유방암 수술기법에 따른 방사선치료계획 기법의 선택 (Selection of radiation treatment plan technique at breast cancer operating technique)

  • Techniques, using physical wedge filter and using dynamic wedge filter and FIF(Field in Field) and ISCT(Irregular Surface Compensating Technique), have been developed according to progress of radiation therapy of breast cancer. Measurement of dose was done to judge the usefulness of technique using three cases, non tissue loss after breast conserving operating and tissue loss after breast conserving operating and mastectomy. Dose indexes of breast tissue, CI (Conformity Index), HI (Homogeneity Index) and QOC (Quality of Coverage), dose index of skin, or dose indexes of lung, volume of 50 percent dose and 20 percent dose were estimated and compared. Using dynamic wedge filter is useful plan at non tissue loss allowing for high dose of lung. FIF and ISCT are useful plan at tissue loss. ISCT is useful plan at mastectomy. Henceforth, we need to apply to valid plan and body type and thorax size.

개인 소음폭로량과 청력손실에 관한 추적조사 (A Follow-up Study on the Personal Noise Exposed Dose and Hearing Loss)

  • For the purpose of presenting the basic data for the establishment of control measures on the long-term noise exposed workers, this study was carried out on the relationship between personal noise exposed dose and hearing loss on the 67 male workers whose hearing threshold had exceeded 40 dB in 4,000 Hz, from 1990 to 1992. Conclusively, the level of hearing loss was significantly related to personal noise exposed dose in follow-up period. We considered that personal noise exposed dose which was measured by the personal noise dosemeter was more efficient rather than the noise level of workplace for the evaluating the long-term change of hearing acuity. And although in the case of not-diagnosed as noise induced hearing loss. it was suspected that the active control programs such as improvement of noisy environment or early transfer to proper workplace were needed on the workers who exposed with over 90 dB in personal noise exposed dose.

광섬유 브래그 격자 센서를 이용한 국내외 상용 단일모드 광섬유의 감마선 영향 연구 (A Study of Gamma-ray Irradiation Effects on Commercially Available Single-mode Optical Fiber using Fiber Bragg Grating Sensor Systems)

  • 본 논문에서는 Ge가 함유된 일반 단일모드 광섬유의 감마선 유입손실을 측정하였다. 방사선 선원은 $Co^{60}$ 감마선 선원을 이용하였으며, 총 4시간동안 0.5 kGy/hr, 2 kGy/hr, 8 kGy/hr의 선량률로 조사하였다. 그 결과 감마선 조사에 의한 단일모드 광섬유의 전송손실이 뚜렷하게 나타났으며, 동일 누적선량에서 선량률이 클수록 전송손실이 증가하는 선량률 효과가 나타났다. 또한 감마선 조사 종료 후 광섬유의 전송손실이 클수록 유입손실의 회복률이 증가하는 어닐링 특성을 보였다. 본 실험결과는 향후 광섬유의 내방사화 연구의 기초자료로 활용할 것이다.

  • The effect of gamma-ray on yeast cells Sacch. cerevisiae, and the leakage of cellular constituents such as carbohydrates, ribose, amino acids, inorganic phosphates and organic phosphates have been studied. The samples of yeast cells washed throughly and starved intensively, radiation effects were compared with those of control (un-starved), the irradiation dose rates are in the range from 24 Kr. up. to 480, Kr. The loss of 260m$\mu$. absorbing material, are also observed. Mechanisms of membrane damage by gamma-irradiation are discussed corelating to permeability changes and loss of substances, then active and passive transport process are also under considerations in discussion. The experimental results are as follows, 1. Carbohydrates of yeast cell leak out by gamma-irradiation, and amounts of loss increase proportionally as the increasing of radiation dose, curve of carbohydrates loss in starved cells is parallel with those of non-starved cells. 2. Ribose leak out less than that of carbohydrate from irradiated cell, the dose response curve of loss is straight and proportional to the increasing of radiation doses, slope of the curve is much lower than of carbohydrates. 3. Amino acids also leak out and the curve of losses to radiation is not proportional, it is revealed that there are little losses from yeast at lower doses of irradiation. 4. The losses of inorganic phosphates increase unproportionally to the increasing of irradiation doses, there are little leakage at the lower doses of irradiation. The losses of organic phosphates increase proportionally to the increasing of irradiation doses, and the amount of losses are much more than that of inorganic phosphate at lower doses of irradiation. 5. Leakage from irradiated yeast cells was shown to be due to passive transport process not an energy requiring process of ion transport. 6. Loss of 260 m$\mu$. absorbing material is little more than that of control yeast by the gamma-irradiation dose of 120K.r. and 240K.r.

방사선조사가 백서 이하선의 선세포에 미치는 영향에 관한 전자현미경적 연구 (AN ELECTRON MICROSCOPIC STUDY ON THE EFFECTS OF IRRADIATION ON THE ACINAR CELLS OF RAT PAROTID GLAND)

  • The author studied the histopathologic changes according to a single or a split dose and the time after irradiation on the acinar cells of rat parotid gland. 99 Sprague Dawley rats, weighing about l20gm, were divided into control and 3 experimental groups. In experimental groups, GroupⅠ and Ⅱ were delivered a single dose of l5Gy, 18Gy and Group Ⅲ and Ⅳ were delivered two equal split doses of 9Gy, 10.5Gy for a 4 hours interval, respectively. The experimental groups were delivered by a cobalt-60 teletherapy unit with a dose rate of 222cGy/min, source-skin distance of 50㎝, depth of l㎝ and a field size of l2×5㎝. The animals were sacrificed at 1, 2, 3, 6, 12 hours, 1, 3, 7 days after irradiation and examined by light and electron microscopy. The results were as follows: 1. As the radiation dose increased and the acinar cells delivered a single dose exposure were more damaged, and the change of acinar cells appeared faster than those of a split dose exposure. 2. The histopathologic change of acinar cells appeared at 1 hour after irradiation. The recovery from damaged acinar cells appeared at 1 day after irradiation and there was a tendency that the recovery from damage of a split dose exposure was somewhat later than that of a single dose exposure. 3. Light microscope showed atrophic change of acinar cells and nucleus, degeneration and vesicle formation of cytoplasm, widening of intercellular space and interlobular space. 4. Electron microscope showed loss of nuclear membrane, degeneration of nucleus and nucleoli, clumping of cytoplasm, widening and degeneration of rough endoplasmic reticulum, loss of cristae of mitochondria, lysosome, autophagosome and lipid droplet. 5. Electron microscopically, the change of rough endoplasmic reticulum was the most prominent and this appeared at 1 hour after irradiation as early changes of acinar cells. The nuclear change appeared at 2 hours after irradiation and the loss of cristae of mitochondria was observed at 2 hours after irradiation in all experimental groups.

  • Continued scaling of MOS devices requires the formation of the ultra shallow and very heavily doped junction. The simulation and experiment results show that the degradation of pMOS performance in logic and SRAM pMOS devices due to the excessive diffusion of the tail and a large amount of dose loss in the extension region. This problem comes from the high-temperature long-time deposition process for forming the spacer and the presence of fluorine which diffuses quickly to the $Si/SiO_{2}$ interface with boron pairing. We have studied the method to improve the pMOS performance that includes the low-energy boron implantation, spike annealing and device structure design using TCAD simulation.

비인두암 토모테라피 시 체중 감소에 따른 피부선량 변화 분석 (Analysis of Changes in Skin Dose During Weight Loss when Tomotherapyof Nasopharynx Cancer)

  • 항암요법과 방사선치료를 병행하여 두경부암을 치료하는 환자의 경우 항암요법과 방사선치료를 거듭할수록 구토, 메스꺼움, 식욕부진 등의 이유로 환자의 체중 감소가 생기게 된다. 체중 감소는 목의 두께 변화로 나타날 수 있으며 이로 인해 치료하고자 하는 t 타겟과 주위 정상조직의 선량 전달에도 영향을 미치며 방사선이 전달되는 방향에 위치한 피부선량의 변화가 나타날 것으로 사료된다. 특히 비인두암 환자의 경우 비인두 구조가 다른 장기보다 복잡한 구조를 지니고 있고 치료방법인 토모테라피는 수 mm 차이로 급격한 선량 변화가 발생하므로 이러한 체중 감소의 변화에 주의 깊은 관찰이 필요하다. 현재 토모테라피의 경우 매 회 치료 전 정확한 자세 재현을 위한 영상 검증을 통해 이러한 움직임을 확인하고 그 값을 보정하여 치료를 수행한다. 그러나 환자의 체중 감소로 인하여 피부선량에 전달되는 선량 변화의 검증은 되지 않고 있다. 환자의 피부선량이 증가함으로써 나타나는 부작용은 환자의 삶의 질에 영향을 미칠 뿐 만 아니라 2차 암 발생률을 높일 수 있다. 이에 환자의 체중 감소로 인하여 나타나는 환자의 목 두께를 휴먼 팬톰에 볼루스를 올리고 두께를 조절해가며 변화시켜 필름을 이용하여 피부선량의 변화가 어떻게 나타나는지 분석해보고자 한다.

모 자동차 공장의 소음폭로와 혈압에 관한 연구 (A Study on Noise Exposure Dose and Blood Pressure in an Automobile)

  • This study was carried out to investigate age, noise intensity, work period, hearing loss at frequencies, hypertension and to examine correlation of the total quantity of noise exposure(Dose) and blood pressure (Response) in the auto industry during May 1987-December 1987. To perform this study 65 workers were tested. Results of this study were as follows: 1. In frequency analysis, the CS-dip phenomenon occurred around 4000 Hz. 2. Between under of ten years group and over of ten years group, average hearing loss value was statitically significant (P < 0.05). 3. The prevalence of hypertension of 65 workers was 7.84% . 4. At 90dB(A) over and 10 years under and workers in the 20's group, the total quantity of noise exposure (Dose) and systolic blood pressure (Response) were statistically significant (P < 0.05). 5. At under 10 years of work period group, the total quantity of noise exposure(Dose) and diastolic blood pressure (Response) were statistically significant(r =0.234, P < 0.1).

  • To evaluate the radiation degradation of ethylene propylene rubber (EPR), radiation effects on EPR were investigated by using dielectric analysis and thermal-gravimetric analysis. Permittivity, loss factor, tan$\delta$, and thermal decomposition temperature were observed for ${\gamma}$-ray irradiated EPR. As the radiation dose was increased, the peak temperature of the loss factor and tans of EPR were increased and loss factor and tan$\delta$ at peak temperature were decreased. Activation energies were calculated using loss factor and thermal decomposition for ${\gamma}$-ray irradiated EPR as well. The trends of both calculated activation energies showed the same tendencies as radiation dose was increased.

전뇌 방사선치료 시 치료방법에 따른 두피선량평가 (Scalp Dose Evaluation According Radiation Therapy Technique of Whole Brain Radiation Therapy)

  • 목 적: 뇌전이 환자에게 시행하는 고전적인 헬멧(Helmet field)형태의 대향2문방사선조사는 환자두피에 과 선량을 일으키며 이는 탈모의 원인이 된다. 이에 본 연구에서는 환자두피를 보호하기 위한 두피보호 형태(Scalp-shielding shape)의 대향 2문 조사와 토모 치료법을 고전적 방사선치료법과 비교하여 보다 효과적인 탈모 예방의 두피선량을 정량적으로 분석하고자 한다. 대상 및 방법: 두피선량은 RANDO 팬톰을 사용하여 열형광선량계를 전두엽의 중심선에 따라서 5개를 위치시킨 후, 세 가지 치료방법(HELMET, MLC, TOMO)으로 피부선량을 측정하였고, 전산화치료계획장치(Pinnacle3, Philips Medical System, USA), 6MV X선(Clinac 6EX, VARIAN, USA)을 이용하여 방사선치료계획을 수립한 후, DVH로 선량분포변화와 두피선량을 비교 분석하였다. 결 과: 열형광선량계를 사용하여 두피의 표면선량을 측정한 결과 기존의 HELMET field 치료방법과 비교했을 때 MLC technique에서는 각 포인트 지점에서 평균 87.44% 두피선량이 감소하는 것으로 측정되었으며 TOMO에서는 평균 88.03% 두피 선량이 감소하는 것으로 측정되었다. 또한 세 가지 치료방법의 두피내의 과다선량 영역(Hotspot)의 존재정도를 평가하기 위해 선량체적히스토그램(Dose volume histogram, DVH)을 사용하여 처방선량의 95%, 100%, 105%가 받는 용적의 백분율(Percentage of volume: V95, V100, V105)을 계산한 결과 MLC technique과 TOMO plan에서 과다선량 없이 Dose coverage가 우수함을 보여주었다. 결 론: 전뇌 방사선치료를 받는 환자에게 탈모현상을 줄여주는 것은 환자의 삶의 질을 높여주는 측면에서 중요하다. 이에 본 실험결과를 바탕으로 두피보호 형태(Scalp-shielding shape)의 대향 2문 조사와 토모 치료법을 사용하여 환자의 두피를 보호함으로써 환자의 탈모현상을 줄여주는 효과를 기대할 수 있을 것이라 사료된다.
