  • Objectives : The Purpose of this study is to evaluate the effect of the deep thermo-conductive acupuncture therapy for the patients with lateral epicondylitis of the elbow. Methods : Thirteen patients with lateral epicondylitis were included from April 2011 to December 2012 and all the patients were treated with the deep thermo-conductive acupuncture therapy. Then we checked visual analog scale(VAS) scores of the patients at every hospital visit, and compared the differences for before and after treatment. We also divided them into 3 age groups(in 40s, 50s, 60s) and 4 groups of the duration of illness(~1 week, ~1month, ~6 months, 6 months ~), and compared the effects for 3 age groups as well as 4 groups of duration of illness. Results : 1. VAS scores had been changed significantly from $6.0769{\pm}1.5390$ to $3.3077{\pm}1.9207$(p=0.005). 2. No significant differences were found between 3 age groups(p=0.968) as well as 4 groups of the duration of illness(p=0.724). Conclusions : This study suggests that deep thermo-conductive acupuncture therapy is effective for healing of the lateral epicondylitis of elbow. But the age and the duration of illness seem to have no influence on results.

  • Objectives: The purpose of this study was to investigate characteristics of patients who were admitted to an oriental medicine hospital by traffic accident. Methods: The medical charts of 346 patients admitted to an oriental medicine hospital from June 1, 2017 to May 31, 2018 were analyzed. The Numbering Rating Scale (NRS) and duration of hospitalization were used to evaluate characteristics of the patients. Results: Acupuncture, Moxibustion, Infralux were used to treat all the patients. The most frequently used herbal medication was Danggwisu-san(22.25%). 87 patients(25.14%) visited the outpatient department after being discharged from the hospital. The most frequent complaint in terms of pain was cervical pain(82.7%) and of systemic symptom was headache(23.7%). Men and younger aged patients showed higher therapeutic effect than women and older ages. The most common duration of hospitalization was 2~4 days(42.73%) and positively correlated with therapeutic effect. The most frequent interval between time of injury and visit to the hospital was from 0-1 days(68.90%) and showed no relationship with therapeutic effect. The most frequent admission pathway was "Directly to the hospital"(57.51%). Admission pathway was proportionally associated with duration of hospitalization and treatment results were not. The most common vehicle type involved in the traffic accidents was a sedan(72.25%), accident type was a rear-end collision(43.64%) and showed no relationship with therapeutic effect. Conclusions: In this study, therapeutic effects were highly correlated among men, younger ages, and duration of hospital stay, and was not for interval days, admission pathway, vehicle type, and accident type.

  • Objectives: We analyzed retrospective clinical data of Korean medical institutes for infertility care and investigated current status and outcome of the Korean medical treatment of infertility as a part of foundational research for verifying validity of constructing national support system and developing appropriate policy on Korean medical treatment of infertility. Methods: We investigated data uploaded on the homepage of The Society of Korean Medicine for Subfertility (http://www.okinfertility.org) by Korean medical institutes for infertility care to get informations such as patients' age, body height, weight, methods of Korean medical treatment, cost and duration of treatment, success or failure of pregnancy and result of treatment. Results: The average age of patients was $33.1{\pm}3.8$ and the average body height was $161.2{\pm}5.3cm$ and the average body weight was $55.2{\pm}8.5kg$. The method of Korean medical treatment was Herbal medicine (97.5%), acupuncture (80.4%), moxibustion (57.7%), cupping (32.7%). The average cost of treatment was $1,160,625{\pm}882,499$ won, and the average medical cost per visit was $357,845{\pm}241,602$ won. The average duration of treatment was $11.26{\pm}10.58$ weeks, and the average number of visits per patient was $4.78{\pm}6.10$ times. The average duration of treatment was the longest in the group of infertile patients with the highest average medical cost per visit. The average pregnancy success rate was 30.9% overall, and that of the group of infertile patients was 25.8%. Conclusions: When we develop the standard project model with expectation for about 25% success rate of pregnancy and delivery in the group of patients who have idiopathic and ovulatory factors, it is desirable to perform acupuncture and moxibustion treatment 1-2 times a week with herbal medicine. The treatment period is set to at least 12 weeks. Average treatment fee maybe calculated by converting the averaged treatment cost of clinic's one month worth of daily treatment fee and medical hospital's ten day treatment fee into weekly treatment cost.

  • Background: The study describes the changes resulted from imposition on tertiary hospital outpatient coinsurance rate rise policy and in tertiary or general hospital drug coverage rise policy on healthcare service utilization. Methods: Accordingly, the hypothesis about outpatient healthcare utilization after rise policy in outpatient coinsurance rate and drug coverage was established, using interrupted time-series analysis and segmented regression analysis to test the hypothesis. 5-year analysis period (2007. 3-2012. 3) from the outset year was designated, the data about most common 10 high-ranking of the main diseases targeting visiting patient from age of 6 to 64 were collected. Results: The summary on the major research is followed. First, the medical expense and duration of treatment tends to be increased in case of imposition about rise policy in outpatient coinsurance rate in the tertiary hospital under the interrupted time-series analysis. It showed temporary increase and slow down on account of influenza A even after the policy enforcement. In segmented regression analysis, duration of visit and medical expense in the tertiary hospital increased temporally right after the policy implementation and the decreased rapidly depends on period. Both rise and fall is statistically significant. The second, In case of tertiary or general hospital outpatient drug coverage rise policy, all of the tertiary hospital healthcare service utilization variables by the interrupted time-series analysis, drug coverage policy in the general hospital deeply declined according to decreasing trend before policy implementation. The third, in case of segmented regression analysis, the visit duration and medical expense statistically declined right after the policy implementation in both the tertiary and general hospital. Meanwhile, administration day was statistically meaningful only for the decrease right after the policy implementation. Otherwise, general hospital changes are not statistically meaningful. And the medicine cost was statistically, meaningfully decreased after the increase in drug coverage. Conclusion: Finally, the result demonstrated according to the analysis is only 1 hypothesis is denied, the other 2 are partially supported. Then, tertiary hospital outpatient coinsurance rate increase policy comparatively makes decrease effect on long-term healthcare utilization, and tertiary or general hospital outpatient drug coverage policy showed partially short-term effect is assured.

  • Authors reviewed 73 cases of primary skin cancer arising from head and neck, including comparative clinical data of 63 cases of squamous cell carcinoma and basal cell carcinoma retrospectively. There was no significant sexual predilection in each type of cancer and 77% of all cases were occurred after age of 50. The data of occurring site and occupation(farmer; 41%) implied that sunlight exposure strongly concerned in development of skin cancer. Comparing the data of 63 cases of squamous cell carcinoma and basal cell carcinoma, authors found out no remarkable difference in mean diameter of primary lesion at first visit of clinic, but some difference in mean duration, the percentage of the cases less than 1 yearduration and percentage of T1 lesion with no statistical significance(p>0.05). Combination of surgery and radiotherapy had relatively lower recurrence rate (11%) than surgery alone (13%). Squamous cell carcinoma had higher recurrence rate(34%) and metastasis rate(28%) than basal cell carcinoma; 6% and 3%, respectively(p<0.05).

  • Objectives: This study aimed to develop and validate machine learning (ML) models using H2O-AutoML, an automated ML program, for predicting medication-related osteonecrosis of the jaw (MRONJ) in patients with osteoporosis undergoing tooth extraction or implantation. Patients and Methods: We conducted a retrospective chart review of 340 patients who visited Dankook University Dental Hospital between January 2019 and June 2022 who met the following inclusion criteria: female, age ≥55 years, osteoporosis treated with antiresorptive therapy, and recent dental extraction or implantation. We considered medication administration and duration, demographics, and systemic factors (age and medical history). Local factors, such as surgical method, number of operated teeth, and operation area, were also included. Six algorithms were used to generate the MRONJ prediction model. Results: Gradient boosting demonstrated the best diagnostic accuracy, with an area under the receiver operating characteristic curve (AUC) of 0.8283. Validation with the test dataset yielded a stable AUC of 0.7526. Variable importance analysis identified duration of medication as the most important variable, followed by age, number of teeth operated, and operation site. Conclusion: ML models can help predict MRONJ occurrence in patients with osteoporosis undergoing tooth extraction or implantation based on questionnaire data acquired at the first visit.

  • Two experiments were conducted to compare the feed intake patterns and growth performance of Meishan and Yorkshire growing pigs. Experiment 1 was carried out over a 6-wk period and used 48 barrows with equal numbers of purebred Meishan (M) and Yorkshire (Y). Pigs were allocated to four groups of 12 pigs consisting of equal numbers of M and Y. Initial BW were $36.4{\pm}0.32kg$ and $42.1{\pm}1.41kg$ for M and Y, respectively. Experiment 2 was carried out over a 5-week period and used 48 pigs consisting of equal numbers of both barrows and gilts and of crossbred Meishan$\times$Yorkshire (MY) and purebred Yorkshire (Y) animals. Pigs were allotted to 6 pens of 8 pigs, with 4 single- and 2 mixed-genotype groups (initial $BW=28.5{\pm}0.99kg$). In both experiments, pigs were given ad libitum access to a grower diet (17% crude protein, 0.9% lysine, 3365 kcal/kg ME) via feed intake recording equipment (F.I.R.E.). Pigs carried an ear-tag transponder with an unique identification which allowed the time, duration, and size of individual meals to be recorded. In Exp. 1, Y had higher ADG (721 vs 353 g, p<0.01), daily feed intake (DFI; 2.338 vs 1.363 kg, p<0.01), made more frequent visits to the feeder per day (NFV; 18.5 vs 7.7, p<0.01), had a shorter feeder occupation time per visit (FOV; 7.4 vs 12.9 min, p<0.01), and ate less feed per visit (FIV; 130 vs 177 g, p<0.01) than M pigs. Feed consumption rates (CR) were greater for Y compared to M (19.3 vs 14.8 g/min, p<0.01). Feeder occupation time per day (FOD) was longer for Y than M (114.3 vs 82.8 min/pig, p<0.01). Yorkshire pigs visited the feeder more frequently between 0800 and 1100 h. Meishan pigs showed more frequent feeder visits between 0600 and 0800 h, and between 1600 and 2100 h when feeding competition with Y was reduced. In Exp. 2, there was no effect of genotype or group composition on DFI, ADG or gain:feed ratio. Crossbred pigs (MY) made fewer feeder visits (12.6 vs 17.7, p<0.01), and had greater FIV (124 vs 98 g/visit, p<0.01), and longer FOV (8.11 vs 7.24 min/visit, p<0.01) and FOD (112 vs 100 min, p<0.05) than Y pigs. Results of this study suggest substantial genetic variation in feeding patterns as well as in growth performance.

  • Purpose: To evaluate correlation between the clinical results and causative bacteria in diabetic foot patients with lower extremity amputation. Materials and Methods: One hundred twenty nine patients(131 feet) of diabetic foot amputations were followed for more than one year. Wound cultures were done by deep tissue or bone debris at first visit to our clinics. Retrospective analysis was performed using chart review and interview with the patients. Depending on the culture result, level of amputation, reinfection, duration of treatment, death rate, patient satisfaction and admission dates were evaluated. Results: Microorganisms were confirmed in 114 cases. In the other 17 cases, there were no cultured microorganisms. In bacterial growth group, Methicillin-sensitive Staphylococcus aureus was the most common pathogen and accounted for 34 cases. As other common pathogens, there were Methicillin-resistant Staphylococcus aureus(24 cases) and mixed infection(14 cases). Mortality is no difference in each infected group. Mixed bacterial infected patients have higher reinfection, longer hospital day and duration of treatment, but there is no difference in patients satisfaction and pain at last follow up. Conclusion: The most common pathogen in diabetic foot patients with lower extremity amputation was Methicillin-sensitive Staphylococcus aureus, and mixed bacterial infected patients have higher reinfection rate, longer admission date and duration of treatment than other bacterial infected patients.

  • The study was clinically performed in the 65 outpatients with pruritus who visited the Dept. of Dermatology, Oriental Medicial Hospital, Kyung Hee University and oriental medical clinic of Hang Ki Park from Jan. 1985 through Dec. 1986. The results were summarized as follows: 1. Among 65 outpatients, male was 20 ($30.8\%$) and female was 45 ($69.2\%$). In the distribution of age, 10S was $21.5\%,\;and\;20S\;20\%$ at the first visit. 2. In the distribution of season, the peak incidence occured in Spring ($35.4\%$) and next incidence in Winter ($20\%$). 3. In the distribution of duration, $41.5\%$ was between 1 year and 5 years, $20\%$ between 1 month and 6 months, $12.3\%$ between 6 months and 1 year, $9.2\%$ between 5 years and 10 years, $7.7\%$ over 10 years. 4. In regard to the itching lesion, $66.2\%$ showed pruritus throughout the whole body, including partially $15.4\%$ in their heads and faces, $12.3\%$ in their hands and legs, and $6.3\%$ in their shoulders, sides and hips. 5. The most frequently used prescription, was Gosanhomasan (苦蔘胡麻散) which occupied $60.7\%$ of all, in next order of frequency, Gamisamooltang (加味四物湯 $21.4\%$) Gupoongchongkisan (祛風淸肌散 $12.5\%$) and Bangpoongtongsungsan (防風通聖散 $5.4\%$). 6. In regard to the therapeutic duration, $46.2\%$ was showed under 10 days, $18.5\%$ between 10 days and 20 days, and $16.9\%$ between 30 days and 60 days. 7. In the 35 patients, who were confirmed to show apparent result by continual treatment, curative rate was more than $57.6\%$. According to above results, we can confirm that efficacy of therapeutic measures by the oriental medicine on pruritic dermatoses was increased in proportion to therapeutic duration and we feel sure that the therapeutic efficacy can be gradually increased as we expect to make a deep study of the disease and to give correct medical treatment to the nature of the disease.
