistry/practice, preclinical stage, emergency treatment and introduction to dental hygiene should be integrated in to clinical dental hygiene. The courses in clinical area should be combined into clinical dentistry, and in the field of medical management, dental insurance, hospital management and medical relation law should be incorporated into management of dental clinic. 3. In the 11-stage integrative ladder, the subjects in the same field could perfectly be incorporated as the stages advanced. Each of the subjects was less emphasized, and communication and joint plans among teachers who were respectively in charge of the courses were increasingly considered important. Specifically, there should be a consensus among the teachers in regard to the outline of teaching programs, order of education. objects and objectives of programs and what and how to evaluate.

영유소아기 가정사고의 원인과 예방에 관한 연구 (A Study on Home Accidents of Preschool Children (from 1 to 6) in Korea and Prevention Measures)

  • Necessity and purpose of this study: In a large number of countries it has been founded that children′s domestic accidents are at great risk year by year In the United States, they publish detailed accident statistics at regular intervals. In Korea. there have been just a few studies on Accidents-At-Home of preschool children. But it can not be said that there have been any systematic statistics about this area. and any study accounting for the relations of home accidents and preschool: children in detail, Therefore, the purpose of this thesis was focused on the inquire of these relations so as to make a little contribution to Korean preschool children′s health and security measures. So, the detail-purposes are to study following questions and to testify following hypothesis. Prob. 1. What the types of accidents of Preschool children, where the place accidents occurred\ulcorner Prob. 2. What the cause of accidents and, the main factors of the cause\ulcorner Prob. 3. How about the number of their children. the disparity of age among their children and mother′s age in each case of accidents\ulcorner hypothesis 1. There will be differences in the density of protection of parents according to the number of their children. hypothesis 2, There will be differences in accident-types and first-aid methods according to parents socio-economic background. Method; This study employed the interviewing survey method, in which 130 preschool children ware random.sampled, who visit hospital to have medical care. These children (from 1 to 6 years olds) were selected at the emergency room of five hospitals in Seoul (Hosp: Severance, Woosok, Medical Center, Hanyang Medical College Hospital and Seoul Medical Col1age Hospital during study-period (from Aug. to Oct, 1973). Four head nurses in above Hospitals were employed as accident members for this study. Concerning research analysis, the method of hypothesis verifying is used. Conclusion: As two American experts on this subject. Dr, Raymond Neuter and Mr. Ross Mc Garland have drawn attention to "minor epidemics of accidents" that could be avoided by fairly simple measures. preschool children′s accidents could be avoided by parents fair attentions. In other words, one of the most common causes of preschool children′s accidents derived from their parent′s inattention. Therefore, one important task on this subject is to instruct the parents fairly about the children′s accidents. Many accidents could be avoided by the exorcist of a little self-discipline. Also, as much the prevention of accidents is important, as the first-aid Is Important and necessary at the case of the accidents. So, the methods of proper first-aid treatment must be emphasized, and must be taught in school, especially in girls school. And there could be other means available for prevention of accidents. Firstly, the public authorities can take legal measures. More stringent safety standards can be made enforceable by law. Building materials and equipment for domestic us: ought to meat minimum safety criteria at all times. Next the public itself has to understand the seriousness of the problem, and here the dissemination of information is of great importance. All mass media should be brought into play to promote greater public awareness of the question. At last, it will be needed to obtain more detailed epidemiological data through additional surveys and statistics after this study.

여성 구급대원의 현장 스트레스 경감방안 모색에 관한 연구 (A Study on Searching for Reduction of Women EMTs's Stress)

  • The rate that women's EMTs(emergency medical technicians) in fire department occupy is increasing, but my general plan is aimed at suggesting the improving plan for the working environment of women's EMTs paying attention to the duty stresses of increasing women's EMTs, because improvement for the working environment of women's EMTs is being proceeded slowly. I suggested my improving plan for solving the working stress on the basis of a survey of the Recognition against women's EMTs as follows. Firstly, I suggested that physical strength of women's EMTs should be managed well continually via program establishment and a defined evaluation not only by employing a professional person in charge for physical strength in firehouse, but also by replenishing the suitable number for the urgent mobilization. Secondly, I suggested that the suitable number for the urgent mobilization should be increased to 3 including an ambulance driver. Thirdly, I suggested necessity such as the fruitful education and the creation of circumstances that can focus on education for improving participation rate, development of various new educational program, incentive program and penalty. finally, I suggested the fact that is related to the actual re-amendment of the concerned law and ordinance such as not only the smooth treatment of penalty for a traffic signal violation of ambulance and the legal remedy system due to aggravation of a patient's condition during a first-aid course, but also workshop and meeting among the periodical EMTs and administration of special therapy program and sufficient recess.

수형자 교정에 대한 개념분석 (A Concept Analysis of Prisoner Corrrection)

  • 본 연구는 수형자 교정에 대한 개념분석을 통해 그 속성을 파악하고 개념을 명확히 하여 간호중재프로그램 개발시 기초자료를 제공하기 위함이다. 2011년에서 2015년까지 발표된 국내외 수형자 교정에 대한 연구문헌을 PubMed, NDSL RISS을 이용하여 2016년 4월 1일부터 7월 30일까지 검색하였다. 37편의 문헌을 Walker와 Avant의 개념분석방법 절차에 따라 분석하였다. 수형자 교정의 속성은 응보, 자기성찰, 긍정적인 문제해결, 재사회화로 나타났다. 수형자 교정의 정의는 법에 위반되는 반사회적이고 반도덕적인 행위로 교정시설에 수용된 수형자를 대상으로 과학적이고 개별화된 형벌을 집행하고, 수형자의 내면적 성찰을 통해 삶에 대한 긍정적인 태도를 지니게 할 뿐만 아니라 개별적 특성을 고려한 서비스 제공으로 문제해결과 사회적응능력의 향상, 신체적 정신적 건강의 조화를 통한 재사회화와 범죄로부터 국민과 사회를 안전하게 보호하는 것이었다. 본 연구는 수형자 교정의 바람직한 방향을 설정하고, 치료적 교정을 위한 간호중재프로그램 개발 시 이론적 근거를 제공하였다.

심장 스텐트 시술과 의료사고 예방 (Cardiac Intracoronary Stenting vs CABG: Prevention of Medical Accident)

  • 관상동맥 질환은 2017년 고령사회로 진입한 우리나라에서 앞으로 더 많은 관심을 가질 것이다. 고령화가 될수록 고혈압, 당뇨 등 복합적인 질환이 합병되어 혈관상태도 상대적으로 더 나빠져 관상동맥 질환에 걸릴 가능성이 높기 때문이다. 심혈관 질병은 심장외과와 심장내과와의 긴밀한 협진이 필요하다. 따라서 협심증이나 심근경색증환자를 먼저 진료하게 되어 있는 우리나라의 임상현장에서 객관적인 심장내과 의사의 치료방침에 대한 판단은 매우 중요하다. 최근 심장내과의 비수술적 중재술이 발전하고 있지만 무리한 스텐트 시술로 의료사고도 발생하고 있다. 특히 관상동맥 3개혈관이 모두 막힌 삼중혈관이거나 석회화가 심해 혈관 상태가 좋지 않은 경우가 문제이다. 또한 심장외과 의사가 없는 병원에서 무리하게 경피적관상동맥중재술을 실시하다가 응급상황이 발생할 경우 관상동맥이식술 등 외과적 대처가 어려운 경우가 종종 발생한다. 최근 2년간 한국소비자원(소비자분쟁조정위원회) 의료분쟁 조정결정 8사례를 분석한 결과, 심장 중재술을 시행한 병원 중 심장외과 의사가 상주한 곳은 2곳으로 확인됐다. 8사례 모두 심장내과 진료 후 풍선확장술 및 스텐트 삽입한 경우로 7명이 사망했고 이중 5명은 시술 당일에 사망했다. 8사례 중에 3중혈관 환자는 5건이고, 나머지도 석회화가 심하거나 완전폐쇄로 혈관상태가 좋지 않은 상태였다. 2017년 심장내과 스텐트 시술 건수 조사 보고에 의하면 3개 이하 약물 방출 스텐트 시술이 98%로 보고됐다. 2015년 스텐트 시술 건수가 38,922건으로 약800건(2%)은 스텐트가 4개 이상 사용된 것으로 추정된다. 무리한 스텐트 시술로 마지막 여명에 급사함으로써 신변정리 기회상실은 물론 여명단축에 따른 손해로서 '지도 설명의무' 책임을 물어 전 손해에 대한 배상을 신중하게 고려할 필요가 있다. 최근 심평원 보험적용 스텐트 시술 개수 제한규제가 없어지면서 무리한 시술과 심장외과 의사 확충에 대한 문제가 있다. '다학제통합진료' 같은 병원차원의 해결방안은 물론 필수요원에 해당하는 심장외과를 공무원으로 확충하는 등 국가차원의 해결방안이 요구된다.

베버리지 보고서의 의료보장 구상과 NHS를 통한 구현 (The Public Health Welfare Conception of the Beveridge Report and Its Realization via the NHS)

  • 본고는 베버리지 보고서 원문에 담긴 의료보장 및 복지 구상을 검토하고, 베버리지의 기획이 오늘날 영국 NHS를 통해 어떠한 방식으로 현실에서 구현되었는지를 규명하고자 노력한다. 20세기 복지 사회의 근간을 형성한 베버리지 보고서의 영향력을 염두에 둔다면, 보건의료 영역에 관한 베버리지 보고서의 본래적인 기획은 무엇이었으며, 그것이 국가 의료 시스템인 NHS로 얼마나 충실히 계승되어 구현되었는지를 알아보는 과제는 흥미롭지 않을 수 없다. 또한, 보건복지 정책이 현대 국가에서 차지하는 비중을 고려한다면, 베버리지가 제시한 의료보장 기획 및 그 내용이 오늘날까지 이어져 왔는지를 살피는 노력은 시의적절하다. 위의 문제의식을 바탕으로, 본고는 베버리지 보고서에 담긴 의료보장 구상과 NHS를 통한 구현을 다음과 같이 알아본다. 우선 베버리지 보고서의 역사적인 배경을 다루어 베버리지가 제시한 복지체계의 기원을 살피며, 복지 제도의 개혁을 주동한 시대정신의 역할 및 영국 전시생산체제와 응급의료서비스의 경험을 주요 논점으로 부각한다. 그 후 당대의 사회현실로부터 태동한 베버리지 보고서의 의료보장 구상이 무엇인지를 분석하는 단계가 진행되며, 이때 사회복지를 향한 목표와 의료보장에 관한 계획이 주목된다. 마지막으로, NHS의 지향과 운용 방식, 치료 유형, 재활 프로그램을 포함한 현황을 차례로 검토하고 베버리지 보고서와 비교분석하여 저자의 기획이 현실에서 충실하게 구현되었는지를 살핀다. 이 과정에서 본고는 베버리지 보고서 원문은 물론이며, 잉글랜드 NHS 헌법과 1946년 국민보건서비스법을 비롯한 주요 법정책 자료를 참고하여 연구 목적을 달성하고자 노력한다. 본고의 탐구는 단지 베버리지 보고서의 답습에 그치지 아니하고, 현대의 관점에서 복지국가에 대한 베버리지의 기여를 평가하여 되돌아본다는 지점에서 그 의의가 있다. 베버리지 보고서에 담긴 지향과 정책 등을 구조화하여 분석하고, 이를 NHS의 현실에 접목하여 비교분석하는 본고의 서술은 베버리지의 기획이 영국에서 착실하게 구현되었는지를 살피는 적절한 기회를 마련할 수 있다. 더 나아가 본고는 보건의료 분야 복지의 과거와 현재를 비판적인 시각에서 반추하려는 노력의 일환이며, 한국 의료보장 및 복지 관련 법 제도의 미래와 개선을 염두에 두는 건설적인 탐구에 적절한 시사점을 남길 수 있으리라고 기대한다.

일반항공에서의 제조물책임에 관한 연구 - 미국 일반항공진흥법(GARA)을 중심으로 - (The Study on U.S. GARA and Aircraft Products Liability)

  • 미국에서 일반항공산업의 보호를 위해 입안된 GARA는 일반항공의 제조업자를 지나치게 보호한다는 비판에서 지속적으로 위헌성이 제기됨에도 불구하고 현재까지 적용되고 있는 법률이다. 일반항공 사고에 관한 소송에서 우리나라 국민이 원고나 피고가 되었을 때 이 법이 직접적으로 적용될 수 있다는 역외적용의 측면에서 이 법은 우리에게도 큰 의의가 있다. 또한 GARA는 일반항 공기 및 그 부품에 관한 특별법이지만, 제소기간을 18년으로 확정하면서 제소 기간의 의미, 기산점, 적용제외 등을 상세히 규정하고 있어서 일반 제조물책임상 제소기간 규정과의 좋은 비교가 된다. 특히 제소기간 규정과 관련하여 우리 제조물책임법이 일률적으로 10년이라는 제소기간을 설정하면서 이에 관한 구체적인 적용과 관련한 규정을 전혀 두고 있지 않아서 문제이다. 제조물의 개별적 특성을 고려하지 않은 일반규정으로 인하여 일반항공에서 특별히 18년이라는 제소기간을 규정하고 있는 미국보다 한국의 제조물책임법이 더 짧은 제소기간을 운영하여 한국의 일반법이 미국의 특별법 보다 제조업자를 더욱 두텁게 보호하는 결과가 되고 있다. 이러한 문제점의 인식하에 GARA의 제소기간 규정으로부터 우리의 제조물책임법의 향후 개정방향을 다음과 같이 짚어 볼 수 있다. 첫째, 제조물책임법상 제소기간을 일괄적으로 정하는 것 보다는 여러 가지 다양한 제조물을 유형화하여 개별적 제소기간 규정을 두는 것을 고려해 볼 수 있다. 둘째, 실제 소송에서 소모적인 논쟁을 방지하기 위하여 제조물 공급일의 의미를 명확히 규정할 필요가 있다. 셋째, 제소기간 규정을 일률적으로 적용하는 것이 합리적이지 못한 경우를 대비하여 제척기간으로 해석되는 우리의 제소기간 규정이 적용되지 않을 수 있는 예외조항을 둘 필요가 있다. 비교법적인 관점에서 제조물책임상 제조업자와 피해자인 소비자 간의 이익 균형을 맞출 수 있는 합리적인 제소기간 규정의 설정이 모색되어야 하겠다.

국내 메르스(MERS) 사태가 남긴 과제와 법률에 미친 영향에 대한 소고(小考) (The Study of Effectiveness of MERS on the Law and Remaining Task)

  • 2015년 5월 중동 사우디아라비아와 아랍에미레이트를 다녀온 68세의 남자가 고열, 근육통, 기침 호흡곤란으로 자택부근 2개 병원)과 삼성서울병원(SMC) 응급실을 거치면서 시작된 메르스(MERS)라는 전염병은 거의 3개월에 걸쳐 최근까지 대한민국을 유행성 전염병의 공포로 몰아넣었다. 특히 이 전염병은 중동지역 아라비아 반도를 중심으로 2012년 9월부터 현재까지 총 26개국에서 1,392명에게 발생하여, 538명의 사망환자가 보고(유럽질병센터, 2015.7.21. 기준)되었고, 우리나라의 경우에도 총 확진자 186명, 사망자 36명(치사율 19.4%) 격리자 16,693명이라는 기록을 남기고 점차 우리의 뇌리에서 사라져가고 있다. 이 전염병의 파급으로 인하여 현 정부의 전염병 관리체계에 대한 많은 비판과 문제점을 제기하는 계기가 되었다. 특히 병원내 감염문제, 민간의료기관-지역 보건소-질병관리본부를 연결하는 감염병 관리전달체계의 미작동, 정부의 전염병과 관련된 정보 공개 거부, 냉난방 공조설비의 환기 등과 관련된 시설문제, 감염병 격리병상의 절대부족 문제, 지역보건소의 기능 및 권한의 제한문제, 격리자 및 의료기관에 대한 보상문제, 제도적으로는 감염병에 대한 건강보험 수가 개선문제, 법정감염병 재분류 문제, 질병관리본부의 역할 재정립 문제, 전염병 관련 전문인력 양성을 위한 제도적 보완문제, 병원 이용문화의 개선으로서 다인실 조정과 간병인 대체문제, 사회적 후유증을 치유하기 위한 정책적 배려문제 등이 있으며, 이에 대한 광범위한 논의와 해결방안이 필요한 때이다. 국민들은 2015년 5월 이전에는 아주 낯설었던 메르스라는 외부유입 전염병에 국가 방역체계가 흔들리는 모습을 보고 극도의 공포감을 느꼈으며, 외국에서는 중동지방을 제외하고는 전세계적으로 유례가 없는 메르스라는 전염병이 대유행하는 대한민국에 발길을 끊었다. 국민의 건강권이 심각하게 위협받았음은 물론이고 관광산업과 유통업계 등의 경제도 얼어붙었다. 정부는 메르스라는 전염병 확산의 초기대응 실패를 인정하고, 사태를 악화시킨 점에 대한 책임을 통감하여야 한다. 국민들에 대한 신뢰상실을 반성하고 향후 각종 필요한 정보를 공개함으로써, 메르스와 같은 전염병에 대한 정부정책을 투명하게 집행하고, 지방자치단체와 국민들이 지역사회 감염에 대처하고 확산방지에 나설 수 있도록 제도를 개선하며, 변경된 제도를 홍보하여 전염병 전파 예방에 노력해야 할 것이다.

병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.
