Primary Rib Chondrosarcoma in a Dog (원발성 개 늑연골육종 1예)

  • Primary rib tumors occur in dogs at a much lower frequency than appendicular bone neoplasms. A 3-year-old 3.5kg female maltese was referred to the Veterinary Medical Teaching Hospital because of a 2-month history of severe coughing and a 1-month history of dyspnea, gaging, and anorexia. The plain thoracic radiography revealed a huge well-defined mass in the left thoracic cavity, and a diagnosis of primary rib chondrosarcoma was established by histopathologic evaluation of the surgical biopsy specimen. At operation, a 8-x12-x4.5-cm relatively large firm mass was found filling the lumen of the thoracic, which was diffusely attached to the left 2-9th rib. This mass oppressed the heart, left lung and diaphragm.

Surgical Treatment for Aneurysm of Sinus of Valsalva Combined with Ventricular Septal Defect (심실중격결손을 합병한 Valsalva's 동 동맥류 파열의 치험예)

  • This is a case report of surgically treated rupture of Valsalva Sinus aneurysm combined with VSD. He has been relatively healthy until about one month before admission, when during bath, he felt abruptly palpitation, left chest pain and exertional dyspnea. These symptoms have progressed. On admission, thrill was palpable and continuous machinery murmur was audible on 2nd and 3rd intercostal space along the left sternal border. A rupture of Valsalva`s sinus aneurysm was confirmed by aortography and echocardiography but a small VSD was found by cardiotomy in open heart surgery. On 11th Sep. 1978, open heart surgery was performed. Valsalva`s sinus aneurysm came out from right coronary aortic sinus and ruptured into the right ventricle. It sized 1.2X1.5X1.5 cm. Ruptured opening was noted on apex of aneurysm [0.8X0.8cm], VSD [1. 0X0. 3cm in size] was just below the aortic annulus. The aneurysmal sac was removed on neck. After that, VSD and aneurysmal orifice were closed together with interrupted mattress sutures on same plane. The postoperative course was uneventful and discharged three weeks after open heart surgery.

Surgical Treatment of Adult Patent Ductus Arteriosus (성인 동맥관개존증의 외과적 치료)

  • Patent ductus arteriosus in adult is frequently complicated with aneurysm, calcification and pulmonary hypertension. Therefore, surgical treatment of patent ductus arteriosus in adult has some differences from that of infants and children. We have experienced 25 cases of adult patent ductus arteriosus from July 1983 to December 1994. Mean age of the patients was 26.6 years[16-59 years . There were 12 males and 13 females. Exertional dyspnea was the most frequent clinical manifestation. Pulmonary hypertension was present in eight patients. Ligation was done in 20 cases and surgery using cardiopulmonary bypass was done in 5 cases. There was one hospital death, which had a 5x5cm sized main pulmonary artery aneurysm and was operated under cardiopulmonary bypass. Recanalization occured in one patient who received Teflon-felt supported ligation. Although one case of recannalization after ligation was present, we believed the fact that ligation was safe and simple method of operation in adult patent ductus arteriosus without calcification and aneurysm.

Surgical Experience of Idiopathic Hypertrophic Subaortic Stenosis - Report of A case - (특발성 비후성 대동맥판하 협착증의 수술적 치험 1례 보)

  • A 23-y-o male patient was suffered from intermittent fainting and dyspnea on exertion [NYHA Class IIIIV]. 2-D - Echocardiogram and cardiac catheterization with cineangiogram showed typical IHSS findings those were asymmetrical septal hypertrophy [ASH], systolic anterior motion of anterior mitral leaflet [SAM] which induced mild mitral regurgitation [Seller Grade I/IV] and pressure gradient about 60 mmHg between left ventricle and the aorta. Medical treatment with 8-adrenergic blockade [propranolol] and Ca" channel antagonist [Verapamil] had no response. So, we performed trans-aortic ventricular septal myotomy and mymectomy. Resected rectangular muscle bar was 1 Cm x 1 Cm x 4.5 Cm. Post-operative pressure gradient between the left ventricle and the aorta was less than 10 mmHg and SAM. was disappeared with decreased mitral regurgitation grade. Post-operative course was smooth and his symptoms and signs were free without any medication during 12 months follow-up.w-up.

  • A 12-year-old, intact female Jindo was referred to our clinic due to the enlargement of all lymph nodes, as well as severe dyspnea. All palpatible lymph nodes were highly swelling and enlargement. The dog was diagnosed as end stage of multicentric lymphoma with multi-organ metastasis. In addition, the dog was diagnosed as having a pulmonary thromboembolism via computed tomography (CT) and D-dimer concentrations and prothrombin time (PT) results. This case report describes that lymphoma can be associated with pulmonary thromboembolism which is life threatening complication in dogs. The present case was managed successfully with chemotherapy and antithrombotic treatment.

  • In this study, 186 cases of spontaneous pneumothorax experienced at the Dept. of thoracic and Cardiovascular surgery, chosun University Hospital during from Jan. 1978 to Mar. 1987 were analysed retrospectically. The results were as follows: 1. The sex ratio of the stadied patient was 2.5:! in male prominence and the incidence and the incidence was highest in the adolescence between 20-29 year of age. 2. The clinical symptoms were frequently Dyspnea (52.2%) and Chest pain (38.7%). The etiologic factors were as follows, Tuberculous origin in 55.9%, Unknown origin in 17.2%, Bullae in 11.8%. 3. In the method of treatment, closed thoracotomy was the most effective procedure in achieving the expansion of collapsed lung in 87.1%. Other methods were bed rest with high oxygen inhalation, thoracentesis and open thoracotomy. 4. The incidences of complication were developed in 12.4% and recurrent rates were seen in 9.7%.

  • A 73-year-old woman who underwent combined bioprosthetic mitral valve replacement, tricuspid ring annuloplasty, and coronary artery bypass grafting 12 years previously visited our clinic due to aggravated dyspnea caused by structural valve deterioration of the mitral prosthesis. Because aortic or femoral artery cannulation and cross-clamping would have a high risk of stroke owing to severe calcification of the ascending aorta and ilio-femoral vessels, and because there was a risk of redo sternotomy due to the patent bypass grafts, a comprehensive approach including axillary artery cannulation, a minimally invasive right thoracotomy approach, and a clampless hypothermic fibrillatory arrest technique was used during redo mitral valve replacement.

  • This study has been carried out to investigate the Cause and Symptom of Ji-Eum ( 支飮 ) by referring to 34 documents. The results were as follow ; 1. Ji-Eum ( 支飮 ) is caused by the Su-Eum ( 水飮 ) storing up thoracic cavity ( 胸中 ), interdiaphragam ( 膈間 ). SimHa ( 心下 ), and stay lung. 2. The symptom of Ji Eum ( 支飮 ) is as follows ; HaeYouk( 咳逆 ), EuySik( 倚息 ), dyspnea( 短氣 ), sleeplessness ( 不得臥 ) edema ( 基形加腫). WhungMan ( 胸滿 ), SimHaChukGey ( 心下築悸 ), GoMoHyun ( 苦冒眩 ), EumSikBulWha ( 飮食不化 ). Pulse HyeonGin ( 弦緊 ). Greasy white coat of tongue.

A Clinical Study of Spontaneous Pneumothorax (자연기흉의 임상연구)

  • Spontaneous pneumothorax is an accumulation of air in the pleural space with collapse of the lungs in the absence of external chest trauma. In this clinical study were analyzed of 369 cases of spontaneous pneumothorax experienced at the department of Thoracic and Cardiovascular Surgery, College of Medicine, Chosun University during from January, 1986 to December, 1992.The peak incidence of age was in 3rd decade and more predominantly in male than women [5:1]. Pulmonary tuberculosis was most common etiologic condition and frequently symptoms were dyspnea and chest pain. The site of pneumothorax was Rt.in 48%, Lt.in 45% and both in 7%. The common accompanied diseases were hydrothorax, pyothorax and hemothorax. In 166 cases [45%] were treated by closed thoracostomy only, in 43 cases [12%] were treated by closed thoracostomy & chemical pleurodesis with Tetracycline and in 145 cases [39%] were treated by open thoracotomy. The most serious complication, one case of pulmonary edema, was developed after closed thoracostomy and fatal.

Mitral Valve Dysplasia in a Maltese Dog

  • A four month-old intact female Maltese (weighing 1.8 kg) was present with major complaints of severe coughing and dyspnea. Diagnostic imaging studies revealed abnormally elongated chordae tendineae attached to anterior leaflet and shortened chordae tendineae attached to posterior leaflet of mitral valve causing mitral insufficiency and regurgitant blood flow into left atrium, indicating congenital mitral valve dysplasia. Therapy was directed to control existing clinical signs and to prevent further deterioration of cardiac enlargement using conventional cardiac medications. The dog is currently survived and being monitored at regular interval. This is the first case report describing mitral valve dysplasia causing mitral insufficiency of dog in Korea.
