This study aims to present ways to enhance the stabilization of electronic medical records, ensure the commitment to filling in information of the medical record and improve the overall quality Electronic Medical Record(EMR) information. For that purpose, the present state of the incomplete record rate and the doctor's satisfaction in Electronic Medical Record(EMR) have been surveyed by comparing and analyzing Paper-based Medical Record(PMR) and Electronic Medical Record(EMR). The survey was conducted on 31 doctors in charge of EMR system and each PMR and EMR inpatients were collected for a period of 5 months and analyzed. The results showed that the doctor's satisfaction level was higher for EMR, and the rate of incomplete record appeared to be lower in EMR in departments of both internal and external medicine. In this context, it can be said that the higher efficiency of EMR helped accomplish the increase in commitment to completing medical record information and improve the quality of the data.

EMR(Electronic Medical Record)이란 종이문서 방식을 사용하는 의료차트 대신에 모니터로 출력되는 형식의 의료차트를 말한다. 현재 통용되는 EMR은 전문 의료인이 전문적인 용어 써놓은 양식이기 때문에, 그 의료기록의 실제 주인인 환자는 의료인의 부가적인 설명 없이는 자신의 자세한 증세는 물론 병명조차 정확히 확인하기 힘들다. 이에 환자에게 있어 매우 시간 소모적이고 노동집약적인 과정으로 환자가 의료인을 거쳐서 자신의 의료정보를 얻는 방식보다는 환자가 직접 그 정보를 얻는 방식이 필요하다. 따라서 본 연구는 EMR이 갖는 이러한 문제를 해결하기 위해 환자가 의료인을 거치지 않고 직접 자신의 의료정보를 얻기 위한 환자 접근행 EMR 응용 시스템의 비즈니스 모델을 제시한다.

전자정보의 기밀성과 무결성을 유지하는 가장 안전한 보안방법 중의 하나가 공인전자서명이다. 이를 의료분야에 적용하기 위하여 본 논문에서는 전자서명법과 의료법에 기반을 둔 전자의무기록에 대한 공인 전자서명의 구체적인 적용 지침을 개발하였다. 개발된 지침은 공인전자서명의 주체 및 시점, 공인인증서의 유효성 확인, 공인전자서명의 관리 책임, 전자의무기록의 보관 및 관리에 관한 적용 지침과 관련 해설을 그 내용으로 하고 있다. 향후에는 이 지침을 토대로 의료기관과 관련 업체에서 전자의무기록 시스템 구축 시 실제 활용할 수 있는 예시들을 개발하고자 한다. 이는 전자의무기록의 도입을 용이하게 하고, 그 보급을 촉진할 뿐 아니라 의료기관과 관련 업체들이 겪고 있는 애로사항을 해소하는데 크게 기여하게 될 것이다.

The rapid development and distribution of information communication industry facilitates the changes of hospital administration, introducing EMR(Electronic Medical Record) instead of paper-based medical record in the medical field. The developed countries such as U.S. have established EMR system after in the middle of 1970s because the primary advantages of EMR is to store and handle vast amounts of records efficiently and increase the quality of health care. Most of health organizations in Korea also apply medical record system to their administration. As the result, they have accomplished a scientific administration system through the use of medical record to handle a variety of patient's information including patient's confidentiality and privacy such as family history, social status, income level, and so on. However, access to and the misuse of EMR causes illegal infringement of patient's information and finally it becomes a very serious medical issue. Potential leakage and misuse of records may seriously infringe patient's privacy rights. In this respect, the related agencies in the public and private sector have been making efforts to prevent patient's records leakages. Especially, the revision bill of Medical Law in 2002 establishes the ways on the security and standards of electronic records. However, it does not provide the proper guidelines which is applied to the rapid changes of the medical environment. One of the most priorities in the hospital administration is the production and maintenance of an accurate medical records fulfilled by medical recorders. Therefore, it is very important for health care providers to hire ethical-based medical recorders. But, unfortunately most of hospitals overlook the importance of their roles. All parts including government, physician and patient must have more concerns on the problems related to EMR. Therefore, this study aims to propose the proper ways to resolve the problems coming from EMR.

최근 국 내외 의료계와 정부는 병원에서 발생하는 진료기록들을 디지털화하여 모든 의료 기관의 전자의료기록을 네트워크로 통합하여 공유하는 첨단 의료정보화에 많은 관심을 가지고 있다. 본 연구는 의료기관의 정보시스템 구축 사례연구로서 조선대학병원의 전자의무기록(EMR: Electronic Medical Record)시스템의 구축 과정을 소개하고 있다. 정보 시스템의 도입에서부터 활용단계까지 Cooper and Zmud(1990)가 제시한 정보시스템 구축 모델을 기반으로 조선대학병원의 EMR 구축 사례를 연구하여 병원정보시스템의 특성과 이슈들을 명확히 하고, 향후 EMR시스템을 도입하고자 하는 의료기관에 유익한 정보를 제공하고자 한다.

The Electronic Medical Record (EMR) is a valuable source of medical data intelligence in e-health systems. The watermarking techniques have been used to authenticate the owner and protect the EMR from illegal copying. The existing distributive strategies, successfully operated to secure the EMR, are found to be inadequate. Blockchain technology, mainly, is employed by a sharing database that allows the digital crypto-currency. It rapidly leads to the magnified expectations acme. In this excitement, the use of consortium adopting the technology based on Blockchain, in the EMR structure, is found improving. This type of consortium adds an immutable share with a translucent record of the entire business and it is accomplished with responsibility, along with faith and transparency. The combination of watermarking and Blockchain technology provides a singular chance to promote a secured, trustworthy electronic documents administration to share with the e-records system. The authors, in this article, present their views on consortium Blockchain technology which is incorporated in the EMR system. The ledger, used for the distribution of the block structure, has team healthcare models based on dissimilar multiple image watermarking techniques.

전자의무기록(EMR)[1]을 도입한 이후 현재 의료장비의 EMR 연동상황이 극히 미비한 상태로 대두되고 있다. 종합병원 의료장비의 약 30%정도는 검사 결과물이 출력이 안되고 보여지기만 하는 구조로 되어 있어 직접 수기로 EMR 에 연동할 수 있는 소프트웨어에 입력하여 EMR 에 연동하고 있으며, 약 60%는 의료장비에서 프린터 혹은 시리얼통신으로 데이터를 출력할 수 있는 구조로 되어있다. 하지만 의료장비의 노후화 및 검사결과물의 인터페이스 특성이 다양하기 때문에 쉽게 연동하지 못하고 있으며, 대부분 종이 출력 결과물을 스캔을 통해 이미지를 저장하거나 받아서 EMR 에 연동하고 있다. 그 외 나머지 10%는 의료장비 자체의 저장장치 또는 네트워크를 통해 검사결과 데이터를 EMR 에 전송하는 구조로[2] 되어있다. 본 논문에서는 의료장비의 프린터를 통한 결과물을 EMR 에 연동하기 위한 인터페이스 솔루션을 구현하고, 검사결과의 출력 데이터를 이미지 복원 및 관리하는 방법을 통해 많은 수의 의료장비의 출력 데이터를 EMR 연동을 하여 인력 및 기타 사무용품의 소모를 줄이며, 검사결과를 실시간 진단할 수 있는 방법을 제시한다.

컴퓨터에 전자적 형태로 저장된 의무기록인 EMR의 표준화에 대한 논의가 활발하다. 이는 EMR 표준화를 통해 의료 서비스의 향상을 기대할 수 있을 뿐만 아니라, 의료와 IT의 융합영역인 의료 빅데이터의 가치가 점차 높아지고 있기 때문이다. EMR 표준화와 관련한 주요 이슈 중 하나는 EMR 표준화의 필요성과 효과성을 이해관계자들에게 설득시키는 일이다. 연구자는 EMR 표준화에 대한 의사들의 인식을 기술 관점과 경제적 관점에서 살펴보고자 설문조사를 실시한 후 이에 대해 통계분석을 실시하였다. 실증분석 결과, EMR 시스템의 기능 품질과 경제적 가치는 EMR 표준화에 대한 유용성 인식과 수용 의도에 정(+)의 영향을 미치는 반면, 상호운용성은 유용성 인식에만 영향을 미치는 것으로 나타났다. 또한, 경제적 가치가 EMR 표준화 필요성에 대한 공감대 형성에 가장 중요한 변수로 확인되었다.

다양한 의료서비스를 정형화되고 효율적인 방법으로 제공하기 위하여 최근 의료기관의 EMR(Electronic Medical Record; 전자의무기록) 시스템의 도입이 급속도로 확산되고 있다. 그러나 국내에서 구축되고 있는 EMR은 초기에 기대했던 EMR의 도입목적을 중분하게 달성하지 못하고 있다. 이는 근본적으로 EMR의 방향성과 구조에 한계를 지니고 있기 때문이다. 국내에서 구축되고 있는 대부분의 전자의무기록시스템은 자료수집원 지향 의무기록 (Source Oriented Medical Record; SOMR) 방식으로 개별 사실을 수집하고 제시하기 위한 관찰자 중심, 서식중심의 시스템이며 차트 이송과 종이를 없애는 것이 주목적이다. SOMR 방식은 EMR의 다양한 도입목적, 특히 임상자료의 효과적 활용을 충족시킬 수 없는 태생적인 한계를 지니고 있기 때문에 문제지향 의무기록 (Problem Oriented Medical Record; POMR) 방식의 EMR과 적절한 혼용전략 (Hybrid Strategy)이 절실히 요구된다. 본 연구의 목적은 SOMR 시스템과 POMR 시스템이 보완적으로 결합된 하이브리드 EMR 아키텍처를 제시하고 아키텍처를 구성하는 데 필요한 기술적 요소들과 그들간의 상호관계 틀 파악한 후 이를 기반으로 개발방법론을 도출하는 것이다. 또한 만성신부전증 환자 사례를 이용하여 앞서 제시된 개발방법론에 따라 프로토타입을 개발하였다. 본 연구는 EMR 시스템이 SOMR 구조에만 편향됨으로써 발생될 수 있는 다양한 문제들을 해결함으로써 진료의 품질 항상, 임상연구의 활성화, 조직적이고 과학적인 진료계획의 수립과 같은 소외되어 왔던 EMR의 목표를 실현할 수 있을 것으로 기대 된다.

본 연구에서는 의료기관에서 진료의 질 향상을 위해 도입하고 있는 전자의무기록 (Electronic Medical Record : EMR)시스템의 수용 및 활용을 결정짓는 요인들을 분석하였다. 전자의무기록에 대한 기존 연구를 바탕으로 하여 측정변수와 요인들을 정의하였고, 현재 EMR시스템을 구축하여 운용하고 있는 종합병원으로 부터 수집된 데이터로 실증분석을 하였다. 요인들 사이의 상관관계를 분석하기 위하여 연구모델을 구축하였고, 이를 바탕으로 요인들 사이의 관계를 규정하는 가설을 설정하였고, 이 가설들을 구조방정식 (Structural Equation Model : SEM)을 통해서 분석하였다. 분석 결과에 의하면 EMR시스템의 수용 결정에 영향을 미치는 요인은 의료기관의 경영전략 이고, EMR시스템의 수용은 EMR시스템의 경영성과에 영향을 미치는 것으로 판명되었다. 본 연구의 결과는 한국의 의료기관들이 EMR시스템을 구축하거나 운용하는데 있어서 중요한 참고자료가 될 수 있을 것으로 기대한다.
