Adequate anesthetic induction dose in a morbidly obese patient based on bioelectrical impedance analysis. -Case report- (병적 비만 환자에서 생체 전기 임피던스 분석을 이용한 적절한 마취 유도 용량 -증례보고-)

  • Background: The dosage of the anesthetic drugs is generally determined by the total body weight of the patients. However, the drugs can be overdosed when the patient is morbidly obese. We have determined anesthetic induction dose based on lean body mass estimated from bioelectrical impedance analysis (BIA). Case: We report a case of morbidly obese patient (161 cm, 138 kg and body mass index 53.1) who had an elective laparoscopic cholecystectomy. The dose of induction agent was determined by lean body mass estimated by BIA, and the sedation was assessed by the observer's assessment alertness/sedation scale. Conclusions: Dose determination through lean body mass measured by BIA is useful in highly obese patients.

  • Opioids profoundly inhibit evoked discharges of spinal nociceptive neurons, thereby inhibiting the transmission of pain. Intrathecal administration of opioids using implantable continuous infusion systems is an effective method of pain relief when other treatments have failed, as well as for patients with adequate analgesia on high dose therapy that produces unacceptable side effects. We report two cases of intrathecal pump implantation performed in patients suffering from intractable chronic pain. A test dose of 3 mg morphine was injected into the epidural space. No side effects were noted and patients experienced considerable pain relief. Implantation was performed one day after the test. The initial intrathecal morphine delivery dose was half of the equivalent dose of daily oral intake opioids and the infusion rate was increased gradually under close observation for opioid side effects. Two days post-implantation, both patients were discharged without any complications.

Evaluation of Anticancer Activity of Curcumin Analogues Bearing a Heterocyclic Nucleus

  • We report herein an in vitro anticancer evaluation of a series of seven curcumin analogues (3a-g). The National Cancer Institute (NCI US) Protocol was followed and all the compounds were evaluated for their anticancer activity on nine different panels (leukemia, non small cell lung cancer, colon cancer, CNS cancer, melanoma, ovarian cancer, renal cancer, prostate cancer and breast cancer) represented by 60 NCI human cancer cell lines. All the compounds showed significant anticancer activity in one dose assay (drug concentration $10{\mu}M$) and hence were evaluated further in five dose assays (0.01, 0.1, 1, 10 and $100{\mu}M$) and three dose related parameters $GI_{50}$, TGI and $LC_{50}$ were calculated for each (3a-g) in micro molar drug concentrations (${\mu}M$). The compound 3d (NSC 757927) showed maximum mean percent growth inhibition (PGI) of 112.2%, while compound 3g (NSC 763374) showed less mean PGI of 40.1% in the one dose assay. The maximum anticancer activity was observed with the SR (leukemia) cell line with a $GI_{50}$ of $0.03{\mu}M$. The calculated average sensitivity of all cell lines of a particular subpanel toward the test agent showed that all the curcumin analogues showed maximum activity on leukemia cell lines with $GI_{50}$ values between 0.23 and $2.67{\mu}M$.

Estimation of Electron Dose Rate using CCD Camera (CCD 카메라를 이용한 전자빔 조사량의 예측)

  • We report a useful method to estimate the electron dose rate which may be a decisive factor to characterize sample properties. Even though most mircoscopes have their own exposure meters, there are several practical concerns when such exposure meters are used to measure the electron dose rate: 1) Specimen should be avoided within the entire area of exposure meter; 2) beam current has to be always recorded whenever the operation mode is changed; 3) the electron dose rate can not be calculated for the beam current beyond the detectable range. To overcome these limitations, we suggest a useful method which utilize a CCD (charge coupled device) camera which is now a popular detector to obtain the final electron micrographs. We have evaluated the CCD sensitivity using the linear relationship between electron current on the exposure meter and counter ratio on the CCD camera which are built in KBSI-HVEM (high voltage electron microscope). Applying the new method, we obtained the CCD sensitivity which are approximately 0.039 counts/$e^-$ and 1.37 counts/$e^-$ for the Top-TV and the HV-GIF CCD cameras, respectively.

  • This study investigated the size specific dose estimates of difference localizer on pediatric CT image. Seventy one cases of pediatric abdomen-pelvic CT (M:F=36:35) were included in this study. Anterior-posterior and lateral diameters were measured in axial CT images. Conversion factors from American Association of Physicists in Medicine (AAPM) report 204 were obtained for effective diameter to determine size specific dose estimate (SSDE) from the CT dose index volume (CTDIvol) recorded from the dose reports. For the localizer of mid-slice SSDE was 107.63% higher than CTDIvol and that of xiphoid-process slices SSDE was higher than 92.91%. The maximum error of iliac crest slices, xiphoid process slices and femur head slices between mid-slices were 7.48%, 17.81% and 14.04%. In conclusion, despite the SSDE of difference localizer has large number of errors, SSDE should be regarded as the primary evaluation tool of the patient radiation in pediatric CT for evaluation.

  • Hemorrhagic complications associated with aspirin use occur primarily at skin or gastrointestinal sites but can occasionally occur in the central nervous system. In particular, spontaneous spinal epidural hemorrhage (SSEH) associated with aspirin is very rare. We report a case of low-dose (100 mg daily) aspirin-related SSEH that was successfully treated with medical management. Our case indicates that low-dose aspirin could induce SSEH and that conservative treatment with close observation and repeated imaging studies should be considered in cases with neurological improvement or mild deficits.

  • The purpose of this study was to report dose-independent pharmacokinetics of KR-31543, a new neuroprotective agent for ischemia-reperfusion damage, after intravenous and oral administration and first-pass effects after intravenous. intraportal, intragastric, and intraduodenal administration in rats. After intravenous (10, 20 and 50 mg/kg) and oral (10, 20 and 50 mg/kg) administration, the pharmacokinetic parameters of KR-31543 were dose-independent. (omitted)

Case Report of Potassium Bromide Therapy for Canine Epilepsy Patient (Potassium bromide를 적용한 개의 epilepsy 치료에 관한 증례)

  • Six year-old female shihtzu was presented as primary epileptic seizure. We had been treated with high dose phenobarbital sodium (6.5 mg/kg), but could not achieved adequate control seizures. In an add-on therapy, the dog was given potassium bromide at a dose of 20 to 40 mg/kg daily for a period of 24 months. This dog has showed a greater than 50% reduction in seizure frequency. The therapeutic range of serum concentration of potassium bromide was from 0.7 to 1.3 mg/ml at 6weeks after treatment. To begin an add-on therapy with potassium bromide it can be recommended at a daily dose of 20 to 40 mg/kg.

  • Praziquantel is the drug of choice for clonorchiasis. Since clonorchiasis is endemic in most river basins, praziquantel has been widely used for 30 years in Korea. A 54-year-old Korean woman suffered from hypersensitive reactions, such as nausea, dyspnea, rash, and urticaria after taking the first dose of praziquantel to treat clonorchiasis. She ingested one dose again and the same symptoms appeared, and she was treated at a clinic with anti-histamines. She tried one more dose with anti-histamines but found the same symptoms. Later, she was found to pass eggs of Clonorchis sinensis and medicated with flubendazole. The hypersensitive reaction to praziquantel is rare but occurs. This is the 5th case report in the world.

Region-wise evaluation of gamma-ray exposure dose in decontamination operation after a nuclear accident

  • The gamma-ray exposure doses in decontamination operation after a nuclear accident were evaluated with a consideration of various geometrical conditions and specific gamma-ray energies. The calculation domain is organized with three residence types and each form is divided into two kinds of geometrical arrangements. The position-wise air KERMA values were calculated with an assumption of evenly distributed gamma-ray source based on Monte Carlo radiation transport analysis using the MCNP code. The radioactivity is initially set to be unity to be multiplied by the deposition value measured in the actual accident condition. The workforce data set depending on the target object was determined by modifying the Fukushima report. The external exposure doses for decontamination workers were derived from the calculated KERMA values and the workforce analysis. These results can be used to efficiently determine the workforce required by the characteristics of the area and the structure to be decontaminated within the dose limits.
