임상 1급응급구조사의 특성과 업무범위 간의 상관관계 (Correlation between scope of practice and clinical paramedic properties)

  • 본 연구의 목적은 임상 응급구조사 특성과 업무범위 간의 상관관계를 밝히는데 있다. 2012년 4월 9일부터 5월 12일까지 181명의 1급응급구조사가 성별, 학력, 경력, 실행업무, 의료지도 등의 조사내용에 무기명으로 답변한 자료를 대상으로 1종오류를 범할 확률 ${\alpha}$=0.05 수준에서 양측 검정으로 상관관계 등을 분석하였다. 실행하고 있거나 필요하다고 보는 업무는 32가지로 구분되었으며 응급환자를 위한 처치로 의료인이 행하는 침습적인 처치가 주를 이루었다. 1급응급구조사 업무범위에 대한 수행능력이 우수하다고 판단하면 법에 따른 업무, 실행하고 있는 업무, 필요한 업무에서 통계적으로 유의하게 업무범위가 확대되는 것으로 나타났다. 경력이 늘면 법에 따른 업무, 필요한 업무에서 통계적으로 유의하게 업무범위가 확대되는 것으로 나타났다. 그렇지만 전반적으로 상관계수는 0.238이하로 낮았다. 능력과 경력에 따라서, 1급응급구조사로 하여금 의사-환자 사이의 매개역할을 원활히 형성하도록 지원해 주는 의료지도체계가 빨리 확립되어 의료서비스의 질을 높이고 효율성을 기해야 하며, 1급응급구조사가 지도의사의 교육, 훈련, 지침, 근무규정 등에 의해 업무를 수행하게 하고 처치 활동은 질관리를 하면 처치의 안정성을 높일 수 있을 것으로 보인다.

물리치료(학)과 학생들의 임상실습 중 대인관계에 관한 연구 (A Study on Interpersonal Relations on Clinical Practice of Physical Therapy Students)

  • Background : The purpose of this study is to provide information for making positive interpersonal relations by understanding the degree of difficulties of personal relations that physical therapy students may have on clinical practice. Methods : The study has been made based on the survey results of 10 university that completed clinical practice physical therapy students. The survey was conducted through direct contact and mail between October 1 and December 10, 2013, and analysed 291 survey sheets. Results : The results reveal that clinical instructor's attitude toward students was satisfying(71.82%). The results reveal that students felt convenient when faced with other department staffs in the hospital(70.44%). The results reveal that students didn't feel uncomfortable when they addressed themselves to patients(83.85%). The results reveal that students didn't feel uncomfortable when they were working with other college students(74.57%). Conclusions : The results of the study indicate that educators who are developing clinical practice programs and clinical practitioners should consider stressors during clinical practice and the educational implications to physical therapy students.

A Systematic Review of Domestic Research on Clinical Practice in Emergency Medical Technicians

  • This study analyzes research trends by systematically examining research about domestic emergency medical services' clinical practice, and it is aimed to present the basic data needs in development plan in clinical practice education in the future. The thesis was searched through the electronic data research (Science Direct, PubMed, Medline, and 55 academic DB interworking) from the library of Konyang University from 2010 to 2021. The main keywords were "Emergency Medical Technician(EMT) Student" and "Clinical Practice," and 6 pieces of researches were selected, finally. As the result of analyzing the qualitative level of selected research, all the 6 pieces of research subjects(100.0%) were pertinent to level IV(survey research) which is low-qualitative level. As the principal subjects, "Clinical practice experience" got the highest frequency as 6(100%), "Satisfaction of clinical practice" was 3(50%), "Self-efficient, Major satisfaction" and "Stress, Depression, Coping" showed 2 (33.3%), and "Change after clinical practice", "Clinical practice improvement plan" was 1(16.7%) each. From this time on, it is confirmed that the quantitative and qualitative growth in domestic emergency medical clinical practice is necessary. This is expected to contribute to establishing a practical and systematic development plan of clinical practice education.

신생아집중치료실 미숙아를 위한 직접모유수유 프로그램의 효과: 비동등성 대조군 전후 설계 (Effect of Direct Breastfeeding Program for Premature Infants in Neonatal Intensive Care Unit)

  • Purpose: This study aimed to identify the effects of a direct breastfeeding program for premature infants in neonatal intensive care units (NICUs). Methods: This quasi-experimental study was conducted during August 2016 to April 2017. Sixty mothers of premature infants were assigned to the experimental (n = 31) or control groups (n = 29). The program was comprised of breastfeeding education and direct breastfeeding support. The experimental and control groups were provided with education and counseling on breastfeeding at the time of admission and discharge. In the experimental group, the mothers initiated oral feeding with direct breastfeeding and engaged in breastfeeding at least seven times during the NICU stay. The collected data were analyzed by the χ2-test and repeated measures ANOVA using an SPSS program. Results: The experimental group showed a higher direct breastfeeding practice rate (χ2 = 19.29, p < .001), breastfeeding continuation rate (χ2 = 3.76, p < .001), and self-efficacy (F = 25.37, p < .001) than the control group except for maternal attachment. Conclusion: The direct breastfeeding program in the NICU has significant effects on the practice and continuation rate of breastfeeding and breastfeeding self-efficacy. Therefore, this program can be applied in the NICU settings where direct breastfeeding is limited.

  • There is a difficulty for many practioner in anterior direct restroation with composit resin. Because its result is various according to patient, a practioner have a fear about that unpromisable result. Moreover in esthetic region, there is difference in satisfaction by patient character. That is one of difficulty in this practice. But if we make a manual for parctice it will be easier. So I will summarize the process and things to note in direct anterior composite resin restoration.

학령기 아동의 안전생활 실천행동에 관한 구조모형 (A Structural Model for the Practice of Life Safety Behavior in School-age Children)

  • Purpose: This study is an examination of the paths in which the primary factors of anxiety, impulsiveness, knowledge of life safety practice, attitudes towards life safety practice, interpersonal support, and self-efficacy from Pender's Health Promotion Model influence the practice of life safety behavior in school-age children. Methods: The sample consisted of 489 5th and 6th grade students recruited from five elementary schools in Seoul City and four provinces, South Korea. Data were analyzed using descriptive statistics, correlations, factor analysis, and structural equation modeling. Results: Attitudes towards life safety practice, interpersonal support, self-efficacy and impulsiveness directly influenced practice of life safety behavior. Anxiety did not have a direct influence on practice of life safety behavior, but indirectly affected it. In this modified model, 52.0% of the practice of life safety behavior was explained by the primary factors. Conclusion: To facilitate the practice life safety behaviors in late childhood, a positive attitude towards life safety needs to be developed along with decreasing impulsiveness and enhancing self-efficacy.

스마트워크 도구를 이용한 팀 기반 학습에서 비공식적 커뮤니케이션과 마음챙김 실천이 수업외적인 행동에 미치는 영향: 자기효능감 지각의 매개효과를 중심으로 (Effects of Informal Communication and Mindfulness Practice on the Non-Class-Related Behavior in the Team-Based Learning using Smartwork Tool: Focusing on Mediating Effects of Perceived Self-Efficacy)

  • 본 연구의 목적은 스마트워크 도구를 이용한 팀 기반 학습에서 비공식적 커뮤니케이션과 마음챙김 실천이 수업외적인 행동에 미치는 영향에 있어서 자기효능감 지각의 매개효과를 조사하는 것이었다. 이를 위해 대학생 80명을 대상으로 설문조사를 실시하였다. 본 연구의 실증결과를 요약하면 다음과 같다. 첫째, 비공식적 커뮤니케이션은 수업외적인 행동에 직접 영향을 미치는 것이 아니라 자기효능감 지각을 매개로 하여 영향을 미치는 것으로 나타났다. 둘째, 마음챙김 실천도 수업외적인 행동에 직접적으로 영향을 미치는 것이 아니라 자기효능감 지각을 매개로 영향을 미치는 것으로 나타났다. 셋째, 비공식적 커뮤니케이션과 마음챙김 실천은 자기효능감 지각에 긍정적인 영향을 미치는 것으로 나타났다. 본 연구의 결과는 스마트워크 도구를 이용한 팀 기반 학습 정책 수립시 자기효능감 지각의 중요성을 제시하였다는데 의의가 있다 하겠다.

비탈면 안정을 위한 참싸리 뿌리의 토양보강 효과 (Analysis on Soil Reinforcement by Lespedeza cyrtobotrya Roots for Slope Stability)

  • To examine the soil reinforcement by the shrub with shallow root systems for slope stability, we developed insitu apparatus for direct shear test and conducted the insitu field tests for Lespedeza cyrtobotrya, a representative revegetation species for artificial hillslopes. The insitu field tests were conducted for two different soil conditions (the rooted soils and non-rooted soils) and we then compared the experimental values with those calculated from the Wu model. The results showed that the soil reinforcement derived from the insitu field tests ranged from 0.01525 to 0.1438 $kgf/cm^2$ while the one calculated from the Wu model ranged from 0.1952 to 0.2696 $kgf/cm^2$. Our finding suggests more field tests are needed to collect the related parameters in the model application thereby predicting the reliable soil reinforcement by the shrub root systems.

우리나라와 캐나다 약학대학 실무실습 교과과정 비교 (Comparison of Pharmacy Practice Experience in Pharmacy School between Korea and Canada)

  • With the changes in the pharmacy curriculum from a 4 year program to a 6 year program in 2009, a new subject called Pharmacy Practice Experience (PPE) has been launched into the curriculum. The purpose of introducing this subject is to increase the competency of new pharmacy graduates in providing quality healthcare to the community. This study has been done, via comparison among different pharmacy schools in Canada and Korea, to ensure that the competency of future Korean pharmacists can be increased with the introduction of this subject. In general, the Introductory Pharmacy Practice Experience (IPPE) in Korea consists of 60 hours while Canada consists of 320 hours (minimum). Furthermore, the Advanced Pharmacy Practice Experience (APPE) required in Korea is 1340 hours while Canada requires minimum of 960 hours. Specifically, comparing the Korean PPE curriculum to the PPE curriculums of University of Toronto (UT) and University of Waterloo (UW), UT and UW required a minimum of 75% and 89% direct clinical patient care experience respectively, either in hospital or community setting, compared to 45% in Korea; the remaining percentage in any of the universities can be fulfilled by taking other electives that may not require direct patient care experience. Observing these differences, it seems clear that the current PPE experience in Canada takes more of a patient focused approach than in Korea. Thus, with the recent movement in the Korean pharmacy community towards a more patient focused approach rather than a product focused approach, it would be beneficial to learn the differences between the PPE curriculums in Korea and Canada and apply any new understandings to the relatively newly introduced PPE program in Korea to further enhance the value of the new curriculum in helping to deliver quality patient care.
