Bilateral Chylothorax after Left Modified Radical Neck Dissection : Two Cases (좌측 광범위 경부 림프절 청소술 후 발생한 양측 유미흉 2예)

  • Bilateral chylothorax as a complication of radical neck dissection is extremely rare. Early diagnosis of chylothorax is important due to the consequences on metabolic & cardiopulmonary conditions. This present report describes our recent experiences with two cases of bilateral chylothorax following left modified radical neck dissection that was treated successfully by conservative management.

Clinical Experiences of Multiple Organ Failure after Surgery for Acquired Cardiovascular Disease

  • A serious problem after cardiovascular surgery known as Multiple Organ Failure[MOF] whereby several vital organs successively demonstrate dysfunction in spite of intensive postoperative treatment has recently arisen. We have made a retrospective study of the clinical records of 137 patients who underwent cardiovascular surgery during past two years [1987-1988]. Fourteen patients [10%] developed multi-organ failure postoperatively with the results of seven death [50%]. In fatal group, preoperative poor cardiac function [Cardiac Index<2.0L/min/m2] was considered important prognostic factor and infection 5 disseminated intravascular coagulation complicating gastrointestinal bleeding were the leading cause of death. In conclusion, evaluation of multiple factors concerning multi-organ failure demonstrates preoperative poor functional preservation of vital organs is the main factor. So early diagnosis k management for each of the failing organs & prevention of infection are mandatory of the treatment of these critically ill patients.

A Study on the Middle Age Women's Needs of Educare Services (중년 여성의 보육서비스에 대한 요구도)

  • The purpose of this study was to find out middle age women's needs of the educare services. Furthermore this study was done to serve as the basic data for guidance and the preparation of effective educare services. For these purposes, the data were collected by using questionnare from to 305 middle age women living in T city. The data were analyzed by frequency, percentile, chi square analysis. The major findings of this research are as follows: 1. Most subjects of present study needs for the daycare center. Furthermore there were significant differences in the daycare management, caring agent, day care cost according to the school career. 2. They wantd nere home daycare center and part time daycare center. And there were no significant differences in general enviroment of daycare center according to the school career. 3. There were significant differences in 1)the standards of selection for teacher 2)teacher's educational level, 3)teacher's age according to the school level. And they wanted 'low school career but caring experiences' daycare teacher.

  • The Centralized Traffic Control(CTC) System of Urban Railway is an element(subsystem) of the Urban Railway system to aim safety operation and efficient management of rolling stock in accordance with a train scheduling program in train operation ways. That's why the CTC System must be developed considering systematic and safety-guaranteed methods such as RAMS(Reliability, Availability, Maintainability, and Safety) refereed to IEC standards. The CTC System is consists of rolling stock, signaling, power supply, communication, and mechanical equipment and preforms control and remote monitoring functions. The technical activities considering safety process are very important at such an early stage of development in a railway project. This paper introduces RAMS activities and an independent safety assessment by $3^{rd}$ Party focused on a safety applied to a development of the CTC system of Urban Railway and proposes the experiences as well practices.

A Study on Health as Expanding Consciousness of the Low-income Elderly Living Alone (저소득 독거노인의 의식확장으로서의 건강연구)

  • Purpose: This study was to explore health experiences of the low-income elderly living alone reflected in Newman's Health as expanding consciousness theory. Method: The researcher used Newman's praxis methodology because it is good for showing the process of interaction between the researcher and the low-income elderly living alone. Results: The significant characteristics of early health experience during a participant's lifetime were demonstrated that blamed themselves, being burden of themselves hopeless of their lives. However, after a turning point in health experience. The health experience of most of the participants evolved as expanding consciousness. Conclusion: This study has provided support for Newman's theory of health. Most of the participants recognized meanings in their patterns and authentic caring relationships with the nurse as researcher, pattern recognition as a nursing practice was a meaningful transforming process in the participant-nurse partnership. This participatory approach expands the scope of sharing health experience with the elderly living alone and with caring community people.

  • Literacy acquisition is a social phenomenon. Children in a literate society grow up with literacy as an integral part of their personal, familial, and social histories. Because it is language, children learn written language in ways similar to oral language. However. because it is written, the ways in which written language differs from oral language in terms of its different functions and forms affect the way in which children learn written language. Written language is likely to be more decontextualized than spoken language. The ability to use decontextualized language seems to be crucial to successful participation and progress in school. Experiences identified as contributing to preschool children's literacy development contribute to their ability to use language in a decontextualized way. Teale and Sulzby's(1986) metaphor of emergent literacy has provided a conceptual scheme for understanding the nature and process of literacy acquisition in early childhood.

  • This research used Q-Methodology to investigate kindergarten teachers' perception style regarding children's creativity. The 30 participants had various teaching experiences and educational backgrounds and were teaching in a variety of types of kindergartens in the cities of Busan and Ulsan. Individual teacher's subjective perspective was the main target for the analysis. Results showed four types of teachers' perceptions of creativity : 1) perceiving creativity as children's creative thinking ability, 2) focusing on creative curriculum development 3) perceiving creativity as children's creative personality, and 4) focusing on either teacher's thorough understanding of children's creativity or teachers' creativity. These results indicate that teachers tend to perceive creativity in narrow and specific domains. These findings are discussed with respect to their educational implications.

  • Participant observations, in-depth interviews, and analyses of documents were used for collecting data. Children separated from their parents were living with grandparents because their parents were working abroad. Results included socio-cultural and psycho-social factors. (1) The socio-cultural grounded factors or the common characteristics of young children's family life in both living circumstances included early childhood academic achievement orientation, demands of responsibility and obedience, limited opportunity for social development, and societal phenomenadeveloping wide family concept affected by China's Confucianism and Korean-Chinese social culture. (2) Psycho-social grounded factors included the present care-givers' perceptions of the child's agency and their beliefs in the importance of play and friendship. This affected interactions between child and care-giversand child's self-esteem and friendships.

  • The hypothesis was tested that children whose families differ in socioeconomic status(SES) and educational level differ in their rates of productive language development because they have different language-learning experiences. Naturalistic interaction between mothers and their children was video taped. Transcripts of these interactions provided the basis for estimating the growth in children's productive vocabularies and properties of maternal speech. The sixty children from age 1 to 3 were selected in Yanji, China. The results show that the high educated mothers' children grew more than the low educated mothers' children in their mean length of utterances. Properties of maternal speech that differed as a function of mother's educational level fully accounted for this difference. Implications of these findings for mechanisms of environmental influence on child development are discussed.
