인터넷 기반의 수신자 맞춤형 재난경보 전달시스템 연구 (A Study on Internet Emergency Alert Distribution System Adaptive to the Receiver Characteristics)

  • 효과적으로 경보를 전달하면 재난 피해를 크게 줄일 수 있다. 경보 전달 채널을 다양화 하는 것이 경보 전달 효과를 향상시킬 수있는 방법 중 하나이다. 본 논문에서는 기존 경보시스템을 분석하여 문제점을 보완한 새로운 경보시스템을 제안한다. 기존에 운영 중인 CBS (Cell Broadcasting System) 재난문자방송 서비스와 지상파 디지털멀티미디어방송 자동재난경보 시스템(이하 T-DMB 재난경보방송)은 콘텐츠의 제한, 수신 기기의 제한, 단방향 서비스라는 단점이 있다. 이러한 문제점을 해결하기 위해 RSS (Rich Site Summary)를 활용한 인터넷 기반의 수신자 맞춤형 재난경보 전달시스템을 제안한다. 이를 위해 기존 사례를 분석하고, 시나리오를 작성하여 요구사항을 도출하였다. 요구사항을 기반으로 시스템을 설계하고, 설계한 결과를 테스트베드를 구현하여 검증하였다. 제안한 시스템을 활용하면 다양한 기기에서 사용자가 원하는 풍부한 정보를 얻을 수 있을 것이다.

우리나라 병원의 경영전략 실태 (Management Strategy of Hospitals in Korea)

  • This paper investigates the current feature of management strategy of hospitals in Korea, and examines the relationships between adoption of a particular strategic orientation and the hospitals environmental and organizational characteristics, strategic behaviors and management improvement activities, and financial performance. Data were collected from CEOs of 88 hospitals among 650 hospitals for a 13.5% response rate using the self-administered questionnaire by mail survey. The major findings that obtained are as follows: 1. Only 37.2% of response hospitals carried out strategic planning, Most of these hospitals established the first strategic planning in 1991(81.3%) and renovated strategic planning by 4 or 5 years(56.3%), and modified strategic planning with flexibility(59.4%). Most strategic plans were documented, but informalized(68.8%). And only 29.0% of these hospitals had independent planning division. 2. Hospital services that CEOs assessed rank ordered for their impact on profitability are as follows: i)diagnostic ultrasound facility, computerized tomography scanner, obstetric inpatient unit, therapeutic X-ray, and physical therapy at present. ii)diagnostic ultrasound facility, physical therapy, computerized tomography scanner, emergency department, and health screening at future. And the services rank ordered that CEOs hoped to introduce are as follows: emergency department, physical therapy, health screening, volunteer services, and computerized tomography scanner. 3. Using a typology developed by Miles and Snow(l978), the strategic orientation of response hospitals are shifting significantly from defenders in the past to analyzers in the present, and to prospectors in the future(p<.01). 4. With regard to hospital environmental and organizational characteristics such as ownership, physician training, location, bed size, and hospital management training career and specialty of CEOs, the four strategic orientation archetypes varied not significantly. But, hospitals with a analyser orientation in the present and a reactor orientation in the future perceived competition significantly higher than the other three archetypes(p<.05). 5. The four archetypes rank ordered in terms of appling strategic behaviors and management improvement activities are as follows: prospector, analyzer, reactor, and defender. 6. The four archetypes differed significantly in terms of their financial performance using revenue per bed(p<.05). Reactors and prospectors in terms of total revenue per bed, prospectors in terms of outpatient revenue per bed, and reactors and prospectors in terms of inpatient revenue per bed had the best performance.

다량 객혈을 합병한 폐동정맥루 (Pulmonary Arteriovenous Fistula)

  • Pulmonary arteriovenous fistula is a congenital malformation of the pulmonary vasculature in which there is a persistence of one or more sizable communications that bypass the pulmonary capillary bed that thus directs unoxygenated pulmonary arterial blood directly into the pulmonary venous system. The developmental cause of pulmonary arteriovenous fistula is unknown, but it is postulated that the abnormal channels represent persistent vessels of the primitive splanchnic capillary bed that do not resorb during growth and maturation. We have experienced a case of pulmonary arteriovenous fistula. The 45 years old woman admitted to Kosin Medical College Hospital via emergency room because of massive hemoptysis, and was diagnosed pulmonary arteriovenous fistula by twice postoperative histopathologic examination. The postoperative course was uneventful and she was discharged on the 16th, second postoperative day in a healthy condition.

병원내 심폐소생술 모형에서 환자와 구조자의 거리 및 위치에 따른 가슴압박의 질 비교 (Comparisons of the qualities of chest compression according to various positions of rescuer to patient at the in-hospital cardiopulmonary resuscitation model)

  • Purpose: The purpose of the study is to evaluate the distance and location of the rescuer to patient for the effective chest compressions qualities. Methods: The subjects were 42 students who earned the basic lifesaving technique and had informed consents to participate in the study from May 1 to 20 in 2013. The position of the rescuers included model-0(reference point), model-1(10 cm distance), model-2(20 cm distance), and model-kn(kneeling up). Results: The mean depth of compression was $50.6{\pm}6.6mm$ in Model-0, $48.7{\pm}8.2mm$ in Model-1, $44.2{\pm}10.4mm$ in Model-2, and $51.8{\pm}6.0mm$ in Model-kn. There were statistically significant differences between each Model(p<.001). Conclusion: The closer distance between rescuer and patient could provide more effective chest compressions. Kneeling on the bed stance provided the deeper chest compression consistently than the stool stance.

Performance Analysis of Water Systems under Hazardous Conditions

  • The performance analysis of water systems is very important to urban disaster mitigation. It will benefit the task of preparedness and emergency response through a more practical and more quantitative approach. In this research work, an overview of hydraulics of water system has been provided. A methodology for such implementation based on scenario simulation and hydraulic analysis has been developed. The water system of Taipei Water Department was selected as a test bed for case study. Its serviceability following a major earthquake around Taipei metropolitan area has been quantified.

중증외상환자의 치료경과 시간과 활력징후에 따른 응급처치 및 간호활동 (Emergency Treatment and Nursing Activities of Severe Trauma Patients according to Elapsed Time and Vital Signs)

  • Purpose: The aim of this study was to identify necessary emergency treatment and nursing activities for severe trauma patients according to elapsed time and vital signs. Methods: A survey was conducted with 121 patients over 15 points ISS on EMR from June 1, 2011 to May 31, 2012. Collected data were analyzed with descriptive statistics, applying McNemar's test using SPSS 12.0. Results: Almost all of the subjects were men and the mean age was 46.9. Run-time for primary diagnosis, treatment decision, and leaving for the hospital room was 0.19, 4.36, and 4.21 hours, respectively, and stayover time was 9 hours. Regardless of vital signs, emergency treatments involving ambu-bagging, intubation, ventilator, and central vein catheterization insertion were offered within an hour. Central venous pressure, Foley catheter/Levin tube preparation and maintenance were performed in cases of unstable vital sign patients within an hour. Unrelated to vital signs, nursing activities for consciousness assessment, skin assesment and wound care, bed sore/fall down assesment and care, intravenous injection insertion and maintenance were conducted for all severe trauma patients within an hour. Foley catheter/Levin tube drainage care was performed for patients who had unstable vital signs within an hour. Conclusion: Emergency treatment and nursing activities for severe trauma patients were specific according to elapsed time and vital signs.

Virtual calibration of whole-body counters to consider the size dependency of counting efficiency using Monte Carlo simulations

  • The counting efficiencies obtained using anthropomorphic physical phantoms are generally used in whole-body counting measurements to determine the level of internal contamination in the body. Geometrical discrepancies between phantoms and measured individuals affect the counting efficiency, and thus, considering individual physical characteristics is crucial to improve the accuracy of activity estimates. In the present study, the counting efficiencies of whole-body counting measurements were calculated considering individual physical characteristics by employing Monte Carlo simulation for calibration. The NaI(Tl)-based stand-up and HPGe-based bed type commercial whole-body counters were used for calculating the counting efficiencies. The counting efficiencies were obtained from 19 computational phantoms representing various shapes and sizes of the measured individuals. The discrepancies in the counting efficiencies obtained using the computational and physical phantoms range from 2% to 33%, and the results indicate that the counting efficiency depends on the size of the measured individual. Taking into account the body size, the equations for estimating the counting efficiencies were derived from the relationship between the counting efficiencies and the body-build index of the subject. These equations can aid in minimizing the size dependency of the counting efficiency and provide more accurate measurements of internal contamination in whole-body counting measurements.

진단검사의학과의 검사기능과 공간구성에 관한 연구 - 1000병상급 종합병원을 중심으로 (A Study on the Laboratory Function and Spatial Organization for Laboratory Medicine - Focused on the Tertiary Level General Hospitals having more than 1000 Beds)

  • Purpose: As the structure of the disease has been changed and the infectious disease has been increased, the demand for diagnostic examination has been increasing. So, the department of laboratory medicine in hospital has playing the important role accounting for about nineteen percent of total medical expenses in korea. This study is to investigate the laboratory function and spatial composition of the department and figure out the space area and space organization. Methods: Explore the literature review to identify the laboratory function. Limit to five cases of tertiary level general hospital having about a thousand bed and analyze the space layout and floor area to confirm the spatial composition. Classify the exam function and check the spatial composition and spatial organization. Results: This study allows 5 conclusions to be summarized. Laboratory medicine divided into nine part in regulation, but in space allocated into core lab, emergency lab and six part lab. Total laboratory area is $2,036m^2$ in average, and is composited with 60% for lab, 17% for office and 20% for public. Lab area per a bed presents $1.88m^2$. Microbiolgy and molecular lab area are getting large. Laboratory space organized into the four zone, like an entrance zone, core zone, rear zone and peripheral zone. Emergency and transfusion lab are allocated in entrance zone, hematology chemistry and immunology in core lab, microbiology and molecular lab in rear, support offices in peripheral zone. The most important point was to check the spatial composition of the Laboratory Medicine according to the inspection function. Implications: This study can be used as a useful data in planning and designing a Laboratory Medicine Department.

심폐소생술에서 두 개의 가속도 센서를 활용한 흉부 압박 깊이 추정 (Estimation of Chest Compression Depth using two Accelerometers during CPR)

  • During the cardiopulmonary resuscitation (CPR), the correct chest compression depth and period are very important to increase the resuscitation possibility. For the feedback of chest compression depth, the depth monitoring device based on the accelerometer is developed and widely used. But this method tends to overestimate the compression depth on the bed. To overcome this limitation, the chest compression depth estimation method using two accelerometers is suggested With the additional accelerometer between the patient and mattress on the bed, the compression of the mattress is also measured and it is used to compensate the overestimation error. The experimental results show that the single accelerometer estimates as 61.4mm for the actual compression depth of 43.6mm on the mattress. The depth estimation with the dual accelerometer was 44.6mm which is close to the actual depth. With the automatic zeroing in every single compression, the integration error for the depth can be reduced. The dual accelerometer method is effective to increase the accuracy of the chest compression depth estimation.

고령자용 소규모 그룹홈의 생활환경 실태 (Actual conditions on the Living Environment of Small-scale Group-homes for the Elderly)

  • To investigate the actual conditions on the living environment of group-homes for the elderly and their living environment, 4 group-homes were surveyed. As the results, conditions of location for the group-home of the elderly looked positive, on the other hand, safety for criminal prevention, accessability and convenient facilities were not on level satisfied. The average area of each space in the group-home indicated that the bed room was $11.21\;m^2$, the living room $28.13\;m^2$, the kitchen $23.59\;m^2$, and the bath room had an integrated type of bath room and toilet $7.63\;m^2$. The physical environments of each space in the group-home were investigated, In bed room, door, windows and illumination were relatively good but safety was extremely inferior. Living room mostly looked good except the doorsill which needs refurbishment. In kitchen, cooking table, windows, illumination, ventilator were somewhat satisfactory but gas-warning device should have been installed. In addition, in bath room and toilet, location, door, window, illumination and finished material were satisfactory. But the stepped difference between the bath room and other space, safety-bar, emergency bell and floor heating system were seriously deteriorated and must be improved for the safety of the elderly. Conclusively, living environment of group-home for the elderly is mainly satisfactory but it was necessary to take a consideration for their safety.
