Study on the Development of a Standard Instrument of Diagnosis and Assessment for Spleen Qi Deficiency Pattern (脾氣虛證(비기허증) 진단평가도구 개발 연구)

  • Objectives: The purpose of this study was to develop a standard instrument of diagnosis and assessment for spleen qi deficiency pattern. Methods: Reports published in Korea and China related to spleen qi deficiency pattern were selected. Assessments of selected references were performed to select major symptoms of spleen qi deficiency pattern. Korean translation and review by a Korean linguist were performed to create a draft of [Standard instrument of diagnosis and assessment for spleen qi deficiency (for subject)]. The final [Standard instrument of diagnosis and assessment for spleen qi deficiency (for subject)] was completed after assessment on inclusion or exclusion, on importance of items and on validity of translation by an expert committee, consisting of professors from the National College of Korean Medicine. Results & Conclusions: 1. 14 major symptoms were selected by frequency from 45 references which were related to standard identification of spleen qi deficiency pattern, translated into Korean and reviewed by a Korean linguist. 2. 11 symptoms were selected after assessment on inclusion yes or no by the expert group. Items were listed in order of importance: tiredness of extremities (肢体倦怠), sallow complexion (面色萎黃), reduced appetite (食欲减退), abdominal distension after eating (腹胀食後尤甚), inability to eat (納少), pale tongue and white fur (舌淡苔白), lethargy (神疲), emaciation (消瘦), loose stool (大便溏薄), shortness of breath and reluctance to speak (少氣懶言), and weak pulse (脈緩弱). 3. Final [Standard instrument of diagnosis and assessment for spleen qi deficiency (for subject)] was completed after assessment of translation validity, reflection of individual opinions by the expert committee, and application of weighted value computed from assessment on importance of items.

Nutrition Education Status in Industrial Workers with Meal Service by Dietitian Recognized Stage of Nutrition Education (산업체영양사의 영양교육 인식단계별 직장인 영양교육 현황)

  • This study was made to analyze the condition and the needs of nutrition education considering the different stage of nutrition education recognized by the dietitian in industrial area. 165 female dietitian were surveyed by questionnaire and they were divided into 4 groups according to the different recognition stages of nutrition education: Pre-contemplation(PC) 4.8%, Contemplation(CO) 46.7%, Preparation(PR) 22.4%, Action & Maintenance phase(AM) 26.1%. For statistical analysis, SAS(Ver.8.1. for Window) was used to find out the distribution related with nutrition education and to calculate the scores of mean and standard deviation. General characteristics of the subjects are about 20 years old(71.5%), single(69.7%), careers over 5 years(40%) and university graduates(73.9%). The number of meals(p<0.05) and the employment status(p<0.05) were significantly different according to the recognized stage of nutrition education. The practice of nutrition education was different depending on the dietitian in the industry. Many of the factory dietitian were in pre-contemplation stage(87.5%), however, those in the office and service area were more in Action & Maintenance stage(27.9%). In the industrial area, just 26.1% of dietitian operated the nutrition education and most did not due to the work overload and insufficient support of staff(73.3%). The frequency for the most effective nutrition education was once a month(61.2%). The contents for desirable nutrition education were in the order of 'eating habits'(36.1%), 'relation with the daily life'(23.5%), 'food hygiene' (21.7%), 'nutrition knowledge'(9.7%), and 'disease prevention'(9%). In operating nutrition education, dietitian had concerns about 'insufficient support of staff', 'shortage of teaching materials' and 'lack of time'. And, the dietitian who were with lower recognition stage of nutrition education(P<0.05) concerned more about the insufficient educational contents. In conclusion, dietitian in the industry highly recognized the need of nutrition education, but it was so difficult to practice. For more programs and various materials should be developed, and the staff's perception, the view of dietitian and the meal service should be changed, too. For the health improvement of industrial workers, it would be necessary to proclaim the importance of nutrition education nationwide.

Behavioral Characteristics on Season and Group Size of Hanwoo Steer (한우 거세우의 계절 및 군 두수별 행동 특성)

  • This study was carried out to determine the behavioral characteristics on season and group size of Hanwoo steer. Forty eight Hanwoo steers were video-recorded at 15, 17 and 19 months of age for behavioral measurement. Space allowance was kept consistence with either group. The results showed that eating behavior of Hanwoo steers was strongly influenced (p<0.05) during seasonal transition, being the lowest in summer and the highest in autumn. With seasonal transition, standing time would linearly increase (p<0.05). In summer, steers spent more (p<0.05) time tying down but relatively less (p<0.05) time walking and fighting, meanwhile they much less (p<0.05) scratching and rubbing. In autumn, steers showed more grooming behavior (p<0.05) contrasted to the other seasons. Steers spent more (p<0.05) walking time when raised in large group and increased (p<0.05) grooming frequency when raised in small group.

A Preference Analysis of Vegetable Group Foods in Primary School Students and their Mothers (초등학생과 초등학생 어머니의 채소류 식품군 식품의 기호도 조사)

  • The purpose of this study was to investigate the primary school students' and their mothers' vegetables preferences. Total of 360 students and their 360 mothers in Seoul and Gumi-si were participated in survey during July, 2011, and data of 253 students and their 253 mothers(70.3%) were analyzed by SPSS ver 12.0. The preference scores of mothers were higher than those of students in all kinds of vegetables examined, except bean sprout and laver. The preference scores of lower grader(2, 3 grade) were higher than higher graders(5, 6 grade), and those of female student were higher than male students. The preference accordance degree of students and their mothers were highest in seaweeds group, and lowest in root stalk vegetables. The onion, welsh onion, water dropwart, mustard kimchi were showed specially low student-mother preference accordance. Among the 50 vegetables investigated, all mother had chance to eat was 31 vegetables, and all students had chance to eat was 10 vegetables. The 3 kinds of seaweeds such as seastaghorn, ceylon moss and seaweed fusiforme were the foods that more than half of student had no experience to taste. Environmental variables, such as having the sibling, whether or not mother had job, the type of breakfast foods, and food and frequency of eating-out, all had no significant influences on vegetables preferences.

A Study of Health-related Habits, Dietary Behaviors and the Health Status of the Middle-aged and the Elderly Living in the Chonju Area (II) (전주지역 중, 노년층의 생활습관과 건강상태 조사(II))

  • This study investigated the anthropometric and biochemical indices, and the health and nutritional factors influencing the two indices among 194 middle-aged and elderly subjects (108 middle-aged and 86 elderly) residing in a medium sized city for more than 10 years. In the examination of their dietary habits, 8.3% of the middle-aged subjects and 14.0% of the elderly subjects had two meals a day, and more female subjects had two meals per day. Of the subjects who ate meals at regular times, 75.0% were middle-aged and 79.1% were elderly, and the degree of irregularity of meals was greater for female subjects. The study of the dietary behavior of the subjects indicated that 71.3% and 66.3% of the middle-aged and elderly, respectively responded that the amount of food in each meal was sufficient. The subjects ate alone comprised 19.7% of the middle-aged females and 31.5% of the elderly females. The prevalence of smoking among the subjects was 28.1% for the middle-aged, 18.8% for the elderly male and 7.4% for the elderly females. The percentage of the subjects who drank alcohol was 34.4% of the middle-aged males and 13.2% of the middle-aged females. Slightly less than half of the subjects exercised more than once a week, with the male subjects showing a higher rate than the female subjects. The average body mass indices (BMI) were 24.5 and 24.6 for the middle-aged male and female, respectively, and 22.6 and 24.0 for the elderly male and female, respectively. BMI assessment showed that underweight subjects (BMI < 20) comprised 3.7% of the middle-aged, 14.0% of the elderly, and that 40.7% of the middle-aged and 24.4% of the elderly were overweight (25 < BMI < 30) , and 0.9% of the middle-aged and 1.2% of the elderly were classified as obese (BMI $\geq$ 30) . A waist/hip ratio (WHR) greater than 0.8 was found in 89.5% of the middle-aged females and 90.7% of the elderly females, showing high abdominal fat deposition in the majority of females. The average systolic blood pressure of females was 121.1 $\pm$ 17.1 mmHg for the middle-aged and 129.6 $\pm$ 21.3 mmHg for the elderly subjects. The systolic blood pressures showed a significantly difference between the two age groups. Those defined as anemic subjects based on hemoglobin values comprised 13.0% of the middled-aged group and 16.3% of the elderly group. There was a tendency for higher fasting glucose levels among the elderly subjects. An increase in total plasma cholesterol levels with age was shown. The female subjects had higher cholesterol levels than the males'The study of the correlation between the daily habits and health status showed that the amount of food eaten at each meal, the frequency of eating out, and the use of dietary supplements appeared to influence BMI, WHR, the plasma triglyceride and plasma cholesterol levels; omitting one meal had a positive correlation with the systolic blood pressure and plasma cholesterol. These results suggest that desirable dietary habits and concerns for health are contributing factors for maintaining good health, as indicated by normal blood lipid levels.

Nutritional Education Status and Recognition of the Importance of Elementary School Dietitians in the Gyeongnam Area (경남지역 초등학교 영양사의 영양교육 실시현황 및 인식조사 - 공동관리 여부와 경력을 중심으로 -)

  • This study was carried out to investigate the nutritional education status and the recognition of the importance of elementary school dietitians (N=183) in the Gyeongnam area. The results are summarized as follows. All subjects took part in nutritional education program, but the educational methods were passive, as in the case of “home correspondence”(80.8%) and “bulletin board poster”(16.4%), and the education frequency was very low as in “one time/month”(90.2%). The subjects thought “as an independent subject”(41.5%) and “as a related subjects”(35.1%) were suitable teaching venues for the nutritional education. They were very low in individual counseling (4.5%) for school children because of “lack of opportunity”(42.2%) and “heavy work load”(24.1%). However, most of the subjects wished that nutritional counseling could offered in the future (95.5%). The parents' experience of nutritional education was also low (34.3%). Information sources for nutritional education were mainly the “internet”(53.1%) and “re-educational materials” (25.0%). The available instructional materials included “printed materials”(96.7%), “exhibition bulletin materials” (70.3%) and “electronic materials”(46.4%). The preferred education materials were “exhibitionㆍbulletin materials”(32.2%), “printed materials”(29.2%), and “electronic materials”(27.7%). However, materials they wished to purchase were “electronic materials”(54.5%) and “cubic materials”(26.0%). These results show a difference between the preferred materials and the possessed materials. Most school dietitians (98.4%) recognized the necessity of nutritional education with respect to “good table manners”(42.0%), “correction of food prejudices”(30.3%), and “proper nutrition for growth”(21.0%) Although they had a great interest in nutritional education, they had difficulty in cutting their teaming into practice because of “heavy work load”(30.9%), “lack of a systematic curriculum”(25.2%), and “lack of a educational opportunity”(22.8%). Ninety five percent of subjects wished to have nutrition education taught as an independent subject. They pointed out “kindergarten”(60.0%) and “lower grades in elementary school”(33.9%) as the optimal starting times for nutritional education and “school dietitians”(91.3%) as suitable teachers for these programs. The required topics chosen by the subjects for nutritional education for children were “proper eating habits”(54.2%) and “nutritional problem”(31.5%). The dietitians thought “food prejudices”(44.7%), “rat too much processed and instant foods”(36.5%), and “obesity”(11.8%) were the most common nutritional problems among elementary school children. These results suggest the necessity of solving the nutritional problems in children by developing a nutritional education program. Along with this program, if dietitian assisted programs for parents were developed, the effects of nutritional education could definitely be increased.

A study on the awareness and practice of hand washing according to the clinical practice experience of physical therapy students (물리치료 전공 대학생의 임상실습 경험에 따른 손 씻기의 인식도와 실천도에 관한 연구)

  • The purpose of this study was to provide basic data on hand washing of physical therapists by examining their awareness and practice of hand washing according to proper clinical practice. A total of 536 questionnaires were collected from physical therapy students in Gwangju metropolitan city. The general characteristics of the subjects were calculated by frequency analysis, and the awareness and practice of hand washing with and without clinical practice were analyzed by independent t-tests. As a result of analyzing hand washing awareness according to the clinical practice, there was a significant difference in hand washing up to the wrist (p<0.05), and a significant difference in 6 stages of proper hand washing (p<0.01). As a result of analyzing hand washing practice according to clinical practice, there was a significant difference in hand washing before eating and starting clinical practice (p<0.05), a significant difference in washing hands after contacting clinical practice equipment and washing your fingers from your thumb down (p<0.01). As a result of analyzing the correlation between awareness and practice, most showed positive correlation. This means it is important to raise the awareness of proper washing hands. Therefore, it is believed that the training school should repeatedly conduct its own hand washing training within the school and in clinical practice to increase awareness and improve handwashing in clinical practice.

h Study on Dietary Practices of Juvenile Delinquents in Korea (비행청소년과 일반청소년의 식생활 비교 연구)

  • The relationship between dietary practices and juvenile delinquency was studied using a dietary survey. Subjects were selected from juvenile delinquents who were under the supervision of the Seoul Probation Office of the Ministry of Justice. The study group consisted of 52 male and 52 female delinquents. As a control group, 104 exemplary high school students were selected in Seoul. A questionnaire was designed to find out the subjects' general characteristics, dietary habits, lifestyle, eating behavior, food frequency, and nutrient intake using the 24-hour recall method. Compared to the exemplary students, the juvenile delinquents were significantly different in education level, family status, monthly allowance, residence status, breast fed, parents' education level, parents' concern. In dietary habits, fifty-eight percent of the juvenile delinquents ate 2 or fewer meals per day, preferred to eat with friends, and liked hot tasting foods. The juvenile delinquents consumed more ion drinks (OR = 9.26 CI: 3.83 -22.37), rameon (OR = 7.67 CI: 3.21 - 18.33), cola (OR = 6.75 CI: 2.91 - 15.69), soft drinks (OR = 6.12 CI: 2.53- 14.81), steamed korean sausage (OR = 5.34 CI:2.31 - 12.32), hamburger (OR = 5.15 CI: 1.91 - 13.87), kimbab (OR = 3.63 CI: 1.76-7.46), ddokbokgi (OR = 3.17 CI: 1.58-6.38), candy (OR = 3.08 CI: 1.41-6.73), white rice (OR = 2.59 CI: 1.19-5.64), hotdog (OR = 2.52 CI: 1.31-4.86), and less rice mixed with grains (OR = 0.02 CI: 0.01-0.05), tangerine (OR = 0.06 CI: 0.02 - 0.20), milk (OR = 0.29 CI: 0.14 - 0.60), roasted fish (OR = 0.32 CI: 0.28-0.99, anchovy (OR = 0.35 CI: 0.17 - 0.72), seaweed (OR = 0.37 CI: 0.16 - 0.83), and tofu (OR = 0.48 CI: 0.23-0.99) than the exemplary students. With respect to the juvenile delinquents, the nutrient intakes lower than 75% of the Korean RDA were for riboflavin (75.0%) and calcium (47.9%) among the boys, and calcium (46.9%) and iron (60.4%) among the girls. To prevent juvenile delinquency, nutritional education and well-balanced school food service meals should be emphasized so as to improve the management of dietary practices. The Association of Cardiovascular Risk Factors with Nonalcoholic Fatty Liver Disease in Health Checkup Examinees (일부 종합검진 수검자 중 비알코올성 지방간과 심혈관질환 위험요인과의 관련성)

  • Objectives: The purpose of this study was to evaluate the relationship of nonalcoholic fatty liver and cardiovascular risk factors. Methods: This study was conducted to investigate the association of nonalcoholic fatty liver and cardiovascular risk factors for adult men (n=2976) and women (n=2442) who were over 19 years old, after excluding the HBsAg(+) or anti-HCV(+) patients and the men and women with increased alcohol intake (men: 40g/week, women: 20g/week). Results: Compared with the normal liver subjects, the nonalcoholic fatty liver subjects showed a significantly increased frequency of abnormal systolic blood pressure (${geq}120mmHg$), fasting blood sugar (${\geq}100mg/dL$), total cholesterol ($({\geq}200 mg/dL$), triglyceride ($({\geq}150mg/dL$), high density lipoprotein cholesterol (<40 mg/dL), low density lipoprotein cholesterol ($({\geq}130g\; m/dL$) and abdominal obesity in men, and all these measures were significantly increased in the women except for abnormal HDL cholesterol. After adjusting for the body mass index, age, smoking, exercise and a nonalcoholic liver, the odds ratios of an abnormal waist hip ratio were 1.35(95% Confidence Interval=1.05-4.72) in the mild fatty liver, 1.61 (1.19-2.18) in the moderate fatty liver, 2.77(1.57-4.92) in the severe fatty liver compared with a normal liver. The adjusted odds ratios for abnormal fasting blood sugar were 1.26(1.03-1.53) in the mild fatty liver, 1.62(1.27-2.06) in the moderate fatty Iiver and 1.77(1.12-2.78) in the severe fatty liver. The adjusted odds ratios for abnormal triglyceride were 1.38(1.11-1.72) in the mild fatty liver, 1.73(0.33-2.24) in the moderate fatty liver and 1.91 (1.17-3.10) in the severe fatty liver of men. Adjusted odds ratios for abnormal triglyceride were 1.50(1.04-2.15) in mild, 1.71(1.07-2.68) in moderate, 1.81(0.69-4.38) in severe fatty liver of women. Conclusions: The nonalcoholic fatty liver subjects had more cardiovascular risk factors compared with the normal liver subjects. Thus, prevention and treatment of the nonalcoholic fatty liver is necessary by lifestyle modifications such as restriction of alcohol intake, no smoking, exercise and adequate eating habits.

The Effect of Hand and Finger Exercise on Grip Strength and Pinch Pressure in Elderly Women (손과 손가락 근관절운동이 노년기 여성의 악력과 잡기력에 미치는 영향)

  • Introduction : Exercise has been suggested as an important nursing strategy in which to help elderly maintain functional performance and to enhanced quality of life. Most of exercise study has been reported on fitness exercise such as walking, swimming, dance etc for health of elderly. There have been few reports about exercise on the promotion of small and fine movement of elderly. The purpose of this pilot study was to determine an effect of 6 weeks hand and finger exercise in home to improve hand muscle strength such as grip strength and finger pinch pressure. Materials and Methods Design: This pilot study was used one group pre and post-test design. Sample: Twelve elderly women above 60 years of age or older living in community were selected by convenient sampling. Procedure: Signed informed consent was obtained prior to participate in this study. The authors met elderly and taught hand and finger exercise, daily a week for 6 weeks, within 30 minutes per session. But exercise frequency and strength were not same. Instruments: Left and right grip strength were measured by Bulb Dynamometer(made in USA) and left and right pinch pressure were measured by Baseline Hydraulic Pinch Gauge(made in USA). Data analysis: Discriptive data analyses were performed on all variables. Wilcoxon matched-pairs Signed-Ranks test were used to find difference of grip strength and pinch pressure between pre and post exercise using SPSS 10.0 for Window. Results: Samples age ranged from 60 to 73, Mean age was 65.3. All were women. Ten elderly were diagnosed osteoarthritis and one had DM. After six weeks hand and finger exercise, Left and right grip strength were higher than those of before exercise(Z=-2.667, P<0.01 ; Z=-3.065, p<0.01). And left pinch pressure after hand and linger exercise were higher than that of before pinch pressure (Z=-2.315, p<0.05). But Right pinch pressure was not shown significant change(Z=-1.099, p>0.05). Conclusions: Although this study was limited by the sample size and design, the findings provide some important implications for community based exercise nursing intervention. Short term (six weeks) exercise of hand and linger is shown to be useful as nursing intervention to maintain routine daily activities such as eating, writing, grip something for elderly.
