Intracranial pial arteriovenous fistula (AVF) is a rare cerebrovascular lesion that has only recently been recognized as a distinct pathological entity. A 41-year-old woman (Patient 1) presented with the sudden development of an altered mental state. Brain CT showed an acute subdural hematoma. A red sylvian vein was found intraoperatively. A pial AVF was revealed on postoperative angiography, and surgical disconnection of the AVF was performed. A 10-year-old boy (Patient 2) presented with a 10-day history of paraparesis and urinary incontinence. Brain, spinal MRI and angiography revealed an intracranial pial AVF and a spinal perimedullary AVF. Endovascular embolization was performed for both lesions. The AVFs were completely obliterated in both patients. On follow-up, patient 1 reported having no difficulty in performing activities of daily living. Patient 2 is currently able to walk without assistance and voids into a diaper. Intracranial pial AVF is a rare disease entity that can be treated with surgical disconnection or endovascular embolization. It is important for the appropriate treatment strategy to be selected on the basis of patient-specific and lesion-specific factors in order to achieve good outcomes.
Purpose: The purpose of this study was to explore the effects of aquatic exercise program on pain, flexibility, grip strength, self-care activities, helplessness in osteoarthritis patients. Methods: This program was carried out three times a week for 6 weeks by 3 groups of patients among 42 patients in regular swimming pool. Data were analysed by paired t-test for pre and post comparison. Results: There were significantly improved in pain flexibility, grip strength, self-care activities and helplessness after the program. However no significant differences were found in fatigue, difficulty of daily living activities and depression. Conclusion: Aquatic program was proved to be an effective nursing intervention to increase the self-care activities flexibility and grip strength, and to decrease pain and helplessness in osteoarthritis patients.
Purpose: This study aims to identify the ADL and IADL of bed-ridden elderly. Also it is used to show fuctional status, and to investigate the content and the level of nursing services provided. Method: The subjects were 191 elderly who received visiting nurse service through public health centers in the Seoul Metopolitan and Chungnam Province. Data collection was conducted by public health center nurses during four months in 2000. Result: As for daily living activities, 100% of subjects had at least one difficulty in ADL and IADL. Among them, only 0.5% had moderate disabilities and 99.5% had severe disabilities by HFS, 27.9% were in a semi bed-ridden state and 72.1% were completely bed-ridden by JABC. The major service provided was a visiting nurse service which was preferable to the social welfare service. In the visiting nurse service, there was no significant difference according to the elderlys' functional status. In addition there was no standadization about the qualification of the visiting nurse, and single entry point for the nursing service. Conclusion: The researchers urgently suggest that a community based comprehensive service model has to be developed to respond to the needs of the elderly in Korea.
The self-report measure is a useful tool for evaluating self-recognized disability and difficulty in daily living activities. Although many studies and clinics used the Neck Pain and Disability Scale (NPDS) for measuring neck pain and functional impairment, there has not been much adaptation of this for use with Korean patients. The purpose of this study was to establish the reliability and validity of NPDS among Korean neck pain patients. Fifty-five subjects (32 males, 23 females) with neck pain enrolled in this study. They completed standardized self-administered questionnaires. The NPDS measures pain intensity; its interference with vocational, emotional, recreational, social, and functional aspects of living; and the presence and extent of associated factors. Reliability was determined by the intraclass correlation coefficient (ICC) and Cronbach's alpha for internal consistency. Validity was examined by correlating the NPDS scores to the Visual Analog Scale (VAS) score. The test-retest reliability of the translated versions of the NPDS was good ICC (2,1)=.86 (95%CI .76~.92). Cronbach's alpha value for NPDS was found to be .93, and this was statistically significant (p<.05). The criterion-related validity coefficient was .79 (p<.Oll. We conclude that the Korean version of NPDS has been shown to be a reliable and valid instrument for the assessment of neck pain. Successful linguistic and cultural translation will admit appropriate cross-cultural comparison for clinical analysis. Therefore, this study can be expected to be used as an adequate evaluation scale for neck pain related studies and treatments.
This study examines the relationship between mother's stress and cortisol level during late pregnancy and how it changes into postpartum periods. Thirty one families were recruited from Daejeon and Incheon in South Korea. The major findings were as follows: (1) The level of mothers' cortisol followed the typical diurnal pattern of decline throughout the day. Further longitudinal analysis revealed that the level of Korean mothers' cortisol steadily lowered from the 8th prenatal month to the 6th postnatal month. (2) During pregnancy, mothers reported to experience changes in physical activities, daily routines, eating habits, types or amount of social activities and sleeping habits. Among these they were stressed the most by changes in physical activities and social activities. (3) The results of postpartum depression tests conducted at two months after giving birth showed mothers at this time were mostly stressed from difficulty sleeping and unnecessary self criticism. (4) The level of psycho social stress in mothers at six months after giving birth was mainly on social roles and the questions about self reliance marked the highest points. (5) Overall, there was a tendency for the level of maternal stress to decrease definitely from before and after the delivery and even though the level of stress was steadily decreasing up to 6th postpartum month it was not as sharp as those of before and after the birth of the babies. The level of cortisol analysis also showed the same obvious tendency.
Objectives : This study was to investigate the level of oral health, xerostomia, and oral health related activities of daily living(ADL) and the clinical factors influencing oral health related quality of life(OHIP-14) in elderly for visiting healthcare. Methods : Data were obtained from a cross-sectional survey of 106 elderly people(mean age=$77.5{\pm}4.98$). Participants in the study were recruited when they attended the public health center in Cheonan-si. Data were collected by means of a personal interview from July 19 through to August 20, 2010. Oral health related ADL, OHIP-14 and dry mouth was measured by the total 24-items. The data was analysed with t-test, one-way ANOVA, and multiple regression using the SPSS program. Results : Participants had a mean of 13.6 remaining natural teeth, which was significantly with age and perceived oral health. Xerostomia score achieved a mean of 6.39 out of a maximum 12 points. Powerful predictors of OHIP-14 were natural teeth, masticatory difficulty, oral pain, PHP index, perceived oral health, age and living together for the elderly Conclusions : Based on the findings, OHIP-14 has a significantly impact on oral health related ADL. These results suggest that elder people oral health promotion should be developed assessment index which was measured predictors related oral health in the elderly for visiting healthcare.

The purpose of this study was diagnosed to grasp the needs for the elderly-elderly care activity in rural areas. The research was conducted in questions and case studies in 7 farming villages. The major findings of this study as follows: First, the senior citizens in rural areas were not in good health, and the illnesses were revealed to be arthritis, high blood pressure, and diabetes. At the occurrence of an illness, they complained about the discomfort in doing daily life routines and expected eir spouses or offsprings to te care of them. Elders in the rural community responded that they had difficulty and financial problems in family maintenance. Leisure activities included watching TV, playing card/board games, or spending time without any specific activities. Second, recipients of elderly-elderly care expected to receive services from a woman in her 50s $1{\sim}2$ hours, once to twice a week. Third, the service providers wanted to be elderly-elderly care takers, hoping to work once a week for $2{\sim}3$ hours. They wanted the education for the elderly-elderly care to be once a week for 3 hours, with understanding the elderly, consultation, preventing senile dementia, and health as the contents.

The purpose of this study is to clarify the concept of the frail elderly and to obtain theoretical evidence. The research method was conducted using the basic principles for conceptual analysis of Walker and Avant(2005). As a Result of a review of the literature about how to utilize the concept of a frail elderly, frail elderly might be in the intermediate state of health and disease. They can be defined as physically vulnerable in the sarcopenia, inflammation, insulin resistance, and preceding advanced disease, lead to hospitalization, falls, disability, and death. The attributes were physiological, psychological, and socio-environmental and economic factors, so they had multidimensional factors. They were required the assist daily living of another person. Also, their attributes had decreased the amount of recovery time and degree, and exhaustion. The attributes of frail elderly consisted of these facts: dynamic process, multidimensional factors, dependency, vulnerability. The frail elderly was a dynamic process that involves the possibility of change to health and disease, and include physical, mental, cognitive, and social environmental factors. In addition, the frail elderly was difficulty in daily life, physical vulnerability and difficulty in adaption. In conclusion, frail elderly as defined by the results of this study will contribute to the foundation of health care systems, including community visiting nursing to understand the level of frail elderly and systemic management to do not go into long term care.

To study the importance of family meals in adolescents, 251 middle school students were surveyed through a questionnaire on their family meal patterns, dietary behaviors, food intake, and life satisfaction. The family meals were defined as "meals with all family members living together" by 62.2% of the students. For the frequency of family meals, 42.2% of the students replied having family meals "More than once a day". A common reason for the difficulty in having a family meal was a "lack of time" (73.3% of the students). Students tended to respond that they would be most fond in having meals with entire family members with traditional Korean food. Having more frequent family meals was found to benefit both individual and familial dietary behavior. In terms of food intake according to the frequency of family meals, the group having frequent family meals consumed significantly more rice, tofu, legumes, meats, fishes, eggs, green vegetables, seaweeds, fruits, milk, and milk products. This indicates that students can achieve a balanced diet through family meals. In terms of emotional status, the group having more frequent family meals showed a higher satisfaction with their daily life, health, nutritional status, and care from their relatives. In terms of personal mental status, the group having more frequent family meals was also found to be more effective at controlling undesirable emotions such as loneliness, indignation, and lethargy. As a result of this study, students in the group having more frequent family meals were found to have a positive dietary behavior, a balanced nutrition, a higher life satisfaction, and a more stable mental status. This result is useful as nutritional and educational information in schools to impress upon the public the importance of family meals for adolescents.

Objectives : The purpose of this study is to find the effectiveness of Korean medical treatment on sense of balance by cerebral hemorrhage. Gaitview analysis and Berg balance scale(BBS) research were used to evaluate it. Methods : We selected patients who were hospitalized at the Kangnam Korean Hospital, Kyung Hee University Hospital from May 2014 to August 2014 that satisfied inclusion/exclusion criteria. Before taking treatment, we took gaitview analysis and BBS first to analyze the ability of balance and gait of patients. In gaitview analysis, we checked Center of gravity(COG), pressure ratio, ave pressure, gait time ratio, statistic score. For about 10 days, patients were given acupuncture treatment, herbal medicine and physical therapy. Afterwards, we compared patients' progression between before-treatment and after-treatment with the same evaluation scale. Results : In the period of treatment, patients showed static and dynamic index change within the normal range or increase. And in statistic score, 2 cases score are increased. In BBS, all cases score are increased. Conclusions : Korean medical treatment improves imbalance symptom of cerebral hemorrhage patient. So the ability of gait and activity of daily living is improved. But, this study has only 3 cases, and has no comparable study. Prospective and Larger study will be necessary.
