The intensity modulated radiation therapy (IMRT) with a multileaf collimator (MLC) requires the conversion of a radiation fluence map into a leaf sequence file that controls the movement of the MLC during radiation treatment of patients. Patient dose verification is clinically one of the most important parts in the treatment delivery of the radiation therapy. The three dimensional (3D) reconstruction of dose distribution delivered to the target helps to verify patient dose and to determine the physical characteristics of beams used in IMRT. A new method is presented for the pretreatment dosimetric verification of two dimensional distributions of photon intensity by means of Beam Intensity Scanner System (BISS) as a radiation detector with a custom-made software for dose calculation of fluorescence signals from scintillator. The scintillator is used to produce fluorescence from the irradiation of 6MV photons on a Varian Clinac 21EX. The BISS reproduces 3D- relative dose distribution from the digitized fluoroscopic signals obtained by digital video camera-based scintillator(DVCS) device in the IMRT. For the intensity modulated beams (IMBs), the calculations of absorbed dose are performed in absolute beam fluence profiles which are used for calculation of the patient dose distribution. The 3D-dose profiles of the IMBs with the BISS were demonstrated by relative measurements of photon beams and shown good agreement with radiographic film. The mechanical and dosimetric properties of the collimating of dynamic and/or step MLC system alter the generated intensity. This is mostly due to leaf transmission, leaf penumbra and geometry of leaves. The variations of output according to the multileaf opening during the irradiation need to be accounted for as well. These phenomena result in a fluence distribution that can be substantially different from the initial and calculative intensity modulation and therefore, should be taken into account by the treatment planning for accurate dose calculations delivered to the target volume in IMRT.
In this study, we intended to examine the difference in brain activation due to dominant and non-dominant hands using functional near-infrared spectroscopy(fNIRS) in 10 healthy adults. Box & Block Test(BBT) was conducted under two conditions: dominant hand and non-dominant hand. During the experiment, brain activity was measured using fNIRS and signals were analyzed using nirsLAB v2019.04 software after the experiment was completed. As a result, 6 out of 10 people showed activation of the cerebral hemisphere related to the dominant hand, and only 3 out of 10 people showed activation of the cerebral hemisphere related to the non-dominant hand. In other words, both dominant and non-dominant hand cconfirmed that the cerebral hemispheres related to dominant hands were more active. Therefore, it is believed that fNIRS can be used as a fundamental data applicable to children with sensory processing disorders that are difficult to identify dominant hand.
Objective : This study aimed to develop prototypes of "smart stacking cones" by combining ICT to evaluate and train the upper extremity function and dual task performance in patients with central nervous system impairment, and to identify the complementary point to the completion of the device through a usability test. Methods : This prototype comprised of a hardware and software system that enabled the evaluation and training of patients and the management of data obtained from patients' performance. Specific measurement variables were established so that patient performance could be measured correctly. Based on the measurement variables, a the prototype included a 'single task evaluation modes', 'dual task evaluation mode', 'single task training mode', and 'dual task training modes'. Additionally, a usability test was conducted to assess clinical applicability and overall satisfaction for the prototype. Results : The results of the usability test were generally found to be appropriate. The 'content adequacy' in the usability test was the area with the highest level of adequacy and the lowest level of inadequacy. Additionally, overall 'satisfaction' in the usability test was the area with the highest appropriate and inappropriate levels. Hence, the overall satisfaction results were unstable. Conclusion : Future studies should be conducted to identify the clinical effectiveness of the device by applying an upgraded smart stacking cone to an actual patient group.
Objective : The objective of this study was to present the components of frailty by organizing the definitions of frail elderly and analyzing the tools used to screen them. Methods : This study searched for articles at involved frailty screening assessments in the elderly. Databases including CINAHL, Embase, Medline Complete, and PubMed were searched. The search terms were "assess" AND "frailty" AND "screening" AND ("frail elderly" OR "elderly"). Results : A total of 539 articles were identified by the search and 11 articles were selected. Frailty occurs due to the depressed function of multidimensional factors, and a frail elderly person is defined as one at high risk of health degeneration, functional impairment, and occurrence of disability, and having a high level of threat to life. Seven tools were selected from 11 articles. The most frequently used tool was the frailty phenotype, which was used in five articles (45.4%). The identified components of frailty were physical, activity participation, nutrition, psychological, social, overall health, and age. Conclusion : The results confirmed the definition and components of frailty. This study is expected to contribute to the future development of standardized evaluation tools for screening frail elderly individuals and intervention programs for the management of the frail elderly.
Objective : Through the bibliographic analysis method, the flow of research is grasped from a macroscopic point of view and the connection system of key words is conducted. The purpose of this is to provide basic data for conducting research on aging anxiety and lifestyle. Methods : Among the bibliographic analysis methods, a citation analysis method that identifies the association based on the number of citations and a simultaneous appearance word analysis method that identifies the association based on the number of keywords appeared was used. VOSviewer was used to cluster and chart the analyzed information. Results : The frequency of occurrence of papers by year showed a gradual increase until 2017 and a rapid increase from 2018. In the field of research paper study, research was most actively conducted in the field of psychiatry. In the citation analysis, the United States, Australia, and the United Kingdom showed high correlation with each other, and as a result of conducting simultaneous word analysis on major keywords, words with high association with aging anxiety were found to be depression. Conclusion : This study is meaningful in that it grasped the flow of aging anxiety and lifestyle research from a macroscopic point of view using a bibliographic analysis method. Based on this, it is expected to understand the importance of lifestyle from the preventive point of view of aging and to be used as basic data for intervention and related education.
Objective : To develop standardized assessment tools that evaluate the multi-aspect lifestyles of children and adolescents, we aimed to systematically analyze lifestyle assessment tools and lifestyle component-specific assessment tools. Methods : The study period was 2001-2020, with search engines using CINANL, NDSL, PubMed, and RISS. Search key words used 'lifestyle assessment' OR 'lifestyle profile' OR 'lifestyle test', 'physical activity assessment' OR 'physical activity participation profile', 'nutrition assessment' OR 'nutrition profile', 'activity participation assessment' OR 'activity participation profile'. Results : A total of 24 evaluation tools were selected after 5,883 studies reviewed the title, abstract, and full text after deduplication. The detailed results resulted in five lifestyle assessment tools, seven physical activity assessment tools, nine nutrition assessment tools, and three activity participation assessment tools. Conclusion : For the lifestyle assessment tools for children and adolescents, the component items, measurement methods, and age of assessment of each item were analyzed. It is hoped that this will be used as a basis for the development of standardized assessment tools to assess the multi-aspect lifestyles of children and adolescents in the future.
Objective : This study aimed to assess the effects of occupation-based lifestyle intervention programs on older adults in the local community. Methods : Nine community-dwelling older individuals participated in this study using a one-group pre-post design. The occupation-based lifestyle intervention program consisted of 12 group sessions, and one individual session was conducted for seven weeks. Occupational balance, activity occupancy, activity participation, depression, health-related quality of life, and program satisfaction were assessed. Results : The average attendance rate of the nine participants was 10.11 (SD=1.36). Overall occupational balance (p=.012), activity participation status (p=.008), performance (p=.012), and satisfaction with activity participation (p=.008) were increased. Furthermore, the results showed changes in leisure time (p=.008) and rest time (p=.008). Finally, there were some improvements in the overall health-related quality of life (p=.034) and depression scores (p=.012). Conclusions : Occupation-based lifestyle intervention programs positively affected occupational balance, activity occupancy, activity participation, depression, and health-related quality of life in community-dwelling older adults. This research suggests promising benefits and feasibility of the program for community-dwelling older adults.
Objective : The typical decline in function due to aging includes limitations in vision, hearing, cognitive function, and activities of daily living. Therefore, this study evaluated the mediating effect of cognitive function on the relationship between vision and hearing and activities of daily living. Objective : This study aimed to investigate the mediating effect of play on the relationship between toddlers' sleep habits and problem-solving. Methods : In total, 1,734 participants were selected from the 3rd wave of the Panel Study on Korean Children. A structural equation modeling approach was utilized to examine the relationship among toddlers' play, sleep habits, and problem-solving, as well as to investigate the mediating effect of play. Results : The monthly age of the study participants ranged from 23 to 32 months, with 885 (51.0%) boys and 849 (49.0%) girls. The indirect effects of play on problem-solving skills (β = 0.137, p = .006) were statistically significant, but the direct effects of sleep habits on problem-solving skills (β = -.015, p = .871) and the total effect (β = 0.122, p = .057) were not significant. Conclusion : This study indicated that sleep habits did not have a direct effect on problem-solving ability, but that the indirect effects were significant and fully mediated by play. Incorrect sleep habits can negatively affect lifelong development. Therefore, parents would need to be aware of whether their child is developing good sleep habits during the toddler age.
