Dosimetry and MIRD for Re-188 Liquid Balloons (Re-188-DTPA 풍선 주위 선량분포와 의용내부피폭선량)

  • Re-188 is suitable for endovascular liquid-balloon brachytherapy for the prevention of restenosis after angioplasty. Re-188 was concentrated to 3700 MBq/ml and labeled with DTPA. According to dosimetric calculation, it took 420 seconds using Re-188 solution with concentration of 3700 MBq/ml to irradiate 17.6 Gy to the target at 1 mm from the balloon surface. Software was made to estimate the irradiation time. MIRD calculation with dynamic bladder model yielded the whole body dose of Re-188-DTPA as 0.005 mGy/MBq in case of balloon rupture and release of the whole amount into the blood.

Calculation of Tissue-Air Ratios(TAR) in Irregularly shaped Field for Co-60 Gamma Radiation (CO-60 감마선에 대한 부정형조사면의 조직공중선량비 (TAR) 계산)

  • In order to calculate the dose on each interest point in five types of irregularly shaped fields used commonly in radiotherapy, the tissue-air ratios (TAR) in these fields for Go-60 gamma radiation were calculated using the newly devised SAR-chart. The TARs calculated from newly method of using the SAR-chart, computer method and approximation method at the interest point were compared to the TARs obtained from measurement. The result are as follows; In case of the interest points on central axis the calculated TARs in irregularly shaped fields by the above mentioned methods were well agreed within the error of $1\%$, whereas for the interest points on off-axis the calculated TARs were resulted in the maximum errors of $2.4\%,\;2.3\%$ and $8.8\%$ respectively. From these results, the accuracy of calculation method of using the SAR-chart was comfirmed.

  • Purpose : To verify the accuracy of the Ecilpse's dose calculation algorithm(AAA:Analytic anisotropic algorithm) in case of a radiation treatment on Inhomogeneous tissues using FFF beam comparing dose distribution at TPS with actual distribution. Materials and Methods : After acquiring CT images for radiation treatment by the location of tumors and sizes using the solid water phantoms, cork and chest tumor phantom made of paraffin, we established the treatment plan for 6MV photon therapy using our radiation treatment planning system for chest SABR, Ecilpse's AAA(Analytic anisotropic algorithm). According to the completed plan, using our TrueBeam STx(Varian medical system, Palo Alto, CA), we irradiated radiation on the chest tumor phantom on which EBT2 films are inserted and evaluated the dose value of the treatment plan and that of the actual phantom on Inhomogeneous tissue. Results : The difference of the dose value between TPS and measurement at the medial target is 1.28~2.7%, and, at the side of target including inhomogeneous tissues, the difference is 2.02%~7.40% at Ant, 4.46%~14.84% at Post, 0.98%~7.12% at Rt, 1.36%~4.08% at Lt, 2.38%~4.98% at Sup, and 0.94%~3.54% at Inf. Conclusion : In this study, we discovered the possibility of dose calculation's errors caused by FFF beam's characteristics and the inhomogeneous tissues when we do SBRT for inhomogeneous tissues. SBRT which is most popular therapy method needs high accuracy because it irradiates high dose radiation in small fraction. So, it is supposed that ideal treatment is possible if we minimize the errors when planning for treatment through more study about organ's characteristics like Inhomogeneous tissues and FFF beam's characteristics.

Comparison and Analysis of Photon Beam Data for Hospitals in Korea and Data for Quality Assurance of Treatment Planning System (국내 의료기관들의 광자 빔 데이터의 비교 분석 및 치료계획 시스템 정도관리자료)

  • Purpose: Photon beam data of linear accelerators in Korea are collected, analyzed, and a simple method for checking and verifying the dose calculations in a TPS are suggested. Materials and Methods: Photon beam data such as output calibration condition, output factor, wedge factor, percent depth dose, beam profile, and beam quality were collected from 26 institutions in Korea. In order to verify the accuracy of dose calculation, ten sample planning tests were peformed. These Include square, elongated, and blocked fields, wedge fields, off-axis dose calculation, SSD variation. The planned data were compared to that of manual calculations. Results: The average and standard deviation of photon beam quality for 6, 10, and 15 MV were $0.576{\pm}0.005,\;0.632{\pm}0.004,\;and\;0.647{\pm}0.006$, respectively. The output factors of 6 MV photon beam measured at depth of dose maximum for $5{\times}5cm,\;15{\times}15cm,\;20{\times}20cm\;were\;0.944{\pm}0.006,\;1.031{\pm}0.006,\;and\;1.055{\pm}0.007$. For 10 MV photon beam, the values were $0.935{\pm}0.006,\;1.031{\pm}0.007,\;1.054{\pm}0.0005$. The collected data were not enough to calculate average, the output factors for 15MV photon beam with field size of $5{\times}5cm,\;15{\times}15cm,\;20{\times}20cm\;were\;0.941{\pm}0.008,\;1.032{\pm}0.004,\;1.049{\pm}0.014$. There was seven institutions $e{\times}ceeding$ tolerance when monitor unit values calculated from treatment planning system and manually were compared. The measured average MU values for the machines calibrated at SAD setup were 3 MU and 5 MU higher than the machines calibrated at SSD for 6 MV and 10 MV, respectively except the wedge case. When the wedges were inserted, the MU values to deliver 100 cGy to 5 cm depends on manufactures. When the same wedge angle was used, Siemens machine requires more MUs then Varian machine. Conclusion: In this study, photon beam data are collected and analyzed to provide a baseline value for chocking beam data and the accuracy of dose calculation for a treatment planning system.

Evaluation of Beam Modeling Using Collapsed Cone Convolution Algorithm for Dose Calculation in Radiation Treatment Planning System (방사선치료계획시스템의 Collapsed Cone Convolution 선량계산 알고리듬을 이용한 빔 모델링의 정확성 평가)

  • This study aims to evaluate the accuracy of the collapsed cone convolution (CCC) algorithm for dose calculation in a treatment planning system (TPS), CorePLAN$^{TM}$. We implemented beam models for various setup conditions in TPS and calculated radiation dose using CCC algorithm for 6 MV and 15 MV photon beam in $50{\times}50{\times}50cm^3$ water phantom. Field sizes were $4{\times}4cm^2$, $6{\times}6cm^2$, $10{\times}10cm^2$, $20{\times}20cm^2$, $30{\times}30cm^2$ and $40{\times}40cm^2$ and each case was classified as open beam cases and wedged beam cases, respectively. Generated beam models were evaluated by comparing calculated data and measured data of percent depth dose (PDD) and lateral profile. As a result, PDD showed good agreement within approximately 2% in open beam cases and 3% in wedged beam cases except for build-up region and lateral profile also correspond within approximately 1% in field and 4% in penumbra region. On the other hand, the discrepancies were found approximately 4% in wedged beam cases. This study has demonstrated the accuracy of beam model-based CCC algorithm in CorePLAN$^{TM}$ and the most of results from this study were acceptable according to international standards. Although, the area with large dose difference shown in this study was not significant region in clinical field, the result of our study would open the possibility to apply CorePLAN$^{TM}$ into clinical field.

The Development of Air-kerma Strength Calculation Algorithm in Terms of the Absorbed Dose to Water for HDR Ir-192 Source (기준점에서의 물 흡수선량을 이용한 Ir-192 선원의 공기커마 세기 계산을 위한 알고리즘 개발)

  • The aim of this study was to develop the calculation algorithm of source strength of Ir-192 source In terms of the absorbed dose to water instead of an apparent activity (Ci). For this work the Multi Purpose Brachytherapy Phantom(MPBP) was developed, which was designed to locate the source and the chamber precisely at a specific position Inside the water phantom. The reference point of measurement was set at the 5 cm distance along the transverse axis of the source. For a brachytherapy source calibration, the absorbed dose to water calibration factor ($N_{D.W.Q}$) of an lonization chamber were determined and then apply standard protocols of absorbed dose to water. The calibration factor ($N_{D.W.Q}$) of the ion chamber (TM30013, PTW, Germany) was determined using the EGSnrcCPP Monte Carlo Code. The calculated calibration factor ($N_{D.W.Q}$) was 5.28 cGy/nC. The calculated factor was then used to determine the absorbed dose to water from which the air kerma strength for an Ir-192 source can be easily derived at the reference point (5 cm). The calculated air kerma strength showed discrepancies of -0.6% to +1.8% relative to the air kerma strength provided by the vendor, In this work we demonstrated that the air kerma strength ($S_k$) could be determined from the absorbed dose to water calibration factor for Ir-192 source. In audition, this source calibration method could be applied directly to the dose Calculation formalism of AAPM report TG-43.

A Study on the Simulation and the Measurement of 6 MeV electron Beam (6 MeV 전자선의 측정과 모의계산에 대한 연구)

  • Purpose : We compared the calcualted percent depth dose curves of 6 MeV electron beam to that of measured to evaluate the usefulness of Monte-carlo simulation method in radiation physics. Materials and Methods : The radiation dose values of 6 MeV electron beam using EGS4 code with one million histories in water were compared values that were measured from the depth dose curve of electron beam irradiated by medical accelerator ML6M. The central axis dose values were calculated according to the changing field size. such as $5{\times}5,\;10{\times}10,\;15{\times}15,\;20{\times}20cm^2$. Results : The value calculated showed a very similar shape to depth dose curve. The calculated and measured value of $D_max$ at $10{\times}10cm^2$ cone is 15mm and 14mm respectively. The calculated value of the surface radiation dose rate is $65.52\%$ and measured one is $76.94\%$. The surface radiation dose rate has varied from $64.43\%$ to $66.99\%$. The calculated values of $D_max$ are in the range between 15mm and 18mm. The calculated value was fitted well with measured value around the $D_max$ area, excluding build up range and below the $90\%$ depth dose area. Conclusion : This result suggested that the calculation of dose value can be replace the direct measurement of the dose for radiation therapy. Also, EGS4 may be a very convenient program to assess the effect of radiation dose using by personal computers.

Doses of Coronary Study in 64 Channel Multi-Detector Computed Tomography : Reduced Radiation Dose According to Varity of Examnination Protocols (64 채널 Multi-Detector Computed Tomography를 이용한 관상동맥검사의 선량 : 검사 프로토콜 다변화에 따른 환자선량 감소)

  • Purpose : To compare radiation dose for coronary CT angiography (CTA) obtained with 6 examination protocols such as a retrospectively ECG gated helical scan, a prospectively ECG gated sequential scan, low kVp technique, and cardiac dose modulation technique. Materials and Methods : Coronary CTA was performed by using 6 current clinical protocols to evaluate effective dose and organ dose in primary beam area with anthropomorphic female phantom and glass dosimetric system in 64 channel multi-detector CT. After acquiring topograms of frontal and lateral projection with 80 kVp and 10 mA, main coronary scan was done with 0.35 sec tube rotation time, 40 mm collimation ($0.625\;mm{\times}64\;ea$), small scan field of view (32 cm diameter), 105 mm scan length. Heart beat rate of phantom was maintained 60 bpm in ECG gating. In constant mAs technique 120 kVp, 600 mA was used, and 100 kVp for low kVp technique. In a retrospectively ECG gated helical CT technique 0.22 pitch was used, peak mA (600 mA) was adopted in range of $40{\sim}80%$ of R-R interval and 120mA(80% reduction) in others with cardiac dose modulation. And 210 mAs was used without cardiac dose modulation. In a prospectively ECG gated sequential CT technique data were acquired at 75% R-R interval (middle diastolic phase in cardiac cycle), and 120 msec additional padding of the tube-on time was used. For effective dose calculation region specific conversion factor of dose length product in thorax was used, which was recommended by EUR 16262. Results : The mean effective dose for conventional coronary CTA without cardiac dose modulation in a retrospectively ECG gated helical scan was 17.8 mSv, and mean organ dose of heart was 103.8 mGy. With low kVp and cardiac dose modulation the mean effective dose showed 54.5% reduction, and heart dose showed 52.3% reduction, compared with that of conventional coronary CTA. And at the sequential scan(SnapShot pulse mode) under prospective ECG gating the mean effective dose was 4.9 mSv, this represents an 72.5% reduction compared with that of conventional coronary CTA. And heart dose was 33.8 mGy, this represents 67.4% reduction. In the sequential scan technique under prospective ECG gating with low kVp the mean effective dose was 3.0 mSv, this represents an 83.2% reduction compared with that of conventional coronary CTA. And heart dose was 17.7 mGy, this represents an 82.9% reduction. Conclusion : In coronary CTA at retrospectively ECG gated helical scan, cardiac dose modulation technique using low kVp reduced dose to 50% above compared with the conventional helical scan. And the prospectively ECG gated sequential scan offers substantially reduced dose compared with the traditional retrospectively ECG gated helical scan.

AN EVALUATION OF RADIATION DOSES RESULTING FROM THE MEDICAL USE OF HIGH-ENERGY BETA-RAY SOURCES

  • Calculational models to evaluate radiation doses resulting from the medical use of high energy beta-ray sources are presented. The radioactive sources considered are Sr-90/Y-90 used as ophthalmic applicator, Re-188 used for treating restenosis of coronary artery, and Ho-166 used for treating hepatic tumors. Typical therapeutic situations which might induce relatively high radiation doses the medical person involved were considered to compute by using MCNP-4C Monte Carlo code the radiation doses. The calculation results suggest that for all of the cases considered, the evaluated radiation doses are negligible compared to the dose limits. It is also found that the effect of Bremsstrahlung radiations on the total dose is insignificant, and hence the conventional lead gown is also effective in shielding beta-rays.

Neutron Dose Rate Analysis of PWR Spent Fuel Transport Cask Using Monte Carlo Method

  • A shielding analysis for KSC-7, the shipping cask for transporting the 7 PWR spent fuel assemblies, has been carried out. Radiation source term has been calculated on spent fuel with burnup of 50,000 MWD/MTU and 1.5 years cooling time by ORIGEN2 code. The shielding calculation for the cask has been made by using MCNP4A code with continuous cross section data library from ENDF/B-V. As a result of neutron dose rate analysis, another shielding calculational model on spent fuel shipping cask was provided which is using the Monte Carlo method.
