Rhinolith: An incidental radiographic finding

  • Rhinoliths are foreign bodies composed of mineralised deposits in the nasal cavity that are rarely encountered in routine dental practice. The current report presents a case of a 77-year-old female patient who visited a dental treatment centre for a routine dental check-up and prophylaxis. On the clinical examination, the patient did not report any breathing difficulty, nasal obstruction, or any form of discharge. Panoramic radiography showed a horizontally magnified radiopaque structure in the right nasal fossa region. Cone-beam computed tomography (CBCT) revealed a hyperdense entity with a laminated presentation in the right nasal fossa. The hyperdense entity was diagnosed as a rhinolith based on the radiographic findings. Although the patient was asymptomatic, a referral was made to an ear, nose, and throat surgeon for a further evaluation. This report highlights the importance of CBCT imaging in the diagnosis of soft-tissue calcifications.

  • This report describes the remedial fields, symptoms, pathology, dosage, prescriptional constitution of 69 studies related to the use of Radix Asari main blended prescriptions from Dongeubogam. The following conclusions were reached through investigations on the prescriptions that use Radix Asari as a key ingredient. Radix Asari blended prescriptions are utilized for 14 therapeutic purposes, for example, in symptoms of tooth, eye, paralysis, ear-nose, skin infection and carbuncle. In particular, 14.49% of the prescriptions appear in the chapter of tooth, and 10.14% of those appear in the chapter of eye. Prescriptions that utilize Radix Asari as the main ingredient are used in the treatment of dental disease, othalmology, nose and ear disease, throat disease, common cold, dermothology, paralysis, musculoskeletal disease, disease of the head, asthma, genital disease, the injury of the cuts and they are also used for treating 50 different types of diseases. Radix Asari is used in Six pathogenic factors such as wind, wind-cold, wind-heat, wind-dampness heat, wind-dampness, dampness, paralysis, cold-chill, cold, dryness, in visceral pathogenic factors such as stomach, kidney, liver-kidney, liver and in chi-blood pathogenic factors such as chi-blood deficiency, chi-blood stagnation and in extravasated blood pathogenic factors such as impact injury, an injury such as cuts, blood accumulation as well as weakness-cold and parasites. The actual amount of Radix Asari blended has ranged at a wide variety of amounts from 2.5pun(about 0.94gram) to 3don(about 11.25gram). 26.09% of the prescriptions used 1don(about 3.75gram). The function of Radix Asari is to calm down, to get the consciousness back, to bore through, to warm up the meridians and to run it smoothly, to discharge of phlegm from the combination of drugs and prescriptions.

방풍통성산(防風通聖散)의 문헌(文獻) 및 형상의학적(形象醫學的) 고찰(考察) (Documentational Study and Observation from the View of Hyungsang Medicine on Bangpungtongseong-san)

  • The following conclusions were obtained from the studies on Bangpuntongseongsan both from the documents and Hyungsang medicine. Bangpungtongseong-san was created by Yu Wan-so to relieve both interior and exterior of disease from the pathogenic fire, and it cures wind syndrome and dry syndrome. Bangpungtongseong-san is of light herbal combination and it works in the upper part of the body and is mainly applied to skin disease. Perspiration without harming the exterior and purgation without hurting the interior shows that it is not a severe prescription belonging to meditation therapy. It is mostly used for curing the disease of internal heat caused by over drinking and consuming heavy food, and it has special relationship with Yangmyung meridian. It is mentioned in the chapters of spirit, head, face, eye, ear, nose, throat, skin, hair, prescription, wind, dryness, fire, internal damage, epidemic infectious disease, carbuncle and cellulitis, ulceration, and pediatrics of ${\ulcorner}$Donguibogam${\lrcorner}$. It is usually applied to those who belong to Yangmyung type of the six meridian types or wind type, who has excessive heat, people with red complexion, reddened nose, pimples over the face and nose, coarse heel, loss of hair due to wind-heat, and to those who tend to have dandruff. Through examination over the cases treated with Hyungsan medicine, Bangpungtongseong-san was found efficacious in bloodshot eyes, brandy nose, loss of hair, various skin problems, tetanus, acute alcoholism, paralysis of hand and foot, deafness, and tinnitus.

Dental needle foreign body in the neck: a case report

  • Foreign body (FB) ingestion is commonly seen in the ear nose and throat (ENT) field, with different presentations and sequelae. FBs can arrest in the upper aerodigestive tract or continue further down into either the airway tract to the bronchus or the digestive tract to the intestines. The pathway of an FB depends on the size and shape of the FB and how sharp its edges are. Since the 20th century, the use of disposable stainless-steel needles in the oral cavity has proven to be an effective and safe method for performing various intraoral procedures like dental infiltration or a root canal wash. Complications from their use are rare. Generally, dental needle breakages are caused by patients biting the needle, incorrect injection techniques, or inadequate preventative measures. The sudden movement of a patient during a procedure is one of the most common causes of breakage. Occasionally, needles are swallowed during dental procedures such as a root canal. Here, we report a case of a patient that swallowed a broken needle during a dental procedure. A few days later, the patient presented with neck pain, swelling, and a FB sensation. When the patient presented, she claimed that her symptoms had onset after consuming a meal containing duck meat. Initially, the patient was diagnosed as having ingested a duck bone. However, intraoperatively, the FB was discovered to be an injection needle that had migrated from the throat to the neck.

  • This report describes 48 studies related to the use of Bulbus Fritillariae main blended prescriptions from Donguibogam. The following conclusions were reached through investigations on the prescriptions that use Bulbus Fritillariae as a key ingredient. All kinds of infection in a cough and the malignant tumor recorded the largest number of clinical frequency of the prescriptions in therapeutic use when Bulbus Fritillariae was taken as a principle medicine. The Prescriptions are compounded with Bulbus Fritillariae as a principle medicine can aplly to respiratory disease, eye disease, nose and ear disease, throat disease, a boil. Various pathogenic factors such as consumptive disease, phlegm, phlegm-fire, invasion by wind, affective by cold, fire, dyspepsia-fire are put to practical use. The dosage of Bulbus Fritillariae is 5 bun (about 1.88g) to 1 lyang(about 37.5g), however 1 jeon(about 3.75g) has been taken the most for clinical application. With examination the formulae of presciptions containing Bulbus Fritillariae, I found the basic prescriptions as well as the medicines are in conjunction with Bulbus Fritillariae and the theoretic grounds of formula in combining Bulbus Fritillariae as a group of medicine for presciption through this reseach.

Benign paroxysmal positional vertigo as a complication of sinus floor elevation

  • Purpose: Osteotome sinus floor elevation (OSFE) is an often-used technique of great utility in certain implant patients with resorbed posterior maxilla. Recently benign paroxysmal positional vertigo (BPPV) has been reported as an early postoperative complication following OSFE. Although OSFE-induced BPPV commonly resolves itself within a month without treatment, this complication can be a cause of trouble between the implant surgeon and patient. This report presents a case of BPPV following OSFE. Methods: A 27-year-old man without any significant medical problems and missing his maxillary right first molar, was scheduled for OSFE and simultaneous implant placement. Results: The patient suffered dizziness accompanied by nausea immediately after implant placement using OSFE. Following referral to the ear nose throat clinic, "right posterior canal BPPV" was diagnosed. Despite anti vertigo medication and a single episode of the Epley maneuver, the condition did not improve completely. The Epley maneuver was then applied 7 and 8 days later and the symptoms of BPPV disappeared. One year later, the patient remained symptom-free. Conclusions: Before sinus elevation with an osteotome, implant surgeons should screen out patients with a history of vertigo, to diminish the possibility of BPPV. Operators should be aware of BPPV symptoms. As the symptoms may be very incapacitating, immediate referral to an otorhinolaryngologist is recommended.

  • Mobile computing devices (MCDs), such as smartphones and tablets, are revolutionizing medical practice. These devices are almost universally available and offer a multitude of capabilities, including online features, streaming capabilities, high-quality cameras, and numerous applications. Within the surgical field, MCDs are increasingly being used for simulations. Microsurgery is an expanding field of surgery that presents unique challenges to both trainees and trainers. Simulation-based training and assessment in microsurgery currently play an integral role in the preparation of trainee surgeons in a safe and informative environment. MCDs address these challenges in a novel way by providing valuable adjuncts to microsurgical training, assessment, and clinical practice through low-cost, effective, and widely accessible solutions. Herein, we present a review of the capabilities, accessibility, and relevance of MCDs for technical skills acquisition, training, and clinical microsurgery practice, and consider the possibility of their wider use in the future of microsurgical training and education.

Optic foramen location on computed tomography

  • Background: This study aimed to identify the location of the optic foramen in relation to the anterior sphenoid sinus wall, which is essential information for surgeons in planning and performing endoscopic transnasal surgery. Methods: Computed tomography scans of 200 orbits from 100 adult patients with no abnormalities were examined. The results included the location of the optic foramen in relation to the anterior sphenoid sinus wall and the distance between them, as well as the distance from the optic foramen and the anterior sphenoid sinus wall to the carotid prominence in the posterior sphenoid sinus. Results: The optic foramen was anterior to the anterior sphenoid sinus wall in 48.5% of orbits, and posterior in the remaining 51.5%. The mean distance from the optic foramen to the anterior sphenoid sinus wall was 3.82±1.25 mm. The mean distances from the optic foramen and the anterior sphenoid sinus wall to the carotid prominence were 7.67±1.73 and 7.95±2.53 mm, respectively. Conclusion: The optic foramen was anterior to the anterior wall of the sphenoid sinus in approximately half of the orbits examined in this study, and posterior in the remaining half. The mean distance from the optic foramen to the anterior sphenoid sinus wall of the sphenoid sinus was 3.82±1.25 mm.

국어통증척도의 타당도 연구 (A Study for Testing Validity of Korean Pain Measurement Tool)

  • The main purpose of this study was to clarify the validity with patient's general background of Korean Pain Measurement tool. The subjects of this study were 195 patient from the 8 Med-Surgical wards in H. University Hospital in Seoul. The study was conducted over a 40 day period from Oct. 5, 1985 to Nov. 15, 1985. All patients had pain. Korean Pain Measurement tool and simple discriptive pain scale as Graphic Rating Scale were used to measure the pain, The Pearson Correlation Coefficient test was exercised to measure the correlation between the two kinds of pain tools. To clarify the Sensitivity of Korean Pain tool was used frequency with patient's response. To compare the difference in Pain levels with patient's general background, ANOVA and t-test was employed. To compare the difference in pain levels existed due to pain area of the body used mean numbers. The outcome of the study was as follows : 1. A positive correlation did exist between two pain measurement tools. (r=.2028∼.7768, p <0.002) 2. The sensitive subclass in Korean Pain Measurement tools was 7 subclass. The 7 subclass are inflammatory repeated pain, simple stimulating, traction pressure, dull pain, cavity pain, digestion related pain, suffering. related pain. 3. The existence of levels of pain in accordance with patient's general background, the department of hospital, pain area of the body and school age was supported. Age, sex, religion, marrital status, economic status, acute or chronic status was not supported. 4. The existence of higher pain levels of the body area was anus, chest, and lower pain levels of the body area was eye, ear, nose and throat. Based on the above results, it was found that sensitive subclasses of the Korean Pain Measurement tool was 7 subclass among all of 20 subclass. Thus it can be concluded that Korean Pain Mea-surement tool when partialy used and supplemented, can be an effective tool of pain measurement for the patient in Korea.

소아 호흡기감염 외래환자에 대한 항생제 처방양상 (Outpatient Antibiotic Prescription Patterns for Respiratory Tract Infections of Infants)

  • Background: Antibiotic resistance has been becoming serious challenge to human beings. Overuse of antibiotics, especially, for infants is concerned, but studies are very few for the prescribing pattern of antibiotic use for infants. This study analyzes prescribing patterns of antibiotics in outpatients of preschool children with acute respiratory tract infections in South Korea. Methods: Data are used from 2011 Health Insurance Review & Assessment Services-pediatric patients sample. Inclusion criteria is outpatient children (0 to 5 years) with top five frequent diseases. Prescription rates are analyzed by types of disease, provider, specialty, region, and ages. Binary or multinomial logit models are used to analyze determinants of providers' prescription pattern. Results: The main findings are as follows. First, distributions of prescription rates are shown as L-shape or M-shape depending on the types of disease. Second, the prescription variation is so large among providers, where providers are polarized as a group with low prescription rates and the other group with high prescription rates, though the shapes are shown diversified across types of disease. Third, prescription rates appear to be lower in pediatrics and higher in ENT (ear-nose-throat). Fourth, broad spectrum antibiotics are widely used among children. Finally, the logit analysis shows similar results with descriptive statistics, but partly different results across types of disease. Conclusion: Antibiotics for respiratory tract infections of infants are used excessively with a large variation among providers, and especially broad spectrum antibiotics are used. The prescription guideline for antibiotics should be provided for each specific disease to reduce antibiotic resistance in the future.
