A Study on Research Trends and Development Process of Health Policy for the Elderly (노인보건정책의 발전과정 및 연구동향에 대한 일고찰)

  • This paper is to suggest policy issues to be done in the future health policy for the elderly, analyzing contents of results suggested in academic papers published in Journal of the Korean Gerontological Society. Number of Papers published in the Journal is 61, occupying most of papers related with chronic disease, medical expenditures and health delivery system except dental and psychiatric health. Method used to be analyzed is mostly random sampling among community inhabitants or hospital patients. Policy issues are as follows in case of being based in the above analysis results. Firstly, it is necessary that the development of health promotion specialized for the elderly be developed and activated. Secondly, it is necessary that the primary health care system specialized for the elderly be constructed. Thirdly, it is necessary that the subacute care system specialized for the elderly be constructed after acute care services. Finally, it is necessary that the health care system specialized for the elderly in need of long-term care be constructed also.

  • Due to the rapid population's aging, the life span's expansion and social & cultural characteristics, the standards of age classes in senescence are changed. Thus, this study aimed to identify the influential factors on health care utilization in the elderly by age class, and targeted 2,937 adults in their ages over 55 years old from the Korea Health Panel's 2009 Yearly Integrated Data. This study investigated the target health care utilization in the elderly in relation with the gender, the education level, the health-care insurance type, the income, the current job state, the chronic disease. In order to analyze the influential factors on health care utilization in the elderly, the multiple linear regression analysis was conducted to the data. As the results from this study, Concerning the influential factors on the health care utilization, the income, the subjective health state, the chronic disease and the regularly meals for the young-old influenced. Concerning the influential factors on the health care utilization, the subjective health state for the old. Concerning the influential factors on the health care utilization, the subjective health state, the income for the older-old influenced. Concerning the influential factors on the health care utilization, the education level, the spouse, the economic activity and the drinking oldest-old influenced. Therefore, it will need to provide systemic health-care & medical services, to develop health-care & medical programs and the health-care & medical policies and to execute them according to the age classes in senescence.

  • The old couple household is also growing up with the growth of the aged so this is problem that needs to consider the home welfare service aspect. Otherwise, by the decrease of the number of the child and facilities' space availability, the government searching for proper usage of that kind of space in terms of financial and administration. So this study have combining of elementary school and home care for the aged facility especially for the increase of home care for the aged service facilities and improvement of using elementary school's space. That will increase the elderly cares and have need the homewelfare-service in the future, we select study elderly welfare facilities. We use analysis that analyze the trend of an elderly and social changes and conclude for the complexation of elderly welfare facilities, compound form of an elementary school, traffic line, space organization and interaction space organization.

An Analysis of Convergence Factors on the Unmet Health Needs of the Indigent Elderly (빈곤노인의 미충족 의료와 관련된 융합적 요인 분석)

  • The purpose of this study is to identify the convergence factors affecting the unmet health needs of the indigent elderly. The data the study is the Korean medical panel of 2011 and the parameters belonging to each factors were selected based on the Anderson model. We analyzed the general characteristics using frequency analysis and the correlations between variables using cross analysis. Finally, logistic regression analysis was conducted to examine the factors affecting unmet health needs. The indigent elderly with no education and elementary school graduates were 1.5 times more likely to experience unmet health needs than the poverty elderly with high school graduates. The indigent elderly who does not work for income, who were employers and self-employeds were 1.5 times more likely to experience unmet health care need than unpaid family workers. The indigent elderly with disabilities in activites of daily living were 2.9 time more likely to experience unmet health care needs than the indigent elderly with no disability in activites of daily living. The results of this study confirm that the increase in the economic burden of medical care for the indigent elderly can lead to the unmet health needs.

Pain of Elderly Women with Osteoarthritis (골관절염 여성노인의 아픔경험에 대한 현상학적 연구)

  • The purpose of this study was to determine the meaning of the pain and experiences of elderly women with osteoarthritis, by adopting Colaizzi′s phenomenological method. The participants were 7 elderly women over the age of 65. They were selected using a theoretical sampling technique. The Data was collected by in-depth and open-ended interviews from Dec. 1. 1999. to Feb, 28. 2000. The length of the interviews varied from 120 minutes to 180 minutes. Data was recorded and analyzed by a constant comparative method. From the data, significant statements were extracted and then organized into 48 themes, which resulted in 15 clusters of themes and 6 categories. The final descriptions turned out to be valid through the interviewee′ validation process. Essential themes of the pain experiences emerged : "physical discomfortness(disturbances)", "negative state of mind", "influence of the death", "positive change in life", "Cause of pain perceived", and "change of their personal relationships". From these results, it was found that elderly women need nursing care based on a deep understanding of pain, and a reflection on their past is imperative to overcome their given situations. In conclusion, it is suggested that the care givers provide more support to solve the problems experienced by the elderly. Thus the researchers expect to provide understanding of older people and give basic data of holistic care for them.

Nursing Students' Knowledge, Attitudes, and Educational Needs Regarding Sexuality in the Elderly (일부지역 간호대학생의 노인에 대한 성지식과 태도 및 교육요구도)

  • Purpose: This study was investigated the knowledge, attitudes, and educational needs of nursing students regarding the sexuality of elderly individuals. Methods: This was a descriptive study. Data were collected from April $1^{st}$ to $22^{nd}$, 2016. One hundred ninety-three nursing students in Daejeon city and Chungcheongnam-do were surveyed. Data were analyzed using SPSS 21.0. Results: Mean scores on the knowledge, attitudes, and educational needs measures were 15.74/29, 28.45/40, and 36.67/50 respectively. There were significantly differences in major variables according to needs for sexual education of the elderly. There were also significant differences in knowledge and educational needs depending on students' age and year in school. Higher levels of knowledge of sexuality were significantly correlated with more positive attitudes and higher educational needs. Positive attitudes were also correlated with higher educational needs. Conclusion: Nursing students showed a low level of knowledge and poor attitudes towards sexuality in the elderly. This suggests that there is a need for education on the sexuality of the elderly in the nursing curriculum. This education would guide nursing students to obtain correct knowledge and positive attitudes so that they would be able to provide effective and appropriate sexuality care for elderly patients.

The Need for, Satisfaction with, and Availability of Local Healthcare Services for Elderly Individuals Receiving Home-Care Services (재가노인의 지역 보건의료서비스 요구도, 만족도 및 이용도 - G시 보건소를 중심으로 -)

  • This descriptive study investigates the need for, satisfaction with, and availability of local healthcare services for elderly individuals. The subjects included 210 individuals aged 65 years over, and data were analyzed using SPSS. The subjects reported the limited availability of local healthcare services but a high level of satisfaction with and a strong need for these services. The subjects used these services mainly for their vaccination and perceived cost-effectiveness as the most important factor motivating their decision to use these services and a lack of publicity as the most important factor discouraging their use. Spouses, jobs, and health conditions showed significant differences in the level of satisfaction and availability, and diseases showed significant differences only in the need. These results suggest that local healthcare centers should provide services that many elderly patients can use through outreach services, connections to local communities, and active publicity instead of expanding service types.

The Quality of Life and Health Behavior Among the Elderly in Long Term Care Insurance Based on Korea National Health and Nutrition Examination Survey (KNHANES) (노인들의 건강행태와 삶의 질 수준 -국민건강·영양조사 중 노인장기요양보험 수혜 대상자를 중심으로-)

  • The purpose of this study was to assess the level of quality of life and related factors among the elderly in Korea. In particular, we focused on factors affecting the quality of life of the elderly in long term care. We used the third Korean National Health and Nutrition Examination Surveys (KNHANES) from 2005. We sampled a total of 3,571 (10.5%) elderly from the national survey. We compared the mean of quality of life to socioeconomic status, Activities of Daily Living (ADL), health behavior, and disease variables. We used EuroQol-5D among KNHANES to assess the quality of life. In this study, the mean score of the quality of life among the elderly was 2.57. Logistic regression showed that the elderly who were male, with spouses, with health insurance, and with good ADL levels enjoyed higher quality of life scores and odds ratios than those who were female, divorced, uninsured, and with low ADL levels (p<.05). The quality of life of the elderly was affected by socioeconomic, ADL, health behavior and disease variables. To improve long term care and the quality of life among the elderly, we need detailed research to clarify the effects of these factors.

Home Care Nursing Needs of Welfare Solitude Elderly Person in Pusan City (부산시 생활보호대상 독거 노인의 가정간호 요구 조사)

  • This study was carried out to assess home health care needs for welfare of solitude elderly person. The subjects of this study were 90 welfare solitude elderly person in Pusan City. Data were obtained by interview and observation using a structured questionnaire. The collected data were analyzed by t-test, ANOVA with SAS program. The results are as follows. 1. Among the home care nursing needs, environmental nursing needs topped the list, followed by spiritual, physical, psychological, communication & health knowledge and ADL nursing needs. 2. Home care nursing needs showed a significant differences by the general characteristic of the respondents. .In the age, there were significant differences in physical and communication & health knowledge nursing needs. .In the religion, there were significant differences in spiritual nursing needs. .In the marital status, there were significant differences in environmental nursing needs. .In the nursing provider, there were singificant differences in ADL, environmental and communication & health knowledge nursing needs. .In the medical benefit, there were significant differences in ADL, environmental, physical and communication & health knowledge nursing needs. .In the disease, there were significant differences in ADL nursing needs.. .In the household maintenance, there were significant differences in environmental nursing needs.

Needs for Home Care Nursing in the Vulnerable Elderly

  • Purpose. The purpose of this study was to determine the subjects' health status according to the needs of visiting health and the function of the family in home care nursing. Sample and Method. The data collection period was from 07/01/04 to 10/31/04 and the subjects were 488 of those above 60 years of age staying at home or living alone who registered at a visiting health service of public health center at an urban area in Korea. This survey was carried out by visiting health nurses and participation was agreed on by the elderly people. Results. The extent of the subjects' total health status to the general characteristics had differences according to the age, sex, monthly income, perceived health status, known functional disorder, and yes-or-no for disease. At all health status domains, visiting health need care in the group I was very lower than one in II, III, or IV groups. Also the severe dysfunctional family was lower than lightly dysfunctional family and normal functional family in all health status domains. Conclusion. Nurses must provide their characteristics considered nursing intervention for the elderly who have high visiting health needs and severe dysfunctional family with vulnerable health care.
