A Comparative Study of Changes in Cognitive Function, Depression and Activities of Daily Living in Patients with Dementia, Mild Cognitive Impairment and Ischemic Stroke (치매, 경도인지장애, 허혈성 뇌졸중 환자에서 인지기능, 우울 및 일상생활수행능력의 변화 비교)

  • This study aimed to compare changes in cognitive function, depression and ability to perform activity of daily living (ADL) in patients with dementia, mild cognitive impairment (MCI), and ischemic stroke (IS) and to identify factors associated with changes in instrumental ADL. A total of 86 patients (dementia=30, MCI=32, and IS=24) were included to analyse cognitive function, depression, and basic and instrumental ADL obtained at the time of diagnosis and 1 year after baseline. Repeated measures analysis of variance and multiple linear regression were used. A significant group by time interaction was found in executive function (p=.037) and instrumental ADL (p=.023) across groups. The MCI group has little change in executive function and instrumental ADL from the baseline to 1 year after diagnosis while other two groups showed changes with the dementia group showing declines and the group of IS having improvement in these factors over time. Changes in executive function(p=.030) and basic ADL (p<.001) explained 26.9% in the variance of changes in instrumental ADL. These findings showed a different changing pattern in executive function during the first year after diagnosis of dementia, MCI, and IS which have cognitive changes as their main symptoms, probably leading to a different changing pattern in instrumental ADL. Healthcare professionals should routinely assess for executive function and instrumental ADL problems and intervene to maintain and improve these functional outcomes immediately after disease.

The Metabolic Syndrome is Associated with More Aggressive Prostate Cancer

  • Purpose: The aim of this study was to analyze any association between the metabolic syndrome (MetS) and risk of prostate cancer (PCa) and cancer grade among men undergoing radical prostatectomy for PCa. Materials and Methods: 50 patients with MetS and 50 patients without MetS who undervent radical prostatectomy (RP) were included in the study. Age at biopsy, height, weight, digital rectal examination (DRE), pre-biopsy PSA levels, prostate volume, histopathologic diagnosis after surgery and gleason scores were collected data from all patients. Histologic material obtained at biopsy was given a Gleason score; tumours with a Gleason score ${\geq}7$ were considered high grade and <7 were considered low grade. Results: The mean age at the time of biopsy was $63.7{\pm}5.94$ in patients with MetS and $61.6{\pm}6.14$ in patients without MetS. Men with MetS had significantly lower PSA levels (p=0.01) ($7.21{\pm}2.74$ and $8.81{\pm}2.72$, respectively). Also, the men with MetS had higher RP tumor grade (p=0.04). Conclusions: Men with MetS undergoing RP have lower PSA levels and have significantly higher grade PCa. We must be careful for screening PCa in patients with MetS. Although the patients had lower PSA levels, they may have high grade disease.

  • The assessment and rehabilitation of patients with cognitive dysfunction is a field that currently requires assistive technology. While the paper-and-pencil test, such as the line tracing test (LTT), is one of the most commonly used assessment methods for cognitive dysfunction, it has become time-consuming due to its manual characteristic. The aim of this study was therefore to establish a computer-based real-time assessment system for patients without compromising the usefulness of the conventional paper-and-pencil based user tools. A digital pen-based assessment and rehabilitation system, the ePen System, could eliminate the time required for manual assessment while maintaining the measurement accuracy. The proposed system may assist rehabilitation specialists to assess and diagnose patients with unilateral visual neglect. This system can be applied to a range of assessment and rehabilitation modalities based on pen and paper. It can also be used for various patients such as those with Parkinson's disease, stroke, or different forms of brain lesions.

A STUDY OF THE ROOT SURFACE AREA, ANATOMIC STRUCTURE AND LINEAR VARIATION OF THE ROOT SURFACE AREA OF THE MANDIBULAR SECOND MOLAR (하악 제2대구치 치근의 형태학적 구조 및 표면적에 관한 연구)

  • The thirty six mandibular second molars, which were extracted because of hopeless tooth due to advanced periodontal disease, were measured the length of mesial and distal root and the distance from cementoenamel junction to root separation. The molars were cross-sectioned every 1.5 milimeter from cementoenamel junction to root apex perpendicular to long axis and each section was photographed, projected and measured with a calibrated Digital Curvi-Meter(Com Curvi-8. Japan). The root surface area (RSA), percentage of the RSA and the linear variation of the RSA were calibrated for each 1. 5 mm section. The results were as follows. 1. The mean length of the roots was 12. 98mm for mesial root, 11.84 mm for distal root. The mesial root was longer than distal root.(p<0.01) 2. The mean distance from the cementoenamel junction to the point at which the root separate from the root trunk was 3.82mm for the buccal furcation and 4.75mm for lingual furcation. The buccal root separation was coronal than the lingual root separation.(p<0.01) 3. The total root surface area was $317.78mm^2$. 4. The mean surface area of the root trunk was $150.06mm^2$ and averaged 42.54% of the total root surface area. 5. The mean root surface area was $88.79\;mm^2$ for the mesial root, $78.93mm^2$ for distal root, The mesial root surface area was wider than the distal root surface area.(p<0.05) 6. In comparision, the mean root trunk surface area of the mandibular 2nd molar was wider than that of mandibular 1st molar(p<0.01), but each root of 2nd molar was smaller than that of 1st molar(p<0.01).

Treatment of Primary Hypertension by Oryung-san Monotherapy: a Case Report (오령산 단일 투여를 통한 일차성 고혈압 치험례)

  • Objectives : To report a case of the treatment of primary hypertension by Oryung-san (五苓散: Wulingsan in Chinese, Goreisan in Japanese) monotherapy. Methods : The blood pressure (BP) fluctuation was checked of a woman who had sudden BP elevation without a history of hypertension treatment. There were no specific history of disease and results for the laboratory examination and image diagnosis including MRI. The woman was diagnosed with primary hypertension and she was given supple of Oryung-san extract (Hanpoong Pharm Co.) 3 g, three times a day for three months. The BP has been checked with digital sphygmomanometer (HEM-7111, Omron Japan) in brachial artery at home. Results and Conclusions : The patient had had stable BP since three weeks after Oryung-san treatment was initiated. There were no subjective symptoms, then Oryung-san medication also had quit after three months treatments. This shows some possibility to control hypertension using Oryung-san, which resembles thiazide that acts on the distal convoluted tubule and inhibit sodium-chloride reabsorption. For further evaluation of the effectiveness, well-designed randomized controlled trials should be undertaken.

Differentially Expressed Genes by Methylmercury in Neuroblastoma cell line using suppression subtractive hybridization (SSH) and cDNA Microarray

  • Methylmercury (MeHg), one of the heavy metal compounds, can cause severe damage to the central nervous system in humans. Many reports have shown that MeHg is poisonous to human body through contaminated foods and has released into the environment. Despite many studies on the pathogenesis of MeHg-induced central neuropathy, no useful mechanism of toxicity has been established so far. In this study, two methods, cDNA Microarray and SSH, were performed to assess the expression profile against MeHg and to identify differentially expressed genes by MeHg in neuroblastoma cell line. TwinChip Human-8K (Digital Genomics) was used with total RNA from SH-SY5Y (human neuroblastoma cell line) treated with solvent (DMSO) and 6.25 uM (IC50) MeHg. And we performed forward and reverse SSH method on mRNA derived from SH-SY5Y treated with DMSO and MeHg (6.25 uM). Differentially expressed cDNA clones were sequenced and were screened by dot blot and ribonuclease protection assay to confirm that individual clones indeed represent differentially expressed genes. These sequences were identified by BLAST homology search to known genes or expressed sequence tags (ESTs). Analysis of these sequences may provide an insight into the biological effects of MeHg in the pathogenesis of neurodegenerative disease and a possibility to develop more efficient and exact monitoring system of heavy metals as environmental pollutants.

Diabetes Mellitus and HbA1c Levels Associated with High Grade Prostate Cancer

Mean PSA level $7.04{\pm}2.85$ in patients with DM and $8.7{\pm}2.86$ in patients without DM, respectively. Also, diabetic men had higher RP tumor grade than men without DM (p=0.04). We found that HbA1c levels were higher in patients who have high grade prostate cancer (p<0.05). Conclusions: Diabetic men undergoing RP have lower PSA levels and have significantly higher grade PCa. We must be careful for screening PCa in patients with DM. Although the patients had lower PSA levels, they might have high grade disease.

  • To assess the clinical value of time resolved imaging of contrast kinetics(TRICKS) MRA by comparison with conventional time of flight(TOF) MR angiography. Both TOF-MRA and TRICKS-MRA were performed in 17 patients with cerebrovascular disease and in 6 patients with brain tumor. Among 17 cerebraovascular patients, digital subtraction angiography(DSA) data were also obtained in 11 patients. TOF-MRA showed good spatial resolution but short in temporal resolution. Although TRICKS-MRA showed somewhat low spatial resolution, it showed superior temporal resolution by distinguishing vessel and tumor in all patients. Also, from the analysis of vessel-tumor relationship, TRICKS-MRA showed better performance than TOF-MRA. TRICKS-MRA makes it possible to image arterial, capillary and venous phase sequentially with very speedy manner and therefore, the clinical use of this method is highly suggestive for future use.

  • With gradual growth of digital image contents industry connected with computer graphics technology, users require high-quality animations similar to real world and want to observe the type of fishes or their swimming types through cyber aquarium and fish encyclopedia. This study designed and developed fish growth process system to express natural and dynamic movement of fish, which is the critical in expressing submarine environment. This system proposes new mopping technique not presented in existing mopping studies as well as simulation using algorithm that newly supplemented existing fish swimming types. In addition, disease infection status is realistically expressed, which may occur depending on environmental factors during their growth process. With comprehensive fish studies, different from existing fish studies, this study examines the overall features of fish with realistic simulation.

  • With the current rapid growth and increase in the world's population, the demand for nutritious food and fibers and fuel will increase. Therefore, there is a serious need for the use of breeding programs with the full potential to produce high-yielding crops. However, existing breeding techniques are unable to meet the demand criteria even though genotyping techniques have significantly progressed with the discovery of molecular markers and next-generation sequencing tools, and conventional phenotyping techniques lag behind. Well-organized high-throughput plant phenotyping platforms have been established recently and developed in different parts of the world to address this problem. These platforms use several imaging techniques and technologies to acquire data for quantitative studies related to plant growth, yield, and adaptation to various types of abiotic or biotic stresses (drought, nutrient, disease, salinity, etc.). Phenotyping has become an impediment in genomics studies of plant breeding. In recent years, phenomics, an emerging domain that entails characterizing the full set of phenotypes in a given species, has appeared as a novel approach to enhance genomics data in breeding programs. Imaging techniques are of substantial importance in phenomics. In this study, the importance of current imaging technologies and their applications in plant phenotyping are reviewed, and their advantages and limitations in phenomics are highlighted.
