Purpose: This is a phenomenological study to describe the experiences of nursing students on home visiting nursing service as a community nursing practice. Method: Individual interviews were conducted on subjective experiences of 17 nursing students. Data were analyzed through Colaizzi's method in which meaningful statements were extracted and these were clustered into 6 themes. Result: The nursing students started practice with anxiety and expectation at the same time. They were frightened at the clients' inferior environment and their level of loneliness. They also felt pity and experienced complicated feelings for the clients. However, the home visiting practice was a chance for them to discard prejudice on the clients. Positive experiences on visiting nursing practice reported by the nursing students included lively interactions between nurses and the clients, and variable provision of primary nursing care. However, facts such as much limited visiting time, non-professional and limited scope of practice were reported as negative experiences. They felt both worthiness of the home visit service and restricted self-capability at the same time through the practice. They also felt sorry for the clients because the home visit services were carried out during limited time period. Regardless of this, the home visit experience provided them an opportunity of self-growth. This self-growth includes increased awareness of issues for elderly, building of self-identity as a nursing student, self-reflection, and realization of the value of family. Conclusion: This study may provide data for better understanding of nursing students experiences of home visiting nursing services. However, more study on the barriers of their community health practice is needed in the future. Moreover, it is needed to establish desirable practice environment through the collaborative relationships between the university and staffs in the public health center.
As the long term care insurance for the elderly has been established for 3 years it was noticed that the success of the policy is depending on the care workers. This study is to analyze and evaluate what are the characteristics and factors influencing their job satisfaction. The questionnaire survey was conducted over 291 care workers who are employed at 61nursing home for the elderlyin Jeollanamdo. The survey result is as following. Firstly, 93.5% of workers were female, 78% of them were over 40-50 years old, and 68.7% of them werelower than a high school graduate as their academic background. Secondly, 63.2% were contract-based or temporary employees, 53.3% had a work-related education less than 5 times for the past year, and 79.7% had an annual salary lower than KRW 20 million. Thirdly, the overall job satisfaction was 3.19 on the Likert scale of 5. Also, the job satisfaction was higher for the care workers working and getting paid by home than by company, for full-time workers than temporary workers. In addition, it was found that the more workers have work-related education and the higher their annual salary is, the higher their job satisfaction is. The study evaluated the factor influencing on their job satisfaction and suggested the way to improve it based on the survey analysis.
In 2007, Long-Term Care Insurance Law was enacted to share the family burden of caring for the elderly who are unable to perform every day living activities due to their old age and chronic diseases such as senile dementia, diabetes mellitus, stroke and more. Backed by this law, since 2008, care workers have been sent to the elderly care centers or each elder's home to help them not only with their recovery from illnesses, but also with general activities from dressing, eating, bathing, walking even to toileting. However, according to the recently released survey by National Human Rights Commission, it has been found that the caring workers are in a very poor working condition including low income, abused blanket wage system, shortage of welfare services, extra works and even sexual harrassment. It is becoming an important issue due to fast-ageing population, the fact that the care workers have had experiences of violation in their right of labor while they are at work needs to be carefully treated. In that sense, this article presents some difficulties the caring workers face and proposes effective ways to solve these problems through the perspective of human rights and human labor rights based on the report written by National Human Rights Commission. In short, for this law to function properly and to boost the worker's capability of providing better services to beneficiaries, followings can be good answers - enhanced management and supervision on caring plans and care centers, providing immediate counselling and protection for victimized care givers, training courses offered to promote service receiver's sincere respect and strengthened awareness upon care givers.
The purpose of this study is to examine the job characteristics of care workers in elderly care voucher service emphasizing a social service quality management approach. The study sample was composed of randomly-selected 233 centers which dispatched care staffs to clients' home. Descriptive analyses were performed for examining the unique aspects of relationship-based labor of care staffs, and logistic regression analyses were performed for investigating the association between service quality structure and human right violation against staffs. As the first empirical study focusing on staff-side service quality factors, this study found out that human right violation against staffs was mainly influenced by record-keeping and document management capacity of center, risk protection under insurance, compliance of standard contract procedure, and regular supervision. These results suggest particular policy attention should be given to basic protection for and set-up of core activity boundaries of care workers as well as clients-centered rights both for preventing human right violation and improving overall social service quality.
Although the availability of home care rehabilitation services have been greatly increased since community-based rehabilitation was introduced to Korea, there is still a dearth of studies investigating the performance level of ADL (activities of daily living) for the physically disabled in the rural areas. The purposes of this study were to investigate the ADL performance level of disabled persons living at home in the rural areas of Wonju city, Korea, using FIM (functional independence measure) and to identify the specific areas of the ADL to be trained or evaluated by physical therapists or occupational therapists. 298 disabled people were interviewed by 10 physical therapy students. Analysis of the interviews indicated: 1) Forty seven point seven percent of the respondents were elderly-disabled persons whose ages were 61 years old or above, and 69.5% of respondents has the history of chronic disability period of 5 years or more. 2) FIM score of bathing, and stairs climbing showed severe dependent trend 3) FIM scores of self-feeding, urination, defecation, and comprehension ability were mildly dependent. These results revealed that functional evaluation/treatment for discharging from hospitals to the rural areas should be emphasized on the specific ADL performance areas such as bathing, and stairs climbing.
The wireless sensor network (WSN) based ECG and body temperature measuring system for ubiquitous health-care were designed and developed. The system was composed of a wireless sensor network node, base station and server computer for the continuous monitoring of ECG signals and body temperatures of patients at home or hospital. ECG signal and body temperature data, important vital signals which are commonly used in clinical and trauma care, were displayed on a graphical user interface (GUI). The data transfer from sensor nodes on patients' body to server computer was accomplished through a base-station connected to a server computer using Zigbee compatible IEEE802.15.4 standard wireless communication. Real-time as well as historical, ECG data of elderly persons or patients, can also be retrieved and played back to assist the diagnosis. The ubiquitous health care system presented in this study can effectively reduce social medical expenses, which will be increased greatly in the coming aging society.
Purpose: This study aimed to identify the factors affecting the emergency response ability to develop an education program to strengthen the emergency response ability of caregivers having the highest probability of discovering emergency situations. Methods: This study is a descriptive research study conducted by 204 caregivers working in elderly care facilities and home care centers located in one area. The collected data were analyzed by independent t-test, one-way analysis of variance (ANOVA), Pearson's correlation, and multiple linear regression using the Statistical Package for the Social Sciences (SPSS) 25.0 statistical program. Results: First aid knowledge was related to work time (F=3.52, p=.008), number of care people (F=2.58, p=.038), workplace (F=6.76, p=.001), and self-efficacy (F=3.70, p=.026), and the ability to cope with emergency situations showed a statistically significant difference in education level (F=3.58, p=.015). There was a positive correlation between self-efficacy and emergency response ability (r=.179, p<.05), and the factors affecting emergency response ability were education level (β=-.164, p=.050), workplace (β=.290, p=.023), and self-efficacy (β=.103, p=.022). Conclusion : Factors influencing the improvement of caregivers' ability to cope with emergency situations, workplace, education level, and self-efficacy were found. Thus, it is suggested to develop and apply an emergency education program to increase self-efficacy and consider the workplace and educational level.
Objectives: This case study was conducted to assess changes in the concentration of volatile sulfur compounds (VSCs) of older adults following home oral health care interventions based on community care. Methods: The participants were three elderly people with the halitosis. An oral health intervention programs including oral massage, oral hygiene care, and oral muscle training strenghtening were conducted for 12 weeks. Halitosis was measured using an oral malodor-checking device. Results: The program showed positive effects on changes in halitosis. The concentration of VSCs of the first case decreased rapidly from 44.5 Refres Oral Volume (ROV) on pre-test to 15.5 ROV on the first post-test. In the second case, the score decreased from 14.5 ROV on pre-test to 12 ROV on 2nd post-test. In the third case, the score decreased slightly from 6.5 ROV on pre-test to 6 ROV on the first post-test. Conclusions: Oral health care interventions contributed to decreasing the concentration of VSCs and improving the quality of life of older adults. The active promotion and customization of these programs are required.
Purpose: This purpose of study was to investigate the relationships among functional state, self-efficacy, and life satisfaction in the elderly with decreased visual acuity. Methods: The subjects were 162 elderly people from the G university hospital. Functional state was measured by Late-Life Function and Disability Instrument (LLFDI) and Minimum Data Set-Home Care version 2.0 (MDS HC 2.0). Self-efficacy and Life satisfaction were measured by the tool of Rho & Lee (2011) and Yoon (2007). Data were analyzed using t-test, ANOVA, Pearson's Correlation Coefficient, and logistic regression. Results: The daily life function was significantly associated with self-efficacy and vision decrease. The regression model with these two variables explained 35.6% of the variance of daily life function. IADL was significantly associated with vision decrease, age, gender, and self-efficacy. The regression model with the three variables explained 52.9% of the variance of IADL. Life satisfaction is significantly associated with self-efficacy, daily life function, vision decrease and IADL. The last regression model with the four variables explained 51.8% of the variance of life satisfaction. Conclusion: The levels of functional state, self-efficacy and life satisfaction in the elderly with decreased visual acuity were low. Self-efficacy was an important factor that influences on the functional state and life satisfaction. Therefore, nursing interventions that can enhance the self-efficacy are required in order to increase the functional state and life satisfaction in the elderly with decreased visual acuity.
The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.
