Purpose: This study investigated evidence for the effects of community-based cognitive rehabilitation programs for demented elderly. Methods: The literature review covered studies between 2004 and 2014, and searched international databases including Ovid-MEDLINE, EMBASE, and the Cochrane Library. The Risk of Bias Assessment Tool for Non-Randomized Studies was used by two reviewers to evaluate independently the quality of the selected articles. Results: Eighteen studies were selected through a systematic search of the literature. About three quarters of the studies showed that the community-based cognitive rehabilitation programs provided by nurses were significantly effective in enhancing cognition among elderly individuals with dementia. Conclusion: Based on these results, we suggest that nurse is an appropriate profession to manage cognitive rehabilitation or cognitive enhancement programs for demented elderly individuals. Therefore, the current system in which caregivers provide cognitive rehabilitation programs for demented elderly individuals must be reformed.

Presently, the world population of the elderly is growing. By improving health hygiene and welfare indicators, mortality and birth rates decrease and life expectancy increases, making the present century the century of elderly. Aging is one of the main risk factors for development of cancer, which itself is the second cause of death in old people. This study was conducted to assess the prevalence of cancer in the elderly covered by the Islamic Republic of Iran Broadcasting (IRIB) insurance program and to obtain suitable programs for cancer screening and early detection, increase patient survival, improve elderly care and to reclaim the cost of treatment in comparison to the national and international statistics. This is a cross-sectional study conducted on all elderly patients diagnosed with malignancy based on their pathology reports. In this study, of the total 75,500 patients covered by IRIB insurance, 17.2% belonged to the elderly group, males accounting for 53.3%. The most common cancers in old men were prostatic cancer (61.3%), colon cancer (10.3%) cancer of the hematologic system, bladder cancer (9.6%), lung cancer (9.1%), thyroid cancer (3.9%) and brain tumors (1.3%). In the elderly women, the most common cancers were breast cancer (80.1%), colon cancer (5.1%), thyroid cancers (4.4%), bladder and hematologic system malignancies (3.6), lung cancer (2.9%) and brain tumors (0.7%). In addition, the prevalence of cancer was almost the same as national and international statistics. With the exception of non-melanoma skin cancer no difference was shown in prevalence of cancer between IRIB elderly patients and the other groups of cancer patients in Iran.
This study investigates the influence of cane length on the weight distribution of the elderly in a standing position. Thirty participants were evaluated using two different cane lengths based on measurements of distal wrist crease to ground (WC cane), and distance of greater trochanter to ground (GT cane). A limloader was used to determine the weight distribution on the subject. It was found that 6.5% of body weight was supported on a GT cane and 7.7% of body weight supported on a WC cane in a standing position. Results suggests that more weight is distributed on a WC cane than a GT cane in the elderly.
Objective: This study investigated the effect of a video game-based home exercise program on the improvement of balance and muscle strength in the elderly. Design: Randomized controlled trial Methods: Participants were randomly assigned to two groups: a video game-based home exercise training group (experimental group, n=16) and a control group (n=17). The experimental group trained three times a week for 50 minutes per session for six weeks, and the control group did not perform any exercise. To evaluate postural balance, one leg standing test, Berg balance scale, functional reach test, and timed up and go test were performed, and five times sit-to-stand was performed to evaluate lower extremity muscle strength. Results: In the experimental group, there were statistically significant improvements (p<0.05) in one leg standing test, Berg balance scale, functional reach test, and timed up and go test, and five times sit-to-stand after the intervention, but in the control group, the difference before and after the intervention was not significant. The difference between the groups according to the experimental method showed a significant increase in the experimental group (p<0.05). Conclusions: These results suggest that a video game-based home exercise program effectively improves balance and muscle strength in the elderly. A video game-based exercise program can contribute to the rehabilitation of the elderly as a method of home remote rehabilitation.
Purpose: The purpose of this paper was to verify effects of the team approach rehabilitation program on balance, gait, and muscle strength of lower extremities of elderly people with Parkinson's disease. Method: Subjects of this paper were 40 elderly people with Parkinson's disease, 20 control and experimental groups respectively, who could walk independently and were less than the 2.5 Hoehn &Yahr stage. The team approach rehabilitation program was applied to the experimental group for 12 weeks. Results: There was significant decrease in second at timed up &go test (p= .008), but no significant difference in reach length at functional reach test (p= .201) with partial improvement of balance. There was no significant difference in second at 10-meter walk test (p= .070), but showed tendency of improvement of gait. And number of times at 30s-chair stand test, indicating muscle strength on lower extremities, increased significantly (p= .029), Conclusion: The team approach rehabilitation program has demonstrated its effectiveness on improving balance, and muscle strength of lower extremities for the elderly with Parkinson's disease.
Objective: This study aims to conduct a comprehensive review of monitoring systems to monitor and manage physical function of community-dwelling elderly living alone and suggest future directions of unobtrusive monitoring. Design: Literature review Methods: The importance of health-related monitoring has been emphasized due to the aging population and novel corona virus (COVID-19) outbreak.As the population gets old and because of changes in culture, the number of single-person households among the elderly is expected to continue to increase. Elders are staying home longer and their physical function may decline rapidly,which can be a disturbing factorto successful aging.Therefore, systematic elderly management must be considered. Results: Frequently used technologies to monitor elders at home included red, green, blue (RGB) camera, accelerometer, passive infrared (PIR) sensor, wearable devices, and depth camera. Of them all, considering privacy concerns and easy-to-use features for elders, depth camera possibly can be a technology to be adapted at homes to unobtrusively monitor physical function of elderly living alone.The depth camera has been used to evaluate physical functions during rehabilitation and proven its efficiency. Conclusions: Therefore, physical monitoring system that is unobtrusive should be studied and developed in the future to monitor physical function of community-dwelling elderly living alone for the aging population.

The purpose of this study is to examine the utility of central auditory processing tests as early diagnostic tools for identifying the elderly with MCI. Subjects and Methods: This study was conducted on 20 elderly patients with MCI and 20 healthy matched peers. The speech perception ability in a quiet environment and in the presence of background noise and also temporal resolution were assessed by using Speech Perception in Noise (SPIN) and Gap in Noise (GIN) tests, respectively. Results: The results indicated that the ability to understand speech in a quiet environment did not differ significantly between the two groups. However, SPIN at the three signal-to-noise ratios and the temporal resolution scores were significantly different between the two groups (p<0.001). Conclusions: Individuals with MCI appear to have poorer speech comprehension in noise and a lower temporal resolution than those of the same age, but without cognitive defects. Considering the utility of these tests in identifying cognitive problems, we propose that since the GIN test seems to be less influenced by intervening factors, this test can therefore, be a useful tool for the early screening of elderly people with cognitive problems.

노인과 경증 뇌질환(치매 및 뇌졸중 등) 환자들 중 일정 정도의 자력 행동 능력과 인지 능력이 있는 경우에는 운동과 인지 재활을 병행해야만 하는 경우가 많다. 그러나 기존의 재활 시스템은 대부분 운동 또는 인지 재활로 분리 또는 특화 되어 있는 경우가 많아서 두 가지 모두를 연계하는 형태에 대한 필요성이 있다고 생각된다. 본 논문에서는 ICT 기술의 하나인 CAN 통신 프로토콜을 이용하여 운동과 인지 재활을 병행할 수 있도록 하는 게임형 재활시스템을 제안하고자 한다. 이 시스템은 CAN BUS 구조를 이용하여 재활 시스템을 구성하는 각각의 개별 모듈들을 다양한 형태로 분리 및 결합 사용이 가능하도록 한다. 환자(또는 노인)의 인지 능력과 운동 능력의 정도에 따라 다양한 형태의 재활 환경을 구현할 수 있음을 보이고자 제안된 시스템의 프로토 타입을 실제로 제작하였다. 아울러 이 시스템을 이용한 몇 가지 실험을 통하여 여러 환경에 적용 가능한 운동/인지 병행 재활의 구현 가능성을 보이고 있다.
Purpose: The purpose of this study was to identify the risk factors for falls among the elderly living in community. Method: This was a descriptive study. The subjects comprised 80 elderly aged 65 years or over living in D city. The instruments used for this study was the MDS-HC V2.0. The data was analyzed by SPSS Win 14.0 using t-test, ANOVA, and Pearson correlation. Result: Among the socio-demographic characteristics, the significant differences were found in ADL by gender and marital status. Older elderly(${\geq}85$) used more medications than other elderly. The elderly were use average 4.33 medications, but 46.3% of the subjects didn't have the physician reviewed all medications. There was statistically significant correlation between number of medications and accidental falls. Conclusion: Nurses should inspect about the number of medications, drug interaction, and side effects. It would be preventive intervention for falls in elderly.
