A research project, the "Construction of Frailty Cohort for Elderly and Intervention Study," funded by the Ministry of Health and Welfare, has been ongoing since December 2015. The Korean Frailty and Aging Cohort Study (KFACS) aims to identify risk factors for adverse outcomes associated with frailty in community-dwelling older adults, as well as means of prevention. KFACS is a multicenter, longitudinal study, with the baseline survey being conducted in 2016-2017. The sample (n=3,000) consists of those aged 70-84 years, stratified by age and gender, recruited from urban and rural regions nationwide. An in-person interview and health examination are performed every 2 years. This project is also conducting many intervention studies. Project interventions focus on nutrition and exercise. The nutritional intervention study compares the effects of 1.2 g/kg versus 1.5 g/kg daily protein intake on sarcopenia and frailty in older Koreans. The exercise trial examines the effectiveness of an information and communication technology-based program in preventing or reducing frailty.
Purpose: This study was to examine the effect of a taping therapy on pain relief and the improvement of daily living for elderly having degenerative knee arthritis. Method: This study was conducted with a non-equivalent control-group pre-test and post-test design. Data were collected conveniently with 63 elderly who had having(a) pain due to degenerative knee arthritis, and (b) inconvenience in daily life(30 for an experimental group and 33 for a control group). The subjects were recruited from the elderly, participating in welfare programs held in a welfare organization and day-care facilities. The experimental group received an intervention of taping therapy offered twice a week, for 4 weeks. The data collection from the experimental group was done from the beginning of the therapy throughout two weeks later after the end of the therapy. Results: For the experimental group, pain scores came to more decreased significantly, as the periods in which taping therapy was conducted were getting longer. Physical function scores became also more decreased at significantly level, as taping therapy was more conducted. However, compared to the control group, the score change for the experimental was not significantly showed in physical function after the therapy ended. That is, there was no longer-lasting effect on physical function improvement. Conclusion: This study found that this therapy could be a useful self-management method that the elderly with degenerative knee arthritis can use easily at home. Because of insignificant result in longer-lasting effect, this taping therapy would be applied properly with the interval of 2~3days.
Objectives: The purpose of this study was to investigate the influencing factors of removable dentures satisfaction in the elderly. Methods: A self-reported questionnaire was filled out by 256 elderly in Jeollanam-do from September 1, 2013 to June 30, 2014. The questionnaire consisted of general characteristics of the subjects, denture related characteristics systemic health characteristics and dental health behavior. Removable dentures satisfaction was adapted from Ban. The questionnaire for Removable dentures satisfaction included general treatment satisfaction, masticatory function satisfaction, denture retention satisfaction, aesthetic satisfaction measured by Likert 5 scale. Cronbach's alpha was 0.850 in the study. Data were analyzed for a t-test, one-way ANOVA and multiple regression analysis by using SPSS(SPSS 18.0, USA) program. Results: According to subjective and systemic health condition and oral health condition, there was a statistically significant increase in general satisfaction of treatment, masticatory satisfaction of function, denture satisfaction of retention, aesthetic satisfaction. The overall satisfaction for removable denture showed a significant improvement. In the multiple regression analysis, variation of removable denture satisfaction was positively associated with oral health status{good(b=0.736, p=0.000)}, denture treatment services{dental hospital clinic(b=0.327, p=0.023)}, and systemic health status{good(b=0.241, p=0.047)}. Conclusions: Satisfaction of removable dentures may have a positive impact on oral health condition, hospital type, and associated systemic disease. It is necessary to develop incremental care programs for oral health and systemic health and to make public opinion to encourage the program.

The purpose of this study was to investigate spirometric lung pattern, respiratory function and degree of fatigue by lung function tests and fatigue tests of 39 elderly people in a care facility aged 65 and over. The respiratory function tests were used to the Spirovit SP-1 and fatigue tests were used modified Piper fatigue scale(mPFS). Regarding the respiratory function, the FVC was $1.41{\pm}0.36$l, the FVC % predicted was $69.10{\pm}14.98$%, the $FEV_1$ was $1.02{\pm}0.31$l, the $FEV_1$ % predicted was $63.27{\pm}16.05$%, the $FEV_1$/FVC was $72.77{\pm}13.40$%, and the fatigue score was $5.83{\pm}1.09$. As for the spirometric lung pattern, 19 patients had a restrictive pattern(48.7%), followed by 11 with a mixed pattern(28.2%), 5 with an obstructive pattern( 12.8%), and 4 with a normal pattern(10.3%). Regarding the respiratory function and fatigue by spirometric lung pattern, the FVC and the FVC % predicted of patients with a normal pattern or an obstructive pattern were greater than other groups at a statistically significant level. As for the $FEV_1$, that of patients with a normal pattern was significantly higher than others, and for the $FEV_1$ % predicted, that of patients with a normal pattern or a restrictive pattern was significantly higher(p<.001). Fatigue score by patients with a normal pattern was significantly less than patients of other patterns(p<.001). Therefore, pulmonary physical therapy is considered necessary to improve respiratory function and fatigue degradation of elderly in a facility.
The prevalence of undernutrition in hospital populations is known to be high. The presence of malnutrition is associated with depression, infections, sarcopaenia, falls, fractures, reduced autonomy and increased mortality. This study specifically examined the prevalence of malnutrition in patients aged 65 or older at the time of admission as determined by the Mini Nutritional Assessment (MNA) which has been a frequently used nutritional risk screening tools in detecting undernutrition in old people. This study was done for one hundred eight hospitalized geriatric patients in Seoul National University Bundang Hospital, Seoul, Korea. On admission baseline history, anthropometrics measurements, laboratory data and nutritional status by MNA were assessed. Length of hospital stay was obtained by reviewing medical charts. We used one-way analysis of variance to compare the differences in variables. Spearman's rank correlation coefficients were calculated for associations between MNA and variables. On admission, $22.3\%$ of patients were malnourished and $40.7\%$ were at risk of malnutrition according to the MNA. Percent of ideal body weight, anthropometrics data, albumin, and hemoglobin were lower in the malnourished patients (p<0.05). The malnourished patients stayed in the hospital 7.3 days longer, as compared with well nourished patients (p<0.05). Percent of ideal body weight, albumin, hemoglobin and total cholesterol were correlated inversely with nutritional status according to MNA (p<0.05). MNA can be used for nutritional assessment in Korean old people, because MNA significantly correlated with other nutritional assessment parameters, such as, anthropometric and laboratory data in hospitalized geriatric patients. The high prevalence of malnutrition in the elderly was observed and the presence of malnutrition on admission predicted a significant increase in the length of hospital stay in this study. Therefore further studies are needed to determine whether nutritional interventions in old people with low MNA scores can improve clinical outcomes during the hospital course.
Purpose: The purpose of this study to develop a fringed fall prevention program based on King's goal attainment theory and education. This study is applied to the personal, interpersonal, and social systems of fall high-risk patients to test its effects. Methods: This study was a nonequivalent control group pre- and post-test design. There were 52 fall high-risk patients in the experimental group and 45 in the control group. The experimental group received six sessions, with the group sessions lasting 60 minutes and the individual sessions lasting 20~30 minutes. Data were analyzed using descriptive statistics, an ${\chi}^2-test$, a paired sample t-test, and a Wilcoxon signed-ranks test utilizing IBM SPSS software. Results: For the 3-month intervention period, the fall prevention program was found to be particularly effective for patients in the experimental group (from 3.38 to 1.69 per 1000 patient days; p=.044), as opposed to the control group (from 1.94 to 1.49 per 1000 patient days; p=.300). For the 6-month follow up period, the fall prevention program was again found to be effective for patients in the experimental group (from 3.26 to 0.76 per 1000 patient days; p=.049) compared to the control group (from 1.98 to 1.01 per 1000 patient days; p=.368). Conclusion: These results indicate that the fringed fall prevention program is very effective in reducing falls, not only during the intervention period, but also after the intervention period has ended. We can therefore recommend this program for use concerning fall high-risk patients in long-term care hospitals.
본 연구의 목적은 병동 간호사를 대상으로 간호진단을 이용하여 노인환자 간호 교육요구를 파악하는 것이다. 연구대상은 최근 1년 이내 입원 노인환자 간호경험이 있는 병동 간호사 245명으로 국내 온라인 간호사 카페 두 개를 통해 2019년 11월 1일부터 30일까지 자료 수집했다. 연구결과 43개 간호진단은 급성기 간호, 일상생활 간호, 교육 및 상담, 환경 및 자원관리, 건강증진, 노인병관리의 6개 영역으로 분류되었고 노인환자 간호 교육 요구도는 간호사의 연령(F=5.42, p=.005), 성별(F=3.92, p=.049), 결혼 여부(F=3.92, p=.049; F=19.00, p<.001; F=4.18, p=.042; F=8.78, p=.003), 학력(F=3.29, p=.039), 병원 규모(F=5.30, p=.006), 근무경력(F=7.03, p=.001)에 따라 차이가 있었다. 노인 및 노인환자 간호를 효과적으로 수행하기 위하여 연속적인 교육 요구도 조사와 세분화된 프로그램 개발이 계속되어야 할 것이다.
Background and Purpose: Dual walking task such as crossing over an obstacle may serve as an excellent tool for predicting early cognitive decline. Thus, this study aimed to investigate correlation between walking while crossing over an obstacle and executive functions under different gait phases to validate the use of walking with an obstacle for predicting early cognitive decline. Methods: A cross-sectional study was conducted on 48 elderly individuals from 2 day-care centers and 3 welfare-centers in Seoul and Gyeonggi, Korea. Executive function tests (Trail Making Test, Stroop test) and dual walking tests (gait speed, cadence, stance time, gait cycle time) were performed and compared using partial correlation analysis. Results: There were significant correlations between executive function and most of the gait variables (stance time, cadence, and gait cycle time) (p<0.05) when crossing over an obstacle while walking. Especially, stance time exhibited significant correlations with most executive functions (p<0.05). Conclusions: When evaluating executive function during walking with an obstacle, post-obstacle-crossing phase and stance time need to be observed.
Among the building types in which behavioral issues play, the largest role are institutional environments. Such setting share a number of important characteristics ; they serve specific population (e.g. the ill, the elderly , '||'&'||' inmates) : they are directed toward specific social behavioral objectives(e.g medical care, education , '||'&'||' rehabilitation); and they are "Total institution" in which residents spend all of there time. The basic concepts and theories of environment-behavior studies are of direct relevance in understanding such social institutions. In this study, I have examined the relationships between waiting and the specific conditions of the hospital experience. More specifically, this is a study of visitors (outpatient) within the context of the hospital , and the physical setting designated for their use in the outpatient waiting room. Environment behavior studies, with its commitment to individual behavior and the physical environment might well be a particularly way by which a study of waiting rooms can demonstrate the power of this approach and its integrative among disciplines.
The purpose of this study was to investigate what life safety services the elderly living alone in rural areas need and to propose the life safety services that they actually need. The research subjects were 1,000 people aged 65 years and over living in the rural areas of Korea. Among the total 1,000 respondents, 283 elderly people who did not live together with their married children were included in this study. Data were analyzed through frequency analysis, descriptive statistics, and multiple regression analysis. The results of this study were as follows. First, the needs of health care services and vehicle support services (when the elderly went to the hospital or walked out) were high. These were services related to health, which means that the health of the elderly is not good and that they are interested in health. Of course, it is important to treat the disease. However, it is more important to prevent disease and maintain health. An expansion of these services is urgently needed. Second, the need for life safety services was affected by the frequency of contact with neighbors, uncomfortable housing, exercise, the frequency of contact with children, gas accidents, and nutrition variables. Through this analysis, we propose to include the housing improvement service, nutrition improvement services, and expanding social relations services in the life safety services.
