A Case Report on Chronic Nipple Eczema with Deficiency of Qi Blood and Liver Fire Improved by Korean Medicine (기혈양허(氣血兩虛)와 간화(肝火)를 동반한 만성 유두 습진 1례)

  • Objective : This study assessed the effect of a combination of Korean medicine on a chronic nipple eczema patient complaining of stress and immunodeficiency due to long-term steroid use. Methods : Herbal medicine, moxibustion and acupuncture were performed for a year. Treatment outcomes were evaluated with DLQI(Dermatology Life Quality Index), EQ-5D-5L(The 5 level of EuroQol 5 Dimensio scale), EQ-VAS(EuroQol-Visual Analog Scale), ADSI(Atopic Dermatitis Severity Index), pruritus NRS(Numeral Rating Scale), pictures and patient's statements. Results : After the treatment, symptoms have been alleviated, the quality of life has increased, and it has been maintained without further nipple eczema for three months. Conclusion : We suggest that Korean medicine is effective for chronic nipple eczema patient.

A literature study about the comparison of Oriental-Occidental medicine on the Atopic dermatitis (아토피 피부염의 東西醫學的 文獻 考察)

  • The results of a literature study about the comparison of Oriental-Occidental medicine on the Atopic dennatitis were as follows. 1. In Oriental medicine, Atopic dermatitis belongs to the category of the Naesun(내선), Taeryumchang(胎斂瘡), Eczema(濕疹), Chimumchang(浸淫瘡). In Occidental medicine, the other names of Atopic dermatitis are allergic eczema, IgE dermatitis, flexual eczema etc. 2. In Oriental medicine, the definition of atopic dermatitis includes chang(瘡), sun(선), and pung(風). Occidental medicine, is one of the intrinsic eczema classifications. In fact eczema term circumscribes dermatitis in atopic patients. 3. In Oriental medicine, the etiology and pathogenesis of Atopic dermatitis arose from the state of cogenital defect(稟賦不足), internal accumulation of damp and heat(濕熱內蘊) at first, and then invaded pathogenic wind, damp, heat factors again, and combined endo-exoteric pathogenic factors. So it appears in skin. In Occidental medicine, the etiology and pathogenesis of Atopic dermatitis approaches in genetic, allergic and immunologic, pharmacophysioloic aspects. It is only a hypothesis but there is no known facts about radical aetiology. 4. In Oriental medicine, differentiation of syndromes classifies manifestation aspects, etiology and pathogenesis, and invasion period. In Occidental medicine, it divides into an invasion period, and clinical aspect etc. 5. In Oriental medicine, Internal theraphys of Atopic dermatitis are decoction of ingredients(湯劑), pills(丸), and tablet(片). So, it prescribes as treatments on the ground of differentition of syndrome. In Occidental medicine, there is no radical therapy because Atopic dispositions don't change. But steroid, antihistamine as symptomatic tre atments are generally used in Occidental medicine. 6. In Oriental medicine, external therapies are wet dressings(濕敷), lotion(洗劑), powder(散劑), adhesive plasters(膏劑), oil(油劑), smoking(烟薰法), warm over fire therapy(熱烘療法), acupunture and moxibustion therapy(鍼灸療法).

A Case Report of an Adult Atopic Dermatitis Patient Whose Eczema and SCORAD Index Were Improved by Herbal Medicine and Acupuncture Therapy (한약 및 침치료 후 피부습진의 SCORAD 점수를 호전시킨 성인 아토피성 피부염 환자 증례보고)

  • This case reports the effect of herbal medicine and acupuncture therapy on the scoring atopic dermatitis (SCORAD) index for an adult atopic dermatitis patient. An adult male in his 30s with complaints of severe eczema on the whole body was diagnosed with heat toxin blazing exuberance pattern (熱毒內蘊) and treated with herbal medicine, including an orally-administrated decoction, externally-applied decoction, extracted powder capsule, and acupuncture for a total of 30 days. We observed skin symptoms, SCORAD index, follow-up photographs, and any adverse events. After treatment, SCORAD index decreased from 74.5 to 38.7 (51.9% from baseline). Skin symptoms such as redness, itching, and exudation were improved without any adverse events reported. This case suggests that herbal medicine and acupuncture therapy could be effective in improving eczema in adult atopic dermatitis patients.

  • Objectives : The purpose of this study is to report the Korean medicine treatment on whole-body nummular eczema. Methods : This case study was conducted with a 66-year-old female patient who has whole-body nummular eczema treated by Korean medicine. She was treated from 2021.3.30 to 2021.6.28. During the treatment, the two herbal medicines were changed depending on the patient's condition. We treated her with herbal medicine, herbal external preparations and acupuncture treatment. For the evaluation of the results, we used photo comparison, VAS, and DLQI. Results : After the treatment, the nummular eczema lesions of patient was remarkably improved. Also, the VAS grade of other symptoms such as pruritus, burning sense, oozing, crusting and erythema were decreased and DLQI score changed from 27 to 2. Conculsions : This study suggests that Korean medicine treatments are effective for patient with recurrent nummular eczema.

AD can divide three groups, Acute-Type, Semiacute-Type, Chronic-Type, according to it's condition. This study shows that it can be applicate Seungmagalgeuntang(升麻葛根湯), Hoichunryangkyuksan(回春凉膈散), Doghengsan(導赤散), Heungbangpaedoksan(荊防敗毒散), Goomigangharltang(九味羌活湯), Baekhotang(白虎湯), Gaegytang(桂枝湯) for treatment of Acute-Type, Pyungweesan(平胃散) for treatment of Semiacute-Type, and saengryusamultang(生料四物湯), Oncungeum(溫淸飮) for treatment fo Chronic-Type.

Case Report of a Lead Poisoning by Home-made Herb Pills (불법 제조된 환약 복용에 의하여 발생한 연중독 증례)

  • A 23-year-old female has visited university hospital for abdominal colic and constipation. She took home-made herb pills for 15 days as a traditional remedy for skin eczema. On clinical examinations, conjunctiva was pale, sclera was icteric and lead line was visible on gingiva. Laboratory examinations are as follows; hemoglobin was 8.6g/$d{\ell}$, reticulocyte was 4.2% and there was basophilic stippling of erythrocytes in peripheral blood smear. Blood level of lead was elevated as $69{\mu}g/d{\ell}$. The herb pills contained 3.32% lead, with cumulative dose of lead over 3.2g. The herb pills were manufactured by a 70-year-old man who was not aware of the health hazard of lead. We suppose that he made herb pills with the imported raw materials from China that contained a high percentage of lead.

Management of Steroid Rebound Phenomenon in Patients with Eczema (습진 환자의 스테로이드제 반동 현상 관리 및 치험 1례)

  • Objectives : We would like to report what symptoms appear in patients experiencing steroid rebound phenomenon and how to manage and treat them at Korean medicine hospital. Methods : A 20-year-old woman who had been using topical and oral steroids for more than 6 weeks presented to the hospital. She hospitalized for 5 weeks and stopped using steroid. The treatment applied was acupuncture, wet dressing therapy and herbal medicine including Yangdokbackho-tang, Sunbangpaedok-tang and Danguieumja. We assessed symptoms through Inflammation Index and Chronicization Index. Results : Right after she stopped using steroid, symptoms of skin got worse gradually. However, after 5 weeks of treatment, all symptoms except itching and dryness disappeared. When symptom was severe, Inflammation Index and Chronicization Index was 12 and 9 points and after the treatment they were 2 and 4 points. Conclusions : This study shows various symptoms in patients with topical steroid addiction and how to manage at Korean medicine hospital.

A Case of Chronic Nummular Dermatitis Improved by Hwangryunagyo-tang (황련아교탕(黃連阿膠湯) 투여로 호전된 만성 동전 모양 습진 1례)

  • Objectives: This study aims to report a case of chronic nummular dermatitis improved by Hwangryunagyo-tang. Methods: The patient in this study was a 15-year-old female, who visited our hospital with symptoms such as redness, erythema, papule, excoriation, scale, exudation, crusting, lichenification, and pruritus of the extremities and trunk. Considering that the skin lesions were coin-shaped and persisted for more than 6 months, the patient's symptoms were diagnosed as chronic nummular eczema. Based on the patient's symptoms and overall condition, Hwangryunagyo-tang was prescribed. Results: Approximately 4 months after administration of Hwangryunagyo-tang, the exudate disappeared, and overall skin symptoms improved, from objective score 33.7 to 6.6, leaving only slight scale, dryness, and pigmentation. As the pruritus of the skin lesion decreased from numeric rating scale 8 to 1, the patient's sleeping condition also improved from numeric rating scale 9 to 0. Conclusion: This study is significant in that the symptoms of chronic nummular dermatitis were effectively improved after administration of Hwangryunagyo-tang.
