A Review of Gaucher Disease in Korea

  • Gaucher disease (GD, OMIM #230800 OMIM#230800) is a rare, autosomal recessive inherited metabolic disorder caused by mutation in GBA1 encoding the lysosomal enzyme, glucocerebrosidase. The deficiency of glucocerebrosidase leads to an accumulation of its substrate, glucosylceramide in macrophages of various tissues. Common clinical manifestations include cytopenia, splenomegaly, hepatomegaly, and bone lesions. The phenotype of GD is classified into three clinical categories: Type 1 (non-neuronopathic) is characterized by involvements on the viscera, whereas types 2 and 3 (neuronopathic) are associated with not only visceral symptoms but also neurological impairment, either severe in type 2 or variable in type 3. A diagnosis of GD can be confirmed by demonstrating the deficiency of acid glucocerebrosidase activity in leukocytes. Mutations in the GBA1 should be identified as they may be of prognostic value in some cases. Biomarkers including Chitotriosidase, CCL18, and glucosylsphingosine (lyso-GL1) are useful in diagnosis and treatment monitoring. Currently available disease-specific treatment in Korea consists of intravenous enzyme replacement therapy and substrate reduction therapy. For enhancing long-term prognosis, the onset of Parkinson's disease and Lewy body dementia, or the occurrence of a blood disease or cancer (hepatocellular carcinoma) should be monitored in older patients. The development of new strategies that can modify the neurological phenotype are expected, especially in Asia including Korea, where the prevalence of neuronopathic GD is relatively higher than that in western countries.

Clinical Effect of the Subantimicrobial Dose of Doxycycline ( SDD ) on the Chronic Periodontitis (만성치주염 환자에 대한 저용량 독시싸이클린의 임상적 효과)

  • Periodontal disease is a complex infectious disease caused by bacteria in the oral mucosa, which results in gingival inflammation, breakdown of periodontal tissues, bone resorption, and finally tooth loss. Mechanical plaque control methods-scaling and root planing are effective methods to stop the progression of such periodontal disease. It was reported that subantimicrobial dose of doxycycline(SDD) regimen could improve clinical conditions of periodontal tissues without causing the overgrowth of opportunistic organisms that was a typical antibiotic side effect. Therefore pharmacological therapy, used in conjunction with mechanical therapy could be considered a useful treatment modality in the treatment of chronic periodontal disease. In this study, 30 patients diagnosed as moderate to advanced chronic periodontitis were divided into 2 groups. In this double-blind, placebo-controlled study, the patients were administered 20mg doxycycline capsule or placebo capsule b.i.d. for 4months, after scaling and root planing. Clinical parameters-bleeding on probing, pocket depth and clinical attachment level were compared and evaluated between these groups at periods of first visit, 1 month, 2 months, 3 months, 4 months. The results were as follows ; 1. In case of moderate periodontitis, pocket depth showed significant reduction after treatment in both the control & experiment groups, when compared with the baseline values(p<0.01), but in case of advanced periodontitis, only the experiment group showed significant reduction after treatment when compared with the baseline values(p<0.05). Statistically significant reduction in pocket depth was observed in the experiment group compared to the control group(p<0.05). 2. In case of moderate periodontitis, clinical attachment level showed significant reduction after treatment in both the control & experiment groups, when compared with the baseline values(p<0.01), but in case of advanced periodontitis, only the experiment group showed significant reduction after treatment when compared with the baseline values(p<0.05). Statistically significant reduction in clinical attachment level was observed in the experiment group compared to the control group(p<0.05). 3. Bleeding on probing improved after treatment in both the groups. In case of moderate periodontitis, the experiment group showed statistically significant reduction of bleeding on probing when compared with the control group at 1 and 4 months after treatment(p<0.05). In case of advanced periodontitis, treatment resulted in statistically significant reduction of bleeding on probing in both the groups(p<0.05). These results indicate that the use of subantimicrobial dose of doxycycline is a useful supplement to mechanical treatment for periodontal patients in ameliorating the clinical parameters such as periodontal pocket, attachment level, and bleeding on probing.

The Change Analysis about Health Behavior and Examination Result of Periodical Medical Examiner (정기 검진자의 건강행태 및 검진결과 변화분석)

  • The subject of investigation was 13,828 people who took periodical medical examiner in some health examination centers two times in 2009 and 2011 so as to analyze changes of borderline group, disease group for 4 diseases(hypertension, diabetes, liver disease, hyperlipemia) and health behavior of some medical examiner. For the contents of investigation, drinking and smoking habit was investigated as a general characteristic of subjects. For the items of health examination, there were SBP, DBP, AST, ALT, GGT, Glucose, Total Cholesterol and Triglyceride. The main results are as follows. In comparison with the starting year of research, drinking and smoking rate of the final year was reduced respectively by 1.6% and 1.4%. And, borderline group by examination items, hypertension was reduced by 2.2% from 62.0% to 59.8% in the final year in comparison with the starting year of investigation, And, fasting blood glucose was 17.1%, hyperlipemia was 6.6%, LFT was 2.0% for the reduction. In disease group, hypertension was reduced by 0.6% from 6.7% to 6.1%. And, fast blood glucose was 1.9%, hyperlipemia was 4.2%, LFT was 1.3% for reduction. Borderline group showed more reduction in comparison with disease group in all of items. The above results showed borderline group showed more reduction of manifestation rate than disease group in all of 4 diseases. Therefore, it's expected that disease group will show reduction in the future. But, it's necessary to study on future's changes through the continuous management and tracing observation.

Etiology of Rice Seedling Disease in Water-Seeded Rice.

  • Rice seedling disease is one of major problems in water-seeded rice. This disease is known to be caused by several pathogen such as Pythium, Achlya, and Fusarium species. However, seedling disease of rice in water-seeded rice in Korea is not extensively studied. Pythium species have been isolated from Seosan, Yeoju, Icheon areas using Pythium selective media and their pathogenicity was investigated. All of the Pythium isolates showed strong pathogencity causing seedling emergence reduction in water-seeded rice. Seedling emergence was reduced to 0∼9% at 10 days after inoculation of 23 Pythium isolates compared to 60% of noninoculated control in a growth chamber. However, Fusarium species did not cause seedling emergence reduction in water-seeded rice. In contrast, when no water added into water agar or soil, the pathogen caused seedling rot two weeks after planting. These results indicate that Pythium species is a cause of seedling disease in water-seeded cultivation areas in Korea.

Induced Resistance in Tomato Plants Against Fusarium Wilt Invoked by Nonpathogenic Fusarium, Chitosan and Bion

  • The potential of. nonpathogenic Fusarium oxysporum strain Avr5, either alone or in combination with chitosan and Bion, for inducing defense reaction in tomato plants inoculated with F. oxysporum f. sp lycopersici, was studied in vitro and glasshouse conditions. Application Bion at concentration of 5, 50, 100 and $500{\mu}g$/ml, and the highest concentration of chitosan reduced in vitro growth of the pathogen. Nonpathogenic F. oxysporum Avr5 reduced the disease severity of Fusarium wilt of tomato in split plants, significantly. Bion and chitosan applied on tomato seedlings at concentration $100{\mu}g$ a.i./plant; 15, 10 and 5 days before inoculation of pathogen. All treatments significantly reduced disease severity of Fusarium wilt of tomato relative to the infected control. The biggest disease reduction and increasing tomato growth belong to combination of nonpathogenic Fusarium and Bion. Growth rate of shoot and root markedly inhibited in tomato plants in response to tomato Fusarium wilt as compared with healthy control. These results suggest that reduction in disease incidence and promotion in growth parameters in tomato plants inoculated with nonpathogenic Fusarium and sprayed with elicitors could be related to the synergistic and cooperative effect between them, which lead to the induction and regulation of disease resistance. Combination of elicitors and non-pathogenic Fusarium synergistically inhibit the growth of pathogen and provide the first experimental support to the hypothesis that such synergy can contribute to enhanced fungal resistance in tomato. This chemical could provide a new approach for suppression of tomato Fusarium wilt, but its practical use needs further investigation.

Acanthopanax senticosus Reverses Fatty Liver Disease and Hyperglycemia in ob/ob Mice

  • Non-alcoholic fatty liver disease (NAFLD) is common in obesity. However, weight reduction alone does not prevent the progression of NAFLD to end-stage disease associated with the development of cirrhosis and liver disease. In a previous experiment, 50% ethanol extract of Acanthopanax senticosus stem bark (ASSB) was found to reduce body weight and insulin resistance in high fat diet-induced hyperglycemic and hyperlipidemic ICR mice. To evaluate the anti-steatosis action of ASSB, insulin-resistant ob/ob mice with fatty livers were treated with ASSB ethanol extract for an 8 week-period. ASSB ethanol extract reversed the hepatomegaly, as evident in reduction of % liver weight/body weight ratio. ASSB ethanol extract also specifically lowered circulating glucose and lipids, and enhanced insulin action in the liver. These changes culminated in inhibition of triglyceride synthesis in non-adipose tissues including liver and skeletal muscle. Gene expression studies confirmed reductions in glucose 6-phosphatase and lipogenic enzymes in the liver. These results demonstrate that ASSB ethanol extract is an effective treatment for insulin resistance and hepatic steatosis in ob/ob mice by decreasing hepatic lipid synthesis.

Dietary Reference Intakes of sodium for Koreans: focusing on a new DRI component for chronic disease risk reduction

  • Sodium is a physiologically essential nutrient, but excessive intake is linked to the increased risk of various chronic diseases, particularly cardiovascular. It is, therefore, necessary to accomplish an evidence-based approach and establish the Korean Dietary Reference Intakes (KDRIs) index, to identify both the nutritional adequacy and health effects of sodium. This review presents the rationale for and the process of revising the KDRIs for sodium and, more importantly, establishing the sodium Chronic Disease Risk Reduction Intake (CDRR) level, which is a new specific set of values for chronic disease risk reduction. To establish the 2020 KDRIs for dietary sodium, the committee conducted a systematic literature review of the intake-response relationships between the selected indicators for sodium levels and human chronic diseases. In this review, 43 studies published from January 2014 to December 2018, using databases of PubMed and Web of Science, were finally included for evaluating the risk of bias and strength of evidence (SoE). We determined that SoE of the relationship between dietary sodium and cardiovascular diseases, cerebrovascular disease, and hypertension, was moderate to strong. However, due to insufficient scientific evidence, we were unable to establish the estimated average requirement and the recommended nutrient intake for dietary sodium. Therefore, the adequate intake of sodium for adults was established to be 1,500 mg/day, whereas the CDRR for dietary sodium was established at 2,300 mg/day for adults. Intake goal for dietary sodium established in the 2015 KDRIs instead of the tolerable upper intake level was not presented in the 2020 KDRIs. For the next revision of the KDRIs, there is a requirement to pursue further studies on nutritional adequacy and toxicity of dietary sodium, and their associations with chronic disease endpoint in the Korean population.

Bacillus vallismortis EXTN-1-Mediated Growth Promotion and Disease Suppression in Rice

  • Bacillus vallismortis EXTN-1, a biocontrol agent in cucumber, tomato and potato was tested in rice pathosystem against rice fungal pathogens viz. Magnaporthe grisea, Rhizoctonia solani and Cochliobolus miyabeanus. Apart from increasing the yield in the bacterized plants (11.6-12.6% over control), the study showed that EXTN1 is effective in bringing about disease suppression against all the tested fungal pathogens. EXTN-l treatment resulted in 52.11% reduction in rice blast, 83.02% reduction in sheath blight and 11.54% decrease in brown spot symptoms. As the strain is proven as an inducer for systemic resistance based on PR gene expression in Arabidopsis and tobacco models, it is supposed that a similar mechanism works in rice, bringing about disease suppression. The strain could be used as a potent biocontrol and growth-promoting agent in rice cropping system.

Health Psdromotion Strategies under Regional Health Planning (보건소의 건강증진사업과 지역보건의료계획)

  • In many people's minds, health promotion is simply a more modern term covering roughly the same field as disease prevention or life style related reduction of the risk factors of chronic disease. A review of the modern literature of health promotion make it clear that there is more to this term than what is involved in functioning as a synonym for disease prevention. Therefore, in order to reach a clear understanding of what health promotion is, this study suggest the concept of the health balance model. Health balance is represented in terms of an equilibrium between physical, social, and life-style-related health challenges on the one hand and health potential on the other hand. Thus, health promotion strategies encompasses both the reduction of health challenges and the strengthening of health potential. Many elements of reducing health challenge are mainly related to the regulation laws. Aspects of strengthening of health potential are related to activities of health center. Therefore, health promotion strategies at a community level should be included in regional health planning which is implemented by health center.

The Study of Saamchimbeop's Method of Reinforcement and Reduction (사암침법(舍巖鍼法)의 보사수기법(補瀉手技法)에 관한 연구(硏究))

  • Objectives : The purpose of this study is what Saamchimbeop's method of reinforcement and reduction. Methods : 1. We reffered to the Bo-Sa method of DongeuiBo-gam(東醫寶鑑), Uihakim-mun(醫學入門), Uihakjeong-jeon(醫學正傳), Chimgugyeongheom-bang(鍼灸經驗方), Biaoyou-fu(標幽賦) in Cimgudaeseong(鍼灸大成), Nei-Jing(內經). 2. We make a conjecture that Zheng(正), Ying(迎), Sui(隨), Xie(斜) Yingzheng(迎正), Duo(奪), Zhenghuoxie(正或斜), Wen(溫), Liang(凉), JongYang-Inyin (從陽引陰) in Saamchimbeop are another expression of method of reinforcement and reduction and compared with the method of reinforcement and reduction of DongeuiBo-gam(東醫寶鑑), Uihakim-mun(醫學入門), Uihakjeong-jeon(醫學正傳), Chimgugyeongheom-bang(鍼灸經驗方), Biaoyou-fu(標幽賦) in Cimgudaeseong(鍼灸大成), Nei-Jing(內經). Results : 1. Zheng(正) and Xie(斜) are angle of acupuncture manipulation. The descending inserting of Yang-meridian is acupuncture manipulation for the Tonifying effect(補法) and the direct inserting of Yin-meridian is the Dispersing effect(瀉法). 2. JongYang-Inyin(從陽引陰) is the contralateral acupuncture. 3. Ying(迎) and Sui(隨) in the Saamchimbeop are same meaning the method of reinforcement and reduction(補瀉手技法). 4. Saamchimbeop's the final aim is the Wen-Liang(溫凉) according to the disease strong and weak in the Ohaeng-seo of Saam-acupuncture. Conclusions : Saamchimbeop's method of reinforcement and reduction is reinforcement-reduction by lifting and thrusting the needle, breathing reinforcement-reduction method, reinforcing and reducing achieved by rapid and slow insertion and withdrawing of the needles, reinforcement and reduction by opening and closing of needles with contralateral acupuncture by Yin-meridian or Yang-meridian. Saamchimbeop's the final aim is the Wen-Liang(溫凉) according to the disease strong and weak.
