Diagnosis of Pulmonary Tuberculosis: Recent Advances and Diagnostic Algorithms

  • Pulmonary tuberculosis (TB) persists as a great public health problem in Korea. Increases in the overall age of the population and the rise of drug-resistant TB have reinforced the need for rapid diagnostic improvements and new modalities to detect TB and drug-resistant TB, as well as to improve TB control. Standard guidelines and recent advances for diagnosing pulmonary TB are summarized in this article. An early and accurate diagnosis of pulmonary TB should be established using chest X-ray, sputum microscopy, culture in both liquid and solid media, and nucleic acid amplification. Chest computed tomography, histopathological examination of biopsy samples, and new molecular diagnostic tests can be used for earlier and improved diagnoses, especially in patients with smear-negative pulmonary TB or clinically-diagnosed TB and drug-resistant TB.

Effects of immune-challenged domestic silkworm hemolymph on the regulation of SIRT5 and PRDx1 expression

  • SIRT5 and PRDx1 play crucial roles in cancer and are involved in the basic mechanisms of reactive oxygen species detoxification. In our previous studies, we showed that hemolymph extracts of immune-challenged Bombyx mori have antioxidant properties. Following H2O2 stimulation, immune-challenged B. mori hemolymph extracts elicited SIRT5 downregulation activity, reaching effective activity at the highest concentration of 100 ppm. Additionally, cells treated with immune-challenged B. mori hemolymph extracts demonstrated increased PRDx1 mRNA expression compared to that of PBS-treated cells. Therefore, immune-challenged B. mori hemolymph extracts offer a potential auxiliary means of treating drug-resistant tumors through downregulation of SIRT5 and upregulation of PRDx1 expression. Nevertheless, further studies on the effects of B. mori hemolymph on SIRT5 and PRDx1 regulation are pertinent for using it as a food or pharmaceutical material and understanding its therapeutic effect on tumors, including those that are drug-resistant.

우리나라 가축(家畜)에서 분리(分離)한 Salmonella 및 대장균(大腸菌)의 내성인자(耐性因子) R의 분포(分布) (Distribution of R Factors in Salmonella and Escherichia Coli Isolated from Korean Domestic Animals)

  • Recent reports confirm that R factors is widespread in Korea among Enterobacteriaceae isolated from humans. However, no reports have been made concerning the incidence of transferable drug resistance in domestic animals in this country. A total of 211 isolates of Escherichia coli, including 94 strains from dogs, 76 strains from pigs, 30 strains from chickens, and 21 strains from cow milk, were examined for drug resistance and distribution of R factors. And, respective two strains of Salmonella E group and Salmonella cholerasuis which were isolated from dogs and pigs, respectively were also examined for the same purposes. Of 211 strains of E. coli isolated, 66.8% were found to be resistant to 8 antibacterial agents such as streptomycin(SM), tetracycline(TC), chloramphenicol(CP), ampicillin sodium(AP), nalidixic acid(NA), gentamicin(GM), and polymyxin B(PX). Among the isolates, 86.2% of the strains from dogs, 70% of the strains from chickens, 43.4% of the strains from pigs, and 28.6% of the strains from milk, respectively, were found to be resistant to the drugs. The following percentage of resistance of E. coli to each individual drugs was encountered: of 94 strains from dogs, AP, 64.9%; SM, 20.2%; NA, 12.8%, CP and PX, 8.5% each; GM, 2.1% each; GM, 2.1%. Among 76 strains from pigs, 42.2% and 2.6% each were resistant to TC, AP and PX, respectively. Among 30 strains from chickens, 43.3% were resistant to SM, TC, AP, respectively, and no strains were resistant to the other drugs. No strains of the isolated from milk were resistant to the drugs, except that 28.6% were resistant to SM and AP, respectively. Of the strains from dogs, multiply resistant strains(56.8%) were more than singly resistant one(43.2%) and sixteen different drug resistant patterns were observed. The most frequently encountered patterns were AP TC AP and SM CP AP NA. Of the isolates from other sources, the most frequently encountered resistant patterns were as follows: TC among the strains from pigs; SM TC AP from chickens; SM AP from milk. Of the resistant strains from dogs, 32% carried R factors and the most common resistance patterns of R factors were AP TC AP and SM TC CP, whereas 35.2% of the resistant strains from pigs carried R factors of which the most common encountered pattern was TC. Of the resistant strains from chickens, 46.9% carried R factors of which the most common patterns were SM TC TC AP and AP, whereas 50% of the resistant strains from milk carried R factors of which the most common pattern was SM. Of 4 strains of Salmonella isolated, no strains were resistant to the drugs, except that only one strain of Salmonella E group isolated from a dog was resistant to AP. The strain did not harbor R factor.

부산지역의 한 3차 진료기관을 방문한 폐결핵 환자의 약제내성률 (Drug-Resistant Pulmonary Tuberculosis In Kosin Medical Center)

  • 연구목적: 폐결핵 치료에서 약재내성은 큰 문제가 된다. 특히 다제 약제 내성은 환자 자신 뿐만 아니라 예방의학적 측면에서도 큰 문제가 되고 있다. 3차 의료기관에 오는 폐결핵 환자들에서 약제 내성의 발현 정도, 양상 및 관계되는 요인 등을 조사하여 내원 환자들의 향후 치료에 도움이 되는 지표를 알아 보고자 하였다. 대상 및 방법: 1986년 부터 1994년 사이 고신의료원 내과를 방문하여 폐결핵으로 진단되어 약제내성 검사를 한 141명의 환자를 대상으로 약제 내성률과 과거 치료 병력, 흉부 X-선 사진상 폐의 침범 정도와 공동의 유무 등의 관계를 후향적으로 조사하였다. 성적: 141 명중 59명(41.8%)에서 한 가지 약제 이상에 내성야 있었고 INH와 RIF에 동시 내성이 있는 경우는 139명중 29명(20.9%)였다. 과거에 결핵 치료를 63명중 40명(63.5%)에서 약제 내성이 있었고 24명(38.1%)에서 다제 약제 내성이 있었다. 과거 결핵치료 병력이 없는 78명중 19명(24.4%)에서 약제내성이 있었고 5명(6.5%)에서는 다제 약제 내성이 있었다. 약제별로는 INH내성이 39.0%로 가장 높았고 RIF(21.6%), EMB(16.3%), PAS(10.8%), SM(8.7%), PZA(8.0%) 순이었다. INH, RIF 및 PZA에 대한 내성은 과거 치료 병력이 있는 경우에서 높았고(odds ratio 각각 3.3, 7.2 및 10.8), 흉부 X-선 사진상 침범부위가 RIF 및 SM에 대한 내성률이 유의하게 높았다(odds ratio 각각 2.9, 2.8). 결론: 높은 약제 내성률로 보아 약제 감수성검사는 모든 환자들에게 시행해야 하겠으며, 그 결과가 나오기 전까지는 4가지 이상의 약제를 사용하는 것이 안전할 것으로 생각된다. 그리고 특히 과거에 치료한 병력이 있거나 진행된 폐결핵 환자의 경우는 철저한 임상적 및 세균학적인 추적관찰이 필요할 것으로 생각한다.

Luteolin Sensitizes Two Oxaliplatin-Resistant Colorectal Cancer Cell Lines to Chemotherapeutic Drugs Via Inhibition of the Nrf2 Pathway

  • Oxaliplatin is a first-line therapy for colorectal cancer, but cancer cell resistance to the drug compromises its efficacy. To explore mechanisms of drug resistance, we treated colorectal cancer cells (HCT116 and SW620) long-term with oxaliplatin and established stable oxaliplatin-resistant lines (HCT116-OX and SW620-OX). Compared with parental cell lines, $IC_{50}$s for various chemotherapeutic agents (oxaliplatin, cisplatin and doxorubicin) were increased in oxaliplatin-resistant cell lines and this was accompanied by activation of nuclear factor erythroid-2 p45-related factor 2 (Nrf2) and NADPH quinone oxidoreductase 1 (NQO1). Furthermore, luteolin inhibited the Nrf2 pathway in oxaliplatin-resistant cell lines in a dose-dependent manner. Luteolin also inhibited Nrf2 target gene [NQO1, heme oxygenase-1 (HO-1) and $GST{\alpha}1/2$] expression and decreased reduced glutathione in wild type mouse small intestinal cells. There was no apparent effect in Nrf2-/- mice. Luteolin combined with other chemotherapeutics had greater anti-cancer activity in resistant cell lines (combined index values below 1), indicating a synergistic effect. Therefore, adaptive activation of Nrf2 may contribute to the development of acquired drug-resistance and luteolin could restore sensitivity of oxaliplatin-resistant cell lines to chemotherapeutic drugs. Inhibition of the Nrf2 pathway may be the mechanism for this restored therapeutic response.

시스플라틴 내성세포주에서 열충격에 의한 세포사멸에 관한 연구 (Heat Shock Induces Necrosis in Cisplatin-resistant Gastric Cancer Cells through Suppressing JNK1/2 Activation and HSP27 Induction)

  • 항암제 내성을 획득한 암세포는 많은 경우 다양한 세포 독성 물질에 대해 교차 내성을 나타낸다. 그러나 온열 치료가 내성 획득 종양에 적용 될 때의 종양 세포의 사멸 효과는 알려져 있지 않다. 본 연구는 시스플라틴에 내성을 갖는 위암 세포 주, SNU601/Cis2이 열충격에 반응하는 민감도와 세포 사멸 방식을 조사함으로써 약물 내성 종양의 온열 치료 효과를 예측하고자 하였다. 정상 위암 세포 주 SNU601/WT은 열충격에 매우 민감하게 반응하며 apoptosis로 사멸하지만, 내성 위암 세포 주 SNU601/Cis2는 미열충격에 내성을 나타내었으며 고열충격에 노출되자 necrosis로 사멸하였다. 또한 SNU601/Cis2에서 necrosis의 발생은 열충격에 의한 JNK1/2의 활성화와 HSP27의 발현저하 현상과 관련되어 있었다. Necrosis의 유도는 세포막 파괴에 의해 세포 내부 물질의 방출로 인한 주변 조직의 염증반응을 수반하는데, 이러한 염증 반응은 암의 성장을 촉진하고 암의 성상을 심화시키는 것으로 보고되고 있다. 이러한 관점에서, 온열 치료가 약물 치료와 병행 될 경우에는 교차 내성과 necrosis로 인한 역효과를 방지하기 위하여, 그 적용이 주의 깊게 이루어져야 할 것으로 판단된다.

Amycolatopsis sp. KCTC 29142로부터 유래된 siderochelin A의 다제 내성 균주에 대한 항균활성 (Anti-multi drug resistant pathogen activity of siderochelin A, produced by a novel Amycolatopsis sp. KCTC 29142)

  • 본 연구에서는 신규 Amycolatopsis 균주 KCTC 29142를 분리하여 형태학적 관찰, 계통분석 및 화학분류학적 분석 등 다상 분류분석을 통해 분석하였다. KCTC 29142 균주의 에틸아세테이트추출물은 강한 항균활성을 나타났고, 활성물질은 철 이온 킬레이트 물질인 siderochelin A로 동정되었다. 본 연구에서 분리된 siderochelin A는 다제내성균인 Acinetobacter baumanii, methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Staphylococcus aureus (VRSA), 및 Escherichia coli (E. coli)에 대해 강한 활성을 보였고, 임상에서 분리된 다제내성균에 대한 MIC를 결정하였다.

대구지방에서의 장열성 Salmonella의 분포와 항균제내성 (Distribution and Drug Resistance of Salmonella Causing Enteric Fever in Taegu Area of Korea)

  • Salmonella strains isolated from blood in the Dong-san hospital, Taegu during the period from 1971 to 1986 were studied for species distribution, drug resistance, and R plasmids. The number of strains was 2,527 and all of them were classified into S. typhi and S. paratyphi A. Approximately 300 strains were isolated in the period from 1974 to 1976 and 1978, 268 in 1982, and 204 in 1983, but the numbers isolated in the 1980's have a tendency to decrease as compared with those of the 1970's. S. typhi occupied 85% or more of strains isolated until 1976, but the isolation frequency decreased yearly with some variation, and S. paratyphi A increased gradually from 1974. Only 4 strains of S. paratyphi A were resistant to some drugs, and the resistance was not transferred to E. coli by conjugation. S. typhi resistant to drugs were 15 in 1971 through 1973, 24 in 1974, and 13 in 1975, but afterwards only few resistant strains were isolated. These strains were resistant to two or more drugs; chloramphenicol(Cm), tetracycline(Tc), streptomycin(Sm), sulfisomidine(Su), ampicillin(Ap), and kanamycin(Km) and no strain resistant to other drugs tested was found. Strains resistant to 3 or less drugs didn't transfer the resistance to E. coli by conjugation. There were 15 strains resistant to four or more drugs, and were isolated in years from 1972 to 1976. These strains transferred the resistance to E. coli, and the resistance was considered to be mediated by R plasmids. Transfer frequency was higher at $25^{\circ}C$ than at $37^{\circ}C$ and patterns of transferred resistance were Cm, Tc, Sm, Su; Ap, Km; Cm. R plasmids having markers of Cm, Tc, Sm and Su were classified into Inc H1.

Antimicrobial Resistance Profile of Acinetobacter spp. Isolates from Retail Meat Samples under Campylobacter-Selective Conditions

  • Acinetobacter strains are widely present in the environment. Some antimicrobial-resistant strains of this genus have been implicated in infections acquired in hospitals. Genetic similarities have been reported between Acinetobacter strains in nosocomial infections and those isolated from foods. However, the antimicrobial resistance of Acinetobacter strains in foods, such as meat, remains unclear. This study initially aimed to isolate Campylobacter strains; instead, strains of the genus Acinetobacter were isolated from meat products, and their antimicrobial resistance was investigated. In total, 58 Acinetobacter strains were isolated from 381 meat samples. Of these, 32 strains (38.6%) were from beef, 22 (26.5%) from pork, and 4 (4.8%) from duck meat. Antimicrobial susceptibility tests revealed that 12 strains were resistant to more than one antimicrobial agent, whereas two strains were multidrug-resistant; both strains were resistant to colistin. Cephalosporin antimicrobials showed high minimal inhibitory concentration against Acinetobacter strains. Resfinder analysis showed that one colistin-resistant strain carried mcr-4.3; this plasmid type was not confirmed, even when analyzed with PlasmidFinder. Analysis of the contig harboring mcr-4.3 using BLAST confirmed that this contig was related to mcr-4.3 of Acinetobacter baumannii. The increase in antimicrobial resistance in food production environments increases the resistance rate of Acinetobacter strains present in meat, inhibits the isolation of Campylobacter strains, and acts as a medium for the transmission of antimicrobial resistance in the environment. Therefore, further investigations are warranted to prevent the spread of antimicrobial resistance in food products.
