Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis (메타분석을 이용한 임상영양서비스의 비용-효과성 평가)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. Back Analysis for Estimating Tension Force on Hanger Cables (역해석기법을 이용한 현수교 행어케이블 장력 추정)

  • In general, the tension fores of hanger cable in suspension bridges play an important role in evaluating the bridge state. The vibration method, as a conventional one, has been widely applied to estimate the tension fores by using the measured frequencies on hanger cables. However, the vibration method is not applicable to short hanger cables because the frequency of short cables is severely sensitive to the flexural rigidity. Thus, in this study, the tension forces of short hanger cables, of which the length is shorter than 10meters, were estimated through back analysis of the cable frequencies measured from Gwang-An suspension bridge in Korea. Direct approach to rock analysis is adopted using the univariate method among the direct search methods as an optimization technique. The univariate method is able to search the optimal tension forces without regard to the initial ones and has a rapid convergence rate. To verify the feasibility of back analysis, the results from back analysis and vibration method are compared with the design tension forces. From the comparison, it can be inferred that back analysis results are more reasonable agreement with the design tension forces of short hanger cable. Therefore, it is concluded that back analysis applied in this study is an appropriate tool for estimating tension forces of short hanger cables.

  • In this study, direct-search based optimization methods are applied for blade shape optimization of wind turbines and the optimization performances of several methods including conventional genetic algorithm, micro genetic algorithm and pattern search method are compared to propose a more efficient method. For this purpose, the currently available version of HARP_Opt (Horizontal Axis Rotor Performance Optimizer) code is enhanced to rationally evaluate the annual energy production value according to control strategies and to optimize the blade shape using pattern search method as well as genetic algorithm. The enhanced HARP_Opt code is applied to obtain the optimal turbine blade shape for 1MW class wind turbines. The results from pattern search method are compared with the results from conventional genetic algorithm and also micro genetic algorithm and it is found that the pattern search method has a better performance in achieving higher annual energy production and consistent optimal shapes and the micro genetic algorithm is better for reducing the calculation time.

  • Recently, Genetic Algorithm(GA) is widely adopted into a search procedure for structural optimization, which is a stochastic direct search strategy that mimics the process of genetic evolution. This methods consist of three genetics operations maned selection, crossover and mutation. Contrast to traditional optimal design techniques which use design sensitivity analysis results, GA, being zero-order method, is very simple. So, they can be easily applicable to wide area of design optimization problems. Also, owing to multi-point search procedure, they have higher probability of converge to global optimum compared to traditional techniques which take one-point search method. In this study, a method of finding the optimum values of suspension parameters is proposed by using the GA. And vehicle is modelled as planar vehicle having 5 degree-of-freedom. The generalized coordinates are vertical motion of passenger seat, sprung mass and front and rear unsprung mass and rotate(pitch) motion of sprung mass. For rapid converge and precluding local optimum, share function which distribute chromosomes over design bound is introduced. Elitist survival model, remainder stochastic sampling without replacement method, multi-point crossover method are adopted. In the sight of the improvement of ride comfort, good result can be obtained in 5-D.O.F. vehicle model by using GA.

  • Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation method that delivers 1-2 mA of current to the scalp. Several clinical studies have been conducted to confirm the therapeutic effect of major depressive disorder (MDD) patients with tDCS. Some studies have shown tDCS's antidepressant effect, while the others showed conflicting results in antidepressant effects. Our aim of this review is to understand the biological bases of tDCS's antidepressant effect and review the results of studies on tDCS's antidepressant effect. For the review and search process of MDD treatment using tDCS, the US National Library of Medicine search engine PubMed was used. In this review, we discuss the biological mechanism of tDCS's antidepressant effect and the existing published literature including meta-analysis, systematic review, control trial, open studies, and case reports of antidepressant effects and cognitive function improvement in patients with MDD are reviewed. We also discuss the appropriate tDCS protocol for MDD patients, factors predictive of response to tDCS treatment, the disadvantages of tDCS in MDD treatment, and side effects.

  • Due to the plasma applied from the outside, which acts as an etchant during the etching process, considerable heat is transferred to the wafer and a separate cooling process is performed to effectively remove the heat after the process. In this case, a direct cooling method using a refrigerant is suitable for cooling through effective heat exchange. The direct cooling method using the refrigerant using the latent heat exchange is superior to the cooling method using the sensible heat exchange. Therefore, in this paper, AMESim is used to design a direct refrigerant cooling system using latent heat exchange simulator was built.The constructed simulator is reliable compared with the actual experimental results. It is expected that this simulator will help to design and search for optimal process conditions.

A Back-Analysis of Tunnels in Multi-Layered Underground Structures (다층구조계내 터널 거동의 역해석)

  • This study consists of two procedures on back analysis and forward analysis which is a basic tool of the former. For a safe and economical construction of underground structures, it is required to identify the structural parameters and analyze the structural behavior as exactly as possible. In this paper, a boundary element method to analyze the behavior of multi-alyered underground structures is studied, in which body forces and initial stresses are considered. That is, each layer is discritized into subregions using infinite fundamental solutions, and terms of body forces and initial stresses are transformed into boundary integral where the applied direct integral method is used. And the system of equations containing body forces and initial stresses are considered. That is, each layer is discritized into subregions using infinite fundamental solutions, and terms of body forces and initial stresses are transformed into boundary integral where the applied direct integral method is used. And the system of equations containing body forces and initial stresses are composed, then the method to solve unknowns is used with applying compatibility and equilibrium conditions between interfaces. As well, the direct search method is applied in back analysis problems. By Powell's method as a technique to search unknown parameters, assuming displacements calculated from boundary element analysis as in-situ displacements, elastic moduli and initial stresses are presumed. As consequences of this study, the results of boundary element analysis of the behavior of multilayered structure considering body forces and initial stresses are agreed with those of finite element analysis. And results of back analysis of elastic moduli and initial stresses in each layers are agreed with exact values with a little difference. Therefore, it is known that this study can be efficiently applied for analyzing the behavior of underground structures including back analysis problems.

  • In this paper, search method and sliding mode observer are developed for efficiency optimization of induction motor. The proposed control scheme consists of efficiency controller and adaptive backstepping controller. A search controller for which information of input of fuzzy controller is included in efficiency controller that uses a direct vector controlled induction motor. The search controller is based on the "Rosenbrock" method and finds the flux level at the minimum input power of induction motor. Once this optimal flux level has been determined, this information is utilized to update the rule base of a fuzzy controller A sliding mode observer is designed to estimate rotor flux and an adaptive backstepping controller is also used to compensate for mechanical uncertainties in the speed control of induction motor. Simulation results are presented to validate the proposed controller.

  • The study developed an automatic design method of steel frames which uses nonlinear analysis. The geometric nonlinearity was considered using stability functions. Likewise, the transverse shear deformation effect in a beam-column was explained. A direct search method was used as an automatic design technique. The unit value of each part was evaluated using LRFD interaction equation. The member with the largest unit value was replaced one by one with an adjacent larger member selected from the database. The weight of the steel frame was considered as an objective function. On the other hand, load-carrying capacities, deflections, inter-story drifts, and ductility requirement were used as constraint functions. Case studies of a two-dimensional and a three-dimensional two-story frames were presented.

Contrast to traditional optimal design techniques which use design sensitivity analysis results, GA is very simple in their algorithms and there is no need of continuity of functions(or functionals) any more in GA. So, they can be easily applicable to wide area of design optimization problems. Also, owing to multi-point search procedure, they have higher porbability of convergence to global optimum compared to traditional techniques which take one-point search method. The methods consist of three genetics opera- tions named selection, crossover and mutation. In this study, a method of finding the omtimum size and topology of drilling machine is proposed by using the GA, For rapid converge to optimum, elitist survival model,roulette wheel selection with limited candidates, and multi-point shuffle cross-over method are adapted. And pseudo object function, which is the combined form of object function and penalty function, is used to include constraints into fitness function. GA shows good results of weight reducing effect and convergency in optimal design of drilling machine.
