Development and Validation of the Self-Care for Aspiration Pneumonia Prevention Scale in the Community Dwelling Elderly with Risk of Dysphasia (삼킴장애 위험 지역사회 재가노인들의 흡인성 폐렴 예방을 위한 자가간호 측정도구 개발)

  • Purpose: The purpose of this study was to develop and validate a Korean version of the Self-Care for Aspiration Pneumonia Prevention (SCAPP-K) scale in older adults at risk of dysphasia. Methods: The Hertz and Baas model of scale development and validation was used. In the development stage, items were generated via literature review and interviews with medical experts, older adults, and caregivers. Ten experts assessed the items for content validity. Subsequently, 12 older adults participated in a pilot test to determine the comprehensibility and appropriateness of the SCAPP-K scale. The validation stage involved a cross-sectional survey with 203 older adults for exploratory factor analysis (EFA) and 200 older adults for confirmatory factor analysis (CFA) and to determine convergent and discriminant validity. To test the validity and reliability of the scale, EFA using principal component analysis with varimax rotation and CFA were conducted, and convergent and discriminant validity as well as internal consistency reliability were determined. Results: As a result of EFA, three self-care factors (knowledge, resources, behaviors) with 21 items were validated. The CFA and convergent and discriminant validity indicated the applicability of the three-factor self-care scale. The reliability of the SCAPP-K scale was acceptable, with Cronbach's α=.87~.91. Conclusion: The SCAPP-K scale has acceptable validity and reliability and can contribute to clinical practice, research, and education to improve self-care for the prevention of aspiration pneumonia in older adults at risk of dysphasia.

Perception, Attitude, and Knowledge about Physical Restraints among Nursing Personnel in Long Term Care Facilities (노인시설 간호제공자의 신체적 억제대에 대한 인식, 태도 및 지식에 대한 연구)

  • Purpose: The present study was performed to identify the perception, attitude, and knowledge regarding the use of physical restraints among nursing personnel working at long-term care facilities. Method: 289 nurses, nurse aids and private caregivers working at 13 wards from 7 facilities participated in the survey. Perception, attitude and knowledge regarding the use of physical restraints were evaluated using the Perceptions of Restraints Use Questionnaire developed by Evans and Strumpf(1993) and Physical Restraint Questionnaire suggested by Janelli, et al(2006). Results: Wrist restraints were used the most frequently. The perception score regarding the use of physical restraints was 3.35, which is a moderate value. Study respondents considered 'safety measure' and 'safe use of a medical device' as the most important aspects when applying physical restraints to elderly. The respondents reported physical restraints were therapeutic at least to some degree but knowledge level about physical restraints of the respondents was rather low. Conclusion: Perception and attitude toward physical restraints were identified. The lack in knowledge about physical restraints needs to be addressed for more efficient use of them.

Caregiver Burden in Caring for Elders Before and After Long-term Care Service in Korea (장기요양서비스 전.후 가족의 수발 부담감에 영향을 미치는 요인)

  • Purpose: Purposes of this study were: evaluation of family burden of caring for elders who receive long term care services, and examination of differences in burden before and after the introduction of long term care service in Korea. Methods: Data were collected by questionnaires from 416 caregivers of elders who were registered with the Long Term Care Insurance Corporation in six cities. Data were collected in September, 2010 and analyzed using descriptive statistics, paired t-test, and ANOVA with the Scheffe test, and stepwise multiple regression. Results: Family burden decreased significantly after longterm care service was initiated. Subjective burden decreased from 2.93 to 2.69 (t=11.78, $p$<.001), and objective burden, from 3.40 to 3.10 (t=12.73, $p$<.001). Stepwise multiple regression analysis revealed that factors affecting subjective burden were family relations (F=13.60, $p$=.003), age (F=5.47, $p$=.019), job (F=6.98, $p$=.008), and education (F=4.59, $p$=.032), and that factors affecting objective burden were living together (F=17.66, $p$<.001), job (F=13.34, $p$=.003), monthly income (F=6.61, $p$=.010), and type of service (F=6.62, $p$=.010). Conclusion: The results of this first study to investigate caregiver burden after the Korean Long-term Care Insurance System was begun provide positive information for the development of strategies to decrease family burden in long term care.

A Multi-tier Based Lying Posture Discrimination Algorithm Using Lattice Type Pressure Sensors Allocation (격자형 압력 센서 배치 구조를 이용한 다층 기반 누운 자세 판별 알고리즘)

  • Patients with dementia or elderly patients who can not move at all by themselves are at a high risk of falls and bedsore due to lack of caregivers. In this paper, to solve this problem, we propose an algorithm to determine the patient's lying postures by discriminating the main body parts such as head, shoulders, and hips based on the pressure intensity sensed at regular intervals. A smart mat with a lattice structure in which a pressure sensor is arranged so that the body part can be discriminated irrespective of the physical characteristics has been implemented. It consists of two modules of $7{\times}7$ array size. Each module consists of 49 FSR-406 sensors and independently senses pressure. For each module, the body part corresponding to the upper body or the lower body is sequentially discriminated by using a pressure distribution such as a cumulative pressure sum using a filter. The proposed algorithm can identify five lying positions by examining the inclusion relationship between body parts belonging to layer-1 such as head, shoulder, and hip area.

  • The purpose of this study examine to identify the mediating attitudes toward the geriatric patient in the relation between emotional work and burnout. Data were collected by self-report questionnaires targeting 142 caregiver in D city and G city. Data were analyzed with SPSS/PC ver. 21.0 programs. There were significantly positive correlations between toward the geriatric patient, emotional work and burnout. As a result of analyzing the main factors affecting the level of care performance for attitudes toward caring the geriatric attentiveness, health state, work type appeared to be significant explanatory variables, and this model's explanatory power was 31%. Therefore, it is necessary to apply the education program to improve the social awareness in addition to the stable work environment of the nursing care provider in order to raise the attitude of the caregiver to care for the elderly.

Trends of Industrial Injuries among Long-Term Health Care Workers in Korea (한국 요양보호사 산업재해의 연도별 변화추이)

  • Objectives: To investigate the trends of industrial injuries among long-term health care workers in Korea Methods: T7866 injuries were selected from the total industrial injuries approved by the Industrial Accident Compensation Insurance Act(Occupational Safety and Health Act) among long-term health care workers between 2007 and 2016 in Korea. We analyzied the trends of industrial injuries according to work process, occurrence type, and causes. Results: The industrial injuries among long-term health care workers increased since 2012. The mostly occurred area for industrial injuries were low back areas, which is related that the most serious industrial injuries occurred when the one long-term healthcare worker lift manually the recipient, from bed(ondol, Korean floor heating system) to a wheelchair, bed to bath bed, and wheelchair to bath chair. In addition to this, lack of workforce, increased work intensity due to overwork contributed the increasing of occupational injury. Conclusions: This study suggests that the main causes of industrial injuries were Lack of facilities and equipment for small private long-term care institutions, The physical load that goes into lifting the recipient directly, work intensity such as excessive workload and increased work speed. We suggest that the social publicization of long-term care service for the elderly, avoiding ways to lift recipients directly, introducing lifting machines as well as improving working methods, and reducing the workload of caregivers are required.

Relationships Among Musculoskeletal Problems, Sleep Problems, and Self-Rated Mental Health of Home-Care Workers: Focusing on the Mediating Effect of Sleep Problems (방문 돌봄 노동자의 근골격계 자각증상, 수면문제와 주관적 정신건강 간의 관련성: 수면문제의 매개효과를 중심으로)

  • Purpose: This study aimed to identify the relationship among musculoskeletal problems, sleep problems, and self-rated mental health of home-care workers. Methods: Data were collected from 447 home-care workers spanning three occupation types: life supporters for the elderly, home-visit caregivers, and life supporters for the disabled. Musculoskeletal problems, sleep problems, and self-rated mental health were assessed using structured questionnaires. Factors affecting self-rated mental health were analyzed using multiple regression. SPSS was used to test the mediating effects of sleep problems on musculoskeletal problems and self-rated mental health. Results: Among the general characteristics, the variables that showed significant differences in musculoskeletal problems were monthly income level, caring-related career duration, weekly working hours, and occupation type; and the variable that showed significant differences in self-rated mental health was occupation type. Among the occupation types, supporters for the disabled had the most musculoskeletal problems and the lowest self-rated mental health. Musculoskeletal problems among home-care workers had a direct negative effect on self-rated mental health and indirect negative effects on sleep problems. Conclusion: Measures are needed to reduce the differences in working conditions and health status among the occupation types of home-care workers. Considering the relevance between the health issues of home-care workers, the development of a carefully designed health promotion strategy is required.

Design Method of Active Standing-to-Walking Assistive Device for Rehabilitation Therapy (재활치료를 위한 능동형 기립-보행 보조기구 설계 방법)

  • Rehabilitation assistive devices not only assist the rehabilitation therapy and daily life of the disabled and the elderly, but also assist the labor of their caregivers, so various functions are required to improve their quality of life. In this study, a design method considering its practicality is introduced for an active rehabilitation assistive device that can perform both standing and walking assistance by driving various actuators. For this purpose, the force required to assist standing was calculated using statics with the body segmentation method. Also, the overturning stability of the device was verified for various physical conditions and postures. The actuator in the active rehabilitation assistive device was operated by a patient using a graphical user interface in an embedded computer and a touch panel for easy usage. The detailed design was performed for implementation through the help of 3D-CAD and the finite element analysis, and a prototype was produced. Finally, it was proven that the design goal was satisfied by experimental validation.

Deep Learning-based Abnormal Behavior Detection System for Dementia Patients (치매 환자를 위한 딥러닝 기반 이상 행동 탐지 시스템)

  • The number of elderly people with dementia is increasing as fast as the proportion of older people due to aging, which creates a social and economic burden. In particular, dementia care costs, including indirect costs such as increased care costs due to lost caregiver hours and caregivers, have grown exponentially over the years. In order to reduce these costs, it is urgent to introduce a management system to care for dementia patients. Therefore, this study proposes a sensor-based abnormal behavior detection system to manage dementia patients who live alone or in an environment where they cannot always take care of dementia patients. Existing studies were merely evaluating behavior or evaluating normal behavior, and there were studies that perceived behavior by processing images, not data from sensors. In this study, we recognized the limitation of real data collection and used both the auto-encoder, the unsupervised learning model, and the LSTM, the supervised learning model. Autoencoder, an unsupervised learning model, trained normal behavioral data to learn patterns for normal behavior, and LSTM further refined classification by learning behaviors that could be perceived by sensors. The test results show that each model has about 96% and 98% accuracy and is designed to pass the LSTM model when the autoencoder outlier has more than 3%. The system is expected to effectively manage the elderly and dementia patients who live alone and reduce the cost of caring.

  • In Korea, as the elderly population increases, elderly medical facilities and nursing care workers are increasing in labor force, but nursing care workers are avoiding nursing care workers due to poor working conditions and many physical and mental exhaustion of nursing care workers. Therefore, in this study, 284 nursing care workers were surveyed to search resources and to find alternatives to enhance the quality of life of caregivers. The result of research, The first, effected positively on the quality of life in environmental and social on a married women. The more a religion, effected positively on the quality of life in environmental and social, psychological. The higher the salary, effected positively on the quality of life in environmental and social, physical. The second, the support of a Superior effected positively on the quality of life in environmental and physical and the support of a colleague effected positively on the quality of life in social and physical. The third, the support of a Superior moderated by self-efficacy of the quality of life in the social and psychological. This results suggest that can be influential variables the support of social and the self-efficacy as an alternative to burnout of the care workers.
