A technique is presented through clinical cases for restoring anterior teeth to enhance dental esthetics using bonded partial ceramic restorations (BPCRs) without or with minimum reduction of tooth structures. These restorations offer the advantages of the superior properties of ceramic materials and the conservative nature of a direct composite restoration at the same time.
Percutaneous coronary intervention including intracoronary stenting is currently an accepted treatment modality in the treatment of coronary artery disease and is widely performed to treat the patient with multivessel disease with decreased morbidities and less cost compared with conventional coronary rtery bypass grafting(CABG), Repeated interventions due to restenosis even after successful angioplasty are the major disadvantage of the angioplsty especially when the lesion is located inthe left anterior descending artery(LAD) Recently CABG through left anterior small thoracotomy using the left internal thoracic artery to revascularize the LAD territory without cardiopulmonary bypass so called Minimally Invasive Direct Coronary Artery Bypass(MIDCAB) was intrduced and performed with comparable early outcomes. In this regard the integrated approach with percutaneous coronary intervention and minimally invasive direct coronary artery bypass surgery so called 'Hybrid CABG' was suggested to be an effective treatment in suitable patients with multivessel coronary artery disease. We report three cases of Hybrid CABG.

Background: The traditional approach for reduction of frontal sinus fractures is coronal incision. Inherent complications of the coronal approach include long scar, hair loss, and long operation time. We describe a simple approach for the reduction of frontal sinus anterior wall fractures using a suprabrow incision that is commonly used for brow lift. Methods: From March 2007 to October 2016, the authors identified patients with anterior wall frontal sinus fractures treated by open reduction through a suprabrow incision. Only cases with photographic/radiographic documentation and a minimum follow-up of 6 months were included. The incision line was designed to be at the upper margin of the eyebrow. Medical records and radiographic data were retrospectively reviewed. Surgical outcomes, cosmetic results, and complication were assessed. The patient scale of the patient and observer scar assessment scale was used to assess patient satisfaction for incisional scar at the 6-month follow-up. Results: Thirty-one patients underwent fracture reduction through a suprabrow approach during the study period, with a mean follow-up of 41 months. No patients showed any recurrent displacement, eyebrow asymmetry, or infection during follow-up. Thirteen patients reported their forehead paresthesia postoperatively, and 12 of them had preoperative symptom. One patient complained of incisional scar and underwent scar revision. All patients were satisfied with their eyebrow and forehead contour. Conclusion: The suprabrow approach allowed for an accurate reduction of the fractures in the anterior wall frontal sinus by providing direct visualization of the fracture. This transcutaneous approach can effectively restore forehead contour with acceptable postoperative complications and patient satisfaction.
목적: 회전근 개 파열에서의 전외측 도달법을 이용한 소절개 봉합술을 소개하고자 한다. 수술 술기: 전신 마취하에 측와위 자세를 취한 후 후방 삽입구, 전방 삽입구를 이용하여 관절내 병변의 유무를 확인하고, 관절경을 견봉하 공간에 위치시킨 후 외측 삽입구를 이용하여 회전근 개의 파열 형태 및 크기를 파악한 후 관절경하견봉 성형술을 시행한다. 견봉의 전외측 연에서 하방으로 3~4 cm의 피부 절개를 가한 후 전방 및 중간 삼각근의 봉합선을 따라 박리하고 삼각근 견인기를 이용하여 시야를 확보한다. 파열된 건에 견인 봉합을 시행하여 해부학적 복원을 위한 위치를 확인한 후 봉합 나사를 이용하여 일열 혹은 이열 봉합술을 시행한다. 봉합술 후 견봉에서 삼각근이 견열되는 것을 방지하기 위해 1번 흡수봉합사를 이용하여 견봉과 삼각근과의 추가적인 봉합을 시행한다. 결론: 본 술기는 회전근 개의 가장 흔한 파열 부위인 극상건의 전방부에 직접 도달이 가능하고, 전방 및 중간 삼각근 사이로 접근하기 때문에 삽입구 연장 도달법에 비해 비교적 적은 견인으로도 시야를 확보할 수 있으며 전방으로는 견갑하건의 상부와 후방으로는 극하건까지 도달할 수 있는 유용한 술식으로 생각된다.
목적: 58예의 직접전방 접근법을 통한 인공 고관절 치환술의 임상결과와 학습곡선을 분석하여 학습능력이 향상되는 시점에 대하여 알아보고자 하였다. 대상 및 방법: 2016년 11월부터 2018년 11월까지 58명의 환자를 대상으로 직접전방 접근법을 사용한 인공 고관절 치환술을 시행하였다. 초기 시행한 29명과 후기 시행한 29명을 후향적으로 비교분석하였다. 수술시간과 합병증(대전자 견열 골절, 외측 대퇴피신경 손상, 이소성 골화증, 감염, 탈구 등)을 바탕으로 분석하였다. 통계적 방법은 양 군 간에 대응표본 T 검정, 카이제곱 검정과 누적합법(cumulative sum, CUSUM) 분석을 사용하였다. 결과: 수술 시간은 전치환술은 평균 132.1분, 반치환술은 평균 79.7분으로 양 군에서 유의한 차이를 보였다. 수술 기간에 따른 CUSUM 분석을 시행하였고, 전치환술에서는 16번째 증례, 반치환술에서는 14번째 증례부터 수술시간이 각각 평균 수술시간보다 감소하였다. 합병증으로 전반기에 5예, 후반기에 0예로 총 5예의 대전자 견열골절이 있었으며, 외측대퇴피하신경 손상은 전반기 8예, 후반기 2예로 총 10예, 이소성 골화증은 전반기 3예, 후반기 2예로 총 5예, 탈구와 감염은 각각 전반기 1예씩, 기타 합병증 3예가 있었다. 전반기 1년간 수술 중 발생한 대전자 견열골절은 5예(17.2%), 후반기에는 0예(0%)가 있었으며 이를 CUSUM 분석을 통해 모니터링하였으나 증례가 많지 않아 유의한 차이를 보이지는 않았다. 결론: 직접전방 접근법을 이용한 인공 고관절 치환술은 해부학적 이해가 선행되어야 하고, 수술 시야의 확보가 어렵기 때문에 습득하는 데 최소 30예 이상의 학습곡선이 필요하다.
목적: 일반 수술용 침대와 길이가 짧은 대퇴 주대(short stem)를 이용하여 인공 고관절 전 치환술을 직접 전방 도달법(direct anterior approach, DAA)을 이용하여 시행하고, 임상적 영상의학적 결과를 변형된 전 측방 도달법(modified Hardinge; anterolateral approach, ALA)을 시행한 경우와 비교하여 알아보고자 하였다. 대상 및 방법: 2013년 1월부터 2015년 11월까지 단일 술자가 DAA로 인공 고관절 전 치환술을 시행한(DAA group) 102명(102예)과 동 수의 ALA를 사용한 환자(ALA group)를 후향적으로 비교 분석하였다. 수술시간과 출혈량을 비교하였고, 수술 후 통증의 개선 정도와 보행 능력 및 고관절의 기능 회복을 알아보았다. 영상의학적으로 비구 컵과 대퇴 주대의 삽입 위치를 평가하였고, 두 군 간에 발생한 합병증을 알아보았다. 결과: 출혈량은 DAA group에서 유의하게 적었다(p=0.018). 수술 후 3주까지는 하지 근력의 회복이 DAA group에서 유의하게 높았다(굴곡/신전력 p=0.023, 외전력 p=0.031). Harris hip score를 이용한 기능 평가는 3개월까지(p≤0.001), Koval score를 이용한 보행 능력 평가는 6주까지(p≤0.001) DAA group에서 유의하게 나은 결과를 보였고, visual analogue scale score를 이용한 통증의 개선은 7일까지는 DAA group에서 유의하게 높았다(p=0.035). 비구 컵의 경사각(p≤0.001)과 전염각(p≤0.001)은 DAA group에서 ALA group보다 더 안정 범위에 위치하였고, 대퇴 주대의 위치와 하지 길이는 통계적 유의한 차이를 보이지 않았다. DAA group에서 수술 중 2예의 대전자 골절이 발생하였다(p=0.155). 결론: 길이가 짧은 대퇴 주대를 이용하여 일반 수술 침대에서 시행하는 DAA는 수술 후 조기 기능 회복을 보이며, 술자에게 친숙한 해부학적 자세로 수술 중 영상 증폭기 사용이 간편하여 원하는 위치에 인공 관절물을 삽입하는 것과 하지 부동 방지에 유용한 도달법으로 생각된다.
Purpose : Propose a surgical technique in donor harvesting method in free vascularized proximal fibular epiphysis. Methodology : Concerned about growth potentials of the transplanted epiphysis in our long term results of the epiphyseal transplanted 13 cases more than 4 years follow-up, anterior tibial artery which contains anterior tibial recurrent artery is most reliable vessel to proximal fibular epiphysis which is the best donor of the free vascularized epiphyseal transplantation. In vascular anatomical aspect proximal fibular epiphysis norished by latearl inferior genicular artery from popliteal, posterior tibial recurrent artery and anterior tibial recurrent artery from anterior tibial artery and peroneal artery through metaphysis. The lateral inferior genicular artery is very small and difficult to isolate, peroneal artery from metaphysis through epiphyseal plate can not give enough blood supply to epiphysis itself. The anterior tibial artery which include anterior tibial recurrent and posterior tibial recurrent artery is the best choice in this procedure. But anterior tibial recurrent artery merge from within one inch from bifucating point of the anterior and posterior tibial arteries from popliteal artery. So it is very difficult to get enough vascular pedicle length to anastomose in recipient vessel without vein graft even harvested from bifucating point from popliteal artery. Authors took recipient artery from distal direction of anterior tibial artery after ligation of the proximal popliteal side vessel, which can get unlimited pedicle length and safer dissection of the harvesting proximal fibular epiphysis. Results : This harvesting procedure can performed supine position, direct anterolateral approach to proximal tibiofibular joint. Dissect and isolate the biceps muscle insertion from fibular head, micro-dissection is needed to identify the anterior tibial recurrent arteries to proximal epiphysis, soft tissue release down to distal and deeper plane to find main anterior tibial artery which overlying on interosseous membrane. Special care is needed to protect peroneal nerve damage which across the surgical field. Conclusions : Proximal fibular epiphyseal transplantation with distally directed anterior tibial artery harvesting technique is effective and easier dissect and versatile application with much longer arterial pedicle.
Background and Objective:A post-operative hypertrophic scar of the anterior neck is the leading complaint of the patients who underwent conventional thyroid surgery. In order to minimize the post-operative scar of the anterior neck, we performed thyroidectomy via axillary approach using operating microscope and a specialized retractor to determine technical feasibility. Patients and Methods:From January 2005 to December 2006, we performed thyroidectomy via axillary approach under operating microscope(f=400mm, ${\times}2.5$;OPMI $pico^{(R)}$;Zeiss, Germany) for benign unilateral nodule in 25 cases(all female, average age 34.5yrs). Under general anesthesia less than 7cm of skin incision was made in the axilla of ipsilateral side. A subcutaneous tunnel went over the pectoralis major muscle and the clavicle, and then through the sternocleidomastoid muscle and sternothyroid muscle was excised. The area around the thyroid was sufficiently dissected, and then a retractor designed for exposure via axillary approach was placed within the tunnel and under operating microscope thyroidectomy was performed. Results:There were 17 cases of thyroid nodulectomy and 8 cases of subtotal lobectomy. The mean average operative time was 102.64minutes. Postoperative complications included one case of postoperative bleeding, one case of temporary vocal cord paralysis, two cases of delayed wound healing, two cases of paresthesia of shoulder and arm, and two cases of hypertrophic scar of the axilla. Postoperative histopathology includes 17 cases of adenomatous hyperplasia, six cases of cyst, and two cases of follicular adenoma. For all cases hospitalization period was two days. Conclusion:Thyroidectomy via axillary approach under operating microscope has a good cosmetic advantage without a post-operative scar of the anterior neck. The procedure is simple due to direct vision using operating microscope, easy to identify important structures by magnifying them, and therefore surgical time can be reduced.

An intraosseous hemangioma of the frontal bone is typically removed via a coronal incision. This procedure, while effective, can be lengthy and may result in complications such as a prominent scar and hair loss. An alternative approach involves a direct incision in the forehead, which leaves a less noticeable scar and allows a quicker recovery. However, in this specific case, the patient declined both coronal surgery and surgery through a direct forehead incision due to cosmetic concerns. Therefore, we proposed an anterior hairline incision. A 35-year-old woman presented with a firm, non-mobile, palpable mass on her right forehead. Preoperative non-contrast computed tomography revealed a heterogeneous osteolytic lesion. We performed an excisional biopsy through the anterior hairline. Postoperative non-contrast computed tomography was conducted 2 and 6 months after surgery. The wound was clean and free of complications, and there was no local recurrence. Partial resection can reduce scarring for patients who are concerned about cosmetic outcomes. However, the potential for recurrence remains a significant concern. We present this case of an anterior hairline incision for a hemangioma located in the forehead, evaluated using serial computed tomography for both preoperative and postoperative imaging.
Closure of interdental spaces using proximal build-ups with resin composite is considered to be practical and conservative. However, a comprehensive approach combining two or more treatment modalities may be needed to improve esthetics. This case report describes the management of a patient with multiple diastemas, a peg-shaped lateral incisor and midline deviation in the maxillary anterior area. Direct resin bonding along with orthodontic movement of teeth allows space closure and midline correction, consequently, creating a better esthetic result.
