Cancer Patients Are at High Risk of Mortality if Presenting with Sepsis at an Emergency Department

  • Background: Sepsis is an emergency condition with high mortality and morbidity rate. There are limited data on the association of cancer as a risk factor for mortality in sepsis patients in the emergency department (ED). Materials and Methods: This retrospective study was conducted at the ED, Faculty of Medicine, Ramathibodi Hospital, Bangkok, Thailand. The study period was between January 1st and December $31^{st}$, 2014. The inclusion criteria were as follows: adult patients over 15 years of age who presented at the ED with suspicion of sepsis, received treatment at the ED, and whose blood culture was found to be positive. Clinical data were recorded from medical records including the Mortality in Emergency Department Sepsis score (MEDS score). The primary outcome of this study was mortality at one month. Multivariate logistic regression analysis was used to identify independent factors associated with death. Results: During the study period, there were 775 eligible patients. The two most common pathogens identified from blood cultures were Staphylococcus aureus (193 patients; 24.9%) and Escherichia coli (158 patients; 20.4%). At one month after presenting at the ED, 110 patients (14.2%) had died. There were four significant factors for death, having cancer, being on an endotracheal tube, initial diagnosis of bacteremia, and high MED scores. Having cancer had an adjusted OR of 2.12 (95% CI of 1.29, 3.47). Conclusions: Cancer patients have double the risk of mortality if presenting with sepsis at the ED.

고속열차의 주행동특성 개선에 관한 연구 (A study on the Dynamic Behavior Enhancement of the Korean High-speed Train)

  • 본 연구는 고속열차 시운전 시험과 이에 따른 주행동특성 고찰 및 개선에 관한 것이다. 고속열차 시운전 시험을 통한 선행 연구에서 도출한 요댐퍼 설치 방법에 따른 후미 진동 개선 방안을 실제 고속철도 차량의 시운전시에 적용하여 후미 진동 저감 효과가 있음을 확인하였다. 또한 전체 차량에 대한 진동저감 방안을 동역학 해석 소프트웨어를 사용하여 수치 해석적 방법으로 도출하였으며, 그 효과를 시운전 시험을 통하여 확인하였다. 개선된 설계안은 실제 2층 고속열차 객차에 적용되어 주행동특성 문제없이 시운전을 진행하였다. 차세대 고속열차 임계속도에 영향을 미치는 현가장치 파라미터들에 대한 민감도 분석을 수행하였으며, 임계속도에 크게 영향을 미치는 4개의 설계변수를 도출하였는데, 이는 1차 탄성조인트 열차진행 방향 강성, 2차 요댐퍼 시리즈 강성, 2차 횡댐퍼 댐핑계수, 차간 댐퍼 댐핑계수 순이었다. 이 설계변수에 대한 최적화를 통하여 임계속도를 23.3% 향상시키는 현가장치 파라미터를 제시하였으며, 이는 차세대 고속열차 상용화 모델의 설계에 이용될 수 있다.

마티스의 이국취미와 작품에 나타난 색채 (The Exotic Tastes and Characteristics of Colors in Matisse's Works)

  • The purpose of this study is to extract the color chip from the works of Matisse with the exotic tastes, to analyze the characters of adjacent colors, and to suggest the color coordination of artistic character. The research method is based on the literature study which analyzes visual materials. The results are as follows. Considering the colors of Japanese taste, He employs vivid and strong tone in red, yellow, and blue, light tone in pink, purple, and green and deep tone in green, purple, blue and black. His color scheme is very bold and splendid. In his Algerian taste, red is very popular, but an attention should be paid in his development of orange and blue tones. In general, the usage of various tones brings colors into less relief and make the contrast of tones more conspicuous. His Spanish taste, based on red and orange, attracts attentions with strong impressions, but sometimes, dark or adjacent pale tones creates strong, mild and murky atmosphere. The Moroccan taste, based on the development of blue tones which have strong attraction implies the lightness but represents silent and calm. The Rumanian taste, represented in primary colors of red, yellow and blue and achromatic colors is bright and clear. In addition, its light tone in various colors represents peace and romance. The coordination of the exotic colors with the unique adjacent colors created by Matisse is not based on the established color coordination theory, but extracts colors in the aspect of culture borrowing. Therefore, it can double the effects of the color scheme expressed in the existing designs and be used as the global and multi-cultural design sources.

Lactobacillus rhamnosus GG Usage in the Prevention of Gastrointestinal and Respiratory Tract Infections in Children with Gastroesophageal Reflux Disease Treated with Proton Pump Inhibitors: A Randomized Double-Blinded Placebo-Controlled Trial

  • Purpose: Proton-pump inhibitors (PPIs) are frequently used to treat gastroesophageal reflux disease (GERD) in children, but recent evidence suggests a potential association between PPI treatment and some types of infections. The aim of this study was to assess the effectiveness of Lactobacillus rhamnosus GG (LGG) for the prevention of gastrointestinal and respiratory tract infections in children with GERD treated with PPI (omeprazol). Methods: Children younger than 5 years with GERD were assigned by a computer-generated list to receive LGG (109 colony-forming units) or placebo, twice daily, concomitantly with PPI treatment for 4-6 weeks; they were followed up for 12 weeks after therapy. The primary outcome measures were the percentage of children with a minimum of one episode of respiratory tract infection and the percentage of children with a minimum of one episode of gastrointestinal infection during the study. Results: Of 61 randomized children, 59 patients (LGG n=30; placebo n=29, mean age 11.3 months) were analyzed. There was no significant difference found between the LGG and placebo groups, either for the proportion of children with at least one respiratory tract infection (22/30 vs. 25/29, respectively; relative risk [RR] 0.85, 95% confidence interval [CI] 0.66-1.10) or for the proportion of children with at least one gastrointestinal infection (9/30 vs. 9/29, respectively; RR 0.97, 95% CI 0.45-2.09). Conclusion: LGG was not effective in the prevention of infectious complications in children with GERD receiving PPI. Caution is needed in interpreting these results, as the study was terminated early due to slow subject recruitment.

서브파이프의 유무에 따른 워터노즐의 성능특성연구 (Effect of the Presence of Sub-pipes on the Performance of Water Nozzle)

  • 본 논문은 화재의 발생 및 전파를 억제하기 위해 방수총의 내부 유체유동특성을 파악하는 것을 목적으로 한다. 본 연구를 위해 개발 중인 워터노즐을 모델링하였고, 필드실험에서 얻은 경계조건(펌프의 가압력 : 4 bar, 방수총 출구압력 : 대기압)을 적용하였으며, 유동에 대한 운동량 지배방정식을 이용하여 방수총 내부 유동에 대한 속도 및 압력분포를 얻었다. 서브파이프의 유무와 길이를 기준으로 방수총 출구의 노즐에서의 성능 특성에 영향을 미치는 2가지 주요 인자로 고려하였다. 해석 결과 기존 모델의 경우에는 서브파이프의 길이가 변하더라도 방수총의 출구영역에서 성능특성에 그다지 영향을 미치지 않았다. 이에 반해서, 서브파이프를 단관형으로 교체한 경우에는 서브파이프의 입구에서 역류가 발생하였고, 이를 제거하기 위해 서브파이프의 길이를 2배로 변화시켜 역류를 제거할 수 있었다.

  • 정밀한 위치정보의 제공은 GPS가 갖는 가장 큰 특징 중의 하나이다. 기준국과 사용자의 공통된 관측정보를 이용하는 상대측위기법이 일반적으로 정밀위치 정보 산출에 사용되고 있다. 그러나 기준국과 사용자의 기선거리가 멀어지면 GPS 신호가 통과하는 공간상의 매질특성이 서로 다르기 때문에 이중차분에 의해서도 전리층과 대류층 등의 오차요인은 완전히 제거가 되지 않는다. 본 연구는 이러한 특성을 고려하여 전리층과 대류층에 대한 오차보정정보를 추가적으로 생성하여 자료처리에 적용하였다. 그 결과 실험지역 17곳 중 14곳은 오차보정정보 적용이후에 위치정확도가 향상되었음을 알 수 있었다. 또한 기선거리에 따른 위치정확도 향상정도를 분석하였다.

  • 비좌굴가새는 우수한 내진성능을 보이는 시스템이다. 그러나 비좌굴가새는 약진이나 바람과 같은 하중에 대하여 에너지를 소산시키지 못한다. 기존의 비좌굴가새의 풍저항 성능을 개선한 하이브리드 비좌굴가새(H-BRB)는 비좌굴가새와 점탄성댐퍼로 구성된 복합댐퍼시스템의 일종이다. 본 논문에서는 탄성영역에서 H-BRB의 구조성능을 확인하기 위하여 심재가 다른 두 개의 실험체에 대한 실험이 수행되었다. H-BRB 시스템에 대한 탄성영역에서의 거동메케니즘 검증을 위하여 주저항요소와 2차 저항요소의 축변형량과 에너지소산 능력을 비교하였다. 실험결과 댐퍼부에서 이면전단을 사용한 H-BRB는 우수한 구조성능을 보이며, 고층건물의 사용성 수준을 향상시키기 위하여 적용될 수 있을 것이다.

  • Koreans have emphasized education as the primary means to reconstruct our country means to reconstruct our country after two catastrophic events-the Japanese occupation and the Korean War(1950-1953). A Strong belief- in education coupled with sheer deter-mination spurred the period of industrial and eeonomic development that started in the early 1970-'s. The "can-do" attitude of the general public was matched and aided by small S & T communities. Scientists and engineers provided the neeessary expertise to manufacture goods and process the many raw materials imported from overseas. After nearly three decades of reconst-ructing our completely devastated country, Korea has become a modern, cosmo-politan, fast-paced and dynamic country. In order to further increaser our manufac- turing capabilities and double the per-capita income by the beginning of the 21st century, we must improve productivitiy and encourage creatitivity in all sectors of our country. The S & T community's efforts in research and development. education, and interna-tional cooperation will be invaluable in determining our direction and reaching these goals. The general public is eager to peacefully unite our foreign powers since 1945. Toward that end. members of the S & T community are willing to coope-rate with our north Korean counterparts in all areas of S & T with the exception of defense related endeavors. Establi-shing scientific ties with north Korea will result in mutual economic benefit and greater regional stability in East Asia. In particular. scientists and manu-facturers in each province are eager to initiate cordial and professional relation-ships with north Korea in the hopes of securing these benefits. The Korean Federation of Science and Technology Societies(KOFST) has played an important role both in developing S & T policies, and in gostering the frowty of S & T societies. A non-governmental umbrella organization composed of over 251 professional societies, KOFST facili-tates the transfer of research outcomes and technology from research institutions to the various industries. KOFST also seeks to increase cooperation between the countries of the Pacific Rim. As evidence of that, we have pursued joint research and industrial ventures with China. Established projects include those on environmental conditions in the Yellow Sea, and improvement of aircraft manufacturing, telecommunica-tion equipment, and agricultural produ-cts. Increased cooperation between the Pacific Rim countries, particularly in S & T, would foster social and economic development for the region as a whole.

Comparison of intrathecal versus intra-articular dexmedetomidine as an adjuvant to bupivacaine on postoperative pain following knee arthroscopy: a randomized clinical trial

  • Background: Postoperative pain is a common, distressing symptom following arthroscopic knee surgery. The aim of this study was to compare the potential analgesic effect of dexmedetomidine after intrathecal versus intra-articular administration following arthroscopic knee surgery. Methods: Ninety patients undergoing unilateral elective arthroscopic knee surgery were randomly assigned into three groups in a double-blind placebo controlled study. The intrathecal dexmedetomidine group (IT) received an intrathecal block with intrathecal dexmedetomidine, the intra-articular group (IA) received an intrathecal block and intra-articular dexmedetomidine, and the control group received an intrathecal block and intra-articular saline. The primary outcome of our study was postoperative pain as assessed by the visual analogue scale of pain (VAS). Secondary outcomes included the effect of dexmedetomidine on total postoperative analgesic use and time to the first analgesic request, hemodynamics, sedation, postoperative nausea and vomiting, patient satisfaction, and postoperative C-reactive protein (CRP) levels. Results: Dexmedetomidine administration decreased pain scores for 4 h in both the intrathecal and intra-articular groups, compared to only 2 h in the control patient group. Furthermore, there was a significant reduction in pain scores for 6 h in the intra-articular group. The time to the first postoperative analgesia request was longer in the intra-articular group compared to the intrathecal and control groups. The total meperidine requirement was significantly lower in the intra-articular and intrathecal groups than in the control group. Conclusions: Both intrathecal and intra-articular dexmedetomidine enhanced postoperative analgesia after arthroscopic knee surgery. Less total meperidine was required with intra-articular administration to extend postoperative analgesia to 6 h with hemodynamic stability.
