Association of Prescribed Drug intoxication and Neuropsychiatric history (약물 음독과 정신과 과거력의 연관성)

  • Purpose: The purpose of this study was to investigate the relationship between patients with a neuropsychiatric history and features of their suicide attempt, in order to analyze the risk associated with psychiatrist prescribed drugs Methods: We retrospectively investigated cases of intentional overdose drug ingestion in patients greater than 14 years of age who visited OO emergency medical center between January 1, 2008 and July 31, 2010. We evaluated patient medical records to ascertain their age, sex, neuropsychiatric history, and components of ingested intoxicant. Information regarding any suicide reattempt was obtained after discharge through follow up telephone survey. SPSS version 13.0 was used for statistical analysis. Fisher's exact test was performed with p<0.05 considered statistically significant. Results: Ninety-six of 209 patients (46%) had a past history of psychological problems. Among those 96 patients with a history of psychological problems, 46(48%) used an overdose of the medicine prescribed by their psychologist in order to attempt suicide. However, for patients without a history of psychological problems, intoxication by neuropsychiatric drugs was insignificant. Neuropsychiatric patients required greater follow up care after discharge and exhibited significantly more suicide reattempts. Conclusion: This study revealed that patients sometimes use the medicine prescribed by their psychologist to attempt suicide. Therefore, an exhaustive plan to control the medicines prescribed to psychiatric patients should be established.

Perception and Performance of Emergency-room Nurse's Protection Behavior for the Consumer Health Information (응급실 간호사의 환자 의료 정보 보호행위에 대한 인식 및 실천 정도)

  • Purpose: The purpose of this study was to investigate the emergency-room nurse's perception and performance about the protection behavior of the consumer health information. Method: The participants were 191 emergency-room nurses in secondary, tertiary medical center nationally. The data were analyzed by SPSS 12.0 program. Result: First, the total average scores of the degree of the perception and performance by emergencyroom nurses on protection behavior of the consumer health information were $4.24{\pm}0.30$ and $3.51{\pm}0.26$. Second, the degree of the perception and performance on protection behavior of the consumer health information was heavily dependent on the following general characteristics of the participants: age, nurse position, career, and education experience on protection behavior of the consumer health information. Third, there was a positive correlation between the degree of perception and performance about the protection behavior of the consumer health information. Conclusions: The results showed the statistically significant partial correlation between the degree of perception and performance. Therefore, it is suggested to apply the concrete education program to enhance the perception in order to improve the performance.

Emergency Detection System using PDA based on Self-response Algorithm

  • The aged are faced with increasing risk for falls. The aged have more fragile bones than others. When falls occur, it is important to detect this emergency state because such events often lead to more serious illness or even death. A implementation of PDA system, for detection of emergency situation, was developed using 3-axis accelerometer in this paper as follows. The signals were acquired from the 3-axis accelerometer, and then transmitted to the PDA through a Bluetooth module. This system can classify human activity, and also detect an emergency state like falls. When the fall occurs, the system generates the alarm on the PDA. If a subject does not respond to the alarm, the system determines whether the current situation is an emergency state or not, and then sends some information to the emergency center in the case of an urgent situation. Three different studies were conducted on 12 experimental subjects, with results indicating a good accuracy. The first study was performed to detect the posture change of human daily activity. The second study was performed to detect the correct direction of fall. The third study was conducted to check the classification of the daily physical activity. Each test lasted at least 1 min. in the third study. The output of the acceleration signal was compared and evaluated by changing various postures after attaching a 3-axis accelerometer module on the chest. The newly developed system has some important features such as portability, convenience and low cost. One of the main advantages of this system is that it is available at home healthcare environment. Another important feature lies in its low cost of manufacture. The implemented system can detect the fall accurately, so it will be widely used in emergency situations.

Trends in Prevalence of Suicidal Idea, Attempt and Suicide Rate in Korea, 2006-2011 (2006-2011년의 한국의 자살생각률, 자살시도율, 자살사망률의 추세)

  • Objective : The purpose of this study was to describe and compare trends in suicidal ideation, suicidal attempt and suicide in Republic of Korea from 2006 to 2011. Such data are needed to guide policies to reduce suicidal behaviors. Methods : Data came from the 2006-2010 Korea National Health and Nutrition Examination Survey, Korea National Statistical Office and 2006-2011 Korean Epidemiologic Catchment Area Study Replication and 2007-2011 National Emergency Department Information System. Results : No change occurred between 2006-2011 in suicidal ideation, suicidal attempt, whereas suicide rate increased annually. Suicidal attempt events was decreased in age over 50. Suicidal attempts by poisoning and hanging have increased, although suicide caused by poisoning of insecticide have decreased. Especially, suicides caused by hanging have gradually increased in both sex. Conclusions : These finding suggest that fatal suicidal methods may influence increased suicidal rate. Instead of traditional suicidal process, investigation of other pathway about suicidal behaviors should be needed.

Unmarried Mother's Knowledge and Attitudes toward Emergency Contraceptive Pills (미혼모의 응급피임약에 대한 지식과 태도)

  • Purpose: This study was conducted to identify relationships among knowledge and attitudes of unmarried mothers toward emergency contraceptive pills. Methods: Data were collected through structured questionnaires from 135 unmarried mothers enrolled in 7 single mothers' facilities nationwide. Data were analyzed using the SPSS/WIN 17.0 program for descriptive statistics, t-test, ANOVA, Scheffe-test, and Pearson correlation coefficients. Results: For knowledge about emergency contraceptive pills, there were significant differences among who live with her before pregnancy, experience of past pregnancies, state of present pregnancy and preparation in using contraceptives. For attitude toward emergency contraceptive pills, there were significant differences according to age, education level and religion. There were significant positive relationships between knowledge and attitudes toward emergency contraceptive pills. Conclusion: The results of this study suggest that unmarried women should be better informed about emergency contraceptive pills, and reassured about their safety. Efforts are needed to disseminate up-to-date information to experts in sex education including nurses.

A Study on the Spatial Analysis Model to Decide Medical Institutions/Mental Health Centers for Disaster Victims

  • The National Emergency Management Agency of South Korea has established a Disaster Victims Psychology Support Center. The Disaster Victims Psychology Support Center can enable victims who got psychological damage from disasters to return to their daily lives through healing activity, field visits and advice of experts. The previous Psychology Support Center System managed the information of disaster victims through an independent database. However, this paper proposes a system that is developed to identify medical institutions and mental health centers within a distance of radius, based on the potential Hot-Spot areas of disaster victims using the GIS Systems. The proposed system can efficiently support selection of appropriate institutions for disaster victims using their location and age, classification of damage, and damaged parts of the body. Also, this spatial analysis can assist to decide on a policy based on the location of disaster victims and the extent of damage. Therefore, this paper can provide the required information to support decision making based on the concentrated areas for disaster victims.

The changes of prevalence and etiology of pediatric pneumonia from National Emergency Department Information System in Korea, between 2007 and 2014

  • Purpose: Understanding changes in pathogen and pneumonia prevalence among pediatric pneumonia patients is important for the prevention of infectious diseases. Methods: We retrospectively analyzed data of children younger than 18 years diagnosed with pneumonia at 117 Emergency Departments in Korea between 2007 and 2014. Results: Over the study period, 329,380 pediatric cases of pneumonia were identified. The most frequent age group was 1-3 years old (48.6%) and the next was less than 12 months of age (17.4%). Based on International Classification of Diseases, 10th revision diagnostic codes, confirmed cases of viral pneumonia comprised 8.4% of all cases, pneumonia due to Mycoplasma pneumoniae comprised 3.8% and confirmed cases of bacterial pneumonia 1.3%. The prevalence of confirmed bacterial pneumonia decreased from 3.07% in 2007 and 4.01% in 2008 to 0.65% in 2014. The yearly rate of pneumococcal pneumonia also decreased from 0.47% in 2007 to 0.08% in 2014. A periodic prevalence of M. pneumoniae pneumonia (MP) was identified. Conclusion: The increased number of patients with pneumonia, bacterial pneumonia, pleural effusion, and empyema in 2011 and 2013-2014 resulted from an MP epidemic. We provide evidence that the frequency of confirmed cases of bacterial pneumonia and pneumococcal pneumonia has declined from 2007 to 2014, which can simultaneously reflect the effectiveness of the pneumococcal conjugate vaccine.

Task Analysis of Paramedics of Korea Based on DACUM Method (DACUM 기법에 의한 1급 응급구조사의 직무분석)

  • Objective : This study grasps specific task on paramedics who plays a great role in the emergency scene, thereby eliciting definition of job called paramedics and analyzing occupation by DACUM method. Thus, the aim is to suggest working-level guidelines on the task of paramedics. Methods : It targeted paramedics who are working at hospitals, fire stations, and industries in Seoul and Gyeonggi area from Oct. 11, 2010 to Nov. 30. A total of 608 copies of questionnaire were analyzed by DACUM method. A research tool on occupational analysis consisted of 8 pieces for duty, 43 pieces for task, and 149 pieces for task elements. In order to survey performance frequency, importance, and difficulty by element, each task was developed by this research team, and each task was analyzed and finally elicited through workshop of DACUM method. Results : The occupational definition of paramedics, which was defined through this DACUM, was elicited as 'professional job of performing emergency medical care on the scene, during transferring, or within medical institution in order to maintain life and prevent wound deterioration, targeting a person who is put in emergency situation.' Task element, whose performance frequency was indicated to be the highest, was in order of checking mental status($2.76{\pm}0.497$), checking vital signs($2.70{\pm}0.578$), and airway/c-spine immobilization($2.69{\pm}.546$). Especially, defibrillation stood at $2.23{\pm}.655$ points. Task element, whose performance frequency was low, was in order of caring sexual abuse victims($1.32{\pm}0.563$), performing cricothyrotomy($1.36{\pm}0.618$), and caring cardiac arrest victims($1.40{\pm}0.636$). Importance of task was in order of airway/c-spine immobilization ($2.88{\pm}0.338$), maintenance of respiration($2.88{\pm}0.351$), caring cardiac arrest victims($2.87{\pm}0.349$), and performing CPR($2.87{\pm}0.361$). Task element, whose importance is low, was indicated to be in order of enema($2.29{\pm}0.736$), urinary catheterization($2.35{\pm}0.664$), and nasogastric intubation($2.35{\pm}0.667$). Task element, whose difficulty was shown to be the highest, was indicated to be in order of caring cardiac arrest victims during pregnancy ($2.60{\pm}0.559$), caring cardiovascular injury($2.59{\pm}0.546$), and labor management($2.53{\pm}0.533$). Conclusion : Based on the results of this study, the job performance work table(Dacom chart) of paramedics is suggested to be used, thereby being applied to development in education and curriculum of paramedics. It is necessary to evaluate usefulness of the job performance work table by estimating effect of education for paramedics based on the job performance work table of paramedics.

Appropriateness of Selecting the Chest Compression Site by Lay-Persons: Compared to the center of the lower half of the breastbone and above the two fingers on solar plexus

  • This study is to find out the effectiveness by adult CPR method of a single rescuer. For patients with acute cardiac arrest, CPR is the only first aid, and chest compression is the most important of these techniques. The subjects of this study were freshman, 120 subjects who did not learn CPR, and for data analysis, the SPSS 22.0 Version was used. The accuracy of chest compression site selection was significantly higher in the upper part of the two fingers in the pit of the stomach(p<.001), and the ease was also high(p<.001). In the case of the general public, they select the chest compression area easily and quickly, it turns out that it is easier to designate the landmark toward the pit of the stomach. In the future, through various job groups, research to select the chest compression site is needed, and additional studies should be conducted to confirm the maintenance and accuracy during chest compression.

Management of Traumatic Pancreatic Injuries: Evaluation of 7 Years of Experience at a Single Regional Trauma Center

  • Purpose: Traumatic pancreatic injuries are rare, but their diagnosis and management are challenging. The aim of this study was to evaluate and report our experiences with the management of pancreatic injuries. Methods: We identified all adult patients (age >15) with pancreatic injuries from our trauma registry over a 7-year period. Data related to patients' demographics, diagnoses, operative information, complications, and hospital course were abstracted from the registry and medical records. Results: A total of 45 patients were evaluated. Most patients had blunt trauma (89%) and 21 patients (47%) had pancreatic injuries of grade 3 or higher. Twenty-eight patients (62%) underwent laparotomy and 17 (38%) received nonoperative management (NOM). The overall in-hospital mortality rate was 24% (n=11), and only one patient died after NOM (due to a severe traumatic brain injury). Twenty-two patients (79%) underwent emergency laparotomy and six (21%) underwent delayed laparotomy. A drainage procedure was performed in 12 patients (43%), and pancreatectomy was performed in 16 patients (57%) (distal pancreatectomy [DP], n=8; DP with spleen preservation, n=5; pancreaticoduodenectomy, n=2; total pancreatectomy, n=1). Fourteen (31%) pancreas-specific complications occurred, and all complications were successfully managed without surgery. Solid organ injuries (n=14) were the most common type of associated abdominal injury (Abbreviated Injury Scale ≥3). Conclusions: For traumatic pancreatic injuries, an appropriate treatment method should be considered after evaluation of the accompanying injury and the patient's hemodynamic status. NOM can be performed without mortality in appropriately selected cases.
