  • Effects of phenobarbital pretreatment on the pharmacokinetics of enterohepatic recirculating griseofulvin were investigated by comparing normal to bile duct cannulated rats and also the effects of enhanced endogeneous bile flow on the absorption of griseofulvin were studied by means of in situ recirculation method in phenobarbital-pretreated rats. Phenobarbital was administered orally for five days at the dose of 75 mg/kg/day. The influence of phenobarbital pretreatment on the absorption rate constant, area under the plasma concentration-time curve, maximum plasma concentration of orally administered griseofulvin was not found in bile duct cannulated rats. Decreased absorption clearance and apparent partition coefficient of griseofulvin in accordance with the amount of endogeneous bile juice seemed to be due to the decrease of thermodynamic activity of griseofulvin as bile forms the micelle with griseofulvin.

  • Three newly synthesized alkanol esters of d-2-(6-methoxy-2-naphthyl) propionic acid, NAPROXEN were examined for physicochemical properties and biopharmaceutical characteristics. These esters were very stable in solid state, but more than 90% of these esters were hydrolysed to the parent, naproxen in rabbit's liver hornogenates. They showed higher dissolution rate in the artificial gastric and intestinal juice, and significantly greater partition coefficient in n-octanol, when compared with naproxen. The absorption rate constants of these esters were increased, while the elimination rate constants were decreased, comparing with naproxen. The ulcerogenic doses on gastric and intestinal mucosa were increased remarkably, and the antiinflammatory dose against carrageenininduced edema on rat hind paw was decreased markedly in these esters, and thus the safety indexes of these esters were higher than that of naproxen.

Measurements of Developed Patterns by Direct writing of Electron Beam on Different Materials underneath PMMA

  • The developed patterns by direct writing of electron beam are measured by AFM, FESEM and optical profiler of WYKO NT3300. From different measurement methods, the measured linewidths of the patterns are shown a little bit wider than designed pattern size due to electrons scattering effect during direct writing of electron beam. The optimized conditions of these experiments are suggested and explained for the forming of structures below 0.1 ㎛ dimension size. Because of electron scattering effects from the different under layers such as Si, Si$_3$N$_4$ and aluminum, the developed pattern size is also influenced by the accelerated energy of electrons, dose, resist and soft and hard bake conditions in PMMA. The distributions of electron beam and calculations of backscattering coefficient are demonstrated by Monte Carlo simulation. From the measured results, the developed linewidth of PMMA/Al /silicon is shown a little bit wider than that of PMMA/Si$_3$N$_4$/silicon structure due to the backscattering effects.

  • Pivaloyloxymethyl ester of cefoperazone was synthesized by treating sodium cefoperazone with chloromethyl pivalate and its chemical structure was determined by spectroscopic trials. The pharmaceutical properties of the ester were investigated to assess its potential as a prodrug of cefo perazone. Cefoperazone pivaloyloxymethyl ester was microbiologically inactive itself in vitro, but hydrolyzed into the parent drug in vivo. After a single oral dose of each drug to rabbits, serum concentrations of cefoperazone were determined by high performance liquid chromatographic assay. The ester showed higher and more sustained blood level than cefoperazone. Therefore, the total area under the serum concentration-time curve of the derivative was 16.8 times larger than that of the parent drug.

  • Energy spectrum modulation of X-ray source in digital mammography has been studied. In this study, we calculated various filtered spectra using the scattering data. Primary spectra were generated by Molybdenum (Mo) and Tungsten (W) targets. The materials of added filters are Molybdenum and Rhodium (Rh) for 40 kVp Mo. primary spectrum, the amounts of photons over whole energy ranges are attenuated to 0.43 with 0.03 mm Mo filter and 0.38 with 0.06 mm Mo filter while the photons of energy ranged from 17 keV to 20 keV. The photons of low energy ranged below 17 keV are considerably attenuated. This effect brings out reducing the scattered radiation and dose to the patient, and enhancing subject contrast in the image. The results show that filtered spectra are not seriously affected by X-ray tube loadability. Because the energy range from 17 keV to 20 keV is directly transmitted although low and high energies are mainly filtered.

Analysis of Patient Effective Dose in PET/CT; Using CT Dosimetry Programs (CT 선량 측정 프로그램을 이용한 PET/CT 검사 환자의 예측 유효 선량의 분석)

  • Purpose: As PET/CT come into wide use, it caused increasing of expose in clinical use. Therefore, Korea Food and Drug Administration issued Patient DRL (Diagnostic Reference Level) in CT scan. In this study, to build the basis of patient dose reduction, we analyzed effective dose in transmission scan with CT scan. Materials and Methods: From February, 2010 to March 180 patients (age: $55{\pm}16$, weight: $61.0{\pm}10.4$ kg) who examined $^{18}F$-FDG PET/CT in Asan Medical Center. Biograph Truepoint 40 (SIEMENS, GERMANY), Biograph Sensation 16 (SIEMENS, GERMANY) and Discovery STe8 (GE healthcare, USA) were used in this study. Per each male and female average of 30 patients doses were analyzed by one. Automatic exposure control system for controlling the dose can affect the largest by a patient's body weight less than 50 kg, 50-60 kg less, 60 kg more than the average of the three groups were divided doses. We compared that measured value of CT-expo v1.7 and ImPACT v1.0. The relationship between body weight and the effective dose were analyzed. Results: When using CT-Expo V1.7, effective dose with BIO40, BIO16 and DSTe8 respectably were $6.46{\pm}1.18$ mSv, $9.36{\pm}1.96 $mSv and $9.36{\pm}1.96$ mSv for 30 male patients respectably $6.29{\pm}0.97$ mSv, $10.02{\pm}2.42$ mSv and $9.05{\pm}2.27$ mSv for 30 female patients respectably. When using ImPACT v1.0, effective dose with BIO40, BIO16 and DSTe8 respectably were $6.54{\pm}1.21$ mSv, $8.36{\pm}1.69$ mSv and $9.74{\pm}2.55$Sv for 30 male patients respectably $5.87{\pm}1.09$ mSv, $8.43{\pm}1.89$ mSv and $9.19{\pm}2.29$ mSv for female patients respectably. When divided three groups which were under 50 kg, 50~60 kg and over 60 kg respectably were 6.27 mSv, 7.67 mSv and 9.33 mSv respectably using CT-Expo V1.7, 5.62 mSv, 7.22 mSv and 8.91 mSv respectably using ImPACT v1.0. Weight and the effective dose coefficient analysis showed a very strong positive correlation(r=743, r=0.693). Conclusion: Using such a dose evaluation programs, easier to predict and evaluate the effective dose possible without performing phantom study and such dose evaluation programs could be used to collect basic data for CT dose management.

A Study on the Image Quality of Mammography and the Average Glandular Dose (맘모그래피의 화질과 평균유선조직선량에 관한 검토)

  • We came to the following conclusion as the results of experiment on the image quality of mammography and the average glandular dose using 4 apparatuses at 3 hospitals in Seoul. 1. Whereas the measurement of half value layer showed no differences among the apparatuses, the measurement by an attenuation curve method showed some differences by 5.9%. There were 9.1% differences in the measurement by aluminum conversion method. 2. The basic density of an automatic exposure control unit must be D = 1.40, but there was no automatic exposure unit adjusted precisely at any hospitals. The unit at the B hospital exceeded the allowable limit by ${\pm}0.15$. 3. In the photographing using an automatic exposure control unit and the management of an automatic film processor using a sensitometer, most automatic film processors were well kept. But in some cases the mean value of a fluctuation coefficient exceeded the allowable limit. There is a need for more cautious management. 4. The image quality of breast phantom photography was affected by the screen/film system among the hospitals. 5. The average glandular dose at a breast of 4.2 cm thickness depended on the tube voltage, In the case of Mo/Mo, it was measured $0.26{\sim}1.39\;mGy$ less than ACR standard 3.0 mGy.

Evaluation of the Low Tube Voltage in the Computed Tomography Scan Technique using a Low Concentration Contrast Agent (저농도 조영제를 사용한 CT검사에서 저관전압 기법에 따른 유용성 평가)

  • The purpose of this study is computed tomography contrast agent at low concentrations and low tube voltage technique to evaluate the usefulness on the phantom image. By varying the degree of mixture by the contrast medium concentration it was inserted in phantom. It was taken by changing the tube voltage and tube current step by step, and to evaluate the dose and the CT value obtained from the phantom image. As a result, low-contrast, low tube voltage(300 mgI/ml, 100 kV) was reduced by an average 21%(CTDIvol; computed tomography dose indexvol) more standard condition(350 mgI/ml, 120 kV). SNR was increased at all depths of the phantom, respectively 1:10 and 1:20(by diluting a contrast agent and normal saline) 12.2(26%) 6.2(17%). CNR was increased at all depths of the phantom, respectively 1:10 and 1:20(by diluting a contrast agent and normal saline) 11.5(32%), 6.3(26%). Research work on the CT scan is necessary in a variety of studies on the low contrast concentration and low tube voltage techniques for dose reduction and reducing of side effects the contrast agent.

Evaluation of Variation Method to Improve the Sensitivity of Immunoradiometric Assay (면역방사계수측정법의 민감도 향상을 위한 변법의 평가)

  • Purpose The concentration of PSA (Prostate Specific Antigen) after radical prostatectomy in prostate cancer patients is a predictor of biochemical recurrence, and the AUA (American Urological Association) is defined as biochemical recurrence when the concentration of PSA is measured at 0.2 ng/mL or more, and when the concentration is measured at 0.2 ng/mL or more at the retest. This standard is also applied our hospital. In this laboratory, the PSA reagent using IRMA (Immunoradiometric Assay) is used, and the sensitivity at a very low value was not as good as the reagent used in the department of laboratory medicine. This study aims to increase the reliability of the results by improving the precision and sensitivity of very low values. Materials and Methods As a reagent for the study, PSA reagent using IRMA was used. As a method to improve the precision and sensitivity of very low values, a variation method on the serum volume(25 uL, 50 uL, 100 uL, 200 uL) was studied, and variation usefulness evaluation was conducted. The evaluation items were compared the results of precision, analytical sensitivity, recovery rate, dilution test, high-dose hook effect test, parallel test and very low concentration values(n = 20). Results The validation results were displayed in the order of 25 uL, 50 uL, 100 uL, 200 uL. As the serum volume increased, it was confirmed that CV (Coefficient of Variation)(%) improved. Analytical sensitivity(ng/mL) was 0.038, 0.041, 0.017, 0.015 and recovery rate(%) was 101±3, 101±3, 99±2, 97±4. very low concentration values(ng/mL) between each volume(n=20) were 0.135±0.068, 0.076±0.050, 0.048±0.034, 0.046±0.034. and high dose hook effect appeared as the serum volume increased. Conclusion Through the variation usefulness evaluation, it was confirmed that as the serum volume increased, the precision and sensitivity improved at very low concentration values. However, it is necessary to pay special attention to the occurrence of high-dose hook effect as the serum volume increases. In the case of tests that requires very low concentration values, it is thought that the reliability of the result will be increased if the variation method is properly used after the variation usefulness evaluation.

Analysis of Tissue Equivalent Characteristics of Agar Phantom for Hyperthermia Therapy (온열종양치료 한천 팬텀의 조직등가 특성 분석)

  • A tissue-equivalent phantom is necessary for quality control of hyperthermia therapy. However, since there is no phantom for this purpose, phantoms made from agar are being used in various studies. The tissue-equivalent properties of the agar phantom were confirmed by comparison with the tissue-equivalent material bolus in this study. CT images of the agar phantom and bolus were acquired, and tissue equivalent characteristics were analyzed with image analysis and dose calculation using a computerized radiation therapy planning system. The average pixel value was 96.960±10.999 in bolus, 108.559±8.233 in 3% agar phantom, and 111.844±8.651 in 4% agar phantom. Using the SSD technique, 100 cGy was prescribed at a depth of 1.5 cm and 6 MV X -ray was set to irradiated to 10x10 cm2, and the absorbed dose according to depth was calculated from the central axis of the beam. The intraclass correlation coefficient of dose distribution of bolus, 3% agar phantom, and 4% agar phantom was 0.979 (p<.001, 95%CI .957-.991). The density (g/cm3) at the point where the absorbed dose was calculated was 0.990±0.020 at the bolus, 1.018±0.020 at the 3% agar phantom, and 1.035±0.024 at the 4% agar phantom. In this study, the internal density distribution and uniformity of the agar phantom were confirmed to be appropriate as a tissue equivalent material by analysis of CT images and a computerized radiation therapy planning system.
