역동역학해석 및 근전도 신호를 이용한 앉기-서기 동작에서의 하지 관절 모멘트 및 근력 예측 (Estimation of Joint Moment and Muscle Force in Lower Extremity During Sit-to-Stand Movement by Inverse Dynamics Analysis and by Electromyography)

  • 앉기-서기 동작은 인간의 생체역학적 기능 정도를 결정할 수 있는 기본적인 일상생활 동작의 하나이다. 동작 중 발생하는 관절의 운동 정보와 모멘트, 그리고 근력을 예측하는 것은 앉기-서기 동작의 특성을 이해하는데 필요하다. 본 연구에서는 동작 기반 역동역학해석 프로그램과 근전도 기반 근력예측 프로그램을 개발하였다. 개발된 프로그램과 앉기-서기 동작 실험에서 측정한 동작, 지면반력, 그리고 근전도신호 정보를 이용하여 실험적 방법으로 측정하기 힘든 관절각도, 관절모멘트, 및 근력을 예측하고 기존 연구결과와 비교하여 검증하였다. 개발된 기법과 프로그램들은 일상생활 동작에서의 관절 생체역학과 근육 협응에 대한 지식을 얻는 데 사용될 수 있으리라 기대한다.

신발굽 높이와 Wedge 위치 변화가 하지 근활성도에 미치는 영향 (Effect on the Lower Extremity Muscle Activity According to the Heel Height and Wedge Position)

  • 본 연구는 30명의 여성에게 내 외측 wedge 삽입과 함께 flated heel 또는 5cm heel을 착용하도록 한 후, 넙다리네갈래근 중 안쪽빗넓은근(VMO)과 가쪽넓은근(VL)의 근활성도를 측정하여 비교하였다. 연구 결과 내외측 wedge 모두에서 flated heel 군에서는 가쪽넓은근이, 5 cm heel 군에서는 안쪽빗넓은근의 근활성도가 높게 나타났으며 가쪽넓은근에 대한 안쪽빗넓은근의 근활성도비(% VMO/VL) 또한 5cm heel 군에서 유의하게 높게 나타났다. 이는 안쪽빗넓은근과 가쪽넓은근의 근활성도에 영향을 미치는 외적 변수가 wedge의 내외측 위치보다는 힐의 높이에 의함을 알 수 있었다. 차후 무릎통증이나 무릎 불안정성으로 인한 하지 재활훈련 또는 하지보조기 제작 시에 하지 근육의 선택적 활성화를 유도하기 위한 변수로써 heel 높이를 고려할 수 있을 것이다.

Ergonomic Evaluation of Trunk-Forearm Support Type Chair

  • Objective: The aim of this study is to investigate the effects of trunk-forearm supported sitting on trunk flexion angle, trunk extensor fatigue and seat contact pressure. Background: The relationship between sitting posture and musculoskeletal disorders of the trunk extensor fatigue and seat contact pressure has been documented. The trunk-forearm support type ergonomic chair was devised from the fact that trunk-forearm support has been reported to reduce trunk extensor activity and discomfort. Method: Using three different sitting postures, upright ($P_1$), trunk-forearm supported ($P_2$) and normal sitting ($P_3$), six healthy subjects participated in the study. Motion capture system was used to collect head and trunk flexion angle, and surface electromyography (sEMG) was used to collect myoelectric signal of upper trapezius, lower trapezius, erector spinae, multifidus, and pressure mat system was used to measure seat contact pressure. Results: When trunk and forearm were supported by the ergonomic chair, higher head flexion angle showed upright > trunk-forearm supported > normal in order, and muscle fatigue showed less than upright and normal sitting. Mean seat contact pressure decreased 19% than upright sitting. But muscle fatigue was not affected by each condition. Conclusion: Trunk-forearm supported sitting of the ergonomic chair showed positive effect in respect of trunk and head flexion angle, trunk extensor fatigue, seat contact pressure. To acquire comprehensive understanding of the effectiveness of the ergonomic chair, further studies such as anatomical effects from measurement of external applied loading effect to the body from interface pressure analysis are required. Application: The results of the publishing trend analysis might help physiological effects of trunk-forearm support type chair.

최대 및 최대하 벤치프레스 반복운동 중에 근활성도 및 근피로도의 비교 (A Comparison of Muscle Activity and Fatigue Between Maximal and Submaximal Repetitions Exercise During the Bench Press)

  • The study aim was to investigate a comparison of muscle activity fatigue between maximal and submaximal strength gains during the bench press in men. It was hypothesized that maximal voluntary contraction (%MVC) would similar gains between maximal and submaximal strength gains during the bench press, but median frequency (MDF) would increase in maximal compared with submaximal exercise. Employing a randomized, counterbalanced crossover design, 12 men were asked to perform maximal repetitions and submaximal repetitions (concentric: 1-s, eccentric: 1-s, 2-s/repetition) to failure with a load of 85% of 1RM for the bench press, with a 3-minute recovery between the sets. Each subject was tested for the number of repetitions and sets, total work in bench press. Surface electromyography (EMG) was recorded from the pectoralis major, deltoid anterior, and triceps brachii for %MVC and MDF. Total work was significantly higher in the submaximal repetition exercise than that the maximal repetition exercise (p<.05). Muscle fatigue of pectoralis major, deltoid anterior and triceps brachii were significantly smaller in the submaximnal repetition exercise than that the maximal repetition exercise (p<.05, respectively). However, muscle activity of pectoralis major, deltoid anterior, and triceps brachii were not significantly different between exercises. Our study showed that a smaller muscle fatigue in submaximal repetitions, despite higher in total work and a similar in muscle activity were observed. These results suggest that submaximal repetitions during the bench press would be enhanced intensity and/or volume compared with maximal repetitions exercise.

골반 자세 변화에 따른 일어서기동작의 운동형상학적 분석과 근전도 연구 (Kinematic and EMG Analysis of Sit-to-Stand With Changes of Pelvic Tilting)

  • The purpose of this study was to analyze the effects of three different pelvic tilts on sit-to-stand ativities and to suggest a new therapeutic approach for movement reeducation in patients who have difficulty with sit-to-stand activities. The three different pelvic tilts were: (1) comfortable pelvic tilt sit-to-stand (CPT STS), (2) posterior pelvic tilt sit-to-stand (PPT STS) and (3) anterior pelvic tilt sit-to-stand (APT STS). To analyze the kinematic component of STS, a motion analysis system (Zebris) was applied to the ankle, knee, hip joint, and thigh-off area. Also, to determine the onset time of muscle contraction, surface electrodes were placed to the rectus femoris muscle (RF), the vastus lateralis muscle (VL), the biceps femoris muscle (BF), the tibialis anterior muscle (TA), the gastrocnemius muscle (GCM), and the soleus muscle (SOL). One-way repeated ANOVA was used for the statistical analysis. First, significant differences were found in kinematic variables for the hip, knee, ankle joint, and thigh-off among the three activities. Second, there was significant difference in muscle activation pattern in TA. VL. and BF among three activities. In conclusion, the findings of this study suggest the following evaluative and therapeutic approach for STS activity: (1) Changes in knee and ankle joints should be prioritized and recruitment order differences in VL and RF can be generated to accomplish abnormal STS activity. (2) APT STS can be introduced for movement efficiency and functional advantage when abnormal STS is treated.

척추 안정화 운동 방법들에 따른 배근육의 근 활성도 비교 (EMG Activity of Abdominal Muscles During Lumbopelvic Stabilization Exercises)

  • Lumbopelvic stabilization exercise has become the most popular treatment method in lumbar rehabilitation since its effectiveness was shown in some aspects of pain and disability. The abdominal drawing-in maneuver (ADIM) has been extensively implemented to promote lumbopelvic stability. However, performing ADIM correctly is difficult even for healthy subjects, and it is time consuming to train people in ADIM. Thus, the purpose of this study was to compare abdominal muscle [rectus abdominalis (RA), external oblique (EO), and transverse abdominis/internal oblique (TrA/IO)] activity during lumbopelvic stabilization exercises (ADIM only, ADIM with a ball, maximum exhalation only, and maximum exhalation with a ball) performed in a supine position with feet against a wall. Fifteen healthy subjects were recruited for this study. Surface electromyography was used to measure abdominal muscle activity during lumbopelvic stabilization exercises. A one-way repeated-measures analysis of variance was used to determine the statistical significance of RA, EO, and TrA/IO muscle activity during four lumbopelvic stabilization exercises. Both-side TrA/IO muscle activity was significantly greater with maximum exhalation with a ball than with ADIM only or ADIM with a ball (p<.008). The results of this study suggest that maximum exhalation with a ball can be used as an effective lumbopelvic stabilization exercise to increase TrA/IO muscle activity in healthy subjects.

총 비골 신경에 발생한 결절종 - 증례보고 - (Ganglionic Cyst of the Peroneal Nerve - A Case Report -)

  • 결절종으로 인한 총 비골 신경 마비는 매우 드문 예에서 보고되고 있지만, 잘 알려져 있는 질환이다. 결절종으로 인한 비골 신경 포착을 보이는 자기 공명 영상 소견에 대하여 3편이 보고되어 있다. 본 증례에서는 진단을 위해 초음파, 자기 공명 영상, 근전도, 신경전도 검사 및 조직 검사를 시행하였다. 비골 경부 주변의 관상 구조는 자기 공명 영상 소견 상 특징적으로 T2 강조영상에서 고신호를 보이면서 상부 경비골 관절의 아래 부분까지 종축으로 뻗어있는 소견을 보였다. 비골 신경을 노출시키고 결절종에 대해 병소내 절제술을 시행하였다. 수술 후에도 마비 증상이 지속되었으나, 수술 후 4개월부터 점차적으로 마비의 회복소견을 보이기 시작했고 술 후 7개월에 완전한 회복을 보였다. 본 증례에서 자기 공명 영상은 낭종의 범위, 위치, 기원을 밝혀 내는데 도움을 주었고, 외과적 절제술로 좋은 결과를 얻을 수 있다.

뇌졸중 후 편마비 환자의 체위변화에 따른 척수운동신경원 흥분성 변화 (Changes in the Spinal Motor Neuron Excitability Depending on Postural Changes in Post Stoke Hemiplegics)

  • Purpose: The purpose of this study was to measure changes in the H-reflex and V wave under loading conditions (e.g. prone and standing position) and to investigate whether postural change would affect the H-reflex and V wave in post stroke hemiplegic patients. Methods: Thirty persons with hemiplegia resulting from stroke (20 males, 10 females) participated in this study. Electromyography (EMG) was used to electrically stimulate and record the soleus H-reflexes and V waves under various loading conditions. The normality of the distribution of each variable (H latency, $H_{max}/M_{max}$ ratio, $V_{max}/M_{max}$ ratio) was tested using the Kolmogorov-Smirnov test. The means of normally distributed continuous data were assessed by independent t-test (${\alpha}$=0.05). Results: There were statistically significant differences in $H_{max}/M_{max}$ ratio (p<0.01), $V_{max}/M_{max}$ ratio (p<0.01), H latency (p<0.01) among the prone and standing position. Conclusion: We found that the H-reflex and V wave in standing position was more active to weight bearing load than prone position.

불안정한 지지면에서의 균형운동이 건강한 성인의 폐활량에 미치는 효과: 사전 연구 (Effect of the Balance Exercise on the Unstable Surfaces for the Vital Capacity in Healthy Adults: A Preliminary Study)

  • Purpose : This study attempts to examine the effect of the balance exercise on the unstable surfaces for the vital capacity in healthy adults. Methods : A total of 13 subjects was randomly divided into a breathing exercise training group (n=7) and a breathing and balance exercise training group (n=6). Changes in vital capacity and respiratory muscle activity were measured before and after the intervention. The intervention was performed three times a week for four weeks. Forced vital capacity (FVC), forced expiratory volume at one second (FEV1) and maximal voluntary ventilation (MVV) were used as measurement tools for the vital capacity test. Electromyography (EMG) was also used to examine respiratory muscle activity. Result : The breathing exercise training group showed significant improvement in terms of FVC, FEV1 and external oblique (EO), and transverse abdominis/internal oblique (TrA/IO) of MVV. The breathing and balance exercise training group showed significant improvement in terms of FVC, MVV and EO, TrA/IO of FVC and rectus abdominis (RA), EO, and TrA/IO of MVV. However, in comparing changes in vital capacity and respiratory muscle activity before and after the training, the breathing exercise training group and the breathing and balance exercise training group showed a significant difference in terms of MVV. Conclusion : This study is as a preliminary study to find out the relation between a balance exercise and a vital capacity, it is considered to require a further study with several revisions of subjects, duration and time for an intervention.

Comparison of Triceps Surae EMG in Plantar Flexion Test of MMT at Different Knee Angles

  • This study was conducted to examine changes in the muscle activity of the triceps surae, specifically the gastrocnemius and the soleus, depending on the angle of the knee joint during the manual muscle test (MMT) of the plantar flexion of the ankle. The muscle activity of the medial and lateral heads of the gastrocnemius was statistically significantly reduced when the angle of the knee joint was $15^{\circ}$, $30^{\circ}$, and $45^{\circ}$ compared to when the angle was $0^{\circ}$. However, there was no statistically significant difference in muscle activity at the angles of $15^{\circ}$ and $30^{\circ}$ or $45^{\circ}$. There was no statistically significant difference in the muscle activity of the soleus depending on the angle of the knee joint. The ratio of the muscle activity of the soleus to that of the triceps surae showed a statistically significant increase when the angle was $15^{\circ}$, $30^{\circ}$ and $45^{\circ}$ compared to when the angle was $0^{\circ}$. However, there was no statistically significant difference in muscle activity at the angles of $15^{\circ}$ and $30^{\circ}$ or $45^{\circ}$. When the angle of the knee joint was $15^{\circ}$ or higher during the test of the isolated soleus, the muscle activity of the gastrocnemius was reduced. These results indicate that the angle is suitable for the test of the isolated soleus, but there was no statistically significant difference in the muscle activity of the gastrocnemius when the angle was higher than $15^{\circ}$. Therefore, it can be concluded that the most suitable angle of the knee joint for the isolated MMT test of the soleus is $15^{\circ}$.
